Zusammenfassung Die Prognose der Diabeteserkrankung wird entscheidend vom Selbstbehandlungsverhalten der Betroffenen beeinflusst. Psychosoziale Aspekte wie krankheitsspezifische Belastungen, subklinische oder klinische psychische Störungen stellen eine wesentliche Barriere für eine erfolgreiche Diabetestherapie dar. Deshalb ist ein Screening auf mögliche psychische Belastungen sinnvoll. Beim Auftreten psychischer Komorbiditäten ist eine Mitbehandlung durch diabeteserfahrene psychologische oder ärztliche Psychotherapeuten sinnvoll.

Es sollte geklart werden, welche Einstellung Menschen mit Diabetes zur Digitalisierung und fortschreitenden Technologisierung in der Diabetologie haben, welche Vor- und Nachteile sie sehen und wie sie aktuelle Entwicklungen der IT (Informationstechnologie) in der Diabetologie einschatzen. Es wurden 3427 Menschen Personen mit Diabetes online befragt. Die Teilnehmer der Umfrage haben insgesamt eine sehr positive Einstellung zur Digitalisierung (83 % positiv/sehr positiv) und schatzen deren Potenzial zur Optimierung der Diabetologie als sehr hoch ein (79 % positiv/sehr positiv). Als groste Vorteile der Digitalisierung wurden eine „bessere Behandlungsqualitat“ (80 %), eine „grosere Eigenverantwortung und Selbstbestimmung in der Therapie“ (80 %) und „mehr Unterstutzung bei Therapieentscheidungen“ (78 %) genannt. Die grosten Nachteile sind die „Fehleranfalligkeit von digitalen Anwendungen“ (36 %), die „Gefahr des Missbrauchs von Patientendaten“ (32 %) und die Befurchtung, dass „durch die Digitalisierung in vielen Fallen der Arzt ersetzt wird“ (31 %). Als bedeutsamste Themen eingeschatzt werden aktuell AID-Systeme (AID: „automated insulin delivery“), die Moglichkeit der Auswertung und Analyse von Glukosedaten mittels Software sowie eine Verbesserung der Interoperabilitat der verschiedenen Anwendungen. Menschen mit Diabetes stehen neuen Technologien in der Diabetologie sehr positiv gegenuber und verbinden mit diesen grose Hoffnungen, dass sich ihre Diabetestherapie verbessert, vereinfacht und sich die Belastungen im Zusammenhang mit der Erkrankung verringern. Besonders Menschen mit Typ-1-Diabetes bzw. deren Eltern haben hohe Erwartungen an AID-Systeme und erhoffen sich dadurch eine deutliche Verbesserung der Diabetestherapie.

Aim To investigate the longitudinal bi-directionality of diabetes-related distress and depressive symptoms. Methods A total of 509 patients receiving intensified insulin therapy completed the Centre for Epidemiological Studies Depression scale questionnaire for the assessment of depressive symptoms as well as the Problem Areas in Diabetes questionnaire for the assessment of diabetes-related distress at baseline and at 6-month follow-up. Separate logistic and linear regression analyses for incidence and persistence were performed with demographic (age, gender, BMI) and medical (diabetes type, HbA1c, diabetes duration, late complications) control variables. Results Diabetes-related distress at baseline increased the risk of the incidence of elevated depressive symptoms by 2.56-fold (odds ratio 2.56; 95% CI 1.15–5.72; P = 0.02) when controlling for demographic and medical variables. In addition, diabetes-related distress at baseline doubled the chance of the persistence of elevated depressive symptoms (odds ratio 2.04, 95% CI 1.04–3.99; P = 0.04) when controlling for demographic and medical variables. Hypoglycaemia is a common side effect of insulin therapy in diabetes patients, with negative physical and emotional impacts. Despite this, there are few studies investigating the frequency of non-severe hypoglycaemic events from the perspective of patients in the real-world setting. We investigated self-reported NSHE frequency and levels of hypoglycaemia awareness in Germany. Respondents>15 years with Type 1 or insulin-treated Type 2 diabetes (receiving basal only, basal-bolus or other insulin regimens) were recruited using online panels to complete≤4 questionnaires. Questionnaires collected demographics, non-severe hypoglycaemic event rates and patient-reported level of hypoglycaemia awareness. Non-severe hypoglycaemic event rates are reported as respondent-week records and calculated using data from all respondents completing at least one questionnaire. A total of 1 771 respondent-week records were obtained from 614 participants. Mean non-severe hypoglycaemic event rates per respondent-week were 1.6 for Type 1 and 0.6–0.8 for Type 2, with estimated annual rates of 83 and 31–42 respectively. Two-thirds of Type 1 (65%) and Type 2 (61–72%) respondents reported impaired levels of awareness or unawareness of hypoglycaemic events (inability or impaired ability to recognise the symptoms of hypoglycaemia). Respon­dents’ self-reported hypoglycaemia-awareness was significantly associated with the proportion of asymptomatic non-severe hypoglycaemic events; respondents classified as being unaware of hypoglycaemia had a higher proportion of asymptomatic non-severe hypoglycaemic events than aware respondents. Non-severe hypoglycaemic events are common in people with Type 1 or insulin-treated Type 2 diabetes in the real-world setting in Germany but may still be underestimated due to an inability to recognise the symptoms of hypoglycaemia.

