Greenland's peripheral glaciers are losing mass at an accelerated rate and are contributing significantly to sea level rise, but only a few direct observations are available. In this study, we use the unique combination of high-resolution remote sensing data and direct mass balance observations to quantify the contribution of a singular avalanche event to the mass balance of Freya Glacier (74.38° N, 20.82° W), a small (5.5 km2, 2021) mountain glacier in Northeast Greenland. Elevation changes calculated from repeated photogrammetric surveys in August 2013 and July 2021 show a high spatial variability, ranging from −11 to 18 m, with a glacier-wide mean of 1.56 ± 0.10 m (1.33 ± 0.21 m w.e.). After applying a seasonal correction of −0.6 ± 0.05 m w.e., the geodetic mass balance over the entire 8-year period (2013–2014 to 2020–2021) is found to be 0.73 ± 0.22 m w.e. A significant influence over the near-decadal mass balance stems from the exceptional winter mass balance of 2017–2018, which was 2.5 standard deviations above average (1.89 ± 0.05 m w.e.). After heavy snowfall in mid-February 2018, snow avalanches from the surrounding slopes affected more than one-third of the glacier surface and contributed 0.35 ± 0.04 m w.e., which is close to 20 % of the total winter mass balance of 2017–2018. Remote sensing data show that Freya Glacier is also prone to avalanches in other years but to a lesser spatial extent. Due to a gap in mass balance point observations caused by high accumulation rates (buried stakes) and the COVID-19 pandemic, the recently reported glacier-wide annual mass balances are rather crude estimates and show a negative bias of −0.22 m w.e. a−1 compared to the geodetic mass balance. Finally, we speculate that the projected future warming may increase the likelihood of extreme snowfall, thus potentially increasing the contribution of snow avalanches to the mass balance of mountain glaciers in Northeast Greenland.

The negative surface mass balance of glaciers and ice caps under a warming climate impacts local ecosystems, influencing the volume and timing of water flow in local catchments while also contributing to global sea level rise. Peripheral glaciers distinct to the Greenland ice sheet respond faster to climate change than the main ice sheet. Accurate assessment of surface mass balance depends on in situ observations of near-surface climate and ice ablation, but very few in situ observations of near-surface climate and ice ablation are freely available for Greenland's peripheral glaciers. The transect of three automated weather and ablation stations on the peripheral A. P. Olsen Ice Cap in northeast Greenland is an example of these much needed data. The transect has been monitored since 2008, and in 2022, the old weather and ablation stations were replaced by a new standardized setup. In order to ensure comparable data quality of the old and new monitoring station setups, it is necessary to re-evaluate the data collected between 2008 and 2022. This paper presents the fully reprocessed near-surface climate and ablation data from the A. P. Olsen Ice Cap transect from 2008 to 2022, with a focus on data quality and the usability in ice ablation process studies. The usability of the data is exemplified by the data in an energy balance melt model for two different years. We show that the inherent uncertainties in the data result in an accurate reproduction of ice ablation for just one of the two years. A transect of three automatic ablation and weather stations of this length is unique to Greenland's peripheral glaciers, and it has a broad scale of usage from input to climate reanalysis and detailed surface ablation studies. Abstract To address limitations in current approaches for treating large peripheral nerve defects, the presented study evaluated the feasibility of functional material-mediated physical stimuli on peripheral nerve regeneration. Electrospun piezoelectric poly(vinylidene fluoride-trifluoroethylene) nanofibers were utilized to deliver mechanical actuation-activated electrical stimulation to nerve cells/tissues in a non-invasive manner. Using morphologically and piezoelectrically optimized nanofibers for neurite extension and Schwann cell maturation based on in vitro experiments, piezoelectric nerve conduits were synthesized and implanted in a rat sciatic nerve transection model to bridge a critical-sized sciatic nerve defect (15 mm). A therapeutic shockwave system was utilized to periodically activate the piezoelectric effect of the implanted nerve conduit on demand. The piezoelectric nerve conduit-mediated mechano-electrical stimulation (MES) induced enhanced peripheral nerve regeneration, resulting in full axon reconnection with myelin regeneration from the proximal to the distal ends over the critical-sized nerve gap. In comparison, a control group, in which the implanted piezoelectric conduits were not activated in vivo, failed to exhibit such nerve regeneration. In addition, at both proximal and distal ends of the implanted conduits, a decreased number of damaged myelination (ovoids), an increased number of myelinated nerves, and a larger axonal diameter were observed under the MES condition as compared to the control condition. Furthermore, unlike the control group, the MES condition exhibited a superior functional nerve recovery, assessed by walking track analysis and polarization-sensitive optical coherence tomography, demonstrating the significant potential of the piezoelectric conduit-based physical stimulation approach for the treatment of peripheral nerve injury.

