Contains fulltext : 220977.pdf (Publisher’s version ) (Open Access), Purpose: Oncofertility focuses on providing fertility and endocrine-sparing options to patients who undergo life-preserving but gonadotoxic cancer treatment. The resources needed to meet patient demand often are fragmented along disciplinary lines. We quantify assets and gaps in oncofertility care on a global scale. Methods: Survey-based questionnaires were provided to 191 members of the Oncofertility Consortium Global Partners Network, a National Institutes of Health-funded organization. Responses were analyzed to measure trends and regional subtleties about patient oncofertility experiences and to analyze barriers to care at sites that provide oncofertility services. Results: Sixty-three responses were received (response rate, 25%), and 40 were analyzed from oncofertility centers in 28 countries. Thirty of 40 survey results (75%) showed that formal referral processes and psychological care are provided to patients at the majority of sites. Fourteen of 23 respondents (61%) stated that some fertility preservation services are not offered because of cultural and legal barriers. The growth of oncofertility and its capacity to improve the lives of cancer survivors around the globe relies on concentrated efforts to increase awareness, promote collaboration, share best practices, and advocate for research funding. Conclusion: This survey reveals global and regional successes and challenges and provides insight into what is needed to advance the field and make the discussion of fertility preservation and endocrine health a standard component of the cancer treatment plan. As the field of oncofertility continues to develop around the globe, regular assessment of both international and regional barriers to quality care must continue to guide process improvements.

- Abstract
PURPOSE: In the accompanying article, "Analysis of Fertility Preservation Options Available to Patients With Cancer Around the Globe," we showed that specific fertility preservation services may not be offered at various sites around the world because of cultural and legal barriers. We assessed global and regional experiences as well as the legal status of third-party reproduction and adoption to serve as a comprehensive international data set and resource for groups that wish to begin oncofertility interventions. METHODS: We provide data on the legalities of third-party assisted reproductive technologies and other family-building options in the 28 oncofertility-practicing countries surveyed. RESULTS: We found regional and country differences that will be important in the development of tailored resources for physicians and for patient brochures that are sensitive to these local restrictions and cultural norms. CONCLUSION: Because many patients first consult Web-based materials, the formal assessment of the availability of these options provides members of the global oncofertility community with data to which they might otherwise not have ready access to better serve their patients.

The objective of this study was to determine breed additive and heterosis effects on growth curves, total milk yield (TMY), calf weaning weight (WW), predicted energy intake (EI), and cow efficiency (CE) of purebred and crossbred beef cows raised in Southern Brazil. The data were from 175 purebred and crossbred cows representing eight genetic groups: Angus (A), Hereford (H), Nelore (N), A × H (AH), H × A (HA), A × N (AN), N × A (NA), and Caracu (C) × A (CA). Growth of the cows was modeled using the nonlinear Brody function and machine milking was used to assess TMY. WW was linearly adjusted to 210 d. EI was predicted with an equation in which the independent variables were estimates of parameters of the Brody function and TMY. The ratio of WW to EI estimated CE. Taurine?indicine heterosis effects were significant for all traits, and greater than those for taurine breed crosses. In general, crossbred cows were heavier at maturity, matured earlier, produced more milk, weaned heavier calves, and were predicted to consume more energy. Thus, they were more efficient than purebred cows, despite their greater predicted feed intake. Among the purebreds, A cows matured most rapidly, weighed the least at maturity, produced the most milk, weaned the heaviest calves, were predicted to consume the least energy; and were therefore most efficient among the breeds that were evaluated. These results are useful as inputs to bioeconomic models that can be used to predict productive and economic outcomes from crossbreeding and to facilitate recommendations for beef producers of southern Brazil and other similar subtropical climatic regions. - Abstract
PURPOSE: Oncofertility focuses on providing fertility and endocrine-sparing options to patients who undergo life-preserving but gonadotoxic cancer treatment. The resources needed to meet patient demand often are fragmented along disciplinary lines. We quantify assets and gaps in oncofertility care on a global scale. METHODS: Survey-based questionnaires were provided to 191 members of the Oncofertility Consortium Global Partners Network, a National Institutes of Health-funded organization. Responses were analyzed to measure trends and regional subtleties about patient oncofertility experiences and to analyze barriers to care at sites that provide oncofertility services. RESULTS: Sixty-three responses were received (response rate, 25%), and 40 were analyzed from oncofertility centers in 28 countries. Thirty of 40 survey results (75%) showed that formal referral processes and psychological care are provided to patients at the majority of sites. Fourteen of 23 respondents (61%) stated that some fertility preservation services are not offered because of cultural and legal barriers. The growth of oncofertility and its capacity to improve the lives of cancer survivors around the globe relies on concentrated efforts to increase awareness, promote collaboration, share best practices, and advocate for research funding. CONCLUSION: This survey reveals global and regional successes and challenges and provides insight into what is needed to advance the field and make the discussion of fertility preservation and endocrine health a standard component of the cancer treatment plan. As the field of oncofertility continues to develop around the globe, regular assessment of both international and regional barriers to quality care must continue to guide process improvements.