ZusammenfassungIn Deutschland gibt es, wie auch in vielen anderen Ländern, eine kontinuierliche Zunahme von Menschen mit Diabetes. Bereits heute beläuft sich die Zahl der Menschen mit Diabetes auf über 7 Millionen Menschen. Jährlich erkranken ca. 270000 Menschen neu. Die Zahl der Personen mit einem undiagnostizierten Diabetes wird auf 2 bis 3 Millionen geschätzt. Angesichts dieser großen Zahl von Menschen, die bereits an Diabetes erkrankt sind oder künftig daran erkranken werden, sowie der bisher fehlenden Möglichkeit, den Diabetes zu heilen, werden von der UNO, der EU und maßgeblichen diabetologischen Verbände und Patientenorganisationen aus Deutschland die Entwicklung eines „Nationalen Diabetesplans“ gefordert. Damit sollen strukturierte Vorschläge erarbeitet werden, wie die Prävention, Therapie, Versorgung und Forschung des Diabetes verbessert werden kann. Wichtige Handlungsfelder eines Nationalen Diabetesplans sind die Themen „Primäre Prävention“, „Früherkennung des Typ 2 Diabetes“, “Epidemiologie, Diabetesregister“, „Versorgungsforschung, Versorgungsstrukturen, Qualitätssicherung“ sowie „Patienteninformation, Patientenschulung und Empowerment“.

Nahezu 25% der an Diabetes Erkrankten haben zumindest eine beeintrachtigende subklinische Depression, 10% leiden an einer depressiven Storung. Diese beeinflusst die Lebensqualitat, das Krankheitsverhalten und den Verlauf der Diabeteserkrankung. Die diabetesbezogenen Therapieziele sind bei depressiven Patienten anzupassen, da diese infolge der depressiven Symptomatik in der Alltagsbewaltigung eingeschrankt sind. Uberforderungssituationen konnen die depressive Symptomatik aufrechterhalten. Ein Depressionsscreening ist in die Therapie zu integrieren. Der Behandlung der Depression kommt bei Patienten mit Diabetes wegen der multiplen Interaktionen zwischen der Diabeteserkrankung und der Depression eine besondere Bedeutung zu.

Bei der Diabetestherapie kommt dem Patienten die entscheidende Rolle zu, da er die wesentlichen Therapiemasnahmen in seinem Alltag dauerhaft und eigenverantwortlich umsetzen muss. Die Prognose des Diabetes mellitus hangt wesentlich davon ab, inwieweit dies dem Betroffenen auf dem Hintergrund seines sozialen, kulturellen, familiaren und beruflichen Umfeldes gelingt. Entsprechend muss ein Diabetiker

Da die Depression die Lebensqualitat, den Krankheitsverlauf und die Mortalitat bei diabeteserkrankten Patienten erheblich negativ beeinflusst, kommt der Diagnose in der Behandlung der Depression bei Menschen mit Diabetes eine besondere Rolle zu. Screening-Instrumente in Form von sehr kurzen Fragebogen konnen hilfreich bei der Identifizierung der depressiven Symptomatik sein. Die Diagnose wird im Arzt-Patient-Gesprach gestellt. Wesentliche Bausteine der Therapie sind die psychosomatische Basisversorgung, die Pharmakotherapie und die Psychotherapie. Vorgestellt wird in dem Beitrag der aktuelle Stand der Diagnose und Therapie dieser Patientengruppe.