Objectives: In acute coronary syndrome (ACS) patients the focus is on major conventional risk factors - CRF [diabetes, hypertension, elevated low-density cholesterol (LDL-C) and smoking] whereas others - specific metabolic risk factors - MRF [high-density lipoprotein cholesterol (HDL-C), body-mass index (BMI), waist-hip ratio (WHR), and triglycerides, and HbA1c get less attention. Methods: This is a prospective case–control observational study from 15 tertiary care hospitals in India. CRF and MRF in patients presenting with first incidence of ACS (n = 2153) were compared with matched controls (n = 1210). Objective: This is a prospective study of patients with LVEF ≤40%, with the objective of correlating CV events to LGE detected and quantified by CMRI. Methods: Heart Failure (HF) patients with LVEF 30% the corresponding figures were 9 (22.5%) vs 1 (2.8%) and 15 vs 1 respectively (p = 0.02). Conclusion: Demonstration of significant LGE by CMRI indicates high risk occurrence of CV events (CV hospitalization, appropriate shocks and total mortality) in NIDCM & ICM patients with LVEF

Photoaging is an important extrinsic aging factor leading to altered skin morphology and reduced function. Prior work has revealed a connection between photoaging and loss of subcutaneous fat. Currently, primary models for studying this are in vivo (human samples or animal models) or in vitro models, including human skin equivalents (HSEs). In vivo models are limited by accessibility and cost, while HSEs typically do not include a subcutaneous adipose component. To address this, we developed an “adipose-vascular” HSE (AVHSE) culture method, which includes both hypodermal adipose and vascular cells. Furthermore, we tested AVHSE as a potential model for hypodermal adipose aging via exposure to 0.45 ± 0.15 mW/cm2 385 nm light (UVA). One week of 2 h daily UVA exposure had limited impact on epidermal and vascular components of the AVHSE, but significantly reduced adiposity by approximately 50%. Overall, we have developed a novel method for generating HSE that include vascular and adipose components and demonstrated potential as an aging model using photoaging as an example.

ZnO doped with different concentrations (2, 4, 6, 8 and 10%) of ceria was synthesized by the conventional solid–state reaction method. X-ray diffraction spectra confirm that all the samples have a hexagonal structure. The structural properties of the samples were studied from X-ray diffraction data. The surface morphology and elemental composition of the ceria doped ZnO samples were characterized by scanning electron microscopy and energy dispersive X-ray spectroscopy. The variation of the dielectric constant, the dielectric loss and the ac conductivity as functions of frequency in the range of 100 Hz–1 MHz for the as-prepared material were studied at room temperature by using impedance spectroscopy. All the samples exhibit a normal dielectric dispersion behavior, i.e. it decreases with increasing frequency due to the Maxwell–Wagner type of interfacial polarization. The ac conductivity data was used to evaluate the maximum barrier height, the minimum hoping distance, and the density of localized states at Fermi level. Electrical storm (ES) is a life threatening clinical situation. Though a few clinical pointers exist, the occurrence of ES in a patient with remote myocardial infarction (MI) is generally unpredictable. Genetic markers for this entity have not been studied. In the present study, we carried out genetic screening in patients with remote myocardial infarction presenting with ES by next generation sequencing and identified 25 rare variants in 19 genes predominantly in RYR2, SCN5A, KCNJ11, KCNE1 and KCNH2, CACNA1B, CACNA1C, CACNA1D and desmosomal genes - DSP and DSG2 that could potentially be implicated in electrical storm. These genes have been previously reported to be associated with inherited syndromes of Sudden Cardiac Death. The present study suggests that the genetic architecture in patients with remote MI and ES of unstable ventricular tachycardia may be similar to that of Ion channelopathies. Identification of these variants may identify post MI patients who are predisposed to develop electrical storm and help in risk stratification.

