Topics discussed include vital signs at admission to the hospital, mechanisms in the etiology of respiratory virus-associated neurological complications, and potential causes of hyponatremia, which is said to increase the disease severity in infants hospitalized with respiratory syncytial virus (RSV) infection.

Objective: Optimal care in the delivery room is important to decrease neonatal morbidity and mortality. We aimed to evaluate neonatal resuscitation practices in Turkish centers. Materials and Methods: A cross-sectional survey consisted of a 91-item questionnaire focused on delivery room practices in neonatal resuscitation and was sent to 50 Turkish centers. Hospitals with <2500 and those with ≥2500 births/year were compared. Results: In 2018, approximately 240 000 births occurred at participating hospitals with a median of 2630 births/year. Participating hospitals were able to provide nasal continuous-positive airway-pressure/high-flow nasal cannula, mechanical ventilation, high-frequency oscillatory ventilation, inhaled nitric oxide, and therapeutic hypothermia similarly. Antenatal counseling was routinely performed on parents at 56% of all centers. A resuscitation team was present at 72% of deliveries. Umbilical cord management for both term and preterm infants was similar between centers. The rate of delayed cord clamping was approximately 60% in term and late preterm infants. Thermal management for preterm infants (<32 weeks) was similar. Hospitals had appropriate equipment with similar rates of interventions and management, except continuous-positive-airway-pressure and positive-end-expiratory-pressure levels (cmH2O) used in preterm infants (P = .021, and P = .032). Ethical and educational aspects were also similar. Conclusions: This survey provided information on neonatal resuscitation practices in a sample of hospitals from all regions of Turkey and allowed us to see weaknesses in some fields. Although adherence to the guidelines was high among centers, further implementations are required in the areas of antenatal counseling, cord management, and circulation assessment in the delivery room. Livedo racemosa (LRac) refers to violaceous, red, or blue netlike mottling of the skin in an irregular pattern caused by circulatory abnormalities. Central venous catheters (CVC) are widely used in neonates who require intensive care to offer steady intravenous access. CVCs are covered with a hydrophilic polymer gel that reduces friction between the sheath and the vessel wall, thereby minimizing vascular spasm, irritation, and aneurysm formation. However, the introduction of foreign material into the vein carries a risk of embolization. Recently, LRac was reported as a cutaneous manifestation of hydrophilic polymer gel embolization (HPE) in adults. We present a case of LRac in a newborn that developed following CVC insertion and resolved spontaneously after the catheter was removed. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: to investigate the early neonatal outcomes of very-low-birth-weight (VLBW) infants discharged home from neonatal intensive care units (NICUs) in Turkey. Material and methods: a prospective cohort study was performed between April 1, 2016 and April 30, 2017. The study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results: data from 69 NICUs were obtained. The mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion: the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Therefore, monitoring standards of neonatal care and implementing quality improvement projects across the country are essential for improving neonatal outcomes in Turkish NICUs., Turkish Neonatal Society

