Abstract

PURPOSE lthough clinical ophthalmologic findings have been reported, no study documented magnetic resonance imaging (MRI) findings in mucopolysaccharidosis (MPS) type VI. The aim of this study was to determine the ophthalmologic imaging findings of MPS type VI in the pediatric age group retrospectively. METHODS Brain MRIs of 10 patients with MPS type VI and 49 healthy children were evaluated independently by two pediatric radiologists for the following characteristics: globe volume, ocular wall thickness, and optic nerve sheath diameter for each orbit. The means of the measurement of each group were compared by using an independent t-test. Agreement and bias between reviewers were assessed by intra-class correlation coefficients (ICC). RESULTS A total of 59 children [32 girls (54.23%), 27 boys (45.77%); age range, 4-16 years; mean age, 10.37 ± 3.73 years] were included in the study. Statistical analysis revealed smaller eyeballs and thicker ocular walls of patients with MPS type VI (P < .001 and P < .001, respectively). However, there was no statistically significant difference in terms of optic nerve sheath diameter between the two groups (P=.648). CONCLUSION Patients with MPS type VI displayed reduced globe volumes and increased ocular wall thicknesses compared to the healthy children. Therefore, we recommend that ophthalmologic imaging findings might prove to be an auxiliary tool in the diagnosis of MPS patients.

Abstract

Objective: Caffeine is frequently used for the treatment of apnea in preterm babies. Its mechanism of action includes not only antagonism of adenosine receptors on central nervous system but also an increase in electrical activity of the diaphragm. Caffeine’s direct effect on diaphragm was investigated via electromyography, but ultrasound has not been used to show visible changes in muscles after the treatment before. Therefore, we aimed to assess the effect of caffeine on diaphragmatic function through ultrasonographic examination. Materials and Methods: It was a prospective observational study. Fifty-six participants receiving nasal continuous positive airway pressure with less than or equal to 32 weeks’ gestational age born were enrolled. Diaphragmatic thickness, amplitude of excursion, and velocity of movement were measured before and within 5 minutes after caffeine loading dose and compared to each other. The protocol was registered with ClinicalTrials.gov Identifier NCT04483492. Results: Diaphragmatic thicknesses and diaphragmatic velocity of movement did not differ after the treatment. However, amplitude of excursion of the diaphragm was found significantly higher after caffeine loading dose (8.7 mm, 10mm, respectively, P < .05). Conclusion: Diaphragm excursion increased after caffeine treatment in preterm babies, and this finding was potentially supported the direct effect of the caffeine on diaphragm. Another important finding of this study is that it reinforces the utility of ultrasonography in assessing diaphragmatic function in preterm infants. [ABSTRACT FROM AUTHOR]

15. The Relationship Between Serum Growth Differentiation Factor-15 Levels and Clinical Outcomes in Infertile Women Receiving In-vitro Fertilization Treatment.

Abstract

Objective: It has been reported in many studies that Growth Differentiation Factor-15 (GDF-15) has an important role in physiological or pathological processes. This study was aimed to investigate the role of GDF-15 in infertility and its treatment outcomes. Material and Method: According to their ovarian reserve characteristics, 88 infertile women were divided into three groups: normal ovarian reserve (NOR) (n= 42), diminished ovarian reserve (DOR) (n= 22), and polycystic ovary syndrome (PCOS) (n= 24). Serum estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), anti-Mullerian hormone (AMH), and GDF-15 levels were measured in their serum. The antagonist protocol patients' total oocyte, meiosis II (MII) oocytes, embryo count, and clinical pregnancy rates were documented and compared. Results: In terms of serum GDF-15 concentrations, there was no statistically significant difference among the mean values of the three study groups. The mean FSH level at baseline was substantially higher in the DOR group compared to the PCOS group (p=0.006*). The median serum AMH levels of all three groups were found to be statistically different (p=<0.001*). When the groups were compared in terms of averages of total oocytes, MII oocytes, and embryos, it was observed that the NOR group and the PCOS group both had higher values than the AOR group (p=0.001*). Conclusion: In our study, a significant and strong relationship was found between serum GDF-15 level and the number of embryos formed as a result of in-vitro fertilization (IVF) treatment. Thereby, serum GDF-15 level may be considered to be a biomarker for predicting IVF clinical outcomes. [ABSTRACT FROM AUTHOR]

Abstract

Ultrasound and conventional radiographs still remain the first-line radiological tools for most of the kidney disorders. Ultrasonographic image quality has been astonishingly improved in recent years with the development of technology and software algorithms. In line with these developments, ultrasound is not only the primary imaging modality for kidney anatomy and lesions, but also sufficient for definitive diagnosis and follow up for some lesions. The aim of this chapter is to focus on the areas that ultrasound is used primarily about the kidney. These topics include kidney anatomy, anatomic variants that mimic lesions, congenital diseases, kidney stones, nephrocalcinosis, most of the cystic diseases, and some solid lesions, infection, and trauma.