Objective: This is a case series of consecutive patients with past myocardial infarction presenting with Electrical Storm (ES) of unstable ventricular tachycardia (VT) treated by a protocol directed algorithm. Methods: Management protocol involved treatment of reversible causes, ventilatory & hemodynamic support, administration of antiarrhythmic drugs (AAD) & maximally tolerated doses of beta-blockers, stellate ganglionectomy and Radiofrequency ablation (RFA) guided by Electro Anatomic Mapping (EAM). Patients were followed up periodically with review of device data logs. Results: There were 12 patients (mean age = 61.38 ± 6.48 years & mean LVEF = 31.92 ± 4.23%). Presentation was recurrent ICD shocks (n = 5) or VT (n = 7). All were mechanically ventilated. Reversible causes were identified in 4 patients and appropriately addressed. Totally 8 patients underwent endocardial substrate modification by EAM & RFA. Endocardial LV Voltage mapping demonstrated a mean scar area of 70.04 ± 17.63 sq.cm (27.04 ± 6.20% of mapped area). The electrograms targeted for ablation included late potentials, fractionated electrograms, double potentials and channels within the scar. Two patients had stellate ganglionectomy in addition. Ten patients (83.3%) survived to discharge, all of whom are alive at a follow up of 30.12 ± 19 months free of ES. VT free survival at end of follow up was 80%. No patient had hospitalization related to VT. Single episode of VT recurrence was seen in 2 patients at 7 months and 1 year of follow up respectively. Conclusion: In post myocardial infarction patients presenting with ES and unstable VT, a protocol driven approach involving substrate modification targeting abnormal electrograms improves outcomes. ZnO doped with different concentrations of Al2O3 (2, 4, 6, 8 and 10 wt%) is prepared by conventional solid-state reaction method. X-ray diffraction results revealed that the samples were crystalline with a hexagonal wurtzite phase. As the concentration of alumina (Al2O3) increases in ZnO, the X-ray diffraction peaks shifts towards higher angle. This shifting in peak position and decrease in intensity reflect that Al is successfully replaced Zn in ZnO matrix. X-ray peak broadening analysis was used to evaluate the crystallite size and lattice strain by the Williamson–Hall (W–H) method and size-strain plot (SSP) method. The physical parameters such as strain, stress, and energy density values were also calculated using W–H method with different models namely uniform deformation model, uniform stress deformation model and uniform deformation energy density model. The surface morphology and elemental analysis of the prepared samples were characterized by field emission scanning electron microscopy and energy dispersive spectra.

Geometric effects induced by the underlying terrain slope or by tilt errors of the radiation sensors lead to an erroneous measurement of snow or ice albedo. Consequently, artificial diurnal albedo variations in the order of 1–20 % are observed. The present paper proposes a general method to correct tilt errors of albedo measurements in cases where tilts of both the sensors and the slopes are not accurately measured or known. We demonstrate that atmospheric parameters for this correction model can either be taken from a nearby well-maintained and horizontally levelled measurement of global radiation or alternatively from a solar radiation model. In a next step the model is fitted to the measured data to determine tilts and directions of sensors and the underlying terrain slope. This then allows us to correct the measured albedo, the radiative balance and the energy balance. Depending on the direction of the slope and the sensors a comparison between measured and corrected albedo values reveals obvious over- or underestimations of albedo. It is also demonstrated that differences between measured and corrected albedo are generally highest for large solar zenith angles.

