14. Analysis of Complications of a Neglected Disease: 13 Years of Experience with Liver Hydatid Cysts in a High-Volume Hospital.

Background and Objectives: The aim of this study was to evaluate the clinical presentation, treatment outcomes, and complications associated with hepatic hydatid cysts in a pediatric population. Materials and Methods: This retrospective study analyzed 214 pediatric patients with liver hydatid cysts, focusing on clinical presentation, treatment outcomes, and associated complications. Patients were classified based on treatment modality, including non-operative management with albendazole, PAIR, and surgical intervention. This study compared cyst characteristics, recurrence rates, and complications such as cysto-biliary fistulas. Results: Among the patients, 68% (n = 145) had a single cyst and 86% (n = 184) were found to have isolated liver cysts. No significant statistical difference was observed between Group 1 and Group 2 in terms of age, gender, and basic laboratory values and general characteristics of the cysts, such as the lobe where the cyst was located, involvement of multiple organs, number of cysts, the state of cyst rupture, and recurrence; no statistically significant difference was found between the groups (p > 0.05 for each comparison). Cyst rupture incidence was 6%, and the average incidence of recurrence was 2%, with a surgical recurrence incidence of 3%. A total of 37 patients had a laparotomy, while 7 had laparoscopic surgery. In total, capitonnage was performed in 68 patients, omentopexy in 4, and cystostomy in 6. Consequently, among the treated patients (PAIR + surgery), the incidence of cysto-biliary fistula was 11%, anaphylaxis was 2%, surgical recurrence was 3%, and the incidence of reoperation (Clavien–Dindo ≥ 3) was 6%. The average follow-up period was 72 months, during which no mortality was observed. Conclusions: We identified key clinical outcomes related to both non-surgical treatments (cyst rupture and recurrence) and surgical groups (cysto-biliary fistulas, anaphylaxis, the need for reoperation, rupture, and recurrence). 18. How does ChatGPT perform on the European Board of Pediatric Surgery examination? A randomized comparative study

[eng] Purpose: The purpose of this study was to conduct a detailed comparison of the accuracy and responsiveness of GPT-3.5 and GPT-4 in the realm of pediatric surgery. Specifically, we sought to assess their ability to correctly answer a series of sample questions of European Board of Pediatric Surgery (EBPS) exam. Methods: This study was conducted between 20 May 2023 and 30 May 2023. This study undertook a comparative analysis of two AI language models, GPT-3.5 and GPT-4, in the field of pediatric surgery, particularly in the context of EBPS exam sample questions. Two sets of 105 (total 210) sample questions each, derived from the EBPS sample questions, were collated. Results: In General Pediatric Surgery, GPT-3.5 provided correct answers for 7 questions (46.7%), and GPT-4 had a higher accuracy with 13 correct responses (86.7%) (p=0.020). For Newborn Surgery and Pediatric Urology, GPT-3.5 correctly answered 6 questions (40.0%), and GPT-4, however, correctly answered 12 questions (80.0%) (p= 0.025). In total, GPT-3.5 correctly answered 46 questions out of 105 (43.8%), and GPT-4 showed significantly better performance, correctly answering 80 questions (76.2%) (p<0.001). Given the total responses, when GPT-4 was compared with GPT-3.5, the Odds Ratio was found to be 4.1. This suggests that GPT-4 was 4.1 times more likely to provide a correct answer to the pediatric surgery questions compared to GPT-3.5. Chronic lumbar facet pain is commonly treated with fluoroscopy-guided facet medial branch blocks (FMBBs). However, the associated radiation exposure of both patients and clinicians is a growing concern. This study aimed to compare radiation doses and fluoroscopy times between two techniques, i.e., oblique and posterior–anterior (PA) fluoroscopic approaches, while also examining the impact of physician experience on these metrics. A retrospective analysis was conducted on 180 patients treated at Mersin University Hospital Pain Clinic between January and July 2024. Patients were divided into two groups: 90 received the oblique technique (Group O) and 90 received the AP technique (Group A). Radiation dose and fluoroscopy time data were collected for each patient. The AP technique was associated with significantly lower radiation doses (mean 66 mGy) and shorter fluoroscopy times (mean 28 s) compared to the oblique technique (mean radiation dose of 109 mGy and fluoroscopy time of 46 s) (p < 0.001). Physician experience also influenced these outcomes, with more experienced physicians consistently using less radiation. The AP technique should be considered for FMBBs, as it reduces radiation exposure while maintaining procedural efficiency, highlighting the importance of experience in optimizing outcomes. 25. The performance of the inflammatory indexes in predicting double J catheter insertion necessity among children and adult patients with ureteral stone

