The presence of endometrial stroma and/or glandular epithelium in organs other than the uterus is called endometriosis. Urinary system endometriosis is observed in approximately 1% of women with endometriosis and is most commonly seen in the bladder. A 27-year-old female patient was admitted to the urology clinic with cyclic complaints of painful and frequent urination. Ultrasonography revealed a lesion extending to the lumen of approximately 15x10 mm in the left side wall of the bladder. On cystoscopy, a suspicious area with irregular borders was observed, approximately 15 mm, at the left-posterior lateral wall of the bladder, and transurethral resection was performed. The pathological result of the patient was reported as bladder endometriosis. Although rare, bladder endometriosis should be considered in the differential diagnosis, especially in women of reproductive age with cyclic urological complaints. [ABSTRACT FROM AUTHOR]

Objective: In this study, it was aimed to show the changes in the sexual function of the male patients who underwent ureterorenoscopic (URS) and who had JJ stent (DJS) because of ureteral calculi and their spouses before and after the procedure. Methods: The male patients and their spouses who were admitted to the Urology Outpatient Clinic of Eskişehir State Hospital between May and August 2018 due to unilateral ureteral stones were included in the study. In order to determine sexual life, all of the male patients filled the International Index of Erectile Function (IIEF) form while their spouses filled the Female Sexual Function Index (FSFI) form. The patients with DJS and their spouses completed the forms just before the removal of DJS while the patients without DJS and their spouses filled the same forms 4 weeks after the procedure. The patients who were applied DJS after the operation of URS were defined as the study group, the patients who were not applied DJS after the operation of URS were defined as the control group. Results: Thirty one (34.4%) of 90 males who participated in the study were included in the study group. Fifty nine males (65.6%) without DJS included in the control group. There was no statistically significant difference between preoperational and post-operational scores of the male patients and their spouses on the scales of IIEF and FSFI, respectively (p>0.05). There was statistically significant decreases in the postoperational total and subscale scores of the male patients on the IIEF scale and the female patients on the FSFI scale compared to their total and subscale pre-operational scores (p<0.001). Conclusion: The male patients should be informed about their sexual life before endo-urologic procedures while their spouses should be informed about their sexual life after the procedure. DJSs should not be inserted except unavoidable circumstances; if they are inserted, they should be removed as soon as possible for minimizing their effects on the sexual lives of couples. Objective: To evaluate the effect of glanular and urethral catheter fixation to the abdominal skin on wound dehiscence. Material and methods: After a standard tubularized incised plate urethroplasty (TIPU) for hypospadias repair, 128 patients were divided into two groups based on glanular and urethral catheter fixation to the abdominal skin as follows: no glanular and urethral catheter fixation (Group A) and glanular and urethral catheter fixation (Group B). Groups A and B included 61 and 67 patients, respectively. Results: No significant difference was determined between the groups with respect to age, meatal localization, and length of hospital stay. Wound dehiscence was noted in 13.1% and 2.9% patients in Groups A and B, respectively, after surgery (p=0.029, odds ratio=4.9). Patients in Group B had no excessive analgesic usage and unpleasant scarring due to the glans suture. Conclusion: Glanular and urethral catheter fixation to the lower abdominal skin considerably reduced wound dehiscence after hypospadias repair. [ABSTRACT FROM AUTHOR]

Objective: To investigate and compare the effects on pain of intraurethral 2% lidocaine gel and plain lubricating gel in male patients underwent flexible cystoscopy. Material and methods: The data of 220 male patients who underwent flexible cystoscopy between March 2012 and August 2014 were retrospectively analized. The patients were divided into 2 groups according to using intraurethral gel types. Group I included 120 patients who were underwent flexible cystoscopy with 2% lidocaine gel and Group II was consisted from 100 patients who underwent flexible cystoscopy with plain lubricating gel. The groups were compared according to postprocedure data including pain score, procedure time and age of patients. Results: The mean age of the patients in Group I was 50.02±11.87 years while that in Group II was 52.03±13.37 years (p=0.492). The mean procedure times were 6.02±0.787 and 6.28±0.689 minutes in Group I and Group II respectively (p=0.061). Pain perception scores were not statistically different between the groups (Group I: 3.10±0.980, Group II: 3.34±0.789, p=0.132). Conclusion: Use of intraurethral 2% lidocaine gel has no advantage over plain lubricating gel in regard to pain control during flexible cystoscopy in men. [ABSTRACT FROM AUTHOR]

