4. Naïve Scientists and Conflict Analysis: Learning through Case Studies

Abstract

Much of our teaching about conflict relies on theoretical ideas and models that are delivered as finished products. This article explores the supposition that what students need is not already-formed theoretical ideas, but exposure to more real-world cases of conflict from which to build theory. The article presents an experiment in pedagogy: teaching a conflict resolution class using only case studies. This approach was expected to have two benefits: better understanding of the underlying concepts and a significant contribution to students' knowledge about the world. The case-only approach appears to be at least as good as the theory-based version of the class, with some significant side benefits beyond comprehension of the material.

Abstract

Background & Aims: Genome-wide association studies in primary biliary cholangitis (PBC) have failed to find X chromosome (chrX) variants associated with the disease. Here, we specifically explore the chrX contribution to PBC, a sexually dimorphic complex autoimmune disease. Methods: We performed a chrX-wide association study, including genotype data from 5 genome-wide association studies (from Italy, United Kingdom, Canada, China, and Japan; 5244 case patients and 11,875 control individuals). Results: Single-marker association analyses found approximately 100 loci displaying P < 5 × 10–4, with the most significant being a signal within the OTUD5 gene (rs3027490; P = 4.80 × 10–6; odds ratio [OR], 1.39; 95% confidence interval [CI], 1.028–1.88; Japanese cohort). Although the transethnic meta-analysis evidenced only a suggestive signal (rs2239452, mapping within the PIM2 gene; OR, 1.17; 95% CI, 1.09–1.26; P = 9.93 × 10–8), the population-specific meta-analysis showed a genome-wide significant locus in East Asian individuals pointing to the same region (rs7059064, mapping within the GRIPAP1 gene; P = 6.2 × 10–9; OR, 1.33; 95% CI, 1.21–1.46). Indeed, rs7059064 tags a unique linkage disequilibrium block including 7 genes: TIMM17B, PQBP1, PIM2, SLC35A2, OTUD5, KCND1, and GRIPAP1, as well as a superenhancer (GH0XJ048933 within OTUD5) targeting all these genes. GH0XJ048933 is also predicted to target FOXP3, the main T-regulatory cell lineage specification factor. Consistently, OTUD5 and FOXP3 RNA levels were up-regulated in PBC case patients (1.75- and 1.64-fold, respectively). Conclusions: This work represents the first comprehensive study, to our knowledge, of the chrX contribution to the genetics of an autoimmune liver disease and shows a novel PBC-related genome-wide significant locus.

Abstract

Background & Aims: Genome-wide association studies in primary biliary cholangitis (PBC) have failed to find X chromosome (chrX) variants associated with the disease. Here, we specifically explore the chrX contribution to PBC, a sexually dimorphic complex autoimmune disease. Methods: We performed a chrX-wide association study, including genotype data from 5 genome-wide association studies (from Italy, United Kingdom, Canada, China, and Japan; 5244 case patients and 11,875 control individuals). Results: Single-marker association analyses found approximately 100 loci displaying P < 5 × 10–4, with the most significant being a signal within the OTUD5 gene (rs3027490; P = 4.80 × 10–6; odds ratio [OR], 1.39; 95% confidence interval [CI], 1.028–1.88; Japanese cohort). Although the transethnic meta-analysis evidenced only a suggestive signal (rs2239452, mapping within the PIM2 gene; OR, 1.17; 95% CI, 1.09–1.26; P = 9.93 × 10–8), the population-specific meta-analysis showed a genome-wide significant locus in East Asian individuals pointing to the same region (rs7059064, mapping within the GRIPAP1 gene; P = 6.2 × 10–9; OR, 1.33; 95% CI, 1.21–1.46). Indeed, rs7059064 tags a unique linkage disequilibrium block including 7 genes: TIMM17B, PQBP1, PIM2, SLC35A2, OTUD5, KCND1, and GRIPAP1, as well as a superenhancer (GH0XJ048933 within OTUD5) targeting all these genes. GH0XJ048933 is also predicted to target FOXP3, the main T-regulatory cell lineage specification factor. Consistently, OTUD5 and FOXP3 RNA levels were up-regulated in PBC case patients (1.75- and 1.64-fold, respectively). Conclusions: This work represents the first comprehensive study, to our knowledge, of the chrX contribution to the genetics of an autoimmune liver disease and shows a novel PBC-related genome-wide significant locus.

