Abstract

BACKGROUND: Since routine immunization could change the epidemiological profile of hepatitis A virus (HAV) infection in the future, it is important to determine the baseline immunity to HAV across Turkey. OBJECTIVE: The aim of this study was to determine the seroprevalence of hepatitis A among individuals 6 years of age and older in Yozgat, Turkey. DESIGN: Cross-sectional. SETTING: Community in central region of Turkey. PATIENTS AND METHODS: Questionnaires and blood specimens were collected and the presence of hepatitis A IgG antibodies against hepatitis A virus was determined quantitatively by ELISA. MAIN OUTCOME MEASURES: The rates of hepatitis A immunity by age group. SAMPLE SIZE: 1862. RESULTS: Immunity to hepatitis A was 79.1% (n=1473). 2. Retrospective evaluation of pediatric inpatients admitted with thrombosis according to risk factors: single center experience

Abstract

Purpose: Thrombosis in children is a multifactorial disorder associated with both genetic and acquired factors. Idiopathic thrombosis is a rare condition. This study aimed to examine the etiology and risk factors of thrombosis in pediatric patients followed at Baskent University Adana Teaching and Research Center, a reference hospital in the southern part of Turkey. Material and Methods: This study included a sample of 49 pediatric patients who were retrospectively evaluated. The patient records were examined retrospectively in terms of age at diagnosis with thrombosis, gender, family history of thrombosis, localization of thrombosis, underlying primary disease and presence of catheter, as well as indicators of genetic risk factors. Results: The number of children presenting to our clinic during the study period was 10358. The incidence of thrombosis was calculated as 49 cases in 10358 children. The median age of the patients was 5.1 years (ranging from 0 to 17.2 years), and there were 32 (65.3%) males. In the analysis of the genetic factors causing susceptibility to primary thrombosis, only a portion of the patients were studied for genetic mutations. Among these, 5 patients showed factor V Leiden mutation, 13 patients MTHFR 1298C mutation, and 18 patients MTHFR 677CT mutation. Seven patients were found have a combination of multiple mutations. When we investigated any possible correlation between high lipoprotein (a) levels and the distribution of mutations, we found that 7 patients with mutations had normal levels of high lipoprotein (a), while other 10 patients with mutations were found to have elevated levels of lipoprotein (a). Conclusion: We conclude and strongly emphasize that lipoprotein (a) elevation is one of the important genetic risk factors, which might prove a major risk indicator, as valuable as other mutation screening tests performed by molecular genetics [Cukurova Med J 2016; 41(2.000): 285-290]

Abstract

Elevated serum levels of neuron-specific enolase (NSE) was initially assumed to be specific to neuronal tumors (particularly neuroblastoma), but is now known to accompany nontumoral conditions and tumors other than neuroblastomas. There is a need to establish normal ranges for NSE, especially in early infancy. The aims of this study were to determine reference values for NSE in newborns and young infants and to assess whether NSE levels in early infancy (i.e., preterm infants and term infants) differ from the adult reference range for this enzyme. Methods: We enrolled 140 healthy babies, which included 40 preterm newborns (3–15 days old and born at 28–42 weeks gestation), 40 term newborns (< 1 month old and born at term), and 60 young infants 1–3 months old (n = 20 per subgroup of 1-, 2-, and 3-month-old infants). The determination of NSE levels was performed by the electrochemiluminescence immunoassay (ECLIA) method using the Elecysys 2010 device (Roche Diagnostics, Mannheim, Germany). The mean serum NSE levels for the preterm newborns was 21.83 ± 15.06 ng/mL [95% confidence interval (95%CI), 16.95–26.71 ng/mL]; term newborns, 18.06 ± 12.83 ng/mL (95%CI, 13.94–22.19 ng/mL); and young infants, 9.09 ± 4.38 ng/mL (95%CI, 7.96–10.23 ng/mL). Abstract