Objective: Penile torsion is a counterclockwise rotational anomaly of the penile shaft or glans. We aimed to evaluate the outcomes of penile degloving and dorsal dartos flap rotation used for the repair of isolated penil torsion. Material and methods: During evaluation of the patients admitted to our polyclinic for circumcision between January 2013-December 2014, 5 cases of isolated penile torsion were determined. Following the circumcision procedure performed with bipolar cautery, patients undergoing penile degloving were checked whether penile torsion was relieved or not. In case of insufficient improvement, torsion was corrected with application of dorsal dartos flap. Penile torsion was corrected with dartos flap in 2, and penile degloving in 3 cases. Results: The mean age of the patients was 5.6 years (4-7), and the mean operative time 12 minutes (7-20), respectively. The mean operation time was 17.5 (15-20) minutes and 8.3 (7-10) minutes in the dorsal dartos flap and penile degloving groups, respectively The preoperative mean degree of penile torsion was 50° (30°-70°). The mean degree of torsion was 65° and 40° in the dorsal dartos and penile degloving groups, respectively. During the postoperative follow up, 1 case of residual torsion (<10°) was observed in the dorsal dartos flap group. Residual torsion was not observed in other patients. Conclusion: Exploration for isolated cases of penile torsion during the circumcision procedure should not be overlooked. Successful results can be obtained with penile degloving and dartos flap rotation in cases with low and moderate torsion. [ABSTRACT FROM AUTHOR]

Objective: Percutaneous nephrolithotomy (PNL) is the firstline treatment for large and complex renal calculi. In this study, we aimed to compare the efficacy and safety of the totally tubeless PNL versus the standard PNL. Methods: Between January 2012 and July 2013, 73 selected patients were treated standard or totally tubeless PNL, nephrostomy tube and ureteral stent was not placed at the end of the operation in 35 (39.7%) (Group 1) of them and 38 (43.1%) (Group 2) patient underwent standard PNL. Stone disintegration was performed with a pneumatic lithotripter. We retrospectively compared patient and stone characteristics, operation time, duration of hospitalization, analgesia requirements, stone-free rate, operative findings, blood loss, and perioperative complications between two groups. Results: The mean operation time, excluding the preparation course, was 38,3±15,4 minutes vs 51,2±12,9 minutes and mean fluoroscopy time was 4,5±2,4 minutes vs 4,8±2,1 minutes, respectively. No significant intraoperative complication or indication additional access or second-look PNL due to residual stones was observed. In both groups none of the patients demonstrated a urinoma, hemorrhage or residual stones in postoperative ultrasonography and plain radiograph. Blood transfusion was needed only in a patient vs two patients for group 1 and 2, respectively. There were no significant differences in preoperative patient characteristics, postoperative complications between two groups, but the totally tubeless PNL group showed a shorter hospitalization and a lesser analgesic requirement compared with other group. Conclusion: Absence of the nephrostomy tube and ureteral stent may help in keeping the patient comfortable after the operation and reduction in the analgesia requirement and length of hospital stay. Objective: Percutaneous nephrolithotomy (PNL) is the first-line treatment for large and complex renal calculi. The aim of this study is to evaluate the outcomes of totally tubeless PNL operations. Methods: Between October 2009 and August 2013, outcomes of 68 (23.4%) patients treated with totally tubeless PNL method at our clinic were analyzed. Stone disintegration was performed with a pneumatic lithotriptor. Operative findings, duration of hospital stay, analgesia requirement and success rates were retrospectively analyzed performed with totally tubeless PNL. Results: Mean age of patients treated with the totally tubeless PNL was 43.4 ± 13.6 (18-69) years and there were 42 (61,7%) men and 26 (38,2%) women. Mean operation time, excluding the preparation course, was 39.5±14.8 (25-90) minutes and mean fluoroscopy time was 6.04±4.15 (1-12) minutes. None of the patients demonstrated a urinoma, hemorrhage or residual stones in postoperative ultrasonography and plain radiograph. Two patients had undergone ureterorenoscopy for prolonged leakage of urine and treated by retrograde ureteral double-J stent. Blood transfusion was needed only in two patients. The mean hospitalization time was 1.27±0.36 (1-2) days. Conclusion: We think that totally tubeless PNL may be used in properly suitable cases with mil-moderate stone load, intact counterside kidney and without any significant complication and need for second-look. Background: Our aim was to detect the role of radiological abdominal fat parameters by tomography and stone density by plain X-ray on extracorporeal Shock Wave Lithotripsy (SWL) stone-free rate., Methods: The patients who had undergone SWL for a single opaque renal stone < 2 cm in diameter and proximal ureteric stone < 1 cm in diameter were collected retrospectively. The characteristics of patients and stones were recorded. The stone attitude, HU, abdominal fat parameters, paraperirenal fat area, perirenal infiltration and severity of hydronephrosis with pre-treatment Non- Contrast Computed Tomography (NCCT) and stone density with radiography were evaluated by a radiologist. Four weeks after the last SWL; all patients were evaluated by plain X-ray and categorized as Stone Free (SF) and Residual Fragment (RF) group., Results: 51 patients with renal stones and 88 patients with proximal ureteral stones were included in the study. 24 (47%) and 63 (71%) patients were classified as SFfor renal and ureteral stones respectively. Only stone size was an independent predictor for stone-free rates after SWL for renal and proximal ureteral stones on multivariate analysis. The Receiver Operating Characteristic (ROC) curves for renal calculi revealed that creatinine level, stone size, stone attitude, Houns-Field Unit (HU) and Skin-to-Stone Distance (SSD) were the predictive factors for the SWL outcome (p< 0.05). The ROC curve for ureteral calculi demonstrated that HU, stone size and attitude were the predictive factors (p< 0.05)., Conclusion: All abdominal fat parameters and the stone density were not related to SWL failure.