21. Narrative as process and artefact and some implications – the use and role of motif

Narrative is typically conflated with story with an attendant lack of consideration for what might be important distinctions between the two. Similarly, narrative is typically considered as an object but the recognition of the object as an artefact - a cultural tool - is disregarded. Rarely is narrative considered as a process and even less so as both a process and an artefact. In this paper we draw on the work of scholars from both Psychology and Humanities in an attempt to demonstrate that narrative is both a process and an artefact and consider some of the implications for this understanding. In particular, we present an understanding of the role of motif. Motif, we suggest, is a powerful tool used to both build and represent shared meanings and to recognise recurring patterns. Of course, the former understanding is widely acknowledged in literary studies, but less so in other disciplinary studies of narrative. Our current work demonstrates how a linguistic motif is both used to influence policy in a global field (healthcare) and by recognising and identifying the motive we can draw insights into the functions of ‘the system’. This parallels work in neuroscience and cognitive science where motifs are recognised as an important artefact for understand complex chemical interactions in the brain. We argue that work is required to de-conflate narrative and story, and to distinguish between the role of narrative as process and the forms of narrative as artefacts (e.g. story, account, chronology, anecdote, motif, &c. &c.)

On the night of 22–23 February 2006, 444 transient luminous events (TLEs), 86% sprites, were observed above a prolific mesoscale convective system (MCS) over Argentina, as part of the third sprite campaign in Brazil. GOES infrared (IR) cloud top temperatures (Tc) and Tropical Rainfall Measuring Mission (TRMM) radar (PR) and microwave (TMI) data were used to investigate the MCS convective characteristics and their relationship with World Wide Location Network (WWLLN) detected cloud‐to‐ground (CG) lightning and TLE activity. The MCS had a minimum lifetime of 20 hours, 8.5 as a MCS, a maximum extent of ∼430,000 km2, and gusty winds of ∼39–50 km/h. It had several distinctive characteristics: exceptionally high TLE rate, multicellular structure with 19 distinguishable convective regions, and cloud tops temperatures (Tc) ∼10–20 °C higher than regular TLEproducing MCSs over the central USA and South America. Most TLEs occurred above “individual stratiform regions”, where Tc varied from −45 °C to −53 °C from the beginning to the end of the night, surrounding the areas of strong convections, with convective cores at Tc −59 °C to −74 °C, which did not extend up to or overshoot the tropopause, estimated at −75 °C (∼17.1 km) as normally observed for TLE‐producing MCS in these regions. The moderated convection is contrary to the expectation that large charge production is accompanied by vigorous updrafts within deep convection that give rise to cold cloud overshooting tops, thus prompting a detailed study of this prolific TLE‐producing thunderstorm. On the basis of a charge moment change threshold of 350 Ckm and estimated 5 km charge removal altitude, a lower threshold of ∼4,300 C/h was estimated for the hourly charge transfer rate necessary for the observed sprite production (383 events), which is twice the rate for an average TLE‐producingMCS (70 events), also estimated.TMI/TRMM data for the storm at early development showed a low brightness temperature of 84 K, indicative of significant ice content, which is important for cloud electrification processes. We suggest that the unusually high incidence of TLEs in this moderately convective MCS may be related to other local geophysical phenomena such as a large tropospheric aerosol concentration due to smoke from forest fires. Satellite fire count data showed that there were ∼200 fires between 20 and 22 February immediately north of the MCS initiation region and a transport simulation with the Coupled Aerosol‐Tracer Transport model from the Brazilian developments on Regional Atmospheric Modeling System (CATT/BRAMS) model showed a large PM2.5 aerosol concentration, 10,000 mg/m2 (column integrated), at the region where the MCS developed. The aerosols present in the smoke may have been a source of ice nuclei affecting the production of ice particles that get positively charged, accounting for the charge transfer rate necessary to originate the observed TLE production.