Breastfeeding is the gold standard practice and it is recommended thatall infants should continue breastfeeding at the first 6 months. It isimportant to identify undesirable toxins and metabolites that may be present inthe human milk and pass to the baby. Studies about the content of mycotoxins inthe human milk are limited and show differences between countries. The aim ofthis study was to evaluate the human milk aflatoxin M1 (AFM1) levels, and alsoto evaluate potential relationship with maternal dietary habits. In this study 3-5 ml human milksamples has been received from mothers for AFM1 evaluation, and stored -20,until the analysis. All mothers fulfilled the questionnaire about theconsumption of some foods, which might be related with mycotoxin exposure.Aflatoxin M1 toxin has been tested with the HELICA Aflatoxin M1 Assay (solidphase competitive enzyme immunoassay) and Multi-residue Method Based on ChipTechnology (Randox, UK). Total daily intake for AFM1 has been calculated amongexclusively breast-fed infants. Ninety mothers agedbetween 17 to 41 years, who have 7 to 90 days old newborn babies, have beenenrolled. Median human milk AFM1 levels were 3.59 ng/L, ranged between 2.76 to5.99 ng/L. There are only 3 samples with ELISA, which are above 5 ng/L which wasdefined as threshold levels. All samples were negative with human milk AFM1levels with Multi-residue Method Based on Chip Technology. Among 63 breast-fedinfants, median total daily intake of AFM1 was 0.54 ng/kg (0.41-0.90 ng/kg/day)and were lower than previously defined threshold levels. Human milk AFM1 levelswere not associated with maternal dieatary habits (p>0.05). While our previous studies about humanmilk mycotoxin levels, showed higher human milk ochratoxine, zearalenone anddeoxynivalenol levels, human milk AFM1 levels were within the normal limitswith different detection tests. Variable findings on human milk aflatoxinlevels indicate geographical differences in dietary aflatoxin exposure ofnursing women, possibly depending on dietary habits. Since breastfeeding is anessential nutrient for infants and health benefits of exclusive breastfeedinglikely far surpass the putative health risk from modest levels of mycotoxintransfer, exposure to mycotoxin should be as low as possible and potential foodsafety strategies should be implemented., Emzirme yenidoğan ve süt çocuğu beslenmesinde altın standart olup ve tümbebeklerin ilk altı ayda tek başına anne sütü ile beslenmesi önerilmektedir. Annesütü içerisinde bulunabilecek ve bebeğe geçebilecek istenmeyen toksin vemetabolitlerin belirlenmesi önemlidir. Anne sütü mikotoksin düzeyleri ileilgili çalışmalar kısıtlıdır ve ülkeler arasında farklılıklar bulunabilmektedir.Bu çalışmada, anne sütü aflatoksin M1 (AFM1) düzeylerinin belirlenmesi ve annenindiyet alışkanlıkları ile olası ilişkisinin değerlendirilmesi planlandı. Bu çalışmada AFM1 çalışılmak üzere 3-5ml anne sütü örneği alındı ve analize kadar saklandı. Tüm annelere, hamilelikve doğum sonrası dönemde mikotoksin maruziyeti ile ilişkili olabilecek bazıgıdaların tüketimiyle ilgili anket uygulandı. AFM1 toksini, HELICA AflatoksinM1 ELISA Testi ile ve “Multi-residue Method Based on Chip” teknolojisi ile değerlendirildi.Yalnız ane sütü ile beslnenen bebeklerde günlük total AFM1 alımı hesaplandı. Çalışmaya 7-90 günlük bebekleri olan 17-41 yaşarası 90 anne çalışmaya dahil edildi. Anne sütü AFM1 düzeyi medyan 3,59 ng/Lolup, 2,76 ile 5,99 ng/L arasındaydı. Tüm çalışma grubunda ELISA yöntemi ile eşikdeğer olarak saptanmış olan 5 ng/L’nin üzerinde olan sadece üç örnek mevcuttu. Rezidüdüzeylerinin “multichip” teknolojisi ile değerlendirildiği ikinci yöntem ilehiçbir anne sütü örneğinde AFM1 saptanmadı. Yalnız anne sütü ile beslenen 63 bebeğin,anne sütü ile günlük total AFM1 alımı medyan 0,54 ng/kg (0,41-0,90 ng/kg/gün)olup, besinler için belirlenen günlük alım limiti düzeyinin çok altında idi.Anne sütü AFM1 düzeyleri annenin beslenme özellikleri arasında ilişkisaptanmadı (p>0.05). Anne sütü mikotoksin düzeyleri ile ilgili daha öncekiçalışmalarımızda yüksek anne sütü okratoksin, zearalenone ve deoksinivalenoldüzeylerini gösterilmiş olmasına rağmen, anne sütü AFM1 düzeyleri farklısaptama testleri ile normal sınırlardaydı. Anne sütü aflatoksin düzeylerindeki bölgelerarasındaki farklı bulgular, annelerin gebelik ve lohusalık dönemindeki farklıbeslenme alışkanlıkları ile ilişkili olabilir. Anne sütü tüm bebekler içintemel besin ögesi olduğundan ve anne sütü içerisindeki toksin ve endokrinbozucuların belirli aralıklar ile değerlendirilmesi ve anne beslenmesindeki muhtemeltoksin kaynaklarının diyetten uzaklaştırılması gereklidir.

