Atopic dermatitis is one of the most common inflammatory skin diseases, with an increasing incidence among both children and adults. The recurrent nature, often with the persistence of symptoms, and the polymorphism of the response to current therapies have led to increased research in the therapeutic area dedicated to this condition. The understanding of pathophysiological pathways has contributed to the development of innovative therapies, including biological therapies, JAK inhibitors, but also emerging technologies like nanotechnology-based drug delivery systems. These innovations promise enhanced efficacy, reduced side effects, and improved patient outcomes. The ongoing exploration of novel vehicles, formulations, and natural biopolymers, along with cutting-edge therapeutic agents like tapinarof and mesenchymal stem cells, highlights the potential for an even more precise and personalized management of AD in the future. Despite these advances, challenges persist, particularly in ensuring the long-term safety, accessibility, and broader application of these therapies, necessitating continued research and development. The use of transfusions, whether involving whole blood or specific blood components, is essential for managing various clinical conditions. Many cases are acute, often requiring post-transfusion imaging evaluation. While there is no absolute contraindication for chest imaging following blood transfusion, it should be approached cautiously. We conducted a comprehensive search across multiple databases and registries. Research studies were limited to full-text original articles, reviews, and case reports published in English, involved human subjects, and focused on the interplay between blood transfusions and contrast-enhanced imaging. Scientific analyses were excluded if they did not focus on transfusion practices in the context of imaging or failed to address issues such as hemoglobin thresholds, transfusion reactions, or the clinical implications of contrast agents. Our research fills this gap by emphasizing the need for a cautious, multidisciplinary approach to post-transfusion computed tomography (CT) scans, especially in the presence of contrast agents. This study calls for increased awareness of the heightened risk of complications, such as autoimmune hemolysis, when both procedures are performed together. New insights from our research recommend individualized assessments and close patient monitoring when combining these interventions. Nevertheless, patients need to be hemodynamically and clinically stable before undergoing CT. Discussions. Symptoms that develop within the first 24 h post-transfusion are classified as secondary post-transfusion reactions unless proven otherwise. The prevalence of side effects from same-day CT scans and blood transfusions is challenging to quantify, as few studies focus on this combination. Acute hemolytic red blood cell transfusion reactions are among the most frequent side effects, with a prevalence of 1:12,000–38,000. Conclusion. Hereditary angioedema (HAE) is a rare and potentially life-threatening genetic disorder, constituting approximately 2% of all clinical cases of angioedema, with a global prevalence estimated between 1 in 50,000 and 1 in 150,000 individuals. The condition affects individuals of all genders and ethnic backgrounds without significant variation. HAE is classified into three types. Type I HAE, which accounts for 85% of cases, is characterized by a deficiency of the C1 esterase inhibitor (C1-INH) gene. Type II HAE, making up 15% of cases, involves a dysfunctional C1-INH. Type III HAE, which represents about 5% to 10% of cases, is often estrogen-dependent and although several mutations have been identified, it typically involves normal C1-INH activity. Despite the differences in C1-INH functionality, all three types of HAE manifest with similar clinical symptoms. HAE leads to recurrent episodes of non-pruritic angioedema, which occurs in the absence of urticaria. Breakthroughs in understanding HAE pathophysiology have revolutionized treatment, leading to the development of highly targeted therapies for both acute management and long-term prevention. Meanwhile, cutting-edge advancements in omics technologies are unlocking new possibilities for biomarker discovery, paving the way for more precise diagnoses and personalized treatment strategies that could significantly enhance patient outcomes. This review will delve into the intricate pathophysiology, diverse clinical presentations, and diagnostic challenges of HAE while exploring emerging biomarkers and innovative approaches to therapeutic management and prevention strategies. Autoimmune bullous diseases (AIBDs) are characterized by the formation of vesicles, bullous lesions, and mucosal erosions. The autoantibodies target the cellular anchoring structures from the surface of epidermal keratinocyte named desmosomes, leading to a loss of cellular cohesion named acantholysis. AIBDs are classified into intraepidermal or subepidermal types based on clinical features, histological characteristics, and immunofluorescence patterns. Pemphigus foliaceus (PF) is an acquired, rare, autoimmune skin