1. Maternal‐fetal transmission of delta‐9‐tetrahydrocannabinol (THC) and its metabolites following inhalation and injection exposure during pregnancy in rats

Abstract

Cannabis use during pregnancy has increased over the past few decades, with recent data indicating that, in youth and young adults especially, up to 22% of people report using cannabis during pregnancy. Animal models provide the ability to study prenatal cannabis exposure (PCE) with control over timing and dosage; however, these studies utilize both injection and inhalation approaches. While it is known that Δ9-tetrahydrocannabinol (THC; primary psychoactive component of cannabis) can cross the placenta, examination of the transmission and concentration of THC and its metabolites from maternal blood into the placenta and fetal brain remains relatively unknown, and the influence of route of administration has never been examined. Pregnant female rats were exposed to either vaporized THC-dominant cannabis extract for pulmonary consumption or subcutaneous injection of THC repeatedly during the gestational period. Maternal blood, placenta, and fetal brains were collected following the final administration of THC for analysis of THC and its metabolites, as well as endocannabinoid concentrations, through mass spectrometry. Both routes of administration resulted in the transmission of THC and its metabolites in placenta and fetal brain. Repeated exposure to inhaled THC vapor resulted in fetal brain THC concentrations that were about 30% of those seen in maternal blood, whereas repeated injections resulted in roughly equivalent concentrations of THC in maternal blood and fetal brain. Neither inhalation nor injection of THC during pregnancy altered fetal brain endocannabinoid concentrations. Our data provide the first characterization of maternal-fetal transmission of THC and its metabolites following both vaporized delivery and injection routes of administration. These data are important to establish the maternal-fetal transmission in preclinical injection and inhalation models of PCE and may provide insight into predicting fetal exposure in human studies.

2. Characterising ‘the munchies’; effects of delta-9-tetrahydrocannabinol (THC) vapour inhalation on feeding patterns, satiety, and macronutrient-specific food preference in male and female rats

Abstract

With approximately 4% of the world’s population using cannabis, there is need to fully understand how cannabis impacts our health. It is universally known that increased food intake is a side effect of cannabis use, also known as ‘the munchies’, and it has been established that delta-9-tetrahydrocannabinol (THC), the major psychoactive cannabinoid in cannabis, drives these feeding effects. These appetitive effects of cannabis or THC have been modeled in humans and rodents but have not been fully explored. Therefore, the aim of this study was to use a translational pre-clinical model to characterise how inhalation of vapour from a THC-dominant cannabis extract alters daily feeding patterns and macronutrient-specific food preferences, with focus on microstructural feeding pattern analysis and potential sex differences.We exposed adult male and female Sprague-Dawley rats to THC-dominant cannabis vapour or vehicle vapour daily, then gave rats access to different foods (chow, high-carbohydrate, and/or high-fat food) and post-vapour feeding patterns measured. To study macronutrient-specific food preferences, rats were given a post-vapour choice between a high-carbohydrate and a high-fat food. To assess satiety, rats were given pre-vapour access to a palatable preload in which they readily consume to become satiated. For some animals, blood and brain samples were collected post-vapour to measure phytocannabinoid and metabolite levels using mass spectrometry.We show that THC vapour inhalation increases food intake in the first hour after vapour exposure, an effect that is not dependent on food type, sex or whether animals are satiated or not. These feeding effects were a result of THC vapour decreasing feeding latency and increasing feeding bout frequency. Consistently, these acute feeding effects were compensated for through reductions in subsequent food intake, and THC vapour did not promote weight gain. THC also altered macronutrient-specific food preferences, increasing high-fat food preference in standard conditions, and increasing high-carbohydrate food preference in satiated conditions so that rats no longer significantly preferred one food over the other. Following vapour exposure, female rats had higher THC and metabolite concentrations in plasma and the hypothalamus than males and showed a stronger high-fat food preference following THC vapour compared to males when given a food choice.Overall, this study complements and builds upon previous clinical and pre-clinical data to fully characterise the effects of THC inhalation on feeding patterns and is the first to directly examine how THC alters macronutrient-specific food preferences and assess if appetitive THC-driven sex differences exist. This research sheds light on whether cannabis use can have energy-balance effects, information which is beneficial for recreational and medical cannabis users.