31. Noonan syndrome: Neuroimaging findings and morphometric analysis of the cranium base and posterior fossa in children.

Abstract

Background and Purpose: There are a few studies regarding intracranial findings in neonates with Noonan syndrome (NS); however, there are no quantitative analyses in a pediatric population. The aim of this study was to find characteristic intracranial abnormalities and to quantitatively analyze the posterior fossa and cranium base in children with NS. Methods: A total of 30 patients (11 females and 19 males, mean age 13.1 ± 4.3 years) were retrospectively identified between July 2017 and June 2022. Twenty‐one patients had MRI. Age at MRI examination, sex, genetic mutations, and clinical findings were noted. In patients with MRI, the presence of white matter lesions, basal ganglia lesions, corpus callosum abnormalities, sellar/parasellar lesions, and tonsillar ectopia was noted. For morphometric analysis, cerebellar diameter, vermis and clivus heights, cranial base, tentorial and infratentorial angles, and McRae's and Twining's lines were each measured twice by two radiologists individually. Results: The most common lesions were focal white matter lesions, followed by abnormalities of the splenium of the corpus callosum. The cerebellar diameter, vermis and clivus heights, Twining's line, and infratentorial angle were significantly smaller; cranial base angle and tentorial angle were significantly larger in NS (p <.05). Interrater and intrarater agreements were the highest for cerebellar diameter and the lowest for tentorial angle measurements. Conclusion: Children with NS had characteristic callosal and tentorial findings and neuroimaging findings similar to other RASopathies. This study also shows that a small posterior fossa and flattening of the cranial base are present in children with NS, which may aid in diagnosis. [ABSTRACT FROM AUTHOR]

Abstract

Objective: The prevalence of acute pancreatitis and acute recurrent pancreatitis in children has increased over the years, and there are limited data about imaging findings. This study aimed to reveal the imaging findings of acute pancreatitis and acute recurrent pancreatitis in children at a tertiary care hospital. Materials and Methods: The patients with acute pancreatitis and acute recurrent pancreatitis diagnosed between January 2007 and December 2018 were included. Demographic and clinical features, follow-up period, and interventions were noted. Imaging features were evaluated for pancreatic enlargement, peripancreatic fluid, and biliary ducts for initial examination and pancreas parenchymal necrosis, peripancreatic collection, walled-off necrosis, pseudocyst, parenchymal atrophy, and biliary ductal dilatation for follow-up. Results: The study included 74 patients with a mean age of 9 ± 4.9 years. The most common causes of acute pancreatitis and acute recurrent pancreatitis were biliary tract anomalies (n = 21), biliary ductal stones (n = 9), and cystic fibrosis (n = 8). Findings consistent with acute pancreatitis were determined by ultrasound in 40.5% (n = 30/74), whereas by magnetic resonance imaging in 60% (n = 39/65). Forty-one percent of the patients (n = 16) with positive magnetic resonance imaging findings did not show any findings on ultrasound. Acute recurrent pancreatitis was seen in 32 patients (43.2%). Follow-up imaging was performed in 55 patients (74.3%) between 2 months and 11 years. At follow-up, 8 patients had peripancreatic collections (6 walled-off necrosis and 2 pseudocysts). Conclusion: Recognizing the imaging findings of acute pancreatitis and its complications is crucial. Magnetic resonance imaging should be preferred as a second option following ultrasound, with the advantages of biliary ductal system delineation and better characterization of complications. Abstract

