1. A web-survey assessed attitudes toward evidence-based practice among psychotherapists in Austria

Abstract

Abstract Evidence-based practice (EBP) means integrating the best available scientific evidence with clinical experience and patient values. Although perceived as important by many psychotherapists, there still seems to be reluctance to use empirically supported therapies in clinical practice. We aimed to assess the attitudes of psychotherapists in Austria toward EBP in psychotherapy as well as factors influencing the implementation of EBP. We conducted an online survey. To investigate attitudes toward EBP, we used two subscales (“Limitations” and “Balance”) of a translated and validated short version of the Evidence-Based Practice Attitude Scale-36 (EBPAS-36). Participants provided perceived barriers and facilitators as answers to open-ended questions. We analyzed the responses mainly using descriptive statistics. Open answers were analyzed using a thematic analysis. In total, 238 psychotherapists completed our survey (mean age 51.0 years, standard deviation [SD] = 9.9, 76.9% female). Psychotherapists scored on average 2.62 (SD = 0.89) on the reversed EBPAS-36 subscale “Limitations,” indicating that the majority do not perceive EBP as limiting their practice as psychotherapists. They scored 1.43 (SD = 0.69) on the reversed EBPAS-36 subscale “Balance,” indicating that psychotherapists on average put a higher value on the art of psychotherapy than on evidence-based approaches. Organizational factors such as lack of time and access to research studies as well as negative attitudes toward research and a lack of skills and knowledge kept respondents from implementing EBP. Our study highlights that EBP is still not very popular within the psychotherapy community in Austria. The academization of psychotherapy training might change this in the future.

Abstract

Enhanced recovery after surgery (ERAS) protocols are widely established in adult urology, especially for cystectomies and procedures involving the (small) bowel. However, data concerning pediatric surgery and pediatric urologic surgery are scarce. Individual items of ERAS protocols have been well-studied and significant benefits have also been shown for children without increased risk of complications. In a number of small series the safety and efficacy of implementing ERAS protocols have been reported recently. A broad consensus and a guideline for a common pediatric ERAS protocol is not yet available.

Abstract

This study aimed to estimate the genetic parameters for lactation milk yield (LMY), lactation fat yield (LFY), and lactation protein yield (LPY) in the Sahiwal cattle breed of Pakistan. Performance data of 3364 purebred Sahiwal cattle with 9964 herd test records were collected from the public and/or private herds. Various edits were applied, such as herd test day records < 3, unknown calving and drying dates, etc., which reduced the data set to 1039 animals with 4489 herd test records. Lactation yields for milk, fat, and protein were calculated using the test interval method. Preliminary data analysis was performed using PROC MIXED in SAS. The animal model under the REML method was used to estimate the genetic parameters of the milk yield, milk fat, and milk protein. A multivariate model (3-traits) was fitted, which included the fixed effects of parity besides random effects of herd-year-season of calving (HYS), animal and residuals for all three traits. Der Hodenhochstand ist die haufigste genitale Fehlbildung, die bei neugeborenen Jungen diagnostiziert wird. In den letzten Jahren haben sich die Empfehlungen zur Diagnostik und Therapie durch die Implementierung neuer wissenschaftlicher Erkenntnisse teils deutlich verandert. Diese Arbeit soll einen Uberblick uber die aktuellen Empfehlungen der nationalen und internationalen Leitlinien zur Behandlung des Hodenhochstands geben. Die wesentlichen kontroversiellen Punkte werden im Licht der aktuellen Literatur diskutiert. Die Leitlinien der „European Association of Urology“ (EAU)/„European Society for Pediatric Urology“ (ESPU), der „American Association of Urology“ (AUA), des Arbeitskreises Kinderurologie der Osterreichischen Gesellschaft fur Urologie (OGU), der „International Consultation on Urological Disease“ (ICUD) und der Deutschen Gesellschaft fur Urologie (DGU)/der Deutschen Gesellschaft fur Kinderchirurgie (DGKCh) wurden hinsichtlich der wesentlich erscheinenden Punkte in Diagnose und Therapie verglichen. Bezuglich der meisten wesentlichen Schritte und Entscheidungen in der Behandlung des Hodenhochstands besteht weitgehend Konsensus. Aspekte der bildgebenden Diagnostik, die neoadjuvante Hormontherapie und der chirurgische Zugang bei nicht tastbaren Hoden sind die Themen, bei denen sich teils wesentliche Anderungen ergaben und grostenteils geringe Diskrepanzen bestehen.