Introduction: There is high incidence of SCD in the early period following STEMI. We compared the temporal patterns and predictors of SCD amongst patients with LVEF ≤35% and LVEF >35%. Methods: Data from STEMI patients was prospectively collected. SCD cases formed the study cohort and were categorized into 2 groups based on their LV function. Results: There were 929 patients (mean age 55 ± 17 years) with a follow up of 41 ± 16 months. 154 pts (16.6%) had LVEF ≤35% (Group A, LVEF-29.9% ± 6%) and 775 pts had LVEF >35% (Group B, LVEF – 49% ± 14%). The two groups were similar with respect to sex distribution, age, prevalence of hypertension, and mean period of presentation. They differed in incidence of anterior wall MI (77.2% vs 55%), reperfusion (69% vs. 75%), prevalence of diabetes (50.6% vs 42%), and medication non-compliance (34% vs. 13%). Introduction: Intracoronary (IC) papaverine which is one of the commonly used agents for Fractional Flow Reserve (FFR) estimation has been reported to cause transient ST elevation in some patients. This phenomenon has not been systematically studied. Material and methods: This is a prospective, observational study. Consecutive patients, who underwent FFR at our institute using IC papaverine from May 2012 to April 2013, were included. FFR was done when clinically indicated. The procedure involved administration of 20 mg papaverine (Paparin® – Troikaa, Ahmedabad) as a fast bolus by intracoronary route followed by a 10 cc contrast flush, following which pressure measurements were made. Continuous ECG recording by Philips Hemodynamic Laboratory was obtained for all patients throughout the procedure. Post procedure, they were observed for any delayed effects and eventual outcome was documented. Fischer's mid-p test was used for statistical analysis. Result: Twenty-five patients (18 males, 7 females, mean age 57.9 ± 20 years) underwent FFR using Papaverine. The mean LVEF was (51 ± 15%). Fourteen patients (56%) developed transient ST elevation ≥0.5 mm in one or more leads which resolved spontaneously in all cases without any sequelae. The presence of a significant lesion either in the coronary artery being evaluated or in a remote coronary artery did not predict the ST elevation. 70.5% of diabetics (p = 0.02), 75% of hypertensives (p = 0.008) and 75% of patients with LVH (p = 0.008) had ST elevation. None of the 5 patients without any one of these comorbidities showed ST elevation. Conclusion: Transient ST elevation occurs in a significant proportion of cases receiving IC papaverine which is not associated with any adverse clinical outcomes. Micro vascular dysfunction is the most likely mechanism of this phenomenon.

Modern readers who investigate religious theories and practices are exposed to diverse truth claims and worldviews. Such claims are often conflicting and subject the readers to various misconceptions and misguidance. In Buddhism, the Buddha is said to have awakened to the true nature of existence and attained final liberation from suffering, referred to as “enlightenment.” How was he able to convince his disciples of his self-claimed enlightenment? Can his reasoning be applied to modern readers, who are well-educated, but overloaded with the incessant proliferation of digital information? The Buddha, specifically in the Kālāma Sutta, presents empirically testable guidelines, termed the “Ten Criteria,” which were formulated as an integrated interplay of reasoning and morality. This essay examines the Buddha’s strategy, which is empirical and pragmatic in nature and embraces the fundamental principles of modern science. We contend that his proposed methodology is verifiably evocative of a moral discipline, while presenting a pedagogical approach to the teacher−student dynamic. Serving as a reference point, this view may help modern readers in differentiating the right truth from the biased post-truths, which appeal to emotion and personal belief.

A mesocosm experiment was conducted to evaluate the influence of photosynthetic performance on the energetic balance of coastal phytoplankton, in relation to community production and autotrophic phytoplankton biomass in future coastal oceans. Natural phytoplankton assemblages were incubated in field mesocosms under ambient condition (control: ca. 400 μatm CO2 and ambient temperature), and two sets of potential future ocean conditions (acidification: ca. 900 μatm CO2 and ambient temperature; greenhouse: ca. 900 μatm CO2 and 3 °C warmer). The photosynthetic performances were estimated by in vivo fluorometry (effective quantum yield (ΦPSII), steady-state light response curves (LCs)) and in situ incorporation of 14C (photosynthesis-irradiance curves). The ΦPSII and rETRm,LC (relative maximum electron transport rate) clearly reduced under acidification, in particular, when phytoplankton were exposed to high light levels. However, PBmax (maximum photosynthetic rate) was the same in the ambient and acidification conditions. Thus, phytoplankton utilized less light under acidification condition, but could still assimilate a similar amount of carbon compared to the ambient condition. The PBmax and α (photosynthetic efficiency) under greenhouse condition were significantly higher than those under ambient condition without any difference in ΦPSII, rETRm,LC and α,LC (electron transport efficiency) between the treatments. Therefore, phytoplankton utilized the same amount of light under greenhouse condition, but could assimilate more carbon than under ambient condition. As a result, Chl a normalized primary production was higher in greenhouse than in other conditions. Nevertheless, the community production did not change between the experimental treatments. The main reason for the lack of a change in primary production under future climate conditions is the control of autotrophic phytoplankton biomass by grazing. Consequently, acidification and greenhouse environments have a potential to increase growth and primary production of phytoplankton by enhancing inorganic carbon assimilation efficiency when top-down regulation is negligible.

