Earthquakes are catastrophic natural disasters that cause extensive damage to infrastructure and disrupt the lives of millions worldwide. Beyond the immediate physical and psychological damage caused by earthquakes, these events can significantly impact respiratory health. The inhalation of dust, smoke, particulates, toxic gases, and asbestos exposure can lead to various respiratory health pathologies. These include respiratory infections, exacerbations of pre-existing respiratory diseases, chest traumas, and pulmonary and venous thromboembolism. Longitudinal studies are necessary to assess the long-term respiratory health effects in affected populations. By addressing these knowledge gaps, future mitigation strategies and preparedness measures can be developed to minimize the respiratory health impacts of earthquakes and improve the well-being of affected communities. Robust building infrastructure and comprehensive earthquake preparedness are emerging as the most important determinants for not only mitigating building collapse but also significantly reducing the potential health impacts that follow. This comprehensive review aims to provide a systematic overview of the lung health impacts of earthquakes. It highlights the need for further research to identify specific pollutants, air contaminants, and environmental factors contributing to respiratory health issues following earthquakes. Aim: In this study, the role of lung cancer symptom screening in smokers aged 30 and over in Duzce city was investigated.Materials and Methods: 500 cases over 30 years of age with a smoking history randomly selected from 700 households were included in the study. The smoking habits and symptoms of lung cancer cases were evaluated by a questionnaire with a face to face manner. It was emphasized that smoking is an important risk factor for lung cancer. Smoking habits were re-questioned by phone in 353 of 474 active smokers 6 months later.Results: Of the 353 smokers, 32 (9.1%) were female and 321 (90.9%) were male. It consisted of cases with a mean age of 46±9 and an average of 36±24 pack/year smoking history. 130 cases (%36.8) were intended to smoking cessation and 65 (%18,4) had tried smoking cessation. In the 6 months follow-up 17 (%4.8) of them had given up smoking, 59 (%16) of them had decreased, and 2 (%0,6) of them had increased it. The cases who had given up smoking were not the ones who had an intention to smoking cessation and had tried to quit at the first interview. Those who quit smoking had a higher mean age (p=0.044) and had a longer smoking history (p=0.001). The presence of hemoptysis and clubbing symptoms significantly increased smoking cessation rates (p=0.048, p=0.013, respectively). In this study, the quit rate after 6 months of lung cancer symptom questioning (4.8%) was significantly higher than the spontaneous smoking cessation rate (1%) (p=0.001).Conclusion: Smoking cessation rates can be increased by screening for lung cancer symptoms. Along with smoking cessation outpatient clinics, such approaches can contribute to the fight against smoking., Amaç: Bu çalışmada Düzce ilinde 30 yaş ve üzeri sigara içenlerde akciğer kanseri semptom taramasının sigara bırakmadaki rolü araştırıldı.Gereç ve Yöntemler: Randomize 700 hane seçilerek 30 yaş üzeri sigara anamnezi olan 500 olgu çalışmaya alındı. Olguların sigara alışkanlıkları ve akciğer kanseri semptomları bir anket formuyla yüz yüze görüşmeyle sorgulandı. Sigaranın akciğer kanseri için önemli bir risk faktörü olduğu vurgulandı. Halen aktif sigara içen 474 olgunun 353'üne 6 ay sonra telefonla sigara alışkanlıkları yeniden sorgulandı.Bulgular: Toplam 353 sigara içen olgunun 32’si (%9,1) kadın, 321’i (%90,9) erkekti. Yaş ortalaması 46±9 olan olguların, ortalama 36±24 paket-yıl sigara anamnezi vardı. 130'u (%36,8) sigarayı bırakmayı düşünüyordu ve 65'i (%18,4) sigarayı bırakmayı denemişti. 