PURPOSEIn children the assessment of solid tumors’ response to chemotherapy is based primarily on size reduction, which can be unreliable and a late marker, in the presence of necrosis. We aimed to establish whether apparent diffusion coefficient (ADC) values of childhood neuroblastomas show proportional changes in relation to chemotherapy response.METHODSWe evaluated 15 pediatric patients with abdominopelvic neuroblastomas, who had undergone MRI before and after chemotherapy. Two radiologists retrospectively analyzed all images by drawing a round uniform region-of-interest in the solid/contrast-enhancing portion of the lesions in consensus. The ADC values from pre- and postchemotherapy images were compared.RESULTSPostchemotherapy ADC values were significantly higher than those obtained before treatment (P < 0.05, for minimum, maximum, and median ADC values).CONCLUSIONOur results support diffusion-weighted MRI as a promising noninvasive biomarker of therapeutic responses. To the best of our knowledge, this is the first report to compare diffusion- weighted imaging findings before and after chemotherapy in childhood neuroblastic tumors.

Abstract

Cyproheptadine hydrochloride (CYP HCl) {4-(5H-dibenzo[a,d]-cyclohepten-5-ylidene)-1-methylpiperidine hydrochloride} is a first-generation antihistamine with additional anticholinergic, antiserotonergic, and local-anesthetic properties. The geometry optimization, Mulliken atomic charges and wavenumber and intensity of the vibrational bands of all of the possible modes of CYP HCl have been calculated using ab initio Hartree–Fock (HF) and density functional theory (DFT) employing the B3LYP functional with the 6-311G(d,p) basis set. We have compared the calculated IR and Raman wavenumbers with experimental data. Quantum-chemical calculations of the geometrical structure, energies, and molecular electrostatic potential and NBO analysis of CYP HCl have been performed using the B3LYP/6-311G(d,p) method. The electric dipole moment ( μ ), static polarizability ( α ) and the first hyperpolarizability ( β ) values of the title compound have been computed using HF and DFT methods. The study reveals that the antihistaminic pharmacological property of CYP HCl has a large β value and, hence, may in general have potential applications in the development of non-linear optical materials. The experimental and calculated results for CYP HCl have also been compared with those for mianserin HCl.

Abstract

Mirtazapine (±)-1,2,3,4,10,14b-hexahydro-2-methylpyrazino(2,1-a)pyrido(2,3-c)(2)benzazepine is a compound with antidepressant therapeutic effects. It is the 6-aza derivative of the tetracyclic antidepressant mianserin (±)-2-methyl-1,2,3,4,10,14b-hexahydrodibenzo[c,f]pyrazino[1,2-a]azepine. The FT-IR and FT-Raman spectra of mirtazapine have been recorded in 4000–400 cm−1 and 3500–10 cm−1, respectively. The optimized geometry, energies, nonlinear optical properties, vibrational frequencies, 13C, 1H and 15N NMR chemical shift values of mirtazapine have been determined using the density functional theory (DFT/B3LYP) method. A comparison of the experimental and theoretical results of mirtazapine indicates that the density-functional B3LYP method is able to provide satisfactory results for predicting vibrational and NMR properties. The experimental and calculated results for mirtazapine have also been compared with mianserin.

Abstract

The molecular structure, linear and non-linear optical properties, and electronic properties of the anti-depressant drug mianserin (MIA) and its salts (MIA HCl and MIA HBr) have been investigated using the density functional theory (DFT/B3LYP) method with a 6-311G(d,p) basis set. The calculated first hyperpolarizability values are comparable with the reported values and are attractive objects for future studies of non-linear optics. FT-IR (4000–400 cm −1 ) and FT-Raman (3500–50 cm −1 ) spectra of mianserin HCl have been measured and analyzed. The assignment of bands observed in vibrational spectra have been made by comparison of their theoretical vibrational frequencies obtained using a B3LYP/6-311G(d,p) basis set. The scaled frequencies resulted in good agreement with the observed spectral patterns.

