Abstract

Background The correct management of immunocompromised patients with pneumonia is debated. We evaluated the prevalence, risk factors, and characteristics of immunocompromised patients coming from the community with pneumonia. Methods We conducted a secondary analysis of an international, multicenter study enrolling adult patients coming from the community with pneumonia and hospitalized in 222 hospitals in 54 countries worldwide. Risk factors for immunocompromise included AIDS, aplastic anemia, asplenia, hematological cancer, chemotherapy, neutropenia, biological drug use, lung transplantation, chronic steroid use, and solid tumor. Results At least 1 risk factor for immunocompromise was recorded in 18% of the 3702 patients enrolled. The prevalences of risk factors significantly differed across continents and countries, with chronic steroid use (45%), hematological cancer (25%), and chemotherapy (22%) the most common. Among immunocompromised patients, community-acquired pneumonia (CAP) pathogens were the most frequently identified, and prevalences did not differ from those in immunocompetent patients. Risk factors for immunocompromise were independently associated with neither Pseudomonas aeruginosa nor non–community-acquired bacteria. Specific risk factors were independently associated with fungal infections (odds ratio for AIDS and hematological cancer, 15.10 and 4.65, respectively; both P = .001), mycobacterial infections (AIDS; P = .006), and viral infections other than influenza (hematological cancer, 5.49; P < .001). Conclusions Our findings could be considered by clinicians in prescribing empiric antibiotic therapy for CAP in immunocompromised patients. Patients with AIDS and hematological cancer admitted with CAP may have higher prevalences of fungi, mycobacteria, and noninfluenza viruses.

Abstract

An accurate knowledge of the epidemiology of community-acquired pneumonia (CAP) is key for selecting appropriate antimicrobial treatments. Very few etiological studies assessed the appropriateness of empiric guideline recommendations at a multinational level. This study aims at the following: (i) describing the bacterial etiologic distribution of CAP and (ii) assessing the appropriateness of the empirical treatment recommendations by clinical practice guidelines (CPGs) for CAP in light of the bacterial pathogens diagnosed as causative agents of CAP. Secondary analysis of the GLIMP, a point-prevalence international study which enrolled adults hospitalized with CAP in 2015. The analysis was limited to immunocompetent patients tested for bacterial CAP agents within 24 h of admission. The CAP CPGs evaluated included the following: the 2007 and 2019 American Thoracic Society/Infectious Diseases Society of America (ATS/IDSA), the European Respiratory Society (ERS), and selected country-specific CPGs. Among 2564 patients enrolled, 35.3% had an identifiable pathogen. Streptococcus pneumoniae (8.2%) was the most frequently identified pathogen, followed by Pseudomonas aeruginosa (4.1%) and Klebsiella pneumoniae (3.4%). CPGs appropriately recommend covering more than 90% of all the potential pathogens causing CAP, with the exception of patients enrolled from Germany, Pakistan, and Croatia. Abstract

There are innovative informatics solutions made for efficient healthcare delivery and electronic medical record system (EMR) is one of such. The immense opportunities it holds for efficient patient care, hospital economics, clinical audit and medical research affirms its superiority over paper-based records. However, implementing functioning EMRs in environments like ours is froth with many practical and peculiar challenges. Using the experience garnered from the Wesley Guild Hospital EMR project, this paper highlights these challenges and how some of them were overcome. Abstract

Background: Research has shown that social problems do influence clinical decision making. Primary healthcare providers, such as doctors are also prone to social influences both from self and patients, which may affect clinical patient management. Aim: To find out how often perceived social problems of self and patients influence doctors' decisions and management in a primary care setting. Methods: The study was a survey of doctors involved in primary care in a semi-urban tertiary hospital. The doctors completed self- administered questionnaire comprising perceived social influences which often affect their decision making. Fifty doctors comprising consultants and residents involved mostly in primary care, were sampled. Results: 94% of doctors were influenced by social problems of patients in the choice of management. 88% of respondents considered financial status of patients the most influencing factor, while only 24% regarded patient loneliness as a factor in clinical decision making. Other factors that notably influenced management decisions were patient's unemployment (74%), and mental and physical stress of doctors from poor work environment (54%). When perceived social problems influenced clinical decisions, the commonest types of management options offered were considerations on affordability of medication (90%), extra time for consultation (84%), issuance or prolongation of sick leave (22%). Conclusion: In this environment, perceived social problems of both doctors and patients do significantly influence choice of management in 9 out of every 10 doctors in their consultations and clinical management. Abstract

Objectives: To determine the level of computer proficiency, accessibility to the internet and the use of computer-based resources of respondents. Methodology: A questionnaire based descriptive survey of seventy-four doctors in Wesley Guild Unit of OAUTHC was carried out. The questionnaire was self administered and the results were analyzed using SPSS package (Version 11). Frequencies and Pearson's correlation test of significance were done and the results displayed in tables and Charts. Results: Of the 74 doctors studied, 51(68.9%) and 23(31.1%) were males and females respectively. Mean age was 34.3±11 years. 73 % of respondents could use a word processor, 51.4% a statistical package while 42% could prepare power point slides. Also 44.6 % of respondents could access the internet at home, 28.4 % at work, 31.1% via mobile phones while 47.3 % use cyber cafes. Reading and sending emails is the predominant internet based activity (51.4 % of respondents). Of the 74 respondents, 59.5 % had electronic versions of some of their text books but only 52.7 % use them. Even though 45% were aware of free online self learning modules only 40% of the respondents participate. In all, only 12.2% of respondents felt paper based health records are better than electronic records. Conclusion: In today's world of increasing change and complexity, doctors in this study need to acquire more computer-related competencies and internet skills. [ABSTRACT FROM AUTHOR]

Abstract

Aim: To assess the attitude of clinical medical students in Obafemi Awolowo University towards rural medical practice in Nigeria. Materials & Methods