These results have since been reproduced in real-world prevention programmes. Even a delay of a few years in the progression to diabetes is expected to reduce diabetes-related complications, such as heart, kidney and eye disease and, consequently, to reduce the cost to society.A comprehensive approach to diabetes prevention should combine population based primary prevention with programmes targeted at those who are at high risk. This approach should take account of the local circumstances and diversity within modern society (e.g. social inequalities). The challenge goes beyond the healthcare system. Der langfristige Verlauf des Diabetes hangt entscheidend von einer erfolgreichen Selbstbehandlung des Patienten ab, die von diesem eigenverantwortlich und dauerhaft im Alltag umgesetzt werden muss. Schulungsmasnahmen, in denen die Patienten die notwendigen Kompetenzen und Selbstmanagementfertigkeiten im tagtaglichen Umgang mit ihrer Erkrankung erwerben, sind daher ein wesentlicher und unverzichtbarer Bestandteil jeder Diabetestherapie. In diesem Artikel soll ein Uberblick uber die Zielsetzung, Indikation, Inhalte, Umfang, aktuelle Umsetzung und Effektivitat von Schulungs- und Behandlungsprogrammen bei Diabetes gegeben werden. Die vom Bundesversicherungsamt (BVA) und/oder der Deutschen Diabetes-Gesellschaft (DDG) zertifizierten Programme werden beschrieben und hinsichtlich ihrer Qualitat bewertet.

ZusammenfassungPRAEDIAS (“Diabetes vermeiden - selbst aktiv werden”) ist ein strukturiertes Schulungs- und Behandlungsprogramm zur Prävention des Typ-2-Diabetes. Es wurde auf der Basis der bereits evaluierten amerikanischen und finnischen Präventionsstudien (DPP-/DPS-Studie) entwickelt und fokussiert auf eine nachhaltige Lebensstilmodifikation mit den Zielen einer Gewichtsreduktion, Veränderung des Ernährungsverhaltens und Steigerung der körperlichen Aktivität. Bei PRAEDIAS handelt es sich im Gegensatz zu den Referenzstudien um ein Gruppenprogramm (sechs bis zehn Teilnehmer), das eine bessere Kosten-Nutzen-Relation und damit einhergehend eine bessere praktische Umsetzbarkeit erreichen soll. PRAEDIAS umfasst insgesamt zwölf Kursstunden über einen Zeitraum von einem Jahr. Diese unterteilen sich in eine zweimonatige Veränderungsphase (Kernintervention, acht Gruppentreffen) sowie eine Stabilisierungsphase (vier Gruppentreffen und Begleitmaßnahmen). Mit Hilfe einer Toolbox wird im Rahmen der Nachbetreuung besonders auf individuelle Problemkonstellationen eingegangen. Die Effektivität wird in einer randomisierten, prospektiven Studie evaluiert. Die Daten eines Modellprojekts im Zusammenarbeit mit der DAK, dem Zentralinstitut der KV (ZI) und der Universität Dresden belegen die Effektivität, Effizienz und Umsetzbarkeit von PRAEDIAS in einem 6-Monats-Follow-up unter realen Versorgungsbedingungen. Durch PRAEDIAS konnte im Prae-post-Vergleich eine signifikante Gewichtsreduktion, Blutdrucksenkung, Steigerung der körperlichen Aktivität, Veränderung von Ernährungs- und Bewegungsverhalten sowie eine Verbesserung des psychosozialen Wohlbefindens erreicht werden. Im Vergleich zur DDP- und DPS-Studie ist PRAEDIAS kostengünstiger.