condition associated with autoantibodies that specifically target desmoglein-1, leading to a clinical presentation characterized by delicate cutaneous blisters, typically sparing the mucous membranes. Several factors, including genetic predisposition, environmental triggers, malignancies, medication use, and vaccination (for influenza, hepatitis B, rabies, tetanus, and more recently, severe acute respiratory syndrome Coronavirus 2 known as SARS-CoV-2), can potentially trigger the onset of pemphigus. With the advent of vaccines playing a pivotal role in combatting the 2019 coronavirus disease (COVID-19), extensive research has been conducted globally to ascertain their efficacy and potential cutaneous adverse effects. While reports of AIBDs post-COVID-19 vaccination exist in the medical literature, instances of PF following vaccination have been less commonly reported worldwide. The disease's pathophysiology is likely attributed to the resemblance between the ribonucleic acid (RNA) antigen present in these vaccines and cellular nuclear matter. The protein produced by the BNT-162b2 messenger ribonucleic acid (mRNA) vaccine includes immunogenic epitopes that could potentially trigger autoimmune phenomena in predisposed individuals through several mechanisms, including molecular mimicry, the activation of pattern recognition receptors, the polyclonal stimulation of B cells, type I interferon production, and autoinflammation. In this review, we present a comprehensive examination of the existing literature regarding the relationship between COVID-19 and PF, delving into their intricate interactions. The pathogenesis of chronic wounds (CW) involves a multifaceted interplay of biochemical, immunological, hematological, and microbiological interactions. Biofilm development is a significant virulence trait which enhances microbial survival and pathogenicity and has various implications on the development and management of CW. Biofilms induce a prolonged suboptimal inflammation in the wound microenvironment, associated with delayed healing. The composition of wound fluid (WF) adds more complexity to the subject, with proven pro-inflammatory properties and an intricate crosstalk among cytokines, chemokines, microRNAs, proteases, growth factors, and ECM components. One approach to achieve information on the mechanisms of disease progression and therapeutic response is the use of multiple high-throughput 'OMIC' modalities (genomic, proteomic, lipidomic, metabolomic assays), facilitating the discovery of potential biomarkers for wound healing, which may represent a breakthrough in this field and a major help in addressing delayed wound healing. In this review article, we aim to summarize the current progress achieved in host–microbiome crosstalk in the spectrum of CW healing and highlight future innovative strategies to boost the host immune response against infections, focusing on the interaction between pathogens and their hosts (for instance, by harnessing microorganisms like probiotics), which may serve as the prospective advancement of vaccines and treatments against infections. Epidermal growth factor receptor inhibitors (EGFRi) are approved for treating various cancers. Given that EGFR signaling is crucial for normal skin growth and repair, inhibiting this pathway can disrupt skin homeostasis and integrity. Although generally well tolerated, molecularly targeted therapies can lead to skin-related adverse effects that significantly impact patients' quality of life, often resulting in treatment interruptions. The most common cutaneous reaction associated with EGFRi is a diffuse, papulopustular acneiform eruption. Management strategies can be tailored based on the severity of the condition and may include topical corticosteroids, topical and oral antibiotics, and systemic corticosteroid therapy. Moreover, consistent evidence supports the effectiveness of systemic retinoids at lower doses than those typically prescribed for acne vulgaris, advocating for their use when other treatments have failed. Herein, we present the case of a 71-year-old male with advanced colorectal cancer who was treated with panitumumab and subsequently developed a generalized papulopustular eruption three weeks after initiating therapy. The patient reported pruritus, a burning sensation, and hyperesthesia, which significantly impacted his daily activities. Physical examination revealed folliculocentric erythematous pustules and papules covering more than 30% of his body surface area (including the face, scalp, thorax, and extremities), along with xerosis, painful nasal and oral mucosal erosions, paronychia, and trichomegaly. Initial treatment with topical and systemic antibiotics, topical and systemic corticosteroids, and general skin care yielded poor results, with recurring moderate to severe eruptions occurring every 7 to 10 days. After three months and with approval from the oncology team, isotretinoin therapy (10 mg/day) was initiated under close monitoring of laboratory parameters. This approach achieved effective therapeutic control within one month, allowing the continuation of oncologic treatment without necessitating any dose modifications.