Glacier-wide mass balance has been measured for more than sixty years and is widely used as an indicator of climate change and to assess the glacier contribution to runoff and sea level rise. Until recently, comprehensive uncertainty assessments have rarely been carried out and mass balance data have often been applied using rough error estimation or without consideration of errors. In this study, we propose a framework for reanalysing glacier mass balance series that includes conceptual and statistical toolsets for assessment of random and systematic errors, as well as for validation and calibration (if necessary) of the glaciological with the geodetic balance results. We demonstrate the usefulness and limitations of the proposed scheme, drawing on an analysis that comprises over 50 recording periods for a dozen glaciers, and we make recommendations to investigators and users of glacier mass balance data. Reanalysing glacier mass balance series needs to become a standard procedure for every monitoring programme to improve data quality, including reliable uncertainty estimates.

Intra-cardiac extension of tumour thrombus of follicular carcinoma of thyroid is uncommon. We report a case of advanced thyroid carcinoma where tumour thrombus was well profiled with CT scan and transesophageal echo images and extension was noted from SVC into right atrium, with clinical features of superior vena cava syndrome. The clinical significance of the “ring sign” is discussed.

The Buddha is said to have awakened to the true nature of existence and attained final liberation from suffering through the practice of Satipaṭṭhāna. This practice begins by addressing sensations from the processes of body and mind, as characterized by ‘bare attention’ and ‘clear comprehension’ through non-judgmental observation, ultimately effecting a transformation into a unique religious experience. During its transmission to East Asian countries, particularly in the Chan tradition, the essence of Satipaṭṭhāna-sutta has become transformed, while maintaining the theme of intense concentration, perhaps in the form of ‘counter-illumination’—an extended equivalent of ‘bare attention’. Not much has been written on which aspects of the Indian contemplative tradition were passed on to the Chan/Seon schools. In the Korean Ganhwa Seon practice, however, there are some indications that the spirit of Satipaṭṭhāna, resonating as a role of sustained attention with mindfulness, has been partially manifested, having crystallized into the mindful hwadu called Sisimma, or ‘Sati-Sisimma’. To substantiate this, this paper investigates how the two seemingly different practices can be seen to link together in the Korean Seon tradition, and proposes pari passu meditative parallels, Satipaṭṭhāna and Sati-Sisimma, recommending for an ‘attentive’ mode and a ‘non-attentive’ mode respectively, in modern meditative practices.

Sudden Cardiac death (SCD) is a major clinical event causing adverse impact on global economy. This review summarizes the available epidemiological data on SCD from different parts of the world. It contrasts the Indian and global perception on the issues influencing data collection, burden of SCD and sudden deaths occurring following Myocardial Infarction. The differences in data from India and rest of the world are highlighted.

Sera from 326 Norwegian reindeer (NR) and from 40 Svalbard reindeer (SR) were examined for antibodies to reindeer herpesvirus (RHV), bovine viral diarrhoea virus (BVDV) and parainfulenza type 3 virus (PIV-3). No antibodies to any of these three viruses were detected in sera from SR. Sixty-three percent of sera from 101 adult NR (> 12 months old) and 15% of 225 NR calves (6 months old) had antibodies to RHV; corresponding values for BVDV were 41% and 6%, respectively. Twenty-seven percent of adult NR and 1% of NR calves had antibodies to both viruses. No antibodies to PIV-3 were detected in any NR sera.