Background In a randomized, prospective multi-centre trial, the effect of a specific training programme (HyPOS) for patients with hypoglycaemia problems was compared with a control group (CG), receiving a standardized education programme aiming at avoidance of hypoglycaemia by optimization of insulin therapy. Methods A total of 164 type 1 diabetes patients (age 46.0 ± 12.5 yrs, HbA1c 7.3 ± 1.0%, 50% male) were randomized. Hypoglycaemia awareness was measured by the hypoglycaemia awareness questionnaire (HAQ) and by a visual analogue scale (VAS). There were no baseline differences. Results After a 6-month follow-up, hypoglycaemia awareness significantly improved in HyPOS compared to that in the CG (Δ HAQ 0.7 [95% CL 0.1–1.2], p = 0.024, Δ VAS 0.8 [95% CL 0.2–1.2], p = 0.015). In HyPOS, the threshold for detection of low blood glucose (Δ 0.2 mmol/L [95% CL 0.03–0.04], p = 0.02) and the treatment of low blood glucose (Δ 4.6 g [95% CL 1.6–7.6], p = 0.03) increased significantly. The number of undetected hypogylcaemic episodes (Δ− 1.4 episodes per week [95% CL 0.4–2.5], p = 0.01) and the rate of mild hypoglycaemia dropped significantly in HyPOS (Δ 2.1% [95% CL 0.5–5.3], p = 0.015). The numbers of severe (Δ 0.3 events per patient per year [95% CL − 0.04–1.0], p = 0.037) and very severe hypoglycaemic episodes (Δ 0.3 events per patient per year [95% CL − 0.1–0.7], p = 0.09) were lower in HyPOS, but these differences were not significant. Conclusion Compared to the CG, HyPOS demonstrates additional benefits in terms of improving impaired hypoglycaemia awareness, reducing mild hypoglycaemia, detecting low blood glucose, and treating low blood glucose. Aims To compare the properties of the two most commonly used assessment tools for diabetes distress, the Problem Areas in Diabetes Scale (PAID) and the Diabetes Distress Scale (DDS), in order to discriminate their psychometric capabilities and functions. Methods Six hundred and twenty-eight people with diabetes (67% Type 1, 33% Type 2) were cross-sectionally assessed with the PAID, the DDS and further self-report scales regarding coping, quality of life, depressive symptoms and self-care, and medical data were gained. We analysed the PAID and DDS for areas of contentual/psychometric divergence in assessing diabetes distress and compared their associations with criteria of interest. Results Content analysis: The PAID covers a greater variety of emotional concerns and shows a stronger focus on food-related problems and complications. The DDS is more reflective of physician-related distress and problems concerning diabetes self-management. Psychometric analysis: Exploratory factor analyses revealed four-factor structures of both scales, explaining 60% (PAID) and 67% (DDS) of variance. Confirmatory factor analyses confirmed that single-factor and four-factor models fit the data. Total scales proved high and subscales mostly satisfactory reliability. Associations with criteria of interest: The PAID was significantly more strongly associated with dysfunctional coping styles, quality of life and depressive symptoms. The DDS showed significantly stronger associations with diabetes self-care and metabolic outcomes. Conclusion Our results support both PAID and DDS as good self-report measures of diabetes distress. The observed contentual/psychometric differences suggest that a justified choice with regard to the intended clinical or scientific purpose can improve the acquisition of the required data.

Background and aims: Hypoglycaemia is an important concern for patients with diabetes and physicians when setting glycaemic targets. The Hypoglycaemia Assessment Tool (HAT) study, the largest and most comprehensive of its kind, assessed self-reported hypoglycaemia and associated predictive factors in a global population of patients with insulin-treated diabetes. Materials and methods: HAT was a non-interventional, multicentre, 6-month retrospective and 1-month prospective study of hypoglycaemic events in 24 countries using self-assessment questionnaires and patient diaries (for 28 days) in people aged ≥18 years with type 1 (T1D) or type 2 (T2D) diabetes using insulin for ≥12 months attending routine clinics. Associations between predictive factors and hypoglycaemia were examined using negative binomial regression models adjusted for period and country. Results: 27,585 patients completed the study (Table 1). 83.4% of patients with T1D and 50.8% of patients with T2D experienced ≥1 hypoglycaemic event in the 4 weeks before baseline (51.5 and 16.5 events per patient year). Higher (p < 0.001) incidence rates were reported in the 4 weeks after baseline (73.3 [T1D] and 19.3 [T2D] events per patient year). A greater percentage of patients with T1D vs. T2D reported any (83.0 vs. 46.5%), nocturnal (40.6 vs. 15.9%) or severe (14.4 vs. 8.9) hypoglycaemia in the prospective period. Conclusion: In this large, multinational population of patients (insulin-treated) with T1D or T2D, rates of overall, nocturnal and severe hypoglycaemia were higher than previously published. An increased incidence of overall hypoglycaemia in the prospective study indicated significant under-reporting of hypoglycaemia.

