Abstract

The postoperative range of motion is one of the crucial factors indicating the outcome of Total Knee Arthroplasty (TKA). Although the correlation between range of knee flexion and posterior condylar offset (PCO) is controversial in the literature, PCO maintains its importance on evaluation of TKA. Due to limitations on PCO measurement, two novel parameters, posterior condylar offset ratio (PCOR) and anterior condylar offset ratio (ACOR), were introduced. Nowadays, the calculation of PCOR and ACOR on plain lateral radiographs is done manually by orthopedic surgeons. In this regard, we developed a software, AutoCOR, to calculate PCOR and ACOR autonomously, utilizing unsupervised machine learning algorithm (k-means clustering) and digital image processing techniques. The software AutoCOR is capable of detecting the anterior/posterior edge points and anterior/posterior cortex of the femoral shaft on true postoperative lateral conventional radiographs. To test the algorithm, 50 postoperative true lateral radiographs from Istanbul Kosuyolu Medipol Hospital Database were used (32 patients). The mean PCOR was 0.984 (SD 0.235) in software results and 0.972 (SD 0.164) in ground truth values. It shows strong and significant correlation between software and ground truth values (Pearson r=0.845 p<0.0001). The mean ACOR was 0.107 (SD 0.092) in software results and 0.107 (SD 0.070) in ground truth values. It shows moderate and significant correlation between software and ground truth values (Spearman's rs=0.519 p=0.0001412). 33. Are Recommendations Followed after Total Hip Arthroplasty? A Questionnaire.

Abstract

Introduction: Total hip arthroplasty (THA) is an operation that is successfully performed in patients with end-stage hip osteoarthritis. In this study, we aimed to evaluate the compliance level of primary THA patients with their doctor's post-operative advice on restricted hip movements and environment modifications to prevent dislocation. Methods: A survey consisting of 14 questions was prepared, questioning 320 patients who underwent THA in our clinic between 2015 and 2021. One hundred and forty three patients agreed to participate in our study, and their answers regarding whether they followed the introductions recommended by the surgeon who performed the surgery were recorded. Results: 99% of the 143 patients who participated in the survey stated that they were given some advice. 46% (n=66) of the patients stated that they were told that these introductions should be applied for life. 60% of the patients did not cross their legs, 74% did not squat, 86% did not sit cross-legged, 76% did not use toilet risers, and 89% do not do unrecommended sports. Conclusion: Most patients complied with the surgeon's recommendations in the early period; however, we observed that compliance decreased over time. [ABSTRACT FROM AUTHOR]

Abstract

Periprosthetic joint infection (PJI) is a rare yet devastating complication following total joint arthroplasty (TJA). Early and accurate diagnosis of PJI is paramount in order to maximize the chances of successful treatment. However, we are yet to identify a single "gold standard" test for the diagnosis of PJI. As a result, the diagnosis of PJI is often challenging. Currently, the 2018 ICM definition of PJI is the only validated diagnostic criteria available. This article will review the importance of serum and synovial biomarkers in the diagnosis of PJI. In addition, it will provide a brief overview of the emerging modalities for the identification of infections in this setting.

Abstract

The study by Porter et al. is elegant and perfectly designed. This study represents a clinically relevant and timely investigation with regard to tranexamic acid (TXA) use following hip fracture. Numerous studies have demonstrated that TXA can be considered safe and efficacious for elective total joint arthroplasty. As a result, TXA usage has gained momentum in patients undergoing arthroplasty. However, the evidence of TXA’s safety in high-risk patients with hip fracture undergoing arthroplasty is controversial. This study aimed to assess whether TXA administration to high-risk patients with hip fracture requiring arthroplasty increased the risk of thrombotic complications or mortality.

Abstract

Introduction: The purpose of the study is to compare the effects of genistein, estradiol, estradiol/progesterone combination on the bone mineral density and biomechanical properties of ovariectomized rats’ bone. Methods: 50 female adult Sprague-Dawley rats were divided into five groups. Bilaterally ovaeriectomy were performed in all groups except the sham-operated group. Groups were a sham-operated group and a control group (water was given), estradiol treated group (17-β estradiol 0.015 mg/kg per day), genistein treated group (genistein 10 mg/kg per day), and an estradiol/progesterone combination group (17-β estradiol 0.015 mg/kg plus drosperinone 0.028 mg/kg per day). The water or hormones were implemented in relevant groups for eight weeks by orogasthric catheter. The bone mineral density and biomechanical properties of the femur were analyzed. Results: Genistein, estradiol, and estradiol/progesterone groups increased bone mineral density significantly compared to the control group. In diaphysis and metaphysis bending test, all groups had higher peak load values than the control group. There were statistically significant differences between the estrogen/progesterone group and control group in diaphysis bending with regard to peak load. There were statistically significant differences between the estradiol and control groups in metaphysis bending with regard to peak load. In axial rotation test, all groups had higher peak torque values than the control groups. Conclusions: Genistein, estradiol and estrogen/progesterone combination improved the biomechanical properties of the ovariectomized rat bone. Genistein which has less side effects may be considered as an alternative in the treatment of postmenopausal osteoporosis.

Abstract

WOS: 000449502000002 PubMed ID: 30393544 Venous thromboembolism (VTE) is a serious complication after major orthopedic procedures. The best options for prevention of the VTE are still debated. The most popular evidence-based guidelines for prevention and treatment of VTE in orthopedic surgery addressed the total hip or knee arthroplasty and hip fractures as the major orthopedic surgeries. Majority of studies have evaluated the different modalities of the VTE prophylaxis in patients undergiong hip or knee arthroplasty. Hip preservation surgeries (HPS) including mini-open femoroacetabular osteoplasty, surgical dislocation of the hip, arthroscopic procedures, and periacetabular osteotomy (PAO) are gained popularity in recent two decades. The majority of these patients are young, healthy and active and may not be considered at high risk for VTE. The frequency of VTE in patients undergoing PAO seems to be low between 0 and 5%. There is a paucity of data regarding rates of VTE in young healthy patients undergoing HPS as well as the optimal prevention methods for VTE. Hence current VTE prevention guidelines do not cover HPS adequately. We aimed to review the available literature regarding VTE events and VTE prophylaxis options after HPS. We discussed the available and potential options for prophylaxis of VTE events in these procedures along with our experience in a large cohort of hip preservation surgery.
