Introduction\ud Medical imaging in pregnancy (antenatal imaging) is routine. However, the effect of seeing fetal images on the parent-fetal relationship is not well understood, particularly for fathers or partners, or when using advanced imaging technologies. This review aimed to explore how parent experience and prenatal attachment is impacted by antenatal imaging.\ud \ud Method\ud Comprehensive searches of 10 electronic databases, with grey literature and hand searches, were performed between September 2020-April 2021 using a systematic search strategy. Wildcard and Boolean operators were used in combination with key words (e.g. fetal, imaging) to generate search queries. Inclusion criteria were English language primary research studies published since 2000, describing or reporting measures of attachment after antenatal imaging in expectant parents. Studies were evaluated against inclusion criteria by two reviewers and critically appraised. The Pillar Integration Process facilitated integrative synthesis.\ud \ud Findings\ud Twenty-three (13 quantitative, 10 qualitative/descriptive) studies were included (2462 participants, including 186 fathers). Six pillar themes were identified: 1) the scan experience begins before the scan appointment; 2) the scan as a pregnancy ritual; 3) feeling actively involved in the scan; 4) parents’ priorities for knowledge and understanding of the scan change during pregnancy; 5) the importance of the parent-sonographer partnership during scanning; and 6) scans help to create a social identity for the unborn baby.\ud \ud Conclusion\ud Antenatal imaging can enhance prenatal attachment. Parents value working collaboratively with sonographers to be actively involved in the experience. Sonographers can help facilitate attachment by delivering parent-centred care tailored to parents’ emotional and knowledge needs.

Introduction: The COVID-19 pandemic placed additional demands and stressors on UK obstetric sonographers, who were required to balance parent safety and service quality, alongside staff safety. Increased pressure can negatively impact a healthcare worker’s well-being and the provision of person-centred care. The aim of this study was to explore obstetric sonographers’ experiences of performing pregnancy ultrasound scans during the pandemic and to assess the impact on burnout, role satisfaction and clinical practice. Methods: An online, anonymous cross-sectional survey was created to capture sonographers’ experience alongside using the Oldenburg Burnout Inventory to evaluate burnout and Clinical Outcomes in Routine Evaluation 10 (CORE-10) to measure psychological distress. Results: Responses were received from 138 sonographers. Of those completing the Oldenburg Burnout Inventory ( n = 89), 92.1% and 91.0% met the burnout thresholds for exhaustion and disengagement, respectively. Sonographers with a higher burnout score also perceived that COVID-19 had a greater, negative impact on their practice ( p < 0.05). The mean CORE-10 score of 14.39 (standard deviation = 7.99) suggests mild psychological distress among respondents. A significant decrease in role satisfaction was reported from before to during the pandemic ( p < 0.001), which was associated with higher scores for burnout and psychological distress ( p < 0.001). Change in role satisfaction was correlated with sonographers’ perception of safety while scanning during the pandemic ( R2 = 0.148, p < 0.001). Sixty-five sonographers (73.9%) reported they were considering leaving the profession, changing their area of practice or working hours within the next 5 years. Conclusion: Job and context-specific interventions are required to mitigate burnout and its consequences on the workforce and service provision beyond the pandemic.