Abstract

In children, complaints of a respiratory disorder are very frequent. Etiology of respiratory illness is a broad spectrum that varies from a simple viral infection to a malignant disorder. Pulmonary Langerhans cell histiocytosis (PLCH) is one of these entities and it is truly rare in children. The aim of this study is to evaluate our patients with PLCH.Patients who had been diagnosed with PLCH were retrospectively evaluated. Features of medical history, onset of the complaints, date of the diagnosis, chest X-Ray and computed tomography (CT) findings, histopathology and other laboratory investigations were considered.There were four cases with PLCH. All of them were male, ages were between 5 months and 16 years. In three cases, major complaints were chronic respiratory problems whereas in one of them there was acute respiratory distress beginning with cough and leading to pneumothorax. In all of the cases, multisystemic involvement was prominent. The diagnosis was proven by histopathology in all of the cases. In two children with smaller age, skin involvement was detected. Time from complaint to diagnosis was minimum 3 months and maximum 3 years.PLCH is a rare disorder in children. Pulmonary involvement is generally a component of systemic involvement but in many cases it might have been detected with early respiratory complaints. So, children with chronic respiratory problems should be carefully evaluated and should be followed up for rare entities like PLCH.

During the 6-year study using the Centers for Disease Control and Prevention's (CDC) U.S. National Healthcare Safety Network (NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 605,310 patients hospitalized in the INICC's ICUs for an aggregate of 3,338,396 days. Although device utilization in the INICC's ICUs was similar to that reported from ICUs in the U.S. in the CDC's NHSN, rates of device-associated nosocomial infection were higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infection in the INICC's ICUs, 4.9 per 1,000 central line days, is nearly 5-fold higher than the 0.9 per 1,000 central line days reported from comparable U. S. ICUs. The overall rate of ventilator-associated pneumonia was also higher (16.8 vs 1.1 per 1,000 ventilator days) as was the rate of catheter-associated urinary tract infection (5.5 vs 1.3 per 1,000 catheter days). Frequencies of resistance of Pseudomonas isolates to amikacin (42.8% vs 10%) and imipenem (42.4% vs 26.1%) and Klebsiella pneumoniae isolates to ceftazidime (71.2% vs 28.8%) and imipenem (19.6% vs 12.8%) were also higher in the INICC's ICUs compared with the ICUs of the CDC's NHSN. Abstract