Abstract

First measurements of hydrodynamic growth near peak implosion velocity in an inertial confinement fusion (ICF) implosion at the National Ignition Facility were obtained using a self-radiographing technique and a preimposed Legendre mode 40, λ=140 μm, sinusoidal perturbation. These are the first measurements of the total growth at the most unstable mode from acceleration Rayleigh-Taylor achieved in any ICF experiment to date, showing growth of the areal density perturbation of ∼7000×. Measurements were made at convergences of ∼5 to ∼10× at both the waist and pole of the capsule, demonstrating simultaneous measurements of the growth factors from both lines of sight. The areal density growth factors are an order of magnitude larger than prior experimental measurements and differed by ∼2× between the waist and the pole, showing asymmetry in the measured growth factors. These new measurements significantly advance our ability to diagnose perturbations detrimental to ICF implosions, uniquely intersecting the change from an accelerating to decelerating shell, with multiple simultaneous angular views.

Abstract

Cryptorchidism is the most common genital malformation in male newborns. In recent years, guidelines concerning diagnosis and therapy have undergone considerable evolution with the implementation of recent knowledge in pathophysiology, diagnosis, and therapy.The aim of this publication is to provide an overview of the current national and international guideline recommendations concerning diagnosis and treatment of cryptorchidism. Critical points are discussed in light of current scientific literature.The current guidelines of the European Association of Urology (EAU)/European Society for Pediatric Urology (ESPU), the American Association of Urology (AUA), the pediatric urologic task force of the Austrian Society of Urology (ÖGU), the international consultation on urological disease (ICUD) and the German Society of Urology (DGU)/German Association of Pediatric Surgery (DGKCh) have been analyzed concerning the most important aspects of treatment and diagnosis.There is broad consensus concerning most steps and decisions for the treatment of cryptorchidism. However, some aspects of diagnostic imaging, the use of hormonal therapy, and surgical access in nonpalpable testis warrant further discussion and are the fields of considerable changes.

Abstract

This paper presents quench/recovery results, both experimental and simulation, of a 10-cm long YBCO test sample subjected to an over-current pulse. The sample was cooled by liquid nitrogen, either in a bath or forced through a narrow channel. Bath provides a better cooling than flow, at least for velocities of 3 and 5 cm/s used in the experiment.

Abstract

This paper presents a reference design of a solid-nitrogen cooled 300-MHz/25 mm Nb/sub 3/Sn NMR magnet based on a novel design/operation concept. The magnet operates in the temperature range 8-10 K with its cryocooler thermally decoupled from the magnet and idled. A mass of solid nitrogen in thermal contact with the magnet prolongs the magnet warm-up time to 17 h, during which NMR measurement may be performed under a quiet environment free of the cryocooler's microphonics. At 10 K, the magnet is re-coupled to the cryocooler for recooling to 8 K and ready for another 17-h measurement.

Abstract

An experimental campaign to tune the initial shock compression sequence of capsule implosions on the National Ignition Facility (NIF) was initiated in late 2010. The experiments use a NIF ignition-scale hohlraum and capsule that employs a re-entrant cone to provide optical access to the shocks as they propagate in the liquid deuterium-filled capsule interior. The strength and timing of the shock sequence is diagnosed with velocity interferometry that provides target performance data used to set the pulse shape for ignition capsule implosions that follow. From the start, these measurements yielded significant new information on target performance, leading to improvements in the target design. We describe the results and interpretation of the initial tuning experiments.

Abstract

Ignition implosions on the National Ignition Facility (NIF) [Lindl et al., Phys. Plasmas 11 , 339 (2004)] are driven with a very carefully tailored sequence of four shock waves that must be timed to very high precision in order to keep the fuel on a low adiabat. The first series of precision tuning experiments on NIF have been performed. These experiments use optical diagnostics to directly measure the strength and timing of all four shocks inside the hohlraum-driven, cryogenic deuterium-filled capsule interior. The results of these experiments are presented demonstrating a significant decrease in the fuel adiabat over previously un-tuned implosions. The impact of the improved adiabat on fuel compression is confirmed in related deuterium-tritium (DT) layered capsule implosions by measurement of fuel areal density (ρR), which show the highest fuel compression (ρR ∼ 1.0 g/cm2 ) measured to date.