5. Dialysat au citrate et mortalité : résultats de l’étude DOPPS (Dialysis Outcomes and Practice Patterns Study : 2012–2017)

Abstract

Introduction L’acidose metabolique est une complication frequente en hemodialyse. Elle est traitee par l’alcalinisation du bain de dialyse, dont le principal composant est le bicarbonate auquel il faut rajouter de faibles quantites soit d’acide chlorhydrique, acetique ou citrique. Ces deux derniers sont metabolises en bicarbonate par le foie. En regard des incertitudes recentes sur les potentiels effets deleteres du citrate, nous avons evalue si son utilisation etait associee a la mortalite toute cause dans l’etude internationale DOPPS. Methodes Les informations detaillees par patient concernant la composition du dialysat ont ete extraites de la cohorte DOPPS 5 et 6 (2012 a 2017). Une analyse de regression de Cox a ete utilisee pour modeliser l’association entre dialysat contenant du citrate ou non et la mortalite. La mortalite entre pays et phases de l’etude DOPPS a ete comparee en retenant les centres ayant au moins 15 patients utilisant un dialysat au citrate. Resultats obtenus ou attendus L’utilisation de dialysat au citrate etait plus frequente au Japon, Italie et Belgique (25, 23, 21 % des patients de la phase 6 de DOPPS) et moins de 10 % dans les autres pays. Parmi les 10 618 patients dans DOPPS (pays/phases) avec au moins 15 patients ayant un dialysat avec citrate, les caracteristiques demographiques, comorbidites et parametres biologiques etaient similaires chez les patients traites par un dialysat enrichi en citrate (14 %) compares aux autres dialysats (86 %). Apres ajustement, nous n’avons pas observe d’association entre l’utilisation de dialysat au citrate et la mortalite RR (IC 95 %) = 1,03 (0,85–1,23), ni de relation effet-dose apres analyse en fonction de la concentration du citrate dans le dialysat 1, 2, et 3+ mEq/L ( Tableau 1 ). Conclusion L’utilisation de dialysat au citrate n’est pas associee avec un risque accru de mortalite toute cause chez les patients hemodialyses ayant participe a l’etude DOPPS.

Abstract

Health programs compete for resources and attention, e.g. community-based intervention programs for physical activity vs. skin cancer control. Children’s physical outdoor environments may favorably impact both and several other factors relevant to the health of an increasing number of young children who spend the lion’s share of their waking hours at preschool. The impact of the outdoor environment on physical activity and sun exposure was studied in various landscapes at different latitudes to explore whether outdoor environments with high play potential due to space, vegetation and topography would promote health by stimulating play, increased physical activity, and sun-protective behavior irrespective of location. Based on the findings, criteria were formulated to be applied in local government. In Swedish municipalities land use policies and administrative management of outdoor land use have been adapted to monitoring the quality of preschool outdoor environments (upgrading and planning). New guidelines have been implemented and integrated. Inexpensive methods for assessing, selecting, and upgrading preschool land could be adapted to latitude, climate, and varying outdoor play policies (including gender aspects). Criteria could be explored worldwide. Their aptitude compiled in a database for monitoring and evaluation should further discussed.

Abstract

Everyday physical environments are an important potential trigger of automated healthy behaviour, particularly in children, as they may target disparate conventional community intervention programs regarding health and sustainability, and converge in one synergetic intervention. Previous studies in Sweden have shown that outdoor preschool environment may have an impact upon physical activity, sun protection and promotion of general physical and mental wellbeing if certain physical qualities regarding space, topography and vegetation are fulfilled. Outdoor preschool environment providing space and vegetation is one important factor for the promotion of sustainability and health in Western societies as many children spend most of their waking hours at preschool. The lack of physical activity and overexposure to ultraviolet (UV) radiation from the sun, and its health consequences are global issues. An environment that limits daily selftriggered physical activity contributes to “physical activity deficiency”, and thus seriously affects the levels of physical activity required to maintain health. This applies to children in particular as sufficient physical activity as part of a lifestyle may be established in preschool age. Space and attractive opportunities, (e.g. extensive landscape with trees, shrubbery and broken ground) are equally important as a trigger of young children’s outdoor physical activity and automatic sun protection behaviour. Approximately 80-90% of skin cancers in Western societies are caused by overexposure to solar UV, in preschool children by inadvertent exposure. Abstract

Odor has a decisive role in the sexual development and mating behavior of infrahuman animals. It is reasonable to assume, therefore, that man, too, would have a similar inborn sensate guarantee for heterosexual arousal. Yet, until recently, aside from literateurs, among them Shakespeare, Shelley, Huysman, and Zola, and a few clinicians who wrote about the sensual response to certain human odors, relatively little attention has been given to the relationship between olfaction and sexuality. In an inclusive review of the subject, Gibbon (1986) commented, “Odors reach into all our emotional life. Odors suggest, stimulate associations, evoke, frighten and arouse us, but they seem to lie below conscious thought until someone like the poet, Baudelaire, parts the curtain” (p. 327).