[eng] Objective: The present study was designed to examine the differences in NLR, PLR, and PNLR between two distinct patient groups, further examining their utility as diagnostic markers. Materials and methods: Patients were then separated into two categories: Group 1 (those not require DJ catheter insertion) and Group 2 (those needed DJ catheter insertion). Factors such as patient age, gender, symptoms, diagnosis, hematological parameters, and some indexes (NLR, PLR, and PNLR) were thoroughly assessed. Results: The research study featured 150 participants, split into two groups of 80 (Group 1) and 70 (Group 2) individuals. Despite average age differences, the distribution of ages was statistically significant. Group 1 had more children, while Group 2 contained more adults. Gender distribution between groups was not significantly different. Group 2 had larger mean stone sizes than Group 1. Stone location showed no significant variation between the groups. WBC and NEU were higher in Group 2, while LYM was higher in Group 1. There was no significant difference in MONO, PLT, and urea levels, but CRE level was higher in Group 2. NLR, PLR, and PNLR were also significantly higher in Group 2. Cut-off values for NLR, PLR, and PNLR had moderate to good predictive abilities. There was a positive correlation between stone size and PNLR index. Conclusions: Our study demonstrates the predictive utility of hematological parameters (NLR, PLR, PNLR) in determining the need for DJ catheter placement in ureteral stone patients., [spa] Objetivo: El presente estudio se diseñó para examinar las diferencias en NLR, PLR y PNLR entre dos grupos distintos de pacientes, examinando además su utilidad como marcadores diagnósticos. Se evaluaron minuciosamente factores como la edad del paciente, el sexo, los síntomas, el diagnóstico, los parámetros hematológicos y algunos índices (NLR, PLR y PNLR). Resultados: El estudio de investigación contó con 150 participantes, divididos en dos grupos de 80 (Grupo 1) y 70 (Grupo 2) individuos. A pesar de las diferencias de edad media, la distribución de edades fue estadísticamente significativa. En el Grupo 1 había más niños, mientras que en el Grupo 2 había más adultos. La distribución por sexos entre los grupos no fue significativamente diferente. El Grupo 2 tenía un tamaño medio de los cálculos mayor que el Grupo 1. La localización de los cálculos no mostró variaciones significativas entre los grupos. Los valores de WBC y NEU fueron superiores en el Grupo 2, mientras que los de LYM fueron superiores en el Grupo 1. No hubo diferencias significativas en los niveles de MONO, PLT y urea, pero el nivel de CRE fue superior en el Grupo 2. Los valores de NLR, PLR y PNLR también fueron significativamente superiores en el Grupo 2. Los valores de corte para NLR, PLR y PNLR tenían una capacidad predictiva de moderada a buena. 26. Laparoscopic versus open splenectomy in children: A comparative study with an extended description of the surgical techniques