6 ay sonraki kontrolde 17 olgu (%4,8) sigarayı bırakmış, 59 olgu (%16) sigarayı azaltmış, 2 olgu (%0,6) ise artırmıştı. Sigarayı bırakanların yaş ortalaması sigara içmeye devam edenlere göre daha yüksekti (p=0,044) ve sigarayı bırakanların daha uzun süredir sigara içme öyküleri vardı (p=0,001). Hemoptizi ve çomak parmak semptomlarının varlığı sigara bırakma oranlarını anlamlı düzeyde artırmaktaydı (sırasıyla p=0,048, p=0,013). Bu çalışmada akciğer kanseri semptom sorgulamasından 6 ay sonraki sigara bırakma oranı (%4,8), kendiliğinden sigarayı bırakma oranından (%1) anlamlı düzeyde yüksekti (p=0,001).Sonuç: Akciğer kanseri semptom taramasıyla sigara bırakma oranları artırılabilir. Amaç: Coronavirus Hastalığı-2019 (COVID-19) nedeniyle Düzce Üniversitesi Sağlık Uygulama ve Araştırma Merkezinde (Hastanesinde- DUHARH) yatarak tedavi gören hastaların Sosyal Güvenlik Kurumu (SGK) perspektifi açısından maliyet analizini yaparak hem hastane yönetimine hem de hastalığın yönetim sürecinde rol oynayan sağlık politikası yapıcılarına yol göstermektir.Materyal ve Metod: Araştırma, DUHARH’da COVID-19 hastalığı nedeniyle yoğun bakım ve diğer kliniklerde Mart/2020-Aralık/2020 dönemi içerisinde yatarak tedavi edilen 582 hastayı kapsamaktadır. Araştırmada örneklem seçilmemiş evrenin tamamı çalışmaya dahil edilmiştir. Retrospektif olarak elde edilen veriler, aşağıdan yukarı, doküman analizi ve çok değişkenli regresyon analiz yönetimiyle analiz edilmiştir. Results: Araştırma yapılan 582 hastanın %60’ı erkek (350 kişi), %40 kadın (232 kişi), ortalama yatış süresi 5,7 gün, Pandemi Yoğun Bakım Ünitesinde %23 (134 kişi) diğer pandemi servislerinde %77 (448 kişi) olduğu tespit edilmiştir. Hastane tarafından SSI’ya fatura edilen toplam tutar 7.378.695,00 TL ($1,052,595) olup bunun %79’nun müdahale maliyeti olduğu ve hasta başı günlük ortalama yatış maliyetin ± 2.099,80 TL ($ 299,54) olduğu saptanmıştır. Ayrıca cinsiyet ayrımının (erkeklerin), yaşlı hastaların, yatış yerinin (yoğun bakım) ve yatış süresi P, Objective: To guide both the hospital management and the health policymakers who play a role in the management process of their disease by analysing the costs of the patients receiving inpatient treatment in Düzce University Health Application and Research Center (Hospital - DUHARH) due to coronavirus disease 2019 (COVID-19) from the perspective of the Social Security Institution (SSI).Material and Method: The study covers 582 patients who received inpatient treatment in intensive care and other clinics in March/2020-December/2020 due to COVID-19 disease in DUHARH. In the study, all sample unselected populations were included. Retrospectively obtained data were analysed using bottom-up, document analysis, and multivariate regression analysis. Results: It was determined that 60% of the 582 patients studied were male (350 people), 40% female (232 people) and that the average hospitalization period was 5.7 days, 23% (134 people) in the Pandemic Intensive Care Unit and 77% (448 people) in other pandemic services. The total amount invoiced to SSI by the hospital was 7.378.695,00 TRY ($ 1,052,595). Preventing air pollution decreases hospital admissions due to respiratory infections in children. The aim of the study is to examine the impact of using more clean fuel for heating purpose on hospital admissions with respiratory tract diseases in children. The records of 126.940 pediatric patients who were admitted to Duzce University Medical Faculty Pediatric Emergency Medicine outpatient clinic between 2014 and 2017 were reviewed. The records of children diagnosed with respiratory diseases (n: 46137) were examined. During the same period, air monitoring measurements of the national air quality network were recorded. Of the children, 55.5% were male and 45.5% were female. Most of the children were among the age group of 2-5 years (44%). 28. Should isoniazid prophylaxis be prescribed to the patients under tumor necrosis factor-alpha antagonists independent of tuberculin skin test?

