Tracheoceles are rare, and usually, they have been described as incidental findings while evaluating patients for other problems. Our patient complained of a mass located in the right supraclavicular region that got larger on coughing and straining; otherwise, he was asymptomatic. His history did not reveal any predisposing factors. Computed tomography showed an air-filled 3×2.5×2 cm mass at the level of the T2–4 vertebrae. Surgical exploration showed an air-filled mass located between the common carotid artery and trachea, communicating with the tracheal lumen via a narrow tract attached to the posterior wall of the trachea. The mass was completely resected, and the defect in the posterior wall was repaired. A literature search revealed only one tracheocele case without any predisposing factors, and our case is a new one. It is different from other tracheocele reports considering the origin side, type, and level of the lesion.

Objective: Literature does not contain a standard procedure about anticoagulant therapy aimed at patients with atrial fibrillation and carotid artery stenosis following carotid endarterectomy. In this paper, we present our therapy modality at these patients in our clinic. Material and Method: The study includes 424 patients and 498 carotid endarterectomy operations performed by two surgeons with the same technique between June 2010 and December 2017. Fourty-five patients had chronic or paroxysmal atrial fibrillation. Therefore, the patients were receiving Coumadin and aspirin. The median age was 63.4±7.9. There were 27 female and 18 male patients. Thirty seven patients were hypertensive and 17 patients were diabetic. Nine patients underwent bilateral carotid endarterectomy operations. Thirty three patients were symptomatic. Eleven patients had coronary artery disease and 5 patients had cardiac valvular pathologies. Results: The whole carotid endarterectomy operations were performed under locoregional anesthesia. Early mortality occured in one patient because of hypertensive intracranial bleeding. The median follow up period was 68.4±19.2 months. One patient was lost due to aging and co-morbid factors and one patient was lost due to malignancy in late follow up period. Three patients required revisions for hematoma at incision region but an active bleeding focus could not be detected. There was no re-stenosis in any patient during follow-up. Conclusion: The large and multi-centered studies are needed for the anticoagulant therapy protocol for the patients with atrial fibrillation following carotid endarterectomy. We prefer combination of warfarin, providing INR value between 2-3, and 100 mg aspirin per day at our patients as therapy modality., Amaç: Karotis arter stenozu ve atriyal fibrilasyonun birarada görüldüğü hasta grubunda karotis endarterektomi sonrası antikoagülan tedavi protokolü ile ilgili standart bir prosedür bulunmamaktadır. Bu hasta grubunda kliniğimizde uyguladığımız tedavi stratejimizi sunmaktayız. Gereç ve Yöntem: Çalışma Haziran 2010 - Aralık 2017 arasında karotis endarterektomisi operasyonunu gerçekleştiren 2 cerrahın verilerinin toplandığı 424 hastayı ve 498 karotis endarterektomisi işlemini içermektedir. Bu hastaların 45’inde kronik veya paroksismal atriyal fibrilasyon mevcuttu. Hastalar warfarin ve aspirin tedavisi almaktaydı. Ortalama yaş 63,4±7,9 idi. Hastalardan 27’si kadın 18’i erkekti. 37 hastada hipertansiyon, 17 hastada diyabetes mellitus mevcuttu. Hastaların 9’una bilateral karotis arter stenozu nedeniyle cerrahi işlem uygulandı. 33 hasta semptomatikti. 11 hastada koroner arter hastalığı, 5 hastada kardiyak valvuler patoloji mevcuttu. Bulgular: Hastaların tamamında lokorejyonel anestezi ile karotis endarterektomisi yapıldı. Erken mortalite 1 hastada hipertansif intrakraniyal kanama sebebiyle gerçekleşti. Hastaları ortalama takip süresi 68,4±19,2 ay idi. Takiplerde 1 hastada komorbid faktörlere bağlı sebepler, 1 hastada malignite nedeniyle geç dönemde mortalite gerçekleşti. Hastaların 3’ünde insizyon böl gesinde hematom nedeniyle revizyon gerekti; fakat eksplorasyonda aktif kanama odağı tespit edilemedi. Hiçbir hastada takipleri sırasında restenoz saptanmadı. Tartışma: Atriyal fibrilasyonu olan hastalarda karotis endarterektomi sonrası antikoagülan tedavi protokolü açısından daha geniş ve çok merkezli çalışmalara gereksinim vardır. Biz hastalarımızda INR değerini 2-3 arasında tutacak şekilde warfarin tedavisi ile günlük 100 mg aspirin kombinasyonunu tedavi stratejisi olarak uygulamayı tercih ettik.

