Abstract

This study investigated in-hospital outcomes and related factors in patients diagnosed with postearthquake crush syndrome after the earthquakes in Kahramanmaraş, Türkiye. One thousand twenty-four adult patients diagnosed with crush syndrome were analyzed. Data on demographic characteristics, clinical presentation, laboratory values, treatments, and outcomes were collected. A total of 9.8% of patients died during their hospital stay. Nonsurvivors were generally older, more likely to have preexisting chronic kidney disease, and faced more severe injuries and complications, including hypotension-shock, arrhythmias, elevated markers of renal dysfunction, and higher rates of acute kidney injury (AKI) and compartment syndrome. In addition, intensive care unit needs were higher. Multivariate analysis confirmed that age, injury severity, shock, high potassium, uric acid, and lactate levels on admission, development of AKI, compartment syndrome, and intensive care unit admission were significant predictors of mortality. Better disaster preparedness and improved health care infrastructure could be potential explanations for improved in-hospital mortality in the current era, as compared to previous earthquakes.

Abstract

Introduction: Peritoneal dialysis (PD) remains understudied in disaster nephrology. This retrospective multicenter study explores the experiences of PD survivors following the February 6, 2023, Kahramanmaraş Earthquake. Methods: Adult PD patients from 11 affected cities were analyzed to assess challenges faced during and postearthquake, alongside clinical outcomes. Results: Among 101 participants (median age: 45 years, median PD duration: 24 months), 57 were female, with 79 on continuous ambulatory PD. Challenges included power outages and water shortages, with primary shelter in kin's houses (33%) and homes (28%). Twelve patients experienced PD program delays, and three lacked assistance postdisaster. Sixteen patients changed PD modalities, with seven experiencing postearthquake peritonitis. Clinical parameters remained stable, except for a slight decrease in hemoglobin levels. Conclusion: Despite challenges, PD survivors exhibited resilience, highlighting the importance of addressing peritonitis and unusual pathogens in disaster preparedness initiatives. Abstract

Introduction: We aimed to evaluate the relationship between the hand fine motor skills of peritoneal dialysis (PD) practitioners and PD‐related peritonitis. Methods: This multicenter prospective observational study was conducted with 120 incident PD patients. Patients were divided into two groups who had PD‐related peritonitis within the first year as Group 1, and those who did not as Group 2. Hand fine motor skills were evaluated by Nine‐Hole Peg Test (NHPT) and Nut Screwing Test (NST). Results: Initial NHPT (28.5 ± 6.0 s vs. 25.8 ± 5.0 s, p = 0.011) and NST (82.3 (61.5–102.8) s versus 65.3 (52.3–88.5) s p = 0.023) scores were significantly higher in Group 1 than Group 2. In multivariate logistic regression analysis, NHPT, Body Mass Index, Mini‐Mental Test, self PD practitioner, and catheter complications were found to be independent variables in predicting PD‐related peritonitis. Abstract

Background: Patients with chronic diseases such as chronic kidney disease (CKD) have been reported to have more adverse outcomes during the coronavirus disease 2019 (COVID-19) pandemic. There are insuficient data on the outcomes of patients with primary glomerular diseases (PGD) afer COVID-19. Methods: We designed a national multicenter observational study that included adult patients with biopsy-proven PGD who survived COVID-19. A control group was created from the same centers, including PGD patients without COVID-19. The clinical and laboratory data of the patients at baseline, frst, and third months afer COVID-19 were recorded. Results: A total of 129 patients from 21 centers were included (COVID-19 group, n = 77). Baseline characteristics were almost similar except the ratio of active disease in the non-COVID-19 group was signifcantly higher than in the COVID-19 group. No patients died during the frst and third months. Respiratory symptoms were signifcantly higher in the COVID-19 group than in the non-COVID-19 group in the frst month (7.8% vs. 0%, P = .039). All other follow-up outcomes, including initiation of chronic kidney replacement therapy and initiation of new immunosuppressive treatment, and the laboratory data were not diferent between the groups in the frst and third months. Conclusion: Primary glomerular disease patients in the post-COVID-19 period had more respiratory symptoms than non-COVID-19 PGD patients, but outcomes, including death and initiation of kidney replacement therapy, were not diferent in the frst and third months post COVID-19. Abstract