Abstract

Turkey with one of the fastest growing economies in the world but a per capita income only about one seventh of that for the industrialised world has allocated approximately 5 per cent of her gross domestic product to defence since 1952. There is evidence that the level and form of military expenditure has been determined not only by internal and external security factors but also by the ideological function of the military which is closely related to its integration into the sphere of production. There is no evidence of military expenditure having been used as a tool of economic policy to control inflation or unemployment. Turkish military expenditure also needs to be understood in relation to U.S. foreign policy, and in particular through the conditions attached to the flows of military and economic aid. Militarism has been instrumental in shaping the form of industria1isation in Turkey and helped maintain the free umregulated conditions under which foreign capital could operate. One of the consequences of the Turkish path to development has been to create a long term dependency on imports of capital goods and raw materials which ultimately constrained growth in the mid-1970s. Arms production in Turkey cannot be a vehicle for industrialisation since domestic linkages would be limited and one form of dependency would be replaced by another. The links between military expenditure and economic growth are theorised in terms of resource mobilisation and resource diversion which are then estimated by two-stage least squares in a series of equations in which the rate of growth is treated as a function of both exogenous and endogenous variables in a dynamic simultaneous system. The results indicate that the impact of military expenditure is transmitted to the economic structure through both direct and indirect channels and that over the period 1952-76 increases in the military burden have been associated with a lower rate of economic growth.

Apesar de vários estudos investigarem a influência de componentes ambientais na longevidade, o papel de variantes genéticas nesta característica ainda não está bem esclarecido. Genes que participam do metabolismo de aminoácidos e genes que atuam em processos inflamatórios, como os genes MTHFD1L e SERPINA3, respectivamente, poderiam influenciar nas variações de tempo de vida de indivíduos de uma população. Assim, este estudo teve como objetivo avaliar se existe associação dos polimorfismos de MTHFD1L (rs11754661) e SERPINA3 (rs4934) com a longevidade humana em indivíduos atendidos em um hospital de referência no atendimento de geriatria da Grande Vitória-ES, Brasil. Foram selecionados 436 participantes, divididos em grupos de longevos (acima de 85 anos) e idosos com idade próxima a expectativa de vida da população (entre 70 e 75 anos). A genotipagem foi realizada por PCR em tempo real e os polimorfismos identificados estavam em equilíbrio de Hardy-Weinberg. As análises estatísticas foram realizadas por meio dos testes de Odds Ratio (OR) com Intervalo de Confiança de 95%, χ² e Fisher, com p-value ≤0,05. Não foi observada associação do polimorfismo rs4934 do gene SERPINA3 com a longevidade. Todavia, o alelo G do polimorfismo rs11754661 do gene MTHFD1L se comportou como um alelo de risco para a longevidade humana na população analisada (p=0,027; OR=1,976; IC 95% variando de 1,080 a 3,616), bem como o genótipo GG (p=0,030; OR=2,006; IC 95% variando de 1,070-3,761), enquanto que o genótipo AG atuou como fator de proteção (p=0,041; OR=0,518; IC 95% variando de 0,275 a 0,974). Os resultados sugerem que o polimorfismo rs11754661 do gene MTHFD1L está associado com a longevidade humana na amostra de indivíduos da Grande Vitória-ES, Brasil e que o polimorfismo rs4934 do gene SERPINA3 não está associado com longevidade na amostra avaliada.

26. Quantifying links between instream woody habitat and freshwater fish species in south-eastern Australia to inform waterway restoration

Abstract

Removal of instream woody habitat (IWH) is one factor attributed to declines in fish populations worldwide. Restoration of IWH to help fish populations recover is now common; however, quantitative predictions about the outcomes of these interventions is rare. As such, quantitative links between IWH and fish abundance is of interest to managers to inform conservation and restoration activities. Links between instream habitat attributes, especially IWH, and selected fish species of recreational, cultural, and ecological significance were explored at 335 sites spanning eight streams across south‐eastern Australia. Data were collected on fish abundance and length, IWH density and a range of other habitat attributes at a scale that incorporated at least one of each of the major mesohabitat types (functional river elements). The data were analysed using Bayesian hierarchical generalized linear mixed models to examine fish habitat associations and used to make quantitative predictions of responses to future restoration. Strong positive relationships were found between fish abundance and IWH density and the strength of this relationship varied between species and waterways. Murray cod (Maccullochella peelii), a species commonly targeted by IWH interventions, displayed the strongest association with IWH density. River blackfish (Gadopsis marmoratus) also showed a significant relationship with IWH, but this effect was waterway specific. Fish length was only related to IWH for river blackfish. These results may reflect differences in the life histories of these two species. We suggest that differences in habitat association through ontogeny may be more relevant at smaller spatial scales. The results generated in this study can be used to guide waterway restoration and develop quantitative predictions about how fish might respond to IWH interventions across south‐eastern Australia. Abstract