Abstract

Ignition implosions on the National Ignition Facility [J. D. Lindl et al., Phys. Plasmas 11, 339 (2004)] are underway with the goal of compressing deuterium-tritium fuel to a sufficiently high areal density (ρR) to sustain a self-propagating burn wave required for fusion power gain greater than unity. These implosions are driven with a very carefully tailored sequence of four shock waves that must be timed to very high precision to keep the fuel entropy and adiabat low and ρR high. The first series of precision tuning experiments on the National Ignition Facility, which use optical diagnostics to directly measure the strength and timing of all four shocks inside a hohlraum-driven, cryogenic liquid-deuterium-filled capsule interior have now been performed. The results of these experiments are presented demonstrating a significant decrease in adiabat over previously untuned implosions. The impact of the improved shock timing is confirmed in related deuterium-tritium layered capsule implosions, which show the highest fuel compression (ρR~1.0 g/cm(2)) measured to date, exceeding the previous record [V. Goncharov et al., Phys. Rev. Lett. 104, 165001 (2010)] by more than a factor of 3. The experiments also clearly reveal an issue with the 4th shock velocity, which is observed to be 20% slower than predictions from numerical simulation.

Abstract

The National Ignition Facility has been used to compress deuterium-tritium to an average areal density of ~1.0±0.1 g cm(-2), which is 67% of the ignition requirement. These conditions were obtained using 192 laser beams with total energy of 1-1.6 MJ and peak power up to 420 TW to create a hohlraum drive with a shaped power profile, peaking at a soft x-ray radiation temperature of 275-300 eV. This pulse delivered a series of shocks that compressed a capsule containing cryogenic deuterium-tritium to a radius of 25-35 μm. Neutron images of the implosion were used to estimate a fuel density of 500-800 g cm(-3).

Abstract

We demonstrate the hohlraum radiation temperature and symmetry required for ignition-scale inertial confinement fusion capsule implosions. Cryogenic gas-filled hohlraums with 2.2 mm-diameter capsules are heated with unprecedented laser energies of 1.2 MJ delivered by 192 ultraviolet laser beams on the National Ignition Facility. Laser backscatter measurements show that these hohlraums absorb 87% to 91% of the incident laser power resulting in peak radiation temperatures of T(RAD)=300 eV and a symmetric implosion to a 100 μm diameter hot core.

Abstract

Dislocation of tunneled hemodialysis catheters is rare. This report describes displacement of a split catheter with the tip of the arterial limb outside and the venous limb inside the jugular vein. Use of the catheter with the limbs in inverse fashion caused severe hemorrhage along the catheter tunnel. To avoid this complication, a short subcutaneous catheter tunnel is recommended in adipose patients.

Abstract

This document describes measurements that characterize an Advanced Research Systems DE204S cryocooler system. The data is relevant to the thermal performance and vibration characteristics of the cold-head. The thermal measurements include heat load mapping of the 1st and 2nd stage, and temperature fluctuation measurement of the 2nd stage heat station. A comparison of fluctuation measurements by four different sensors is also included to support the 2nd stage fluctuation results. Finally, optical measurement of the cyclic 2nd stage heat station deflection is described.

Abstract

Capsule implosions on the National Ignition Facility (NIF) [Lindl et al., Phys. Plasmas 11, 339 (2004)] are underway with the goal of compressing deuterium-tritium (DT) fuel to a sufficiently high areal density (ρR) to sustain a self-propagating burn wave required for fusion power gain greater than unity. These implosions are driven with a carefully tailored sequence of four shock waves that must be timed to very high precision in order to keep the DT fuel on a low adiabat. Initial experiments to measure the strength and relative timing of these shocks have been conducted on NIF in a specially designed surrogate target platform known as the keyhole target. This target geometry and the associated diagnostics are described in detail. The initial data are presented and compared with numerical simulations. As the primary goal of these experiments is to assess and minimize the adiabat in related DT implosions, a methodology is described for quantifying the adiabat from the shock velocity measurements. Results are contrasted between early experiments that exhibited very poor shock timing and subsequent experiments where a modified target geometry demonstrated significant improvement.

Abstract

Lameness is one of the major welfare issues in dairy cattle, but because of the increasing amount of automation in modern dairy farms and consequently a decreasing surveillance rate per individual, lameness often is recognized only when a severe, very obvious change in gait can be observed. This usually occurs after a long period of time during which the animal endures suffering from pain caused by the hoof lesion, seeing as cows will try to hide signs of disease (in this case: an obvious limp) as long as possible. This study evaluated automatically recorded performance and behaviour parameters as possible risk factors for lameness that could be used for an automatic lameness alert in order to detect lameness cases earlier. The study herd consisted out of 60 Simmental cattle housed in a free stall barn. The herd was observed continuously from April 2014 to April 2015 using different sensors to measure performance (milk yield, live weight, feed intake) and behaviour (feeding behaviour, standing, lying and activity index) related parameters, adding up to a total of 27 variables. During the data collection period the whole herd was locomotion scored weekly according to Sprecher et al. (1997) (1-5, 1 = healthy, 5 = severely lame) in order to assess lameness in every individual. After dividing the data into a training and a test dataset, the training set was fitted into a regularized regression method (Elastic Net) including relevant interaction terms. The preliminary model fitted on the training dataset had an AUC (Area under the Receiver Operating Characteristic Curve) of 0.95 with a sensitivity of 0.95 (95 %-confidence interval: 0.92-0.97) and a specificity of 0.83 (95 %-confidence interval: 0.79-0.86) and the same model fitted on the training data resulted in an AUC of 0.84 with a sensitivity of 1.00 (95 %-confidence interval: 0.19-1.00) and a specificity of 0.80 (95 %- confidence interval: 0.71-0.87). The most important risk factors were ‘feeding duration (day/night ratio)’, ‘lying time (day/night ratio)’, ‘milk yield*feed intake’ and ‘milk yield*lying time’.