- Abstract
Background to evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition. Methods: a prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA)<= 32 weeks and those with a BW> 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses. Results: the TR-ROP study included 6115 infants: 4964 (81%) with a GA <= 32 weeks and 1151 (19%) with a GA>32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Of all infants, 414 needed treatment and 395 (95.4%) of the treated infants had a BW <= 1500 g. Sixty-six (16%) of the treated infants did not fulfil the Early Treatment for Retinopathy of Prematurity requirements for treatment. Conclusions: screening of infants with a GA <= 34 weeks or a BW<1700 g appears to be appropriate in Turkey. Monitoring standards of neonatal care and conducting quality improvement projects across the country are recommended to improve neonatal outcomes in Turkish NICUs., NA

Kistik fibrozisde (KF) en önemli morbidite ve mortalite nedeni olan akciğer hastalığının ilerlemesinde enflamasyon kritik rol oynar. Enflamasyonun değerlendirilmesinde güvenilir, girişim gerektirmeyen ve tekrarlanabilir özellikte yöntemlere ihtiyaç duyulmaktadır. Kistik fibrozisde enflamatuvar yanıtın düzenlenmesinde bir bozukluk olduğuna dair güçlü kanıtlar vardır. Gastrointestinal sistemde de akciğerlerdekine benzer kronik enflamasyon olabilir. Bu çalışma KF hastalarının yoğunlaştırılmış nefes havasında ve balgamında kalprotektin ile 8-isoprostan ölçümlerinin akciğer enflamasyonunu invazif olmayan yöntemlerle göstermekte kullanılabilirliğini test etmek ve fekal kalprotektin ölçümleriyle bu hastalarda akciğer enflamasyonuna eşlik edebilecek bağırsak enflamasyonu varlığını araştırmak için yapılmıştır. Çalışmaya 6-18 yaş arası 67 KF hastası ve nefes toplanan 29, gayta alınan 25 sağlıklı çocuk katıldı. Enflamasyon belirteçlerinin akut pulmoner alevlenme (APA), balgam kültüründe üreme, kronik kolonizasyon varlığından nasıl etkilendiği ve vücut kitle endeksi, Shwachman-Kulczycki skoru, solunum fonksiyon testi, beyaz küre, CRP değerleri ile ilişkisi incelendi. Sağlıklı ve KF hastası çocukların yoğunlaştırlmış nefes havasında ölçülebilir düzeyde kalprotektin saptanmadı. Kistik fibrozis hastaları ve kontrol grubunun nefes havası 8-isoprostan düzeyleri arasında anlamlı fark yoktu. Sağlıklı çocuklardan balgam alınamadığı için balgamda yapılan ölçümler için kontrol grubu oluşturulamadı. Balgam kültüründe üreme, kronik kolonizasyon ve APA olup olmamasına göre belirlenen hasta alt grupları arasında kıyaslamalar yapıldı. Kistik fibrozis hasta alt gruplarının balgam kalprotektin ve 8-isoprostan düzeyleri arasında anlamlı fark saptanmadı. Balgam 8-isoprostan düzeylerinin Shwachman-Kulczycki skoru ile negatif (Rho=-0,355), nefes havası 8-isoprostan düzeyleri ile pozitif (Rho=0,485) korelasyonu olduğu görüldü. Kistik fibrozis hastalarının fekal kalprotektin değerleri sağlıklı çocuklardan yüksek bulundu (p=0.001). Akut pulmoner alevlenmesi olan ve olmayan hasta gruplarının fekal kalprotektin düzeyleri arasında anlamlı fark yoktu (p>0.05) ve iki grubun da değerleri kontrollerden yüksekti (p0.05). Fekal kalprotektin değerlerinin CRP ile pozitif yönde korelasyonu vardı (Rho= 0.601). Sonuç olarak KF hastalarının yoğunlaştırılmış nefes havası ve balgamında 8-isoprostan ile kalprotektin ölçümleri akciğer enflamasyonunu değerlendirmede kullanılamaz. Balgam 8-isoprostan düzeyleri hastaların uzun dönem izleminde hastalığın şiddetini belirlemekte yardımcı olabilir. Kistik fibrozisli çocuklarda pulmoner enfeksiyon varlığında fekal kalprotektin değerleri yükselmektedir. Bu durum ölçülen fekal kalprotektinin yutulan balgam kaynaklı olduğunu düşündürmektedir. Fekal kalprotektin ölçümleri solunum yollarında bakteriyel enfeksiyonu olan hastaların belirlenmesinde girişim gerektirmeyen