The aim of this study was to compare nursing students’ clinical competence using the portfolio method with conventional education and evaluation methods in clinical education.Methods: In this pretest–posttest quasiexperimental crossover study, all six-semester undergraduate nursing students of Ahvaz Jundishapur University of Medical Sciences were classified into four groups. They were selected based on the use of portfolio and conventional methods during their apprenticeship. Then, they were educated using conventional and portfolio methods. Their clinical competence was measured by a self-appraisal questionnaire. Data were statistically analyzed with independent and paired t-tests.Results: There were significant differences between average scores of clinical competence in all three domains before and after study in portfolio-based education method (P=0.0001). Furthermore, the average difference in scores before and after internship between the two educational groups in all three domains — cognitive (P=0.002), affective (P=0.0001), and psychomotor — (P=0001) was significant. Similarly, there was a significant difference in total average scores of clinical competence between the two methods (P=0.0001).Conclusion: The results showed the effect of educational method on increasing the clinical competence of nursing students in a clinical unit. This new method can be used as a combination of education, evaluation and student-centered methods.Keywords: clinical competence, education, nursing student, portfolio

7. CinfuMax: An Influence Maximization-Based Model for Predicting Cancer Driver Genes in Gene Regulatory Networks.

Abstract

Background and Objectives: The identification of driver genes, which initiate cancer in cells, holds immense significance within the field of oncology. Discovering these genes is crucial for identifying markers that can indicate specific conditions or diseases, as well as for developing novel systemic and molecular treatment approaches for them. Several computational methods have been developed to identify the genes responsible for cancer based on genomic data. However, many of these methods find key mutations in genomic data to predict which genes are responsible for cancer. These methods rely on mutation and genomic data, but they often exhibit a high rate of false positives in the results. In this study, we propose an influence maximization-based approach, called CinfuMax, which can predict cancer-associated genes without relying on mutation information. Methods: In this method, the concept of influence maximization and the independent cascade model is employed. Firstly, the gene regulatory network for breast, lung and colon cancers was constructed using regulatory interactions and gene expression data. Next, we implemented an independent cascade diffusion algorithm on the networks to calculate the coverage of each gene. Ultimately, genes with the highest coverage were identified and classified as drivers. Results: The proposed method's results were compared to those of 19 other computational and network-based methods, utilizing the F-measure and the number of predicted driver genes as evaluation metricsThe results clearly indicate that the proposed method outperforms other methods. Furthermore, CinfuMax successfully identifies 18, 19, and 22 individual driver genes in breast, lung, and colon cancers, respectively, which were not previously identified by any other methods. Conclusion: Corrected: The results indicate that independent cascading methods for identifying driver genes outperform linear threshold methods. Driver genes were also categorized based on their influence speed, and the genes with the highest diffusion rate in each type of cancer have been identified. Abstract

To compare different antibiotic prophylaxis administered after preterm premature rupture of membranes to determine whether any were associated with differences in obstetric and/or neonatal outcomes and/or neurodevelopmental outcomes at 2 years of corrected age.Prospective, nationwide, population-based EPIPAGE-2 cohort study of preterm infants.France, 2011.We included 492 women with a singleton pregnancy and a diagnosis of preterm premature rupture of membranes at 24-31 weeks. Exclusion criteria were contraindication to expectant management or indication for antibiotic therapy other than preterm premature rupture of membranes. Antibiotic prophylaxis was categorised as amoxicillin (n = 345), macrolide (n = 30), third-generation cephalosporin (n = 45) or any combinations covering Streptococcus agalactiae and90% of Escherichia coli (n = 72), initiated within 24 hours after preterm premature rupture of membranes.Population-averaged robust Poisson models.Survival at discharge without severe neonatal morbidity, 2-year neurodevelopment.With amoxicillin, macrolide, third-generation cephalosporin and combinations, 78.5%, 83.9%, 93.6% and 86.0% of neonates were discharged alive without severe morbidity. The administration of third-generation cephalosporin or any E. coli-targeting combinations was associated with improved survival without severe morbidity (adjusted risk ratio 1.25 [95% confidence interval 1.08-1.45] and 1.10 [95 % confidence interval 1.01-1.20], respectively) compared with amoxicillin. We evidenced no increase in neonatal sepsis related to third-generation cephalosporin-resistant pathogen.In preterm premature rupture of membranes at 24-31 weeks, antibiotic prophylaxis based on third-generation cephalosporin may be associated with improved survival without severe neonatal morbidity when compared with amoxicillin, with no evidence of increase in neonatal sepsis related to third-generation cephalosporin-resistant pathogen.Antibiotic prophylaxis after PPROM at 24-31 weeks: 3rd-generation cephalosporins associated with improved neonatal outcomes.

