Big Data substantially disrupt the medical microcosm up to challenge the paradigms of Medicine Hippocrates as we previously know. Therefore, a reflection on the study of risks associated with ethical issues around personal health data is imposed on us. Our study is based on many field surveys, interviews targeted at different actors, as well as a literature search on the subject. This work led to the realization of an innovative method of alignment of concepts of ontology of the risks to those of ontology of ethical objectives requirements of Big Data in Health. The aim is to make sense and recommendations to realization, implementation and use of personal data in order to better control it.

BackgroundThe aim of our study is to develop a model to analyze, from an ethical point of view, the objectives of an Information System (IS) in the field of healthcare, as perceived by professionals who use it.Materials/Patients and methodsOur field survey was carried out using a questionnaire based on the ethical model that we built to analyze the objectives of an IS. This model consists of four universal ethical principles that we combined with environmental parameters of reality. The study population (n = 87) comprised IS designers, administrative staff, doctors and paramedics of health facilities, as well as consultancy firm managers, IS publishers and medical data hosting services. A unit ethics score over 100 was used to assess the ethical expectations of the participants. The questionnaire includes 40 items that represent the major ethical objectives of an IS.ResultsThe average unit ethics score of all study participants was 78.3. These expectations belong, in order of importance, to the ethical principles of beneficence (84.5), autonomy (79.4), nonmaleficence (78.1), justice (71.3). The ethics scores according to type of professional do not significantly vary: administrative (81.1), publisher (81.0), IS designer (81.0), physician (79.9), hosting service (78.5), paramedical practitioner (73.1) and consultancy (68.3). All these professionals consider that the principle of beneficence is the most important. Conversely, they are less sensitive to the principle of justice. Physicians have the highest ethical score for beneficence. We were able to identify a code of ethics regarding the objectives that surround the design, implementation and use of IS in health. It consists of 33 ethical recommendations.Discussions and conclusionsThe application of our ethical model on the analysis of the IS objectives presents consistent results. This reflects a degree of reliability of the assessment tool used. By emphasizing this ethical modeling, our research aims to identify the elements required for creating IS that integrate the fundamentals of ethics., Introduction : L’objectif de notre étude est de développer un modèle permettant d’analyser d’un point de vue éthique les objectifs d’un système d’information (SI) en santé tels qu’ils sont perçus par les professionnels qui l’utilisent.Matériels/Patients et méthodes : Notre enquête de terrain a été effectuée à partir d’un questionnaire fondé sur le modèle éthique que nous avons construit pour analyser les objectifs d’un SI. Ce modèle se compose de quatre principes éthiques universels que nous avons croisés avec des paramètres environnementaux du réel. L’étude porte sur 87 personnes constituées de concepteurs de SI, d’administratifs, de médecins et de paramédicaux d’établissements de santé, ainsi que de responsables de cabinets de conseil, d’éditeurs de SI et d’hébergeurs de données médicales. Un score éthique unitaire sur 100 fut utilisé afin d’apprécier les attentes éthiques des participants. Le questionnaire comprend 40 items qui représentent les principaux objectifs éthiques d’un SI.Résultats : La moyenne du score éthique unitaire de tous les participants de l’étude est de 78,3. Ces attentes appartiennent par ordre d’importance aux principes éthiques de : bienfaisance (84,5), autonomie (79,4), non-malfaisance (78,1), justice (71,3). Les scores éthiques par type de professionnels varient de façon non significative : administratif (81,1), éditeur (81,0), concepteur de SI (81,0), médecin (79,9), hébergeur (78,5), paramédical (73,1) et cabinet de conseil (68,3). Tous ces professionnels considèrent le principe de bienfaisance comme étant le plus important. A contrario, ils sont moins sensibilisés au principe de justice. Les médecins possèdent le score éthique le plus important pour la bienfaisance. Nous avons pu dégager une charte éthique sur les objectifs qui entourent la conception, la mise en place et l’utilisation d’une SI en santé. Elle se compose de 33 recommandations d’ordre éthique.Discussions et conclusionL’application de notre modèle éthique sur l’analyse des objectifs d’un SI présente des résultats cohérents. Cela traduit une certaine fiabilité de l’outil d’évaluation utilisé. En mettant en avant cette modélisation éthique, notre recherche vise à expliciter les éléments requis pour la constitution de SI intégrant les fondamentaux de l’éthique.