IgG4‐related disease is an inflammatory, multisystemic disease that affects the immune system. The disease progresses to fibrosis due to inflammation. Retroperitoneal fibrosis is a serious complication. Pancreas, biliary tract, glands, thyroid, lymph nodes, etc. may be involved. Prognosis is usually subacute, and seen in middle age and advanced men. It is characterized histopathologically by IgG4 positive plasma cells, lymphoplasmocytic cell infiltration, and storiform fibrosis. In our case, we evaluated a patient who referred to our clinic from an external center with the complaints of generalized pain, itching, tearing and redness in eyes, involvement of bilateral large joints, and impaired renal function. © 2022 Turkish Society of Nephrology. All rights reserved.Objectives: There is limited data about coronavirus disease-19 (COVID-19) characteristics and results in peritoneal dialysis (PD) patients. This study aimed to investigate the characteristics and outcomes among PD patients and compare them with matched hemodialysis (HD) patients and a control group without kidney disease. Methods: We included 18 PD patients and consecutive age- and gender-matched 18 HD and 18 patients without kidney disease (control group) registered into the Turkish Society of Nephrology database including 1301 COVID-19 patients. We compared demographic, clinical, radiological, laboratory data, and outcomes namely intensive care unit (ICU) admission, mechanical ventilation, mortality, and composite outcome (death and/or ICU admission). Results: ICU admission, mechanical ventilation, and mortality rates in PD patients (27.8%, 22.2%, and 22.2%, respectively) and the HD group (16.7%, 11.1%, and 16.7%, respectively) were higher than the control group (11.1%, 11.1%, and 5.6%, respectively), but intergroup comparison did not reveal difference. A total of 11 (20.3%) patients had composite outcome (6 PD patients, 3 HD patients, and 2 patients in the control group). In Cox regression analysis, higher age and higher CRP level were related to increased risk of composite outcome. Adjusted rate of composite outcome in PD group was significantly higher than the control group (P = .050). This rate was similar in HD and control groups (P = .30). Conclusions: Combined in-hospital mortality and/or ICU admission of PD patients with COVID-19 was significantly higher than the control patients. There is a need for careful surveillance of PD patients for infection signs and prompt treatment of COVID-19.

Abstract

BackgroundAcute kidney injury (AKI) is common in coronavirus disease-2019 (COVID-19) and theseverity of AKI is linked to adverse outcomes. In this study, we investigated the factors asso-ciated with in-hospital outcomes among hospitalized patients with COVID-19 and AKI.MethodsIn this multicenter retrospective observational study, we evaluated the characteristics andin-hospital renal and patient outcomes of 578 patients with confirmed COVID-19 and AKI.Data were collected from 34 hospitals in Turkey from March 11 to June 30, 2020. AKI defini-tion and staging were based on the Kidney Disease Improving Global Outcomes criteria.Patients with end-stage kidney disease or with a kidney transplant were excluded. Renaloutcomes were identified only in discharged patients.ResultsThe median age of the patients was 69 years, and 60.9% were males. The most frequentcomorbid conditions were hypertension (70.5%), diabetes mellitus (43.8%), and chronic kid-ney disease (CKD) (37.6%). The proportions of AKI stages 1, 2, and 3 were 54.0%, 24.7%,and 21.3%, respectively. 291 patients (50.3%) were admitted to the intensive care unit.Renal improvement was complete in 81.7% and partial in 17.2% of the patients who weredischarged. Renal outcomes were worse in patients with AKI stage 3 or baseline CKD. Theoverall in-hospital mortality in patients with AKI was 38.9%. In-hospital mortality rate was notdifferent in patients with preexisting non-dialysis CKD compared to patients without CKD(34.4 versus 34.0%, p = 0.924). By multivariate Cox regression analysis, age (hazard ratio[HR] [95% confidence interval (95%CI)]: 1.01 [1.0–1.03], p = 0.035], male gender (HR [95%CI]: 1.47 [1.04–2.09], p = 0.029), diabetes mellitus (HR [95%CI]: 1.51 [1.06–2.17], p =0.022) and cerebrovascular disease (HR [95%CI]: 1.82 [1.08–3.07], p = 0.023), serum lac-tate dehydrogenase (greater than two-fold increase) (HR [95%CI]: 1.55 [1.05–2.30], p =0.027) and AKI stage 2 (HR [95%CI]: 1.98 [1.25–3.14], p = 0.003) and stage 3 (HR [95%CI]:2.25 [1.44–3.51], p = 0.0001) were independent predictors of in-hospital mortality.ConclusionsAdvanced-stage AKI is associated with extremely high mortality among hospitalizedCOVID-19 patients. Age, male gender, comorbidities, which are risk factors for mortality inpatients with COVID-19 in the general population, are also related to in-hospital mortality inpatients with AKI. However, preexisting non-dialysis CKD did not increase in-hospital mor-tality rate among AKI patients. Renal problems continue in a significant portion of thepatients who were discharged.&nbsp