Introduction: The aim of this study was to present the follow-up results of 110 patients who were given anti-tumor necrosis factor alpha (TNF-α) therapy for rheumatic and dermatologic diseases in a country with a high rates of active and latent tuberculosis bacillus infection.Material and Methods: Between February 2008 and January 2015, 110 cases in the age range of 23-77 who are using anti-TNF-α were included in the study retro-prospectively.Results: 52.7% of them (n = 58) were male. The most common diagnoses were rheumatoid arthritis (42.7%) and ankylosing spondylitis (38.2%). Most frequently given treatment were infliximab 37.3% and etanercept 30.9%, respectively. The 65 patients whose first tuberculin skin test (TST) value "5 mm and above" was started daily 300 mg INH prophylaxis for 9 months but 3 patients had not been started because of refusing treatment. In only one case chemoprophylaxis has had to be interrupted because of high liver function test due to the INH prophylaxis. TST conversion was observed in 14 patients. Further follow-up, it was observed that 4 patients had TST's positivity. Isoniazide (INH) prophylaxis was started these 18 patients (42.9%). Although INH prophylaxis has been given in two patients, they developed active tuberculosis in follow-up.Conclusion: Considering the INH resistance in our country, all patients especially the ones with residual lesion and history of previous exposure, should be followed up closely during the anti-TNF-α treatment. Background: Although tumor markers have been used as biomarkers for monitoring response to therapy and detecting early relapse in malignancies, it is well known that most tumor marker increases are not specific enough to be used for diagnosis of cancer. Aim: The aim of this study was to investigate the serum concentrations of α-fetoprotein (AFP), carcinoembryonic antigen (CEA), cancer antigen (CA) 19-9, CA 125, CA 15-3 and IgE in asthma patients. Materials and Methods: Thirty-five newly diagnosed asthmatic patients and 14 healthy subjects were included into the study. Blood samples were drawn from antecubital vein of asthma patients and control subjects. Serum AFP, CEA, CA 125, CA 15-3 and CA 19-9 levels were determined by chemiluminescent and IgE levels by electrochemiluminescent immunometric method on automatic hormone analyzers. Results: Serum levels of AFP, CEA, CA 19-9, CA 125, and CA 15-3 of asthma patients were not significantly different when compared with the control group. In asthma patients, there was a significant negative correlation between serum IgE and CA 125 (r = -0.401; p = 0.017) and a positive correlation between CA 125 and CA 15-3 (r = 0.368; p = 0.029). Conclusions: We concluded that in asthma patients’ sera, AFP, CEA, CA 125, CA 19-9 and CA 15-3 levels were not different from control subjects. Elevation in any of these tumor markers in asthma patients should be a sign for clinicians to evaluate patients for additional diseases. Amaç: Tümör belirteçleri malignitelerin relapsının erken tespitinde ve tedaviye cevabın izlenmesinde biyobelirteçler olarak kullanılmasına rağmen, çoğu tümör belirtecinin kanser tanısında kullanmak için yeterince spesifik olmadığı bilinmektedir. Biz bu çalışmada astımlı hastalarda serum AFP, CEA, CA 19-9, CA 125, CA 15- 3 ve IgE düzeylerini araştırmayı amaçladık. Yöntem ve Gereç: Bu çalışmaya 35 yeni tanı konulmuş astım hastası ve 14 sağlıklı birey dahil edildi. Astımlı ve kontrol bireylerinin antekübital venlerinden kan örnekleri alındı. Otomatik hormon analizöründe, serum AFP, CEA, CA 125, CA 15-3 ve CA 19-9 düzeyleri kemiluminesans, IgE düzeyleri de elektrokemilüminesans immünometrik metodla ölçüldü. Bulgular: Astım hastalarının serum AFP, CEA, CA 125, CA 15-3 ve CA 19-9 düzeyleri kontrol grubundan istatistiksel olarak farklı değildi. Astım hastaları grubunda IgE ile CA 125 arasında negatif (r = -0,401; p = 0,017) ve CA 125 ile CA 15-3 arasında ise anlamlı pozitif korelasyon olduğu gözlendi (r = 0,368; p = 0,029). Sonuç: Astım hastalarının serumlarında AFP, CEA, CA 125, CA 19-9 ve CA 15-3 düzeyleri kontrol grubundan farklı olmadığı sonucuna vardık. Background: Adverse respiratory reactions have been reported with intravascular radiographic contrast media. The aim of the present study was to assess the effects of iohexol on pulmonary functions in patients undergoing diagnostic coronary angiography. Methods: Thirty patients diagnosed as coronary artery disease undergoing diagnostic coronary angiography were