Background Young mothers face mental health challenges during and after pregnancy including increased rates of depression compared to older mothers. While the prevention of teenage pregnancy in countries such as the United States and the United Kingdom has been a focus for policy and research in recent decades, the need to understand young women’s own experiences has been highlighted. The aim of this meta-ethnography was to examine young women’s perceptions of their mental health and wellbeing during and after pregnancy to provide new understandings of those experiences. Methods A systematic review and meta-ethnographic synthesis of qualitative research was conducted. Seven databases were systematically searched and forward and backward searching conducted. Papers were included if they were from Organisation for Economic Co-operation and Development countries and explored mental health and wellbeing experiences of young mothers (age under 20 in pregnancy; under 25 at time of research) as a primary research question – or where evidence about mental health and wellbeing from participants was foregrounded. Nineteen papers were identified and the Critical Appraisal Skills Programme checklist for qualitative research used to appraise the evidence. Following the seven-step process of meta-ethnography, key constructs were examined within each study and then translated into one another. Results Seven translated themes were identified forming a new line of argument wherein mental health and wellbeing was analysed as relating to individual bodily experiences; tied into past and present relationships; underpinned by economic insecurity and entangled with feelings of societal surveillance. There were ‘no straight lines’ in young women’s experiences, which were more complex than dominant narratives around overcoming adversity suggest. Conclusions The synthesis concludes that health and social care professionals need to reflect on the operation of power and stigma in young women’s lives and its impact on wellbeing. It adds to understanding of young women’s mental health and wellbeing during and after pregnancy as located in physical and structural factors rather than individual capacities alone.

OBJECTIVE: During the COVID-19 pandemic fathers in the UK were excluded from many aspects of maternity care to reduce escalating transmission rates. This study explores the experiences of fathers who had a baby during the pandemic to understand what effect these maternity restrictions had on them and their relationship to the baby. \ud \ud DESIGN: A qualitative interview study of the experiences of fathers whose baby was born during the pandemic-related UK maternity restrictions. \ud \ud PARTICIPANTS AND SETTING: Non-probability voluntary response sampling of 20 fathers: including 13 primiparous fathers and 7 multiparous fathers. Eligibility criteria were that fathers lived in the UK and had a baby born on or after the 23rd March 2020; the start of the most severe COVID-19 maternity restrictions. Participants were interviewed remotely via telephone using semi-structured interviews which were transcribed and analysed using thematic analysis. \ud \ud FINDINGS: Four themes, including ten sub-themes, were identified that described fathers' experiences of the maternity restrictions and the father-baby relationship. The themes were: (1) The impact on paternal experience: this theme describes a collective negative paternal maternity experience as a result of the restrictions. Notably, father exclusion produced feelings of isolation and a sense of loss, along with a disconnection from the pregnancy. (2) The impact on the father-baby relationship: this theme discusses the adverse consequence of the restrictions on initial father-baby bonding. (3) Observed impact on mothers: the observed detrimental impact that excluding fathers had on maternal mental health and well-being. Finally, (4) Fatherhood in the 'new normal': the change of daily living during the pandemic aided profound family relationship building, improving long-term father-baby bonding, compared to pre-pandemic conditions. \ud \ud KEY CONCLUSIONS: The findings provide evidence of undesirable consequences the pandemic-related UK maternity restrictions had on birth partners. With restrictions to maternity care implemented across the globe, these concerns may be applicable at an international scale. \ud \ud IMPLICATIONS FOR PRACTICE: This study adds to other contemporary literature on this subject and can inform discussion among maternity services of the importance of including fathers for improved parental well-being and initial infant bonding.

The City Infant Faces Database (CIFD; Webb et al., 2018) is a database of 154 infant emotional expressions for use in experimental studies of infant facial communication, facial expression recognition, and parental sensitivity. The CIFD was validated only in a small sample from the general public and student midwives and nurses in the UK. This study, therefore, aimed to validate it in a larger sample of Croatian students and parents of 1-12 months old infants. Three-hundred and fifty students (Study 1), 422 mothers and 106 fathers (Study 2) were presented with images of Caucasian infant faces. The students rated images from the CIFD and Tromsø Infant Faces. They also completed questionnaires measuring empathy, alexithymia, and perceiving and expressing emotions. The parents rated the valence of facial expressions of images from the CIFD. The results were consistent with the initial validation in both the students and parents’ sample, except that agreement for negative images was lower for Croatian parents than in the UK study. Compared to the UK study, students rated images as more intense, clear, genuine, and reported stronger internal emotion. Furthermore, there was no difference in accuracy between mothers and fathers or between first-time parents and experienced parents. The CIFD is, therefore, a promising tool for research and should be further validated in other countries, focusing on its predictive validity.