Abstract

During bone remodeling, osteoblasts are known to deposit unmineralized collagenous tissue (osteoid), which mineralizes after some time lag. Some of the osteoblasts differentiate into osteocytes, forming a cell network within the lacunocanalicular network (LCN) of bone. To get more insight into the potential role of osteocytes in the mineralization process of osteoid, sites of bone formation were three-dimensionally imaged in nine forming human osteons using focused ion beam-scanning electron microscopy (FIB-SEM). In agreement with previous observations, the mineral concentration was found to gradually increase from the central Haversian canal towards preexisting mineralized bone. Most interestingly, a similar feature was discovered on a length scale more than 100-times smaller, whereby mineral concentration increased from the LCN, leaving around the canaliculi a zone virtually free of mineral, the size of which decreases with progressing mineralization. This suggests that the LCN controls mineral formation but not just by diffusion of mineralization precursors, which would lead to a continuous decrease of mineral concentration from the LCN. Our observation is, however, compatible with the codiffusion and reaction of precursors and inhibitors from the LCN into the bone matrix.

Abstract

Objective To assess the impact of being small for gestational age (SGA) on very preterm mortality and morbidity rates by using different birthweight percentile thresholds and whether these effects differ by the cause of the preterm birth. Study design The study included singletons and twins alive at onset of labor between 24 and 31 weeks of gestation without congenital anomalies from the Models of Organising Access to Intensive Care for very preterm births very preterm cohort in 10 European regions in 2003 (n = 4525). Outcomes were mortality, intraventricular hemorrhage grade III and IV, cystic periventricular leukomalacia, and bronchopulmonary dysplasia (BPD). Birthweight percentiles in 6 classes were analyzed by pregnancy complication. Results The mortality rate was higher for infants with birthweights Conclusions A 25th percentile cutoff point was a means of identifying infants at higher risk of death and a continuous measure better described risks of BPD. Lower birthweights were associated with poor outcomes regardless of pregnancy complications.

RESULTS: The mortality rate was higher for infants with birthweights <25th percentile when compared with the 50th to 74th percentile (adjusted odds ratio, 3.98 [95% CI, 2.79-5.67] for <10th; adjusted odds ratio, 2.15 [95% CI, 1.54-3.00] for 10th-24th). BPD declined continuously with increasing birthweight. There was no association for periventricular leukomalacia or intraventricular hemorrhage. Seventy-five percent of infants with birthweights <10th percentile were from pregnancies complicated by hypertension or indicated deliveries associated with growth restriction. However, stratifying for pregnancy complications yielded similar risk patterns. CONCLUSIONS: A 25th percentile cutoff point was a means of identifying infants at higher risk of death and a continuous measure better described risks of BPD. Lower birthweights were associated with poor outcomes regardless of pregnancy complications.

41. Inclusive Pedagogies: the development and delivery of Australian Indigenous curricula in higher education

Abstract

The construction of Indigenous knowledge within a western framework has been important in translating Indigenous issues and knowledge to mainstream Australia via educational systems. However, the production of Indigenous curricula is often essentialised and framed within binary locations, reliving traditional processes of assimilation and denying cultural identity and the diversity of student experience. This is because dominant educational frameworks intrude, through representation, reproduction and recontextualisation, Indigenous knowledge and identity. As a consequence, the development of genuine, alternative Indigenous curricula and pedagogy are inhibited. This paper explores the important role of inclusive curricula in supporting alternative processes of knowledge production and pluralistic approaches where the emphasis is on enhancement of active learning through collaboration and consultation. Inclusivity in the classroom recognises the diverse needs of learners, offers a range of teaching, learning and assessment approaches, and incorporates Indigenous knowledge systems through authentic learning experience that draws on Indigenous and non-Indigenous voices and knowledge frameworks. The paper also highlights the need to refigure constructions of difference and identity so as to provide meaningful mapping to support the development of content, assessment, methodology and delivery to integrate western and Indigenous frameworks within higher education curricula. This approach presents the opportunity to rethink pedagogic practices, to present diverse perspectives and contexts so as not to restrict or hinder the development of Indigenous curricula and pedagogy.