Streptococcus pyogenes (Lancefield group A streptococci (GAS)) is a common human pathogen which causes a variety of diseases with a great difference in severity.[1] The most common manifestation of S.pyogenes infection is pharyngitis, but invasive disease can be seen also.[2] Invasive group A streptococcal disease (iGAS) is defined as an infection associated with the isolation of S. pyogenes from a normally sterile body site. S.pyogenes is serologically classified according to the identification of T and M cell wall protein antigens.[3] The M protein (emm type) is a major virulence factor of S. pyogenes and different clinical manifestations were related to particular M/emm types.[1,2] Therefore in epidemiological studies of iGAS, the organisms were classified by M/emm type. There are more than 120 different M/emm types of S. pyogenes and many of them are capable of causing severe disease.[2,3] Person-to-person spread of S.pyogenes occurs from an infected person by respiratory droplets or by direct contact with secretions of the nose or throat or by contact with infected wounds on the skin. People are infectious when they are ill, but transmission from asymptomatic carriers is also possible.[4,5] Other patients, family members, healthcare workers (HCWs) and the environment could be the source of the organism. In hospitals, outbreaks affecting multiple patients can occur. When a cluster of GAS cases has been identified, in order to prevent further cases measures should be taken immediately.[5] Mathur et al. in this issue present an outbreak of Streptococcus pyogenes emm type 58 in a high dependency unit of a level-1 trauma center of India.[6] The authors observed four cases of invasive GAS infections in a 28-bedded poly-trauma ward of a level-1 trauma care center in October, 2012. An intensive surveillance and institution of prompt treatment were performed in order to control the outbreak. GAS strains were isolated from blood culture and multiple sites in two patients, tracheal aspirate in one patient and central line tip in another patient. As in one patient the isolate was obtained from central line tip, they suggested that there should be a definite break in infection control measures in the unit. Sequence analysis of emm gene and profiling of exotoxin production was done to identify the similarity and virulence of the strains. PCR was done for detection of exotoxins speA, speB, speC, speF, speG, speI, speH, speL, speJ speM, smeZ and ssa revealed that all the isolates of GAS obtained from the different sources produced speB, speC, speG, speF and smeZ. A sequence analysis of the emm gene was performed and all isolates were found to be GAS emm type 58. The authors declared that for source investigation a total of 146 samples were taken. Of these, 48 were throat swabs of nursing staff in all the shifts, 4 were throat swabs of the house keeping staff, 16 were throat swabs/tracheal aspirates of other admitted patients, five were throat swabs of various attendants of the patients and 73 were environmental samples of various devices/surfaces in the wards. From these 146 samples, GAS were isolated the ventilator tubing of a patient and the tracheal aspirate and the tip of the suction tubing of another patient. GAS was not isolated from any of the health care workers (HCWs). The major limitation of this study was the source of the outbreak could not be determined. Another important limitation was inadequate screening of the HCWs. The physicians who were taking care of these patients were not screened. It is well known that an asymptomatic staff carrier can be identified in most of outbreaks.[7] Because of the number of reported cases of staff-to-patient transmission of GAS, screening all of the HCWs epidemiologically linked to affected patients is an essential part of outbreak investigation.[8]

21. Malign tümör tanısı alan çocuk hastaların epidemiyolojik özellikleri ve sağkalımları

Abstract

Amaç: Bu çalışmada amacımız bölgemizde merkezimize başvuran malign tümörlü hastaların epidemiyolojik özellikleri ve sağkalımlarını değerlendirmektir.Gereç ve Yöntem: Malign tümör tanısı alan, 599 çocukluk çağı kanseri olan hasta epidemiyolojik özellikleri açısından retrospektif olarak değerlendirildi. Bu hastalardan; bir aydan uzun süre merkezimizde takip edilmiş olan 440 hasta sağkalım açısından ayrı olarak analiz edildi. Veriler, çocuk hematoloji ve onkoloji bölümü hasta kayıt dosyaları taranarak elde edilmiş olup, özel istatistik programları ile analiz edilmiştir.Bulgular: Hastanemize başvuran malign tümör tanısı alan kayıtlı toplam 599 hastanın, ortanca yaşı 6 yıl (1 gün-20,4 yıl), erkek kız oranı (E/K) 1,2 saptandı. Tümör tanılarına göre görülme sıklığı sırası ile hastaların %8,4’inde lösemi; %21,2’sinde lenfoma; %22,2’sinde santral sinir sistemi tümörleri idi.. Tüm kayıtlı 599 hasta ve primer takip ve tedavi edilen 440 hastanın epidemiyolojik özellikleri benzer saptanmıştır. Bir aydan uzun takipli 440 hastanın beş yıllık genel ve olaysız sağkalım hızı sırası ile %75,3 ve %66,9 saptandı. Sonuç: Hastalarımızın epidemiyolojik verileri lösemi sıklığı dışında diğer epidemiyolojik çalışmalardaki sonuçlarla benzer saptanmıştır. Merkezimizde lösemi hastaları 2010 yılından itibaren izlenmeye başlanmıştır. Pediatrik onkoloji hastalarında yüksek sağkalım hızları multidisipliner tedavi protokolerinin ve profesyonel ekibin önemini göstermektedir.