In 1961 a research team of the Society of Medical Psychoanalysts completed a nine year study of male homosexuality in which 77 of its members each filled out a detailed questionnaire that tapped information on a sample of 106 male homosexuals and a comparison group of 100 male heterosexuals in psychoanalytic treatment. Since then, the authors have seen well over 1000 male homosexuals and about 100 pairs of their parents in direct, psychoanalytically focused interviews. In all regards, the data obtained are in accord with the research findings, thus strengthening its validity and reliability. The developmental picture and family background of homosexuals have the following salient characteristics: The parents generally have an unsatisfactory relationship with each other but the mother forms an overly close bond with her homosexual son. She tends to dominate and restrict him and she discourages his assertiveness. Though there are variations in the pattern, most such mothers are close, binding and intimate (CBI), sometimes in sexually inappropriate ways. This son is usually her favourite whom she openly prefers to her husband. Within the family system, the CBI mother and son form a romantically tinged dyad, while the father and other siblings remain on the periphery. The son develops a profound dependency on the mother who often remains the most beloved person and focus of his life. The father-son relationship is strikingly different. The fathers are hostile, competitive and do nothing to neutralize deleterious maternal influences. Patients’ reports of their father are almost always negative. He is virtually non-existent, rejecting, hostile, harsh or brutal. The fathers show neither love nor respect for this son. The authors have repeatedly noted that constructive, affectionate fathers preclude the development of homosexuality, though, conversely, hostile fathers do not necessarily produce homosexual sons. Boys who become homosexual, leave childhood with a profound fear and hatred of the father, yet retain a deep longing for paternal affection and acceptance — a central determinant of a homosexual orientation. Relationships with siblings also tend to be disturbed. The siblings’ intense jealousy of the mother's favourite is more corrosive among other sons than daughters and it results in marked rivalry and hostility. Closely similar are the difficulties with same-sex peer groups. Prehomosexual boys are so traumatized by a hostile father and brothers, that they cannot cope with the aggressive, combative behaviour of young boys at play, hence are deprived of the needed ancillary opportunities for masculine identification and support. Often, the first meaningful contact with peers occurs in adolescence when the combativeness of the earlier years has abated. The widely held theory that homosexuals fear women is not supported. They fear only a sexual relationship with a woman, a fear stemming from expectations of retaliation from aggressive men. The bisexual theory, also, is not supported. It asserts that humans are by nature bisexual and that homosexuals, unlike sexually balanced, normal people, have a preponderance of femininity. Homosexuals are physiologically normal. They become oriented to homosexuality as a substitute for heterosexuality because women are psychologically beyond reach. Homosexuality offers complex defensive and reparative mechanisms for coping with fear of aggressive, rejecting men. Sexual arousal cues offer insights into the dynamics of such mechanisms. In a study by I. Bieber, three categories of stimulae were delineated: 1. the displacement of the heterosexual stimulus to a homosexual object; 2. the eroticization of the feared object; 3. arousal associated with affection shown by men. Recent work in sexual identity, gender identity and gender role behaviour emphasizes the view that effeminate homosexuals and transvestites have, in early life, identified with the mother leading, in adult life, to an identification with women. According to the Bieber studies, homosexuals have a basically masculine identification but have a sense of impaired masculinity. This distinction not only influences psychotherapeutic strategy but may extend to decisions involving transsexual change. The authors’ method of treatment is cognitive psychoanalysis, a process which delineates and evaluates irrational beliefs and irrational belief systems associated with fears of threat and injury. For those patients who might derive benefit from a group experience, T. Bieber offers individual sessions in combination with group sessions. The all-male homosexual group is preferred to the heterogeneous group. It has a less threatening, more familiar atmosphere, it facilitates verbalization and affective responses and offers an important peer group support system. Reversal estimates now range from 30% to an optimistic 50%. A shift to heterosexuality does not mean that the potential for homosexual arousal has been totally extinguished, though in some cases this does occur. Should a post-analytic patient be faced with a recurrence of homosexual interest, he may short-circuit it by identifying the situation that has triggered anxiety about heterosexuality. By thoughtfully working out the dynamics by himself or with the analyst in a session or two, homosexual obsessions and other symptoms may be dissipated. The view that homosexuality is but an alternate way of loving and that the psychopathology is a consequence of social discrimination and prejudice is not supported. Societal bias may add to feelings of anguish and unacceptability but as demonstrated in the homosexuality study and other publications, it is the destructive family relationships and other deleterious interpersonal influences that induce a homosexual adaptation.