enrolled in the study. Subjects with chronic obstructive pulmonary disease, asthma, allergic bronchitis, myocardial infarction and documented systolic dysfunction by transthoracic echocardiography were excluded. The respiratory functions of the patients before and immediately after the coronary angiography were measured and arterial blood gas analyses were performed. The subjects were divided into two groups according to results of angiography as having coronary artery disease (Group 1) and without significant coronary artery disease (Group 2). The angiography procedures were performed by a single, experienced angiographer. The presented study was undertaken to investigate the respiratory health problems in family barns with one or more cows and at least one family member working in the barn. Methods. 150 workers (128 female, 22 male) from 4 villages of Yigilca district near the city of Duzce in north-west Turkey were enrolled in this study between October December 2011. An Occupational and Environmental Chest Diseases questionnaire developed by the American Thoracic Society, pulmonary function test, physical examination and investigation for nasal eosinophil were performed in all subjects. Results. The mean age of workers was 47.7 +/- 14.2 years. Cough was present in 24% of subjects. The rates of phlegm, wheezing, chest tightness and dyspnea were 13.3%, 6%, 6% and 27.3%, respectively. Obstructive ventilatory pattern was observed in 37 workers (24.6%). 43 workers (28.6%) showed restrictive ventilatory pattern. Nasal eosinophilia was detected in 47.3% (71/150) of the subjects. Pulmonary functions of workers with nasal eosinophilia did not differ from the other workers. There were statistically significant negative correlations between the duration of working in barns and respiratory functions. Conclusions. Pulmonary functions of barn workers have been found to be decreased related to the duration of barn working. Furthermore, respiratory symptoms increased in relation with both barn working and biomass consumption. Precautions should therefore be taken to ventilate both barns and houses. more...

However during prolonged alveolar hypoxia HPV occurs with many pulmonary diseases. Despite intensive studies, cellular mechanisms of HPV are still not well defined. G proteins are a family of membrane-associated proteins believed to be involved in the transduction of various signals including the regulation of vascular tonus. In this study, we aimed to determine the contribution of G(i) and G(s) proteins in hypoxic vasoconstriction of lamb isolated pulmonary artery rings. Pulmonary arteries were isolated from left lower lobe of freshly slaughtered lamb. Arteries suspended in an organ bath filled with Krebs-Henseleit solution and isometric contraction recorded continuously via an isometric transducer connected to a computerised polygraphy system. The solution aerated with 75% N-2 - 20% O-2 - 5% CO2 (normoxic) and 95% N-2 - 5% CO2 (hypoxic) pO(2) of bathing medium was measured continuosly using an oxygen electrode. Pertussis toxin and cholera toxin were used to investigate the role of G(i) and G(s) proteins. In the present study, we observed that hypoxia had no effect on resting force in large artery rings, but it caused a further contraction (1.7 +/- 0.5 mN/mm(2), n=10) in 3 mu M 5-HT precontracted pulmonary arteries rings. Hypoxic vasoconstriction was inhibited by preincubation with 2 mu g/ml cholera toxins (from 2.6 +/- 0.4 mN/mm(2), to 1.0 +/- 0.4 mN/mm(2), n=6) and potentiated by preincubation with pertussis toxins (2 mu g/ml) (from 0.6 +/- 0.4 mN/mm(2), to 1.7 +/- 0.3 mN/mm(2), n=6). Amaç: Ağır kronik obstrüktif akciğer hastalığı (KOAH) tanısıyla izlenen olgularda gelişebilecek göz komplikasyonlarını araştırmak amaçlanmıştır. Gereç ve Yöntem: Sigara kullanımına bağlı ağır KOAH gelişen toplam 40 hasta oküler bulgular yönünden araştırılmıştır. Elde edilen veriler, yaş ve cinsiyetleri uyumlu, sigara kullanmayan sağlıklı kontrol grubuyla istatistiksel yöntemlerle karşılaştırılmıştır. Sigara kullanımı ve KOAH tedavisinde kullanılan nazal inhaler kortikosteroid gibi ilaçlarla göz bulguları arasındaki ilişki araştırılmıştır. Bulgular: KOAH grubundaki hastaların tümünde 20 yılı aşkın süredir kronik sigara kullanma alışkanlığı mevcuttu. KOAH hastalarının ortalama yaşı 62, kontrol grubunun ise 60 yıldı (p>0.05). KOAH'lı olgularında en sık göz bulgusu, 40 hastanın 16'sında (%40) saptanan katarakttı. (p0.05) Sonuç: Sigara kullanımına bağlı