Summary A 66-year-old male patient was referred to our clinic with severe pneumonia. Bronchoscopy was performed due to clinical worsening despite antibiotics and diuretic therapy, respiratory failure and radiographic progression. Because bacterial cultures of the bronchoalveolar lavage fluid were negative and after using amiodarone for almost one month, we eliminated amiodarone from his medication regimen due to suspicion of amiodarone toxicity. Accordingly, we also initiated systemic steroid therapy. Chest X-ray done after 72 hours showed a significant resolution of lung consolidations and the patient exhibited significant clinical improvement, with decline of his oxygen requirements.

To determine whether an association exists between a known marker of platelet activation, mean platelet volume (MPV), and the severity of acute pulmonary embolism (APE), and to test whether it can be used as prognostic indicator in patients with high-risk pulmonary embolism (PE). The study included 209 consecutive patients with APE (106 male, 103 female, mean age 62.4 ± 15.4 years) and 162 controls (86 male, 76 female) matched for age (60.5 ± 14.3 years) and concomitant diseases. Contrast-enhanced spiral computerized tomography or ventilation-perfusion scintigraphy, echocardiography, and lower extremity venous Doppler ultrasound were performed on the patients with APE. D-dimer level, troponin, arterial blood gases, platelet count, and MPV were measured in serum. The MPV did not differ between the patients with APE and the controls (8.0 ± 1.1 fL, 7.9 ± 0.59 fL, p = 0.22). There were no significant differences in the value of MPV among the groups of patients with massive, submassive, and nonmassive PE (MPV: 8.3 ± 0.9 fL, 8.1 ± 1.0 fL, 7.9 ± 1.1 fL, p = 0.08, respectively). The MPV was higher in non-survivors than survivors (8.6 ± 1.1 vs. 7.9 ± 1.1 fL, p = 0.02). There was a statistically weak correlation between MPV and mean pulmonary arterial pressure (r = 0.25, p

Amaç: Hem aktif, hem de pasif sigara içimi erişkinde astım gelişme riskini artırmakta, solunum semptom ve fonksiyonlarını olumsuz etkilemektedir. Çalışmamızda özelleşmiş astım polikliniğinde aynı hekim tarafından düzenli izlenen ve tedavi edilen astım olgularında aktif ve pasif sigara içme durumları ile sigaranın hastalığın şiddeti ve ataklar üzerindeki etkisi araştırılmıştır. Gereç ve Yöntem: Astım polikliniğine başvuran 131 olguya hastalık öyküsü, şiddeti ve atak sıklığı, ilaç kullanımı, aktif ve pasif sigara içiciliği durumlarını değerlendirmeye yönelik anket uygulanmış, idrarda kotinin düzeyi bakılmıştır. Bulgular: Olguların yaş ortalaması 52.0±11.7 olup 93'ü (%71) kadındır. Pasif içicilik 39 (%30) olguda saptanırken 7 olgu (%5) aktif içicidir. İdrar kotinin düzeyi pasif içici olguların sadece 2'sinde yüksek (>500ng/ml) bulunmuştur. Pasif içicilik anamnezi veren olgularda son bir yılda atak sayısı ortalama 1.5, maruz kalmayanlarda ise 1'dir (p>0.05). Pasif içici olanlarda, olmayanlara göre hafif intermittan (%32'ye karşılık %24) ve hafif persistan olguların sayısı (%53'e karşılık %46) daha yüksek iken orta (%13'e karşılık %22) ve ağır persistan olguların oranı (%3'e karşılık %8) maruz kalmayanlarda daha yüksektir. Sonuç: Sonuçlarımıza göre astım olgularında genel populasyona göre sigara içme oranları düşüktür. Bu durum özelleşmiş astım polikliniğinde hasta eğitiminin önemini vurgulamak açısından önemli olabilir. Öte yandan 1/3'nün pasif içici olması toplumun astım konusunda bilinçlendirilmesi gerektiğini düşündürmektedir., Objective: Both active and passive smoking increase the risk of developing asthma and respiratory symptoms and decrease pulmonary functions. the aim of this study was to investigate the effects of active and passive smoking exposure on asthma severity and exacerbations in patients treated and followed-up regularly by the same physician in a specialized asthma outpatient clinic. Material and Method: We used a questionnaire to assess the history, medications, severity and exacerbations of asthma, besides active and passive smoking exposure in 131 patients attending our asthma outpatient clinic and urinary cotinine levels were measured. Results: Mean age of the patients was 52.0±11.7 [93 females (71%)]. Thirty nine patients (30%) reported passive smoking exposure, whereas seven (5%) patients were current smokers. Urinary cotinine levels were high (>500 ng/ml) in only two patients with passive smoking exposure. the average number of asthma exacerbations was 1.5/year in patients exposed to passive smoking and 1/year in patients without exposure (p>0.05). the number of patients with intermittent (32% vs. 24%) and mild persistent (53% vs. 46%) asthma were higher in passive smoking patients whereas there were more patients without smoking exposure in the moderate persistent (13% vs. 22%) and severe persistent (3% vs. 8%) groups. Conclusion: We concluded that the percentage of current smokers was lower than the general population among asthma patients. This can emphasize the importance of educational therapy in the specific asthma outpatient clinic. on the other hand, we should improve the consciousness of the general public about asthma as one third of the patients were passive smokers.