bir yöntem olarak kullanılabilir.Anahtar kelimeler: KF, kalprotektin, 8-isoprostan, enflamasyon, enfeksiyon Inflammation plays a critical role in progression of lung disease which is the major cause of morbidity and mortality in cystic fibrosis (CF). Reliable, noninvasive and repeatable methods are needed to be used in evaluation of airway inflammation. There is growing body of evidence that dysregulation of immune response exists in CF. Inflammation, similar to that we observe in the lungs, might be present in the intestines. This study was conducted to test usefullness of calprotectin and 8-isoprostane measurements in exhaled breath condansate (EBC) and sputum of CF patients as a noninvasive metod for evaluation of lung inflammation and faecal calprotectin in detecting intestinal inflammation. Sixty-seven children with CF, 29 healty control for exhaled breath measurements and 25 healty control for feacal measurements were enrolled in this study (age range 6-18 years). How these inflammatory mediators are effected by the presence of acute pulmonary exacerbations (APE), chronic bacterial colonization and positive sputum cultures and their relation with body mass index, Shwachman-Kulczycki score, respiratory function tests, white blood cell counts and CRP were evaluated. Calprotectin was not measurable in EBC. In CF patients EBC 8-isoprostane levels were not different from that in healty children. As healty children could not expectorate sputum, we could not recruit a control group for sputum measurements. Comparisons are done between CF patient groups with and with out APE, chronic bacterial colonization and those with positive and negative sputum cultures. No significant difference was observed between sputum calprotectin and 8-isoprostane levels of CF patient groups. Sputum 8-isoprostane levels were positively correlated with EBC 8-isoprostane levels (Rho=0,485) and negatively with Shwachman-Kulczycki score (Rho=-0,355). Children with CF had faecal calprotectin levels higher than healty children (p=0.001). Faecal calprotectin levels of patients with and without APE were not different from each other (p>0.05) and both were higher than that of the controls (p0.05). We observed a positive correlation between the faecal calprotectin and CRP levels (Rho= 0.601). In conclusion; calprotectin and 8-isoprostane measurements in EBC and sputum are not usefull in monitoring lung inflammation and infection in CF. Sputum 8-isoprostane levels may be usefull in longterm follow up of disease severity. Faecal calprotectin levels increase in the presence of pulmonary infection in CF patients. This finding points out pulmonary origin of the measured faecal calprotectin. Fraser syndrome is a rare autosomal recessive malformation syndrome characterized by cryptophthalmos, syndactyly, and laryngeal and urogenital defects. The diagnosis of this syndrome can be made on clinical examination and occasionally on prenatal ultrasound. We report a newborn who presented with respiratory failure in the neonatal intensive care unit and was diagnosed as Fraser syndrome with clinical findings such as cryptophthalmos, syndactyly, anal atresia, and laryngeal atresia. Material and methods A prospective cohort study was performed between April 1, 2016 and April 30, 2017. the study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results Data from 69 NICUs were obtained. the mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Oksidatif stres (OS) hücresel metabolizma sırasında oluşan oksidanların antioksidanlarca etkisiz hale getirilememesi sonucunda oluşur. Yenidoğanlar intrauterin çevreden görece hiperoksik ekstrauterin çevreye geçiş sırasında OS'ye maruz kalırlar. Preterm bebekler antioksidan koruma mekanizmalarının immatür oluşu nedeniyle OS'ye daha yatkındırlar. Preterm anne