Reklamverenler ve reklamcıların en önemli hedefi hedef kitleyi ikna ederek satışları artırmaktır. Reklamcılar reklamlarda izleyiciye imgeler sunar ve izleyicilerin reklamlarda gördükleri imgeler gibi olmak istemelerini beklerler. Alımlama yöntemi medya metinlerinin insanlar tarafından nasıl anlamlandırıldığını incelemektedir. Alımlama çalışmaları, medya içeriklerini açımlamada izleyicinin pasif olmadığını ve bireylerin birbirlerinden farklı anlamlar çıkarabileceğini varsayar. Çalışma, televizyondaki erkek karakterlerin izleyiciye nasıl yansıtıldığı ve izleyiciler tarafından erkek karakterlerin nasıl alımlandığını incelemeyi amaçlamaktadır. Katılımcılara reklamlarda gördükleri erkek karakterin fiziksel, psikolojik durumları ve bu durumun ürünü satın alma noktasındaki etkisi sorulmuştur. Toplumun erkeklere uygun gördüğü özelliklerin reklamlarda yansıtıldığı ve toplumsal rollerin pekiştirildiği, reklamda yer alan erkek imgenin izleyiciye rol model olarak sunulduğu ve tektipleştirilmiş erkek algısı yaratıldığı sonucuna varılmıştır. Media plays a role in the construction and espousal of gender stereotypes. Television advertisements encourage the target audience to consume more and therefore increase the revenue volumes for both advertisers and their customers alike. Advertisers present their viewers certain images and hope to arouse in them the desire to become more and more like these images. The reception theory examines how spectators interpret media texts. Studies of the reception theory reveal that spectators take an active part in interpreting and explaining the meaning of a text and that individuals' interpretations differ from one another. This paper aims to examine the way male characters are pictured in television and how they are perceived by masses. The study has asked its respondents their perception of male characters appearing in television advertisements, in particular their physical and psychological qualities and the extent to which they influence the sale of a certain product. This research has concluded that characteristics attributed by society to males were also reflected in television advertisements, thereby reinforcing genders roles. It has also found that television ads present these images of men to the audience as role models, which, has the effect of creating a masculine prototype for the society. 90

Bu tez çalışmasında bir robotik akustik müzik cihazı olan ?ROMI ni için imitsayon kontrol değişkenlerinin otomatik yakınsama işlemine dayalı yeni bir doğaçlama algoritması sunulmaktadır. ROMI'nin kontrolü için geliştirmiş olduğumuz müzikal durum gösterimi, bir düet halinde çalan insan müzisyenleri benzeten bir özellik ayrıştırıcısıdır. ROMI'nin akıllı kontrol mimarisi aynı zamanda müzisyeni tanıma ve çalma performansını geliştirme kabiliyetine de sahiptir. Bu tez çalışmasında robotik cihaz ROMI'yi kontrol mimarisi, müzikal durum gösterimi ile sunuyor ve düet yapan müzisyenlerin benzetimi için kontrol değişkenlerinin otomatik yakınsaması üzerine odaklanıyoruz. In this thesis we introduce the robotic device ROMI together with its control architecture, the musical state representation and focus on parameter estimation for imitation of duo players by ROMI. ROMI is aimed at jointly playing two instruments that belong to two different classes and improvises while assisting others in an orchestral performance. A new improvisation algorithm based on parameter estimation process for the imitation control is introduced. This new improvisation algorithm adds the capability of velocity control and polyphonic music processing on top of existing improvisation algorithms. The musical state representation we have developed for controlling ROMI is a feature extractor for learning to imitate human players in a duet. ROMI?s intelligent control architecture also has the ability to provide sample owners? identification and performance training. 81