Abstract

Lameness in dairy cows causes significant losses in dairy production and is an important animal welfare issue. The overall size increase of dairy establishments and the subjectivity of lameness scoring mean that farmers possibly recognise only a relatively small percentage of lame animals in their herd. Lameness recognition is often only recognised when the underlying pathological process is already well advanced, causing higher veterinary treatment costs, loss of milk yield and pain for the animal. The purpose of a current project at the Bavarian State Research Centre for Agriculture, Germany is to expand on the results of a previous project, which demonstrated that the association between various automatically recorded performance and activity parameters could be used to detect lameness in dairy cattle. The pedometers (“Track a Cow”, ENGS Corporation, Israel) used in said project are able to measure lying behaviour through accelerometers and feeding behaviour by identifying cows at the feeding bins by way of an induction loop. The same pedometers have recently been updated and are to be used in the current project. Therefore the technology’s reliability and validity will be evaluated. At the same time as the validation the data collected by the ENGS “Track a Cow” (“TAC”) pedometers will be compared to that collected by the more cost intensive “ALT” (Activity-Lying-Temperature, IBS Ing.Büro Schleusener, Mixdorf, Germany) tags which have been used successfully for many years, in order to evaluate the cost-benefit ratio of both devices.

38. Creation of Calculation Models for Estimation of Labour Requirement for Barn Dried Hay Production and Its Feeding on Dairy Farms

Abstract

The feeding of dairy cows with hay and production of so-called “hay milk” becomes an interesting production alternative for dairy farmers in some European countries. To minimise weather risk during hay production and to produce hay of high quality, the interest of farmers in barn hay drying techniques is growing. However, there is limited information available regarding labour requirement for barn dried hay production and its feeding by current used techniques. The aim of this study is to create calculation models (MS-Excel based) for the estimation of labour requirement for loose barn dried hay production (from harvesting until storing) and its feeding to dairy cows. Different variants with loader wagon (in use for harvesting and feeding) and hay crane (in use for filling up, shifting and emptying of drying boxes and also for feeding) are studied. To determine the standard times for individual work elements of interest, the selected tasks are timed on the level of individual work elements by the digital device Multidata (Drigus). Moreover, influence variables and procurement quantities (volumes, distances etc.) necessary for determination of standard times are recorded at each timing. The recorded data are regularly statistically evaluated (based on the data evaluations applied in the software Meza (Drigus) and SigmaPlot) to estimate if further measurements are necessary to be performed to obtain reliable standard times. Reliable standard times should have an accuracy of 10 % and 95 % confidence level. The standard times together with influence variables will be included in calculation models to estimate the labour requirement for the selected tasks. The calculation models should enable to calculate the labour requirement for existing farms as well as for “fictitious” farms depending on herd size and desired farm design. The timing of tasks by hay production will be performed during several cuts in course of this year. The time studies by feeding have started in March this year and will continue over the year. The first measurements were performed by feeding with the hay winch crane with remote control. The works on creation of calculation models for this variant have started as well, and preliminary data for this variant can be presented. For other variants and for tasks during dried hay production, the creation of calculation models will start after first measurements (for other variants by feeding between March and May and by hay production between May and June). The obtained information should enable the farmer to consider labour requirement for investigated systems regarding and thus support him by the determination those best suited to his condition.