Introduction: Congenital long QT syndrome (LQTS) is a genetically transmitted cardiac channelopathy that can lead to sudden cardiac death. This study aimed to report the clinical and genetic characteristics of all young patients diagnosed with LQTS in the only tertiary paediatric cardiology centre in Hong Kong., Methods: This is a retrospective review of all paediatric and young adult patients diagnosed at our centre with LQTS from January 1997 to December 2016. The diagnosis of LQTS was established with a corrected QT interval (QTc) ≥480 ms, Schwartz score of >3 points, or the presence of a pathogenic mutation., Results: Fifty-nine patients (33 males) from 52 families were included, with a mean age of 8.17 years (range, 0.00-16.95 years) at presentation. Five patients had concomitant congenital heart diseases. The mean follow-up duration was 5.33 ± 4.65 years. The mean QTc in the cohort was 504 ± 47 ms. They presented with syncope and convulsion (49%), cardiac arrest (10%), bradycardia and neonatal atrioventricular block (12%). Fifteen (25%) patients were asymptomatic at diagnosis. Thirty-eight (64.4%) patients were confirmed to have a pathogenic mutation for LQTS genes. Forty-five (76.3%) patients received beta blocker therapy. Thirteen (22.0%) patients required implantable cardioverter defibrillator. There was no mortality in the study period. The 1-, 5-, and 10-year breakthrough cardiac event-free rates were 93.0%, 80.7%, and 72.6%, respectively., Conclusion: Identification of the disorder, administration of beta blockers, and lifestyle modification can prevent subsequent cardiac events in LQTS. Genotyping in patients with LQTS is essential in guiding medical therapy and improving prognosis.

Isolated primary hepatocytes from the liver are very similar to in vivo native liver hepatocytes, but they have the disadvantage of a limited lifespan in 2D culture. Although a sandwich culture and 3D organoids with mesenchymal stem cells (MSCs) as an attractive assistant cell source to extend lifespan can be used, it cannot fully reproduce the in vivo architecture. Moreover, long-term 3D culture leads to cell death because of hypoxic stress. Therefore, to overcome the drawback of 2D and 3D organoids, we try to use a 3D printing technique using alginate hydrogels with primary hepatocytes and MSCs. The viability of isolated hepatocytes was more than 90%, and the cells remained alive for 7 days without morphological changes in the 3D hepatic architecture with MSCs. Compared to a 2D system, the expression level of functional hepatic genes and proteins was higher for up to 7 days in the 3D hepatic architecture. These results suggest that both the 3D bio-printing technique and paracrine molecules secreted by MSCs supported long-term culture of hepatocytes without morphological changes. Thus, this technique allows for widespread expansion of cells while forming multicellular aggregates, may be applied to drug screening and could be an efficient method for developing an artificial liver.

Objectives: To examine the molecular pathogenetic mechanisms, (epi)genotype-phenotype correlation, and the performance of the three clinical scoring systems-namely Netchine et al, Bartholdi et al, and Birmingham scores-for patients with Silver-Russell syndrome in Hong Kong., Methods: This retrospective case series was conducted at two tertiary genetic clinics, the Clinical Genetic Service, Department of Health, and clinical genetic clinic in Queen Mary Hospital in Hong Kong. All records of patients with suspected Silver-Russell syndrome under the care of the two genetic clinics between January 2010 and September 2015 were retrieved from the computer database., Results: Of the 28 live-birth patients with Silver-Russell syndrome, 35.7% had H19 loss of DNA methylation, 21.4% had maternal uniparental disomy of chromosome 7, 3.6% had mosaic maternal uniparental disomy of chromosome 11, and the remaining 39.3% were Silver-Russell syndrome of unexplained molecular origin. No significant correlation between (epi)genotype and phenotype could be identified between H19 loss of DNA methylation and maternal uniparental disomy of chromosome 7. Comparison of molecularly confirmed patients and patients with Silver-Russell syndrome of unexplained origin revealed that postnatal microcephaly and café-au-lait spots were more common in the latter group, and body and limb asymmetry was more common in the former group. Performance analysis showed the Netchine et al and Birmingham scoring systems had similar sensitivity in identifying Hong Kong Chinese subjects with Silver-Russell syndrome., Conclusion: This is the first territory-wide study of Silver-Russell syndrome in Hong Kong. The clinical features and the spectrum of underlying epigenetic defects were comparable to those reported in western populations.