Abstract

Amaç:Akut böbrek hasarı (ABH) günümüzde hala önemli bir morbidite ve mortalite nedenidir. Sıklığı, etiyolojisi, prognozu ve mortalitesinde birçok faktör etkili olmaktadır. Çalışmamızda ABH etiyolojisi ve prognostik faktörleri ortaya koymayı amaçladık.Yöntemler:Çalışmaya nefroloji servisine akut böbrek yetmezliği tanısı ile Ocak 2011- Aralık 2015 tarihleri arasında yatarak tedavi edilmiş 272 hasta dahil edildi. Hastaların demografik özelliklerinin yanı sıra klinik ve laboratuvar bulguları retrospektif olarak incelendi.Bulgular:Hastaların %47,4’ü kadın, %52,6’sı erkek idi. Tüm hastaların yaş ortalaması 61,6, ortalama yatış süresi 13,3 gün, hemodiyaliz sayısı 1,24, mortalite oranı %4,8 bulundu. Hemodiyaliz ihtiyacı ve sayısı yaşlı grupta (>65 yaş) anlamlı şekilde yüksekti. Hemodiyaliz ihtiyacı ve sayısı renal ABH’li grupta daha fazlaydı (%48,7, 1,9). Renal ABH’lilerde yatış süresi ve mortalite diğer gruplarla kıyaslandığında daha yüksekti (16,4 gün, %8,8). Enfeksiyonu olanlarda (%42) mortalite, yatış süresi ve hemodiyaliz sayısı anlamlı şekilde artmıştı.Sonuç:ABH’de etiyoloji ve eşlik eden enfeksiyon varlığı mortaliteyi etkileyen en önemli faktörlerdir. Abstract

Background: we aimed to present the demographic characteristics, clinical presentation, and outcomes of our multicenter cohort of adult KTx recipients with COVID-19.Methods: we conducted a multicenter, retrospective study using data of patients hospitalized for COVID-19 collected from 34 centers in Turkey. Demographic characteristics, clinical findings, laboratory parameters (hemogram, CRP, AST, ALT, LDH, and ferritin) at admission and follow-up, and treatment strategies were reviewed. Predictors of poor clinical outcomes were analyzed. The primary outcomes were in-hospital mortality and the need for ICU admission. The secondary outcome was composite in-hospital mortality and/or ICU admission.Results: one hundred nine patients (male/female: 63/46, mean age: 48.412.4years) were included in the study. Acute kidney injury (AKI) developed in 46 (42.2%) patients, and 4 (3.7%) of the patients required renal replacement therapy (RRT). A total of 22 (20.2%) patients were admitted in the ICU, and 19 (17.4%) patients required invasive mechanical ventilation. 14 (12.8%) of the patients died. Patients who were admitted in the ICU were significantly older (age over 60 years) (38.1% vs 14.9%, p=0.016). 23 (21.1%) patients reached to composite outcome and these patients were significantly older (age over 60years) (39.1% vs. 13.9%; p=0.004), and had lower serum albumin (3.4g/dl [2.9-3.8] vs. 3.8g/dl [3.5-4.1], p=0.002), higher serum ferritin (679 mu g/L [184-2260] vs. 331 mu g/L [128-839], p=0.048), and lower lymphocyte counts (700/mu l [460-950] vs. 860 /mu l [545-1385], p=0.018). Multivariable analysis identified presence of ischemic heart disease and initial serum creatinine levels as independent risk factors for mortality, whereas age over 60 years and initial serum creatinine levels were independently associated with ICU admission. 50. Pregnancy and Kidney Transplantation: A Single-Center Experience.

Abstract

Objective: The possibility of pregnancy increases with kidney transplantation in patients with chronic kidney disease. However, graft dysfunction, risk of fetal growth retardation, and fetal anomaly should be monitored closely. In this study, renal and obstetric outcomes were analyzed in pregnant kidney recipients who were followed in our center. Methods: We analyzed 140 reproductive-aged patients who underwent renal transplantation between January 2009 and May 2015, and clinical and laboratory data were evaluated retrospectively. Results: Twenty-four patients became pregnant (17.1%). In pregnant group, median age was significantly lower than nonpregnant group (P = .014). The median age of pregnant group at the time of transplantation was also significantly lower than non-pregnant patients (P < .001). The rate of pregnant patients was 66.7% in 18-25 year age group (P = .008). The rate of urinary tract infection in non-pregnant group was higher than pregnant group (P = .03). Live birth rates were 83.3% and 45.8% of those whose birth weight was higher than 2500 g. The increased level of daily urinary proteinuria and the time from diagnosis of renal failure to transplantation had significant effect on pregnancy (odds ratio = 13.81;95% CI: 2.06-92.45; P = .007 and odds ratio = 3.25;95% CI: 1.11-9.48; P = .031, respectively). Low serum creatinine level had significant protective effect (odds ratio = 0.001; 95% CI: 0-0.30, P = .018). The patients in 18-25 age group were 48.39 times more eligible for pregnancy compared to those in >35 age group (odds ratio = 48.39; 95% CI: 1.26-1860.72; P = .037). Rejection episodes were observed in 1 of pregnant women and 11 of non-pregnant women (P > .05). Conclusion: Pregnancy is possible in kidney transplant recipients of reproductive age. Calcineurin inhibitors and azathioprine seem to be safe. Maternal age, low-serum creatinine, and urinary proteinuria affect pregnancy.