22. Retrospective evaluation of pediatric inpatients admitted with thrombosis according to risk factors: single center experience

Abstract

Purpose: Thrombosis in children is a multifactorial disorder associated with both genetic and acquired factors. Idiopathic thrombosis is a rare condition. This study aimed to examine the etiology and risk factors of thrombosis in pediatric patients followed at Baskent University Adana Teaching and Research Center, a reference hospital in the southern part of Turkey.Material and Methods: This study included a sample of 49 pediatric patients who were retrospectively evaluated. The patient records were examined retrospectively in terms of age at diagnosis with thrombosis, gender, family history of thrombosis, localization of thrombosis, underlying primary disease and presence of catheter, as well as indicators of genetic risk factors. Results: The number of children presenting to our clinic during the study period was 10358. The incidence of thrombosis was calculated as 49 cases in 10358 children. The median age of the patients was 5.1 years (ranging from 0 to 17.2 years), and there were 32 (65.3%) males. In the analysis of the genetic factors causing susceptibility to primary thrombosis, only a portion of the patients were studied for genetic mutations. Among these, 5 patients showed factor V Leiden mutation, 13 patients MTHFR 1298C mutation, and 18 patients MTHFR 677CT mutation. Seven patients were found have a combination of multiple mutations. When we investigated any possible correlation between high lipoprotein (a) levels and the distribution of mutations, we found that 7 patients with mutations had normal levels of high lipoprotein (a), while other 10 patients with mutations were found to have elevated levels of lipoprotein (a).Conclusion: We conclude and strongly emphasize that lipoprotein (a) elevation is one of the important genetic risk factors, which might prove a major risk indicator, as valuable as other mutation screening tests performed by molecular genetics

23. A Newborn with Congenital Mixed Phenotype Acute Leukemia After In Vitro Fertilization

Abstract

Congenital leukemia is a rare disease. The majority of cases of this disease are acute myelogenous leukemia (AML). Congenital acute lymphoblastic leukemia (ALL) is rare and most often is of B cell lineage. Rarely, some cases have been designated biphenotypic or mixed phenotype acute leukemia (MPAL). Herein, we report a preterm newborn referred to us as a result of the appearance of blue-violaceous dermal nodules on her body at birth. She was a twin and the product of an in vitro fertilization (IVF) pregnancy. Physical examination showed jaundice, hepatosplenomegaly, and peripheral facial nerve palsy in addition to dermal nodules. Bone marrow aspiration showed 40% blasts of lymphoid lineage; skin biopsy and its immunohistochemistry revealed myeloblastic infiltration of the dermis. Cytogenetic analysis (46,XX), fluorescence in situ hybridization (FISH) analysis, and cranial magnetic resonance were normal. The patient was diagnosed with congenital MPAL, and an association between IVF and congenital leukemia was suggested.

24. Treatment of intrathecal methotrexate overdose with folinic acid rescue and lumbar cerebrospinal fluid exchange: A report of two cases

Abstract

We report two male cases (4- and 5-years-old) of intrathecal methotrexate overdose. The two boys with acute lymphoblastic leukemia were to receive intrathecal injection of methotrexate. Instead of the prescribed 12 mg, they both received a dose of 120 mg. The initial cerebrospinal fluid samples showed methotrexate concentration of 2.24x10-2M in case 1 and 1.32x10-2M in case 2. The cases were successfully treated with cerebrospinal fluid (CSF) exchange and intravenous folinic acid rescue. The favorable outcome in our cases suggests that CSF exchange is safe and that folinic acid rescue may be adequate to prevent sequelae in patients subjected to intrathecal MTX overdoses up to 120 mg. We propose CSF exchange and intravenous folinic acid as the mainstay of treatment. In addition to the staff’s failure to check the drug label carefully, the marked resemblance of the two dose preparations of MTX may have been contributory.

Abstract

Splenic abscess is a very rare complication of non-typhoidal Salmonella infections. We report a case of splenic abscess caused by Salmonella enteritidis. The patient is a 63-year-old woman with diabetes mellitus and underwent splenectomy. This case suggests that the patients with comorbities are at increased risk for invasive infections in non-typhoidal Salmonella infections.