Objective: To report our experience in the management of catecholaminergic polymorphic ventricular tachycardia in Hong Kong Chinese children., Methods: This case series study was conducted in a tertiary paediatric cardiology centre in Hong Kong. All paediatric patients diagnosed at our centre with catecholaminergic polymorphic ventricular tachycardia from January 2008 to October 2014 were included., Results: Ten patients (five females and five males) were identified. The mean age at presentation and at diagnosis were 11.0 (standard deviation, 2.9) years and 12.5 (2.8) years, respectively. The mean delay time from first presentation to diagnosis was 1.5 (standard deviation, 1.3) years. They presented with recurrent syncope and six patients had a history of aborted cardiac arrest. Four patients were initially misdiagnosed to have epilepsy. Catecholaminergic polymorphic ventricular tachycardia was diagnosed by electrocardiogram at cardiac arrest (n=2), or provocation test, either by catecholamine infusion test (n=6) or exercise test (n=2). Mutations of the RyR2 gene were confirmed in six patients. Nine patients were commenced on beta-blockers after diagnosis. Despite medications, three patients developed aborted or resuscitated cardiac arrest (n=2) and syncope (n=1). Left cardiac sympathetic denervation was performed in five patients and an implantable cardioverter defibrillator was implanted in another. There was no mortality during follow-up., Conclusions: Catecholaminergic polymorphic ventricular tachycardia should be considered in children who present with recurrent syncope during exercise or emotional stress. Despite beta-blocker treatment, recurrent ventricular arrhythmias occur and may result in cardiac arrest.

Approximately 15% of preterm infants may develop postnatal cytomegalovirus (CMV) infection from seropositive mothers via breast milk and are at risk for neurological sequelae in childhood. The aims of this study were to assess the effects and outcomes on growth, neurodevelopmental status, and hearing in very low birth weight (VLBW) premature infants with postnatal CMV infection via breast milk at the corrected age of 12 and 24 months.The prospective follow-up study population comprised all living preterm children (n = 55) with a birth weight ≤1500 g and gestational age of ≤35 weeks, who had been participated in our "postnatal CMV infection via breast milk" studies in 2000 and 2009, respectively. The cohort of children was assessed at 12 and 24 months. Clinical outcomes were documented during hospitalization and after discharge. Long-term outcomes included anthropometry, audiologic tests, gross motor quotient, Infant International Battery, and neurodevelopmental outcomes; all were assessed at postcorrected age in 12 and 24 months during follow-up visits.Of the 55 infants enrolled in the study (4 noninfected infants were excluded because their parents did not join this follow-up program later), 14 infants postnatally acquired CMV infection through breast-feeding (infected group) and were compared with 41 infants without CMV infection (control group). No significant differences were observed between the groups with regard to baseline characteristics, clinical outcomes, anthropometry, or psychomotor and mental development on the Bayley scale of infant development. None of the infants had CMV-related death or permanent sensorineural hearing loss.Transmission of CMV from seropositive mother via breast milk to preterm infants does not appear at this time to have major adverse effects on clinical outcomes, growth, neurodevelopmental status, and hearing function at 12 and 24 months corrected age.

F-FDG PET/CT is a promising tool in detecting aortic graft infection. Present study investigated the value of dual-time-point F-FDG PET/CT imaging (DTPI) with delayed imaging in assessing aortic graft infection.Twenty-nine patients with suspected aortic graft infection were prospectively enrolled in this DTPI study. Two nuclear medicine physicians read all the images and achieved consensus about the measurement of maximal standardized uptake value (SUVmax) and grading of image quality. The percentages of SUVmax change between initial and delayed images were recorded as retention index (RI); sensitivity, specificity, and accuracy were calculated based on reference standard.All the 5 infected aortic grafts had positive RIs, which were generally higher than that of noninfected grafts. Those noninfected grafts had variable RIs. Seven patients had improved image quality in delayed imaging. DTPI with delayed image detected all the infected grafts with improved specificity (88%) and accuracy (90%), providing conspicuous delineation of the infected graft extent.In conclusion, noninfected aortic grafts had more variable RIs than infected ones. DTPI might be useful for detecting aortic graft infection, improving image quality, and enhancing delineation of the infected aortic grafts.