Amaç: Malatya ilinde mevsim ve iklim değişikliği ile PTE görülme insidansı arasında herhangi bir ilişki olup olmadığını araştırmak istedikGereç ve yöntem: 01/01/2013 ile 31/12/2015 tarihlerini kapsayan üç yıllık dönemde, İnönü Üniversitesi Tıp Fakültesi Göğüs Hastalıkları Kliniği tarafından PTE tanısı konmuş 207 hasta kesitsel olarak incelendi. Hastaların demografik özellikleri, laboratuvar verileri ve hastaneye başvuru tarihleri kaydedildi. Bu veriler doğrultusunda; hastaların başvuru tarihine ait basınç, sıcaklık, rüzgar hızı ve nem değerleri tespit edilerek ortalaması alındı. Bulgular: Çalışmaya PTE tanısı konulan toplam 207 hasta alındı. 113´ü kadın (%54,5), 94´ü erkek (%45,5) olan hastaların ortalama yaşları 62,7±15.5 olup, en küçüğü 20, en büyüğü 90 yaşında idi. Erkek hastaların yaş ortalaması 65,6 olup, kadın hastaların yaş ortalaması ise 60,38 olarak tespit edildi. Mevsimlere göre dağılım incelendiğinde hastaların %28 (n=58) ile en sık sonbahar mevsiminde, %19,3 (n=40) ile en az kış mevsiminde izlenildi. PTE görülme sıklığı açısından mevsimler arası istatistiksel olarak anlamlı fark saptanmadı (p>0.05). PTE tanısı alan hastalar, aylara göre görülme sıklığı açısından değerlendirildiğinde; hastaların %11,6'sının (n=24) temmuz ayında, %4.3'ünün (n=9) şubat ayında takip edildiği saptandı. PTE görülme sıklığı açısından aylar arasında istatistiksel olarak anlamlı fark saptanmadı (p>0.05). PTE için predispozan risk faktörü olabilecek ek hastalıklara bakıldı . Virchow triadı dikkate alınarak yapılan bu değerlendirmede KOAH, immobilizasyona neden olabilecek ek hastalık, post operatif PTE, malignite, kalıtsal trombofili, gebelik, vaskülit, kardiyak ve hematolojik hastalığı olan hastalar kaydedildi. Ek hastalığa sahip hasta sayısı 118 (%57) iken, ek hastalık saptanmayan hasta sayısı 89'du (%43). PTE için risk faktörü olmayan hastaların tanı aldıkları mevsimlere göre dağılımları incelendiğinde ise, 25 (%28) ile en fazla hastanın sonbahar mevsiminde, 18 (%20) ile en az hastanın yaz mevsiminde PTE tanısı aldığı görüldü. Risk faktörü olmayan PTE'de görülme sıklığının mevsimlere göre ilişkisi istatistiksel olarak anlamlı bulunmadı (p> 0,05). Risk faktörü olmayan PTE hastalarının, görülme sıklığı aylara göre incelendi. Mayıs ayında 10 hasta (% 11) PTE tanısı alırken, Temmuz ayı, PTE görülme sıklığı açısından en düşük ay idi (n=2, % 3). PTE görülme sıklığı açısından aylar arasında istatistiksel olarak anlamlı fark saptanmadı (p>0.05).Sonuç: Hastaları risk faktörü taşımaları ve taşımamalarına göre ayırarak mevsim ilişkisine baktığımızda anlamlı bir ilişki tespit edemedik. Malatya ilinin mevsim ve iklim değişikliğini değerlendirdiğimizde, elde ettiğimiz sonuçlara göre PTE gelişme riski üzerinde iklim ve mevsim değişikliklerinin anlamlı katkısı olmadığı kanaatine vardık. Objective: We aimed to study whether there is a relationship between season and climate changes and incidence of PTE in city of Malatya. Material and method: 207 patients who have been diagnosed with PTE by Inonu University Department of Chest Diseases in the three years period of 1/1/2013 and 12/31/2015 are studied cross sectional. Demographic characteristics of the patients, laboratory data and dates of application to the hospital were recorded. In the direction of this data; Pressure, temperature, wind speed and humidity values of patients' application date were determined and averages were oBTainedFindings: This study contains 207 patients diagnosed by PTE, 113 (54.5%) of which were female and 94 (45.5%) were male. Average age was 62.7±15.5; with minimum 20 and maximum 90. Average age of male patients were 65.6, while of female patients were 60.38. Seasonal study showed us that patients were most frequently diagnosed in autumn, with 28% share (n=58), while least in winter with 19.3% (n=40). There was no statistically significant difference between the seasons of PTE incidence (p> 0.05).Patients who were diagnosed with PTE were evaluated in terms of the incidence according to month; 11.6% (n = 24) of the patients were followed up in July, and 4.3% (n = 9) of the patients were followed in February. There was no statistically significant difference between the months of PTE incidence (p> 0.05).Additional diseases that may be a predisposing risk factor for PTE were looked at. Patients with COPD, additional disease that may cause immobilization, post operative PTE, malignancy, hereditary thrombophilia, pregnancy, vasculitis, cardiac and hematologic disease were recorded