Abstract Background The six-minute walk test (6MWT) is widely used to assess functional capacity and prognosis in patients with idiopathic pulmonary fibrosis (IPF). However, studies on oxygen saturation recovery after the 6MWT in patients with IPF are rare. In our study, we investigated the relationship between oxygen saturation recovery time and dyspnea, fatigue, quality of life, prognostic markers and pulmonary hypertension (PH). Methods In this cross-sectional study, IPF patients diagnosed according to current guidelines and followed up in our Interstitial Lung Disease Outpatient Clinic between 2021 and 2022 were included. Demographics, data from spirometry, diffusion capacity measurement, arterial blood gas analysis, transthoracic echocardiography and the 6MWT were recorded. The oxygen saturation recovery time, distance saturation product (DSP), gender-age-physiology (GAP) index and composite physiological index (CPI) scores were calculated. Dyspnea severity was assessed by the modified Medical Research Council (mMRC) and Dyspnoea-12 (D-12) scales, fatigue severity by the Multidimensional Fatigue Inventory (MFI-20) and quality of life by the St George’s Respiratory Questionnaire (SGRQ). Results Fifty IPF patients (34 men, 16 women, age: 66.8 ± 7.3 years) were included in the study. The mean FVC was 77.8 ± 19.3%, the DLCO was 52.9 ± 17.1%, the 6-minute walk distance (6MWD) was 385.7 ± 90.6 m, the GAP index was 3.5 ± 1.5, and the CPI was 43.7 ± 14.1. Objective: To evaluate the frequency of sleep apnea in solvent exposed workers and to find out the relationship between exposure level and sleep apnea. Materials and Methods: The study group included street sign painters who were exposed to solvent daily. Solvent exposure level was categorized according to Heiskel’s classification. Neurobehavioral symptoms were assessed using Questionnaire 16. Polysomnographic data and Questionnaire 16 score of the workers were compared with the healthy controls. Results: Sixteen male street sign painters and 16 healthy male as a controls were included in the study. All workers had daily exposure to solvent (exposure time: 7.9±6.8 years). Sleep apnea frequency in workers was slightly higher than in the controls but there was no statistically significant difference (respectively; 44% vs 31%, p=0.716). Apnea-hypopnea index of the workers with sleep apnea was slightly higher than the controls with sleep apnea, but there was no statistical significance (respectively; 25.7±21.03/hours vs 14.7±7.86/hours, p=0.169). In sleep apnea diagnosed workers who exposed to solvent, two of them had central sleep apnea (28.5%), three of them had obstructive sleep apnea, and two of them had rapid eye movement-related sleep apnea. Solvent exposure time of workers with sleep apnea was significantly higher than those with no sleep apnea (respectively; 12.9±7.0 vs 4.2±3.6 years p=0.008). The mean Questionnaire 16 score of the workers was higher than the controls (respectively; 4.5±3.24 vs 2.6±1.6, p=0.047). Questionnaire 16 score did not correlate with solvent exposure time and polysomnographic parameters. Conclusion: Most of the workers with daily solvent exposure had sleep apnea. Workers with sleep apnea had significantly higher solvent exposure time than the workers without sleep apnea. Therefore, workers who are exposed to solvent for many years should be questioned for sleep apnea.

Objective: In Obesity Hypoventilation syndrome (OHS), pulmonary hypertension (PH) is an important morbidity and mortality reason compared to pure Obstructive Sleep Apnea syndrome (OSAS). However, few studies are available in the literature about this subject. For this reason, we evaluated OHS and pure OSAS cases in our study from the point of right heart-related echocardiographic parameters and PH. Materials and Methods: Obese [body mass index (BMI) >30 kg/m2] cases diagnosed by polysomnography were included in the study. The subjects were divided to two groups as OHS and pure OSAS. OHS criteria were defined as obesity (BMI >30 kg/m2) and for another reason unexplained (neuromuscular, chest wall, restrictive or obstructive pulmonary diseases) daytime hypercapnia (PaCO2 >45 mmHg). Patients with severe hypothyroidism, renal and heart insufficiency, cardiac drug anamnesis were not included in the study. Transthoracic echocardiography was performed to all cases and those with left-heart pathology were excluded from the study. Systolic pulmonary artery pressure (PABs) >35 mmHg was accepted as PH. Results: Of the 115 cases studied (mean age: 49.3±10.6/year, female/male: 53/62, BMI: 40.5±6.1 kg/m2), 70 were pure OSAS and 45 were OHS. In the OHS group, PABs, right ventricular diameter and pulmonary velocities were significantly higher than the pure OSAS group (p=0.002, p=0.015, p=0.012, respectively). Background: As obesity increases, the frequency of obstructive sleep apnea and obesity hypoventilation syndrome increases also. However, obesity hypoventilation syndrome frequency is not known, as capnography and arterial blood gas analysis are not routinely performed in sleep laboratories. Aims: To investigate the frequency and predictors of obesity hypoventilation syndrome in obese subjects. Study Design: Retrospective clinical study Methods: Obese subjects who had arterial blood gas analysis admitted to the sleep laboratory and polysomnography were retrospectively analyzed. Subjects with restrictive (except obesity) and obstructive pulmonary pathologies were excluded.