ağır KOAH gelişen olgularda katarakta sıklıkla rastlanmaktadır. Bu olgular, rutin kontrollerde göz bulguları yönünden de değerlendirilmelidir. Objective: We aimed to evaluate ocular complications in patients followed-up for severe chronic obstructive pulmonary disease (COPD). Materials and Methods: Forty patients who were followed-up for severe smoking-induced COPD were evaluated for ocular findings. We noted the ocular findings and compared with the control group which included 40 age and sex-matched, healthy, non-smoking subjects by statistical methods. The relation of cigarette smoking and medications used for COPD with ocular findings were evaluated. Results: All patients in the COPD group were chronic smokers for more than 20 years. The mean age was 62 years old in COPD patients and 60 years old in the control group (p>0.05). The most common ocular finding was cataract which was seen in 16 of 40 (40%) cases with COPD (p0.05). Conclusion: Cataract is the major ocular complication in patients with cigarette smoking-induced severe COPD. Noninvasive mechanical ventilation is widely used for acute respiratory failure in a variety of etiologies. The recommended specific conditions were the exacerbation of chronic obstructive pulmonary disease, cardiogenic pulmonary edema, de novo acute hypoxemic respiratory failure, immunocompromized pneumonia, palliation, postoperative period, weaning and postextubation. Pneumonia and acute respiratory distress syndrome are common causes of acute hypoxemic respiratory failure. Noninvasive mechanical ventilation failure is high in this disease group compared to acute hypercapnic respiratory failure. Noninvasive mechanical ventilation can be recommended in patients with mild even in moderate acute respiratory distress syndrome and not in patients with severe acute respiratory distress syndrome. Due to insufficient evidence in patients with pneumonia and acute respiratory distress syndrome, no recommendation can be given for routine use of noninvasive mechanical ventilation. Although some patients benefit from noninvasive mechanical ventilation, they should be used by a team experienced on noninvasive mechanical ventilation in pneumonia and early acute respiratory distress syndrome. A skilled team, proper place, patient and devices can optimize prognosis. There should be a particular attention to shock, multiorgan failure and change of consciousness. Bu çalışmada KOAH'lılarda istirahat koşullarında ekspiratuar akım hızları ve kan gazı değerlerinin yürüme bandında efor testi parametreleri (treadmill exercise testing parametres: TETP) ile ilişkisi ve bunun hastalığın şiddetini öngörmedeki değeri araştırılmıştır. Bu çalışmaya 45 KOAH'lı hasta (kadın/erkek: 9/36, yaş ortalaması: 61±11yıl) ve 21 sağlıklı birey (kadın/erkek: 4/17, yaş ortalaması: 60±10 yıl) alındı. Tüm KOAH'lı hastalar (I. grup, FEV1 %51.3±7.2) FEV1 değerlerine göre 3 altgruba (II. grup/hafif KOAH, FEV1 %60-79, III. grup/orta düzeyde KOAH, FEV1 %40-59, IV. grup/ileri derecede KOAH, FEV1 < %40 ) ayrıldı. WOS: 000287819600022, Introduction: To investigate the effects of hazelnut exposure on pulmonary functions in a cross-sectional study on hazelnut processing workers. Materials-methods: One hundred fifty hazelnut workers (137 females and 13 males), and seventy controls (63 females and 7 males) were included. A questionnaire regarding the respiratory symptoms was given to all subjects. Standard pre-shift and post-shift forced expiratory pulmonary functions were obtained from all participitants by spirometry. Furthermore, dust particles were measured according to their sizes. Results: The occurences of common respiratory complaints as cough (8%) and dyspnea (6%) were significantly higher in hazelnut workers than the controls (p=0.009 and p=0.029, respectively). Both of the pre-shift and post-shift spirometric values of hazelnut workers were significantly lower than those of controls. Post-shift FVC (2738.9 +/- 652.2) was significantly lower than pre-shift FVC (2765.8 +/- 634.8) in hazelnut workers (p=0.024). Nine hazelnut (6.0%) workers have shown a decrease of more than 10% in post shift FEV 1 whereas none of the controls have had abnormal decrease in post shift values (p=0.060). Conclusion: A significant deterioration in both restrictive and obstructive pulmonary functions was observed in hazelnut workers. Preventive measures against dusty workplaces should be undertaken to protect the respiratory health of hazelnut workers. more...