in this evaluation based on the Virchow triad.The number of patients with additional disease was 118 (57%) while the number of patients without additional disease was 89 (43%). When the distribution of the patients with no risk factors for PTE according to the season they were diagnosed were examined, it was seen that the least patients received PTE diagnosis was in summer with 18 (20%), while the most was in fall season with number of 25 (28%). There was no statistically significant correlation between the frequency of occurrence of non-risk factors PTE and seasons (p> 0.05). The incidence of PTE patients without risk factors was examined according to the month. In May, 10 patients (11%) were diagnosed with PTE while in July, the incidence of PTE was lowest (n = 2, 3%). There was no statistically significant difference between the months of PTE incidence (p> 0.05).Results: We could not detect a significant relationship between the seasons and PTE diagnosis, by separating patients according to whether they carry risk factors. When we evaluate the seasonal and climate change of Malatya Province, we conclude that climate change and seasonal changes do not have a significant effect on the risk of PTE according to the results we have achieved. 84

Amaç: Tütün yasasının yürürlülüğündeki başarı, uygulayıcı kurumların konuya bakışaçısı, bilgi ve farkındalık düzeyi ile yakından ilişkilidir. Bu çalışmada 4207 sayılı kanununişlerliğinde önemli rolü olan il tütün kontrol kurullarında (ITKK) görevli kurumlarda çalışankamu personellerinin tütün yasası hakkındaki farkındalık ve duyarlılık düzeyleri, yasaya bakışaçıları araştırılmıştır.Gereç ve Yöntem: Malatya İl Valiliği, Belediye, Milli Eğitim Müdürlüğü, SağlıkMüdürlüğü, Gençlik ve Spor İl Müdürlüğü, Verem Savaş Dispanseri, İl Müftülüğü veEmniyet Müdürlüğü'nde çalışan 305 kamu personeline anket uygulanmıştır. Ankette olgularındemografik verileri, sigara içme durumları, çalıştıkları kurumlar, yasayı öğrenme yolları,yasadan sonra kendi sigara içme tutumlarında değişiklik, yasa hakkındaki bilgi düzeyleri,yasanın işlerliği hakkındaki görüşleri, yasa hakkında bireysel ve kurumsal sorumlulukhakkındaki düşüncelerini sorgulayan sorular yöneltilmiştir.Bulgular: Olguların yaş ortalaması 35.9±8.9 olup %73.1'i erkektir. Lise ve üniversitemezunu oranı %92.1'dir. %58.4'ü aktif sigara içicidir. Verem Savaş Dispanseri'nde sigaraiçme oranı diğer kurumlardan fazladır (%75). Yasa sonrası sigara içme oranlarının en çokazaldığı kurum %50.8 ile Emniyet Müdürlüğü'dür. Yasa hakkında olguların %77'si medyayoluyla haberdar olurken kurum amiri yoluyla öğrenenlerin oranı %5.9'dur. Olgulardan%81.3'ü yasayı desteklemektedir. %30'u çalıştığı kurumda sigara içildiğini, %22.3'ü yasakyerde sigara satıldığını görmüştür. Çalışmaya katılan kişilerin yasanın uygulanmasındakendilerini ne kadar sorumlu hissettiklerini anlamaya yönelik 3 farklı soru sorulmuştur.?Kapalı alanda sigara içeni gördüğünüzde tutumunuz ne olur? sorusuna katılımcıların %57.8'iikaz ederim, %7'si 155/184'ü ararım, %10.3'ü kurum yetkisine şikayet ederim, %23.2'siilgilenmem, %1.7'si eşlik ederim şeklinde cevap vermiştir. İzmarit atan gördüğündekatılımcıların %65'i ikaz edeceğini, %25.2'si ise ilgilenmeyeceğini belirtmiştir. In thisstudy the points of view of 350 public employees were evaluated within the Tobocco ControlCommittee of Malatya. Their points of view, levels of awareness regarding the tobacco lowwere investigated.Methods: The Government of Malatya, Municipality of Malatya, Director of NationalEducation, Health Directorate, The provincial directorate of Youth and sports, TuberculosisDispansery, Mufti and Police Department stafs were surveyed. The demographic data,smokıng status, their attıtudes to smoking after changing the law their opions about thefunctioning of the law was obtained and how the law was learnt and their thoughts onindividual and corporate responsibility were asked.Invention: The mean age was 35.9 ± 8.9 years-old. 73.1% males, 92.1% high schooland university. 58.4% smokers. The rate of smoking in Tuberculosis Dispansery were higherthan other institutions (75%). The most degradation in the smoking rate after the law was seenin Police Department (50.8%). 77% of the cases were informed through the media about thelaw, while 5.9% of them learned through the authority of the institution, 81.3% of the casessupported the law. 30% know that cigarette can be smoke at their institution and 22.3%realize the cigarette selling despite of banning.