Objective: Ischemic/reperfusion (I/R) causes tissue injury and the leading cause of acute kidney injury. In this study, we aimed to investigate the effects of the long and short-term usage of ALA and short-term DHLA on oxidative stress markers in the experimental renal ischemia-reperfusion model. Methods: Forty male rats (250 to 300 gr) were divided into 5 groups: control; I/R group; long-term ALA+IR group; short-term ALA+IR group; and short-term DHLA+IR group. Ischemia was carried out for 45 minutes followed by reperfusion for 4 hours. Thiobarbituric acid reactive sunstances (TBARM), catalase (CAT), superoxide dismutase (SOD), and glutathione peroxidase (GSH-Px) activities in tissue samples and serum total antioxidant status (TAS) and total oxidative stress (TOS) assayed by the spectrophotometrically. Tissue samples were investigated by histopathological analyzes. Results: TBARM (Control: 0.38±0.05. I/R: 1.37±0.17, long-term ALA-treated group:1.025±0.15, short-term ALA-treated group: 0.68±0.09, short-term DHLA-treated group: 0.38±0.04 (nmol/mg protein); p, Amaç: İskemi/reperfüzyon (I/R) doku hasarına neden olarak akut böbrek hasarına yol açar. Bu çalışmada, deneysel böbrek iskemi-reperfüzyon modelinde uzun ve kısa süreli ALA ve kısa süreli DHLA kullanımının oksidatif stres belirteçleri üzerine etkilerinin araştırması amaçlanmıştır. .Yöntem: Kırk adet erkek rat (250-300 gr) 5 gruba ayrılmıştır: kontrol grubu; I/R grubu; uzun vadeli ALA+IR grubu; kısa vadeli ALA+IR grubu ve kısa vadeli DHLA+IR grubu. 45 dakika süreyle iskemi, ardından 4 saat süreyle reperfüzyon uygulanmıştır. Doku örneklerinde Tiyobarbitürik asid reaktif maddeler (TBARM), katalaz (CAT), süperoksit dismutaz (SOD) ve glutatyon peroksidaz (GSH-Px) aktiviteleri ile total antioksidan durum (TAS) ve total oksidatif stres kapasitesi (TOS) spektrofotometrik olarak ölçülmüştür. *Karaca, Onur ( Aksaray, Yazar ), Aim: The aim of the present study was to evaluate the effect of Pelargonium sidoides (UMCA ®) which was antibacterial, antiviral, anti-inflammatory and antioxidant, on pulmonary contusion (PC) caused by isolated blunt thoracic trauma (BTT) in an experimental rat model. Material and Methods: A total of 24 rats were divided into three groups: control group (CG), sham group (SG), and Pelargonium sidoidesgroup (PSG). PC was induced by isolated BTT for all the groups except the control group. Pelargonium sidoidestreatment was performed by gavage for 72 hours to the PSG after truma. Blood and tissue samples were collected from the groups. Malondialdehyde (MDA), superoxide dismutase (SOD),glutathione (GSH) and arterial blood gas parameters were measured. Lung tissue samples were collected for histopathology. Abstract

OBJECTIVE: To investigate the role of 50-Hz extremely low frequency pulsed (PEMF) and sinusoidal (SEMF) electromagnetic fields on the testis using a rat model. STUDY DESIGN: Twenty-seven male Wistar albino rats were used. The rats were divided into three groups (n=9 each): sham group, SEMF group, and PEMF group. The SEMF and PEMF groups (pulse time: 20 milliseconds, pulse frequence: 50 Hz) were subjected to 1.5 mT, 50 Hz exposure 6 hours a day, 5 days a week for 28 days in methacrylate boxes. Formalin-fixed, paraffin-embedded tissue sections were stained with H-E and PAS. In addition, E-cadherin and type IV collagen expressions were examined immunohistochemically. RESULTS: Seminiferous tubule basement membranes decreased with EMFs, especially those treated with SEMF. In addition, expression levels of E-cadherin on seminiferous epithelium decreased in PEMF and SEMF groups. The expression level of type IV collagen was also decreased in perivascular and seminiferous tubule basement membrane as compared to that in the sham group. CONCLUSION: PEMF and SEMF have adverse effects on the testis at a histopathological level. EMF leads to a decrease in E-cadherin and type IV collagen expression levels. 44. Ultrasound of the Pediatric Gastrointestinal Emergencies

Abstract

With recent technologies, ultrasound has become an extremely useful imaging modality for evaluating children with acute abdominal symptoms. Higher frequency transducers can be used in children than in adults, owing to their small body size, the presence of less fat tissue in the abdominal wall and peritoneal cavity leading to higher resolution than computed tomography in many circumstances without exposure to ionizing radiation. Real-time ultrasound imaging provides information about motion such as peristalsis, and newly developed harmonic imaging enables improved resolution with decreased artifacts. Beyond gray-scale ultrasound, color Doppler ultrasound provides information on vascularity which increases in inflammatory processes. Point-of-care examination includes ability to focus on the symptomatic area of the patient while performing real-time ultrasound imaging. Ultrasound is sufficient for the diagnosis of several gastrointestinal diseases that cause acute abdominal pain in pediatric patients helping to an accurate patient management in the emergency settings. Common gastrointestinal indications for abdominal ultrasound in children are hypertrophic pyloric stenosis, acute appendicitis, intussusception, inflammatory bowel disease, malrotation, midgut volvulus, hernia, and necrotizing enterocolitis. In this chapter, typical sonographic findings of aforementioned diseases, and possible differential diagnoses were discussed.