[eng] Objective: The aim of this study is to compare and present our patients who underwent open splenectomy (OS) and laparoscopic splenectomy (LS). Materials and methods: This research study incorporated patients who underwent splenectomy who were admitted to the Başakşehir Çam and Sakura Training and Research Hospital, Department of Pediatric Surgery, between January 2022, and April 2023. The study included a total of 48 patients who underwent splenectomy. The patients were divided into two groups: Group 1 LS (n=21) and Group 2 OS (n=27). Mentioned data were compared between groups. Results: The study included a total of 48 patients. Out of the participants, 66.7% were male. Hematological diseases were observed in 35 patients (72.9%). hematological diseases. The surgical technique employed for splenectomy was laparoscopic in 21 cases (43.8%) and open in 27 cases (56.3%). Age, gender, and Spleen size were found to be similar between the groups (p>0.05). Regarding cholelithiasis, the laparoscopic group had a higher prevalence of 86% (n=12) compared to 31% (n=5) in the open group (p=0.003). Similarly, the rate of cholecystectomy was higher in the laparoscopic group (60%, n=12) compared to the open group (22%, n=6) (p=0.008). The distribution of diseases showed a significant difference between the two groups (p<0.001). Conclusions: Compared to open surgery, laparoscopic splenectomy is a feasible method in terms of performing both surgeries together if the patient has gallbladder stones simultaneously., [spa] Objetivo: El objetivo de este estudio es comparar y presentar a nuestros pacientes sometidos a esplenectomía abierta (ES) y esplenectomía laparoscópica (EL). Material y métodos: Este estudio de investigación incorporó a pacientes sometidos a esplenectomía que ingresaron en el Hospital de Formación e Investigación Başakşehir Çam y Sakura, Departamento de Cirugía Pediátrica, entre enero de 2022, y abril de 2023. El estudio incluyó un total de 48 pacientes sometidos a esplenectomía. Los pacientes se dividieron en dos grupos: Grupo 1 LS (n=21) y Grupo 2 OS (n=27). Se compararon los datos mencionados entre los grupos. Resultados: El estudio incluyó un total de 48 pacientes. De los participantes, el 66,7% eran varones. Se observaron enfermedades hematológicas en 35 pacientes (72,9%). enfermedades hematológicas. La técnica quirúrgica empleada para la esplenectomía fue laparoscópica en 21 casos (43,8%) y abierta en 27 (56,3%). Se observó que la edad, el sexo y el tamaño del bazo eran similares entre los grupos (p>0,05). En cuanto a la colelitiasis, el grupo laparoscópico presentó una prevalencia más elevada, del 86% (n=12) frente al 31% (n=5) en el grupo abierto (p=0,003). Del mismo modo, la tasa de colecistectomía fue superior en el grupo laparoscópico (60%, n=12) en comparación con el grupo abierto (22%, n=6) (p=0,008). La distribución de las enfermedades mostró una diferencia significativa entre los dos grupos (p<0,001). Conclusiones: En comparación con la cirugía abierta, la esplenectomía laparoscópica es un método factible en cuanto a la realización conjunta de ambas cirugías si el paciente tiene cálculos biliares simultáneamente.

[eng] Objective: In this meta-analysis, our goal was to examine the diagnostic utility of bilirubin in identifying complicated from uncomplicated pediatric appendicitis. Materials and methods: Using the databases Embase, PubMed, Scopus, and Cochrane, we carried out a thorough literature search up to 2022. Studies comparing complicated appendicitis (CA) and simple appendicitis (SA) in terms of hyperbilirubinemia in the pediatric population were included. Results: A total of 5 studies with 2740 acute appendicitis patients (1097 complicated appendicitis and 1643 simple appendicitis) were included in this meta-analysis. Five studies have discussed the diagnostic value of total bilirubin (TB). When compared to simple appendicitis, complicated appendicitis had a significantly higher TB count (I2=94%), (WMD=0.18, 95% CI -0.00 to 0.37; P=0.05), DB count (I2=0%), (WMD=0.11, 95% CI 0.04 to 0.18; P=0.002), and IB count (I2= not applicable), (WMD=0.04, 95% CI 0.01 to 0.07; P=0.02). Conclusions: In conclusion, in this meta-analysis, total bilirubin, direct bilirubin, and indirect bilirubin values were higher in complicated appendicitis compared to simple appendicitis. Se incluyeron estudios que compararan apendicitis complicada (AC) y apendicitis simple (AS) en términos de hiperbilirrubinemia en la población pediátrica. Resultados: Se incluyeron en este metanálisis un total de 5 estudios con 2740 pacientes con apendicitis aguda (1097 complicada y 1643 simple). Cinco estudios han discutido el valor diagnóstico de la bilirrubina total (TB). En comparación con la apendicitis simple, la apendicitis complicada tuvo un recuento de TB significativamente mayor (I2=94 %), (DMP=0,18, IC del 95 %: -0,00 a 0,37; P=0,05), recuento de DB (I2=0 %), (DMP = 0,11, IC del 95 %: 0,04 a 0,18; P = 0,002) y recuento de BI (I2 = no aplicable), (DMP = 0,04, IC del 95 %: 0,01 a 0,07; P = 0,02). Acute gastric dilation is a rare pathology, which is met at diverse ages and has a high mortality rate if left untreated. This occurs due to several reasons, both due to mechanical obstruction of the stomach, and gastroparesis and due to extragastric causes such as eating disorders. Early placement of an oro- or nasogastric tube may help prevent serious complications such as ischemia and perforation of the gastric wall. The article presents a clinical case of a teenager with a severe degree of gastric dilation due to simultaneous overeating of bulky food and soft carbonated drinks. Due to the inefficacy of conservative measures and the development of severe ischemic alterations of the stomach, surgical intervention was performed.