Objective: The aim of this study was to determine burnout state and the coping methods in HCP working in oncology. Methods: Interview Form, Maslach Burnout Inventory and Ways of Coping Inventory were administered total 444 HCP. Results: At the result of study HCP scored 1.76 in emotional exhaustion, 1.05 in depersonalization and 2.25 in lack of personal accomplishment [score range: 0-4]. These scores demonstrate that HCP experience moderate level burnout. It was established that HCP utilize mostly search for social support approach (X: 2.04) and they were least frequently found to use submissive approach (X: 0,97) [range: 0-3] in coping with burnout. It was established that, for emotional exhaustion nurses, other HCP who are consider their income inadequate and feel they have excessive work load form a risk group and for depersonalization with high education level, males, physicians, other HCP who are find their income inadequate run higher risk. It was also found that physicians and HCP with higher level of education, HCP who are feel themselves under excessive work load have a higher risk of experiencing lack of personal accomplishment. Conclusion: Our study demonstrated that it was not possible to avoid burnout in oncology clinics. The administrators should rapidly put into effect projects and regulations which may protect personnel from burnout. more...

Kardiyovasküler sistemdeki etkileri incelendiğinde damar düz kaslarında gevşetici etkisi; çeşitli damar yataklarında endotele bağımlı ve endotelden bağımsız mekanizmalarla vazodilatatör etkilere sebep olduğu bildirilmiştir. Bu çalışmada önkasılım oluşturulmuş izole kuzu pulmoner arter ve venlerinde, hipoksinin damar yanıtlarına etkisi ile hipoksiye bağlı vazokonstriksiyonda resveratrolun etkisi araştırıldı. Krebs-Heinseleit solusyonu ile doldurulmuş organ banyosuna asılmış izole kuzu pulmoner damarları izometrik transduser aracılığı ile bilgisayarlı poligraf sisteminde sürekli kayıt edilmiştir. Çözelti oda havasıyla (normoksik) veya %95 N2 + %5 CO2 karışımıyla (hipoksik) havalandırılmıştır. İzole organ banyosundaki solüsyonun oksijen konsantrasyonu oksijen elektrodu ile ölçülmüştür. Ön kasılım oluşturmak için arterlerde Serotonin (5-HT), (10-5 M) ve venlerde de bir tromboksan analogu olan U46619 kullanılmıştır. Sonuçlarımız; RSV (20 ?M)’ün, U46619 ile önkasılım oluşturulmuş pulmoner venlerde hipoksik pulmoner vazokonstriksiyonu anlamlı şekilde inhibe ettiğini, ancak 5-HT ile önkasılım oluşturulmuş pulmoner arterlerde damar yanıtlarını anlamlı olarak etkilemediğini göstermiştir. Resveratrol (RSV) is a natural phytoalexin with many biological effects, including antioxidant, anti- platelet, anti- atherogenic, anti- inflamatory, estrogenic properties, immunomodulation and chemoprevention. In the cardiovascular system, it has been shown to cause relaxation of vascular smooth muscle, and reports indicate that RSV induces vasodilatory effects in several vascular beds by acting in endothelium dependent and independent manners. The aim of the study was to determine the role of RSV on hypoxia induced vasoconstriction in precontracted isolated pulmonary arteries and veins. Isolated lamb pulmonary vessels were suspended in an organ bath filled with Krebs Heinseleit solution and isolated contractions were recorded continuously via an isometric transducer connected to a computerized polygraph system. The solution was aerated with room air or a gas mixture of 95% N2 + 5% CO2 (hypoxic). The oxygen concentration of the bathing medium was measured using an oxygen electrode. Serotonin (5-HT)(10–5 M) for arteries and U46619, a thromboxane analog (TXA2) for veins, were used as pre contractile agents. Our results showed that in the presence of RSV (20 μM), hypoxia induced pulmonary vasoconstriction decreased in pulmonary veins pre contracted with U46619, but not in pulmonary arteries pre contracted with 5-HT. more...