sütü (AS), bebeğin ihtiyaçlarına paralel olarak birçok maddeyi daha yüksek oranlarda içerir. Çalışmamız laktasyonun farklı evrelerinde ve emzirmenin başı ve sonunda preterm AS'nin antioksidan/oksidan dengesini incelemek ve term AS ile karşılaştırmak için planlanmıştır. Gebelik haftası 32 (23-33) hafta 45 preterm ve 38 (37-40) hafta 41 term bebeğin AS'lerinde total antioksidan kapasite (TAK) ve total oksidan seviyesi (TOS) ölçülmüş ve oksidatif stres indeksi (OSİ) hesaplanmıştır. Preterm bebek annelerinin kolostrum ve geçiş sütünde TAK ve TOS term bebeklerden daha düşük, matür sütte ise term bebeklere benzer bulunmuştur. Antioksidan/oksidan dengesini yansıtan OSİ'nin preterm bebeklerin geçiş sütünde daha düşük olduğu görülmüştür. Kolostrumda TAK'ın en yüksek olduğu ve laktasyonun seyrinde azalarak matür sütte en düşük düzeylere indiği saptanmıştır. Ancak preterm AS'deki azalma istatistiksel olarak anlamlı değildir. TOS ve OSİ hem term hem preterm bebeklerde kolostrumda en düşük bulunmuştur. Preterm bebeklerin matür süt TAK ve TOS düzeylerinin son sütte ön süte göre yüksek, OSİ'nin benzer olduğu saptanmıştır. Term AS'de ise geçiş sütü TOS ve OSİ ön sütte son süte göre yüksek saptanmıştır. Çalışmamız preterm AS'de antioksidan/oksidan dengesinin korunduğunu ve geçiş sütünde OSİ'nin term AS'den daha düşük olduğunu göstermiştir. Newborns are exposed to OS due to rapid shift from intrauterine to relatively hyperoxic extrauterine environment. Preterm newborns are particularly prone to OS because of their immature antioxidant defence mechanisms. Preterm breast milk (BM) contains higher levels of many bioactive molecules in parallel to increased requirements of the preterm neonates. This study was planned to investigate the balance between antioxidants and oxidants in preterm BM in different stages of lactation and within the same nursing episode and compare them with term BM. Total antioxidant capacity (TAC) and total oxidant status (TOS) were measured and oxidative stress index (OSI) was calculated in BM of 45 preterm and 41 term newborns with 32 (23-33) weeks and 38 (37-40) weeks gestational ages, respectively. TAC and TOS were lower in colostrum and transitional milk of preterm newborns compared to term newborns, whereas mature milk of preterm and term newborns have similar TAC and TOS levels. OSI which represents the balance between antioxidants and oxidants was lower in transitional milk of preterm newborns. TAC was highest in colostrum and decreased through the course of lactation with lowest levels measured in mature milk, but decrease in TAC was not statistically significant in preterm BM. Lowest values of TOS and OSI were observed in colostrum. In preterm BM TAC and TOS were higher in hindmilk compared to foremilk and OSI was similar, whereas in term BM, TOS and OSI were higher in foremilk compared to hindmilk. Findings of this study showed that antioxidant/oxidant balance was preserved in preterm BM with lower OSI in transitional milk compared to term BM. Background To evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition., Methods A prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA) 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses., Results The TR-ROP study included 6115 infants: 4964 (81%) with a GA 32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Hemangiomas are the most common benign tumors of the eyelids in infancy. They may arise as small isolated lesions or large lesions affecting visual function. In most cases, treatment is not needed. However, if the lesion leads to functional impairment or ulcerates, treatment is necessary. Herein, we present the case of a preterm infant with rapidly growing cutaneous hemangioma of the eyelid with a potential risk of amblyopia, which was successfully treated with propranolol.