Amaç: Bu meta-analizinde, pentraxin 3 (PTX3) ve iskemi modifiye albümin (İMA) biyobelirteçlerin akut appendisitte tanısal değerini gözden geçirmeyi amaçladık. Yöntem: PTX3 ve IMA’nın appendisit tanısındaki değerine odaklanan çalışmalar, PubMed veritabanını kullanılarak tarandı. Sadece pediatrik yaş grubunun randomize kontrollü prospektif klinik çalışmaları bu araştırmaya dahil edildi. Makale seçimi, özetler esas alınarak yapıldı. Bulgular: Tarama sonrasında, appendisitte IMA’nın tanısal değeriyle ilişkili 6, PTX3’ün tanısal değeriyle ilişkili 5 çalışmanın sonuçları gözden geçirildi. Toplamda 5 çalışma dahil edildi. 385 hastanın verisi derlendi. PTX3’ün akut appendisit tanısındaki duyarlılığı %73-92, özgüllüğü %88-100 saptanırken, İMA duyarlılığı %89-96,7, özgüllüğü %26-99,7 saptandı. Gastric volvulus (GV) is a rare pathology in children, and therefore such clinical cases cause certain difficulties not only in diagnosis, but also in treatment. However, this problem is a clinical reality in pediatric surgery and seems to be underdiagnosed because of its rareness. These patients are threatened by the development of irreversible ischemic disorders in the affected organs (not only stomach), with a high degree of probability, they may need to perform an organ-removal operation at late admission. This article presents two clinical observations of pediatric gastric volvulus of various types, with different etiopathogenesis.

This study aimed to determine the expression of nuclear factor kappa B (NF-κB) pathway related miRNAs in experimental acute respiratory distress syndrome (ARDS) induced by lipopolysaccharide (LPS) in rats, and to elucidate the underlying molecular mechanism. Twenty four sprague dawley rats were randomly divided into two groups; LPS (n = 12) and control (n = 12). Experimental ARDS was induced by intraperitoneal injection of E. coli LPS in LPS group. Intraperitoneal saline was administered in control group. Serum and lung samples were collected from both groups. Immunohistochemistry staining was performed for interleukin 1β (IL-1β), tumor necrosis factor α (TNF-α), CD 68, and caspase-3 in lung samples. Intensity of staining was scored as strong, moderate, weak, and no for evaluation of IL-1β and TNF-α. In addition, caspase-3 and CD68-positive stained cells were counted in sections. Expressions of 9 miRNAs were determined by quantitative real-time PCR in serum samples. IL-1β and TNF-α staining scores were significantly higher in the LPS group in comparison with the control group (P = 0.04 and P = 0.02, respectively). In addition, caspase-3 and CD68-positive stained cells were significantly higher in the LPS group (P = 0.02). Expressions of seven miRNAs were significantly changed in the LPS group in comparison with the control group. While six miRNAs (miR-7a-5p, miR-7b, miR-9a-5p, miR-21-5p, miR-29a-3p, and miR-138-5p) were up regulated, only miR-124-3p was down regulated. This study suggests that these miRNAs may have a role in the pathogenesis of ARDS related to NF-κB. However, this relationship needs to be examined in new studies by evaluation of pathways and target genes.

Objective: The Quality of Recovery-15 questionnaire is a self-rated questionnaire used to assess the quality of the postoperative recovery and health status of patients in the early period following surgery. The aim of this study was to assess the reliability, validity, and responsiveness of the Turkish version of the Quality of Recovery-15. Methods: After approval by the Maltepe University local ethics committee, this observational study was conducted among patients who received surgical interventions at Mersin University Hospital between July 2019 and January 2020. Reliability, feasibility, and validity were assessed to validate the Turkish version of the Quality of Recovery-15. Results: The completion rate of the form was determined to be 92% and a total of 200 patients were enrolled in the study. The Cronbach’s alpha of the global Turkish version of the Quality of Recovery-15 was 0.927. Test–retest reliability was 0.84 [CI 95%: 0.75-0.90] and Cohen’s effect size was 0.319. The total standardized response mean was determined as 0.53. Conclusions: This is the first study in which the Quality of Recovery-15 scale was translated into Turkish with our knowledge. The Turkish version of the Quality of Recovery-15 showed satisfactory reliability and validity in evaluating the quality of recovery after surgery in the Turkish population [ABSTRACT FROM AUTHOR]