The biochemical response to ursodeoxycholic acid (UDCA)—so-called “treatment response”—strongly predicts long-term outcome in primary biliary cholangitis (PBC). Several long-term prognostic models based solely on the treatment response have been developed that are widely used to risk stratify PBC patients and guide their management. However, they do not take other prognostic variables into account, such as the stage of the liver disease. We sought to improve existing long-term prognostic models of PBC using data from the UK-PBC Research Cohort. We performed Cox's proportional hazards regression analysis of diverse explanatory variables in a derivation cohort of 1,916 UDCA-treated participants. We used nonautomatic backward selection to derive the best-fitting Cox model, from which we derived a multivariable fractional polynomial model. We combined linear predictors and baseline survivor functions in equations to score the risk of a liver transplant or liver-related death occurring within 5, 10, or 15 years. We validated these risk scores in an independent cohort of 1,249 UDCA-treated participants. The best-fitting model consisted of the baseline albumin and platelet count, as well as the bilirubin, transaminases, and alkaline phosphatase, after 12 months of UDCA. In the validation cohort, the 5-, 10-, and 15-year risk scores were highly accurate (areas under the curve: >0.90). Conclusions: The prognosis of PBC patients can be accurately evaluated using the UK-PBC risk scores. They may be used to identify high-risk patients for closer monitoring and second-line therapies, as well as low-risk patients who could potentially be followed up in primary care. Aims: To estimate the impact of existing high-coverage needle and syringe provision (HCNSP, defined as obtaining more than one sterile needle and syringe per injection reported) and opioid substitution therapy (OST) on hepatitis C virus (HCV) transmission among people who inject drugs (PWID) in three UK settings and to determine required scale-up of interventions, including HCV treatment, needed to reach the World Health Organization (WHO) target of reducing HCV incidence by 90% by 2030. Design: HCV transmission modelling using UK empirical estimates for effect of OST and/or HCNSP on individual risk of HCV acquisition. Setting and participants: Three UK cities with varying chronic HCV prevalence (Bristol 45%, Dundee 26%, Walsall 19%), OST (72–81%) and HCNSP coverage (28–56%). Measurements: Relative change in new HCV infections throughout 2016–30 if current interventions were stopped. Scale-up of HCNSP, OST and HCV treatment required to achieve the WHO elimination target. Findings: Removing HCNSP or OST would increase the number of new HCV infections throughout 2016 to 2030 by 23–64 and 92–483%, respectively. Conversely, scaling-up these interventions to 80% coverage could achieve a 29 or 49% reduction in Bristol and Walsall, respectively, whereas Dundee may achieve a 90% decrease in incidence with current levels of intervention because of existing high levels of HCV treatment (47–58 treatments per 1000 PWID). If OST and HCNSP are scaled-up, Walsall and Bristol can achieve the same impact by treating 14 or 40 per 1000 PWID annually, respectively (currently two and nine treatments per 1000 PWID), while 18 and 43 treatments per 1000 PWID would be required if OST and HCNSP are not scaled-up. Abstract

Background: Treatment guidelines recommend a stepwise approach to primary biliary cholangitis: all patients begin treatment with ursodeoxycholic acid (UDCA) monotherapy and those with an inadequate biochemical response after 12 months are subsequently considered for second-line therapies. However, as a result, patients at the highest risk can wait the longest for effective treatment. We determined whether UDCA response can be accurately predicted using pretreatment clinical parameters. Methods: We did logistic regression analysis of pretreatment variables in a discovery cohort of patients in the UK with primary biliary cholangitis to derive the best-fitting model of UDCA response, defined as alkaline phosphatase less than 1·67 times the upper limit of normal (ULN), measured after 12 months of treatment with UDCA. We validated the model in an external cohort of patients with primary biliary cholangitis and treated with UDCA in Italy. Additionally, we assessed correlations between model predictions and key histological features, such as biliary injury and fibrosis, on liver biopsy samples. Findings: 2703 participants diagnosed with primary biliary cholangitis between Jan 1, 1998, and May 31, 2015, were included in the UK-PBC cohort for derivation of the model. The following pretreatment parameters were associated with lower probability of UDCA response: higher alkaline phosphatase concentration (p<0·0001), higher total bilirubin concentration (p=0·0003), lower aminotransferase concentration (p=0·0012), younger age (p<0·0001), longer interval from diagnosis to the start of UDCA treatment (treatment time lag, p<0·0001), and worsening of alkaline phosphatase concentration from diagnosis (p<0·0001).