Fragestellung: Die diabetesadaptierte Fusbettung (DAF) ist Mittel der Wahl, um bei Patienten mit diabetesbedingten Fusproblemen der Risikogruppe III durch eine Druckumverteilung Rezidive/Ulzerationen zu verhindern. Personen mit Diabetischem Fussyndrom (DFS) der Risikogruppe III sollten nur Diabetesschutzschuhe mit DAF tragen. Bisher gibt es keine objektivierbaren Daten zum Trageverhalten von diesen Schuhen mit DAF. Ziel dieser Studie war es, die Tragedauer von Schuhen mit DAF bei Risikopatienten fur ein DFS uber eine Temperaturmessung objektiv zu quantifizieren. Methodik: Die Schuhe von 26 Patienten mit Typ-2-Diabetes und DFS (67,5 ± 10,8 Jahre; 35% weiblich; BMI: 30,3 ± 4,7 kg/m2; Diabetesdauer: 10,4 ± 6,8 Jahre; HbA1c: 7,7 ± 0,6%) wurden mit einer DAF und einem Temperatur-Sensor ausgestattet. Ab einer Schuhinnentemperatur von 25 °C wurde davon ausgegangen, dass der Schuh mit DAF getragen wurde. Ergebnisse: Im Mittel konnte das Trageverhalten von Diabetsschutzschuhen mit DAF an durchschnittlich 133,5 Tagen ausgewertet werden. Die Tragedauer pro Tag betrug im Median 3,4 Stunden (25 – 75% Quartil: 0,5 – 6,9 Stunden). Im Median wurde der Schuh mit DAF an 51% der Tage nicht getragen (25 – 75% Quartil: 16,9 – 81,8%). Schlussfolgerungen: Die Studie liefert erstmals objektive Daten zum Trageverhalten von Schuhen mit DAF. Bei einer mittleren taglichen Tragedauer von nur 3,4 Stunden, ist die Tragezeit der Schuhe mit DAF eindeutig zu kurz, um einen ausreichenden Praventionseffekt fur das Auftreten/Verschlimmerung eines DFS erreichen zu konnen. Besonders problematisch war, dass an der Halfte der beobachteten Tage, die Schuhe mit DAF nicht getragen wurden. Es bedarf daher edukativer und technologischer Anstrengungen, um DFS-Patienten zu unterstutzen, Diabetsschutzschuhe mit DAF regelmasig zu tragen. Maintaining the quality of life (QoL) of diabetics is a decisive outcome variable in the treatment of diabetes. It should be used as an important indicator of quality in assessing efficacy and effectiveness of therapeutic measures. QoL of diabetics is reduced compared with non-diabetics. Diabetics with impaired QoL more frequently suffer from subclinical or clinical depression, which clearly impairs the short-term and chronic prognosis of the disease. The WHO-5 questionnaire has proven valuable for measuring QoL and in screening for depression. It is a simple and valid method of assessment which is recommended in international and national guidelines.

Adoleszente und junge Frauen mit Typ 1-Diabetes mellitus zeigen eine überzufällige Koinzidenz mit der Bulimia nervosa. Aber auch die Prävalenz von Ess-Störungen, die nicht alle Kriterien der Bulimia nervosa erfüllen, ist im Vergleich zu Frauen dieser Altersgruppe ohne Diabetes mellitus erhöht. Die Komorbidität von Typ 1-Diabetes und einer Ess-Störung stellt einen Risikofaktor für die Entwicklung späterer diabetischer Folgeerkrankungen dar. So zeichnen sich essgestörte PatientInnen mit Typ 1-Diabetes durch eine unzureichende Stoffwechselkontrolle und die frühzeitigere Entwicklung von diabetischen Spätschäden wie der Retinopathie aus. Die Binge Eating-Störung, eine zunächst unter Forschungsgesichtspunkten definierte neue Ess-Störungsentität ist insbesondere bei Übergewicht und Adipositas zu beobachten. Auch wenn die Binge Eating-Störung bei Menschen mit Typ 2-Diabetes im Vergleich zu stoffwechselgesunden Menschen nicht häufiger auftritt, stellt sie doch einen Risikofaktor für eine akzellerierte Gewichtszunahme dar, welche in der Regel mit einer Zunahme der Insulinresistenz einhergeht.

We compared the screening performance of different measures of depression: the standard clinical assessment (SCA); the Beck Depression Inventory (BDI); the Center of Epidemiological Studies-Depression Scale (CES-D); and the Problem Areas in Diabetes (PAID) questionnaire, which assesses diabetes-specific distress. We also studied the ability of these measures to detect diabetes-related distress.A total of 376 diabetic patients (37.2% type 1; 23.9% type 2 without insulin treatment, 38.8% type 2 with insulin) completed the BDI and CES-D; patients who screened positive participated in a diagnostic interview, the Composite International Diagnostic Interview (CIDI). Also, all patients completed the PAID questionnaire. Results of the SCA that related to depression diagnosis were reviewed to correct for false negative screening results.The prevalence of clinical depression was 14.1%, with an additional 18.9% of patients receiving a diagnosis of subclinical depression. Sensitivity for clinical depression in SCA (56%) was moderate, whereas BDI, CES-D and the PAID questionnaire showed satisfactory sensitivity (87, 79 and 81%, respectively). For subclinical depression, the sensitivity of the PAID questionnaire (79%) was sufficient, whereas that of SCA (25%) was poor. All methods showed low sensitivity for the detection of diabetes-specific emotional problems (SCA 19%, CIDI 34%, BDI 60%, CES-D 49%).The screening performance of SCA for clinical and subclinical depression was modest. Additional screening for depression using the PAID or another depression questionnaire seems reasonable. The ability of depression screening measures to identify diabetes-related distress is modest, suggesting that the PAID questionnaire could be useful when screening diabetic patients for both depression and emotional problems.