Abstract

Aim of study: The main objective of this study is to introduce a reliable system for in vitro seed germination. Because Cedrus deodara stands are gradually decreasing in different regions of the world due to overexploitation, natural disasters and lower seed viability.Area of study: Swat is situated at the northwestern corner of Pakistan; with total area of 5337 square km. Total cultivated land is 95281 hectares in 2007, while the area under forest cover is 135427 hectares.Material and Methods: We used MS-medium with or without PGRs for overall in vitro seed germination. To enhance germination frequency, we applied different photoperiods (16hrs-Dark/8hrs-Light and 16hrs-Light/8hrs-Dark) and sterilization reagents (mercuric chloride and ethanol). Synthetic free radical of DPPH (1, 1-diphenyl-2-picrylhydrazyl) was applied for the determination of antioxidant activityMain results: Maximum shoots length (3.6 cm) and root length (2.6 cm) were recorded on MS-medium augmented with BA (1.0 mg L-1) and (GA3) 0.5 mg L-1) under light incubation (16hrs-Light/8hrs-Dark) after 2-3 weeks of inoculation. Without PGRs, maximum root length (4.0 cm, with shoots of 3.2 cm) was observed in dark incubation (16hrs-Dark/8hrs-Light), whereas light incubation produced maximum shoot length (3.5 cm) and minimum root length (1.5 cm). Lower concentration of HgCl2 (0.1%) showed a lower inhibitory effect on shoot and root length (2.4 cm and 2.5 cm) as compared to higher concentrations. The antioxidant potential was also investigated in different Cedrus organs and tissues.Research highlights: These results suggested that this simple protocol is useful for Cedrus deodara conservation and plantlets production for commercial purposes.Key Words: Cedrus deodara; Seed germination; Callus; 6-Benzyladenine; Antioxidant.

Abstract

The results of the first clinical trials of left ventricular assist devices are not encouraging. The only indication at present is in patients with cardiac failure after cardiac operations. The acute type of ellipsoid heart described in this paper-displays a canulation technique where the inflow canula inserted in the left ventricular cavity via the left atrium and the outflow canula is directly connected to the aortic canula of the cardiopulmonary bypass. The haemodynamic response shows the efficacy of this system. The circulation can be maintained in severe cardiac failure, as well as in ventricular fibrillation and cardiac arrest. On the basis of data obtained in experiments on 7 female calves this system is considered worthy of clinical trial.

Abstract

The serum level of the myocardial-specific isoenzyme of creatine phosphokinase, CK-MB was determined in 50 patients with severe multiple injuries or following extensive surgery. It is concluded that whenever the proportion of CK-MB exceeds 10% of the total creatine phosphokinase activity myocardial damage has occurred, especially since equivalent ECG changes were registered in this group of patients.

Tetanus is subdivided into 3 degrees of severity according to the course. When death occurs, we differentiate those cases due to the disease itself (16) and those resulting from late complications or avoidable technical defects (12). 82 of our 111 patients-79 men and women-had been neither passively nor actively immunized and none of the supposedly actively immunized had been inoculated properly. The horse serum available in earlier years was considered unsuitable while an intravenous human serum recently produced by Messrs Immuno of Vienna justified greater expectations. Sedation or prolonged sleep with and without relaxation, tube feeding and parenteral feeding, prolonged intubation or trachectomy, duration of the life-threatening phase of the disease, breaking off treatment ("stealing out"), mobilization and finally, the question of expense, bear no relation to the extremely small cost of a confirmed triple inoculation. An urgent consequence of this is obligatory active immunization in early childhood if an earlier inoculation has not been done, but also at any time afterwards.

A case of periodic paralysis is reported that had to be treated by mechanical ventilation. Symptoms and course of this attack are demonstrated and it is shown how diagnosis was reached. Provocative factors of periodic hypokaliemic paralysis are discussed and their importance in anaesthesia is emphasised.

Im Hochgebirge gibt es nicht nur Berge, sondern auch Talerl In den Talern, in welchen neben prachtigen Asphaltstrasen bekanntlich auch die „Traum“-Autobahn Europas durch unser Land (Tirol) fuhrt, gibt es auch zahlreiche Strasenverkehrsunfalle. Hinsichtlich Erstversorgung und Transport dieser Verkehrsverletzten bestehen keine wesentlichen Unterschiede zwischen unserem Gebirge und Ihrem Flachland. Wohl aber gibt es in unseren relativ dicht besiedelten Talern wesentlich mehr Orte, die nur wenige Kilometer von einanander entfernt liegen und in denen jeweils ein Praktischer Arzt niedergelassen ist. Er wird entweder direkt von anderen — voruberkommenden — Verkehrsteilnehmern oder von der herbeigeholten Gendarmerie zur Unfallstelle gerufen, insbesondere dann, wenn es sich um Schwerverletzte handelt. Gleichzeitig oder auch schon vorher wurde bei der nachsten Einsatzstelle des Roten Kreuzes ein Rettungswagen angefordert. Dieser bringt dann den Verletzten auf arztliche Anweisung in das nachste Krankenhaus.