The adsorbed cesium can be eluted from the hydroxide by washing. In the case of aluminum hydroxide, an ammonium salt solution is advantagcous because of its stabilizing effect on the precipitate. The cesium retained by chemisorption on polyuranate can also be desorbed with an ammonium salt solution, which replaces the cesium bound to the precipitate. The cesium can be quantitatively isolated from the solution which contains iron, aluminum, and uranium in excess by precipitation of the mixture with ammonia and washing of the precipitate with ammonium nitrate solution. 49. Prevalence and Outcomes of Chronic Kidney Disease Associated Pruritus: International Results from PDOPPS.

Abstract

Background: Pruritus is common in hemodialysis (HD) patients. Less is known about the prevalence and outcomes of pruritus among patients receiving peritoneal dialysis (PD). Herein, we describe the prevalence of pruritus and its associations with patient-reported outcomes (PROs) and mortality/transfer to HD., Methods: We analyzed a multicenter, international cohort of PD patients enrolled in the Peritoneal Dialysis Outcomes and Practice Patterns Study (PDOPPS) from 2014-2022. Pruritus was assessed at entry into PDOPPS with a single-question Likert Scale capturing the extent to which patients were bothered by itch ranging from 1: Not at all, to 5: Extremely. The KDQOL-36 and the Centre for Epidemiological Studies Depression Scale assessed various PROs. Moderate to extreme pruritus was defined as a Likert scale score ≥3. The associations of pruritus with PROs were assessed using linear/logistic regression where appropriate. Death or HD transfer was assessed using multivariable Cox regression models., Results: Overall, 5535 patients from seven countries were included; 43% had moderate to extreme pruritus which was highest in Thailand (50%) and lowest in the United States (33%). The adjusted odds ratios (aOR) of moderate to extreme pruritus were higher for diabetes, low albumin, and elevated phosphorus but lower for residual urine volume (aOR= 0.98 per 200 ml increase in 24-hour urine volume, 95% confidence interval [CI]; 0.96-1.00, P=0.05). Patients with extreme pruritus had the lowest mental and physical health component scores and a higher burden of other PROs including restless legs and disturbed sleep. Overall, 899 patients died and 1221 transferred to HD. Patients with moderate to extreme pruritus were at higher adjusted risk for death or HD transfer (adjusted hazard ratio [aHR] 1.12, 95% CI 1.02-1.23, P=0.02) with similar point estimates for each subcomponent of the composite outcome., Conclusions: Pruritus is highly prevalent in PD and associated with poor health outcomes. Abstract

Early peritoneal dialysis (PD)-related infection is a severe complication. This study investigated the relationship between patient-doctor contact (PDC) duration and early PD-related infection. In the Peritoneal Dialysis Outcomes and Practice Patterns Study (PDOPPS) Korea, incident dialysis patients receiving PD were divided into two groups based on PDC duration (< 15 min versus ≥ 15 min), which was defined as the duration a nephrologist typically spends with a patient receiving PD during each visit according to the facility practice pattern. Early risks of PD-related infections, such as peritonitis and catheter-related infection (onset within 3 and 12 months of PD), were compared to the PDC duration using Cox regression. The study included 276 patients (184 [66.7%] in the shorter PDC group [< 15 min] and 92 [33.3%] in the longer PDC group [≥ 15 min]). The average age did not differ between the groups. The incidences of 3- and 12-month PD-related infections were significantly lower in the longer PDC group than in the shorter PDC group (3 months: 1.1% versus 9.8%, P = 0.007; 12 months: 9.8% versus 23.4%, P = 0.007). Longer PDC was independently associated with a lower risk of PD-related infections at 3 and 12 months (3 months: adjusted hazard ratio [aHR], 0.11; 95% confidence interval [CI], 0.02-0.85, P = 0.034; 12 months: aHR, 0.43; 95% CI 0.19-0.99, P = 0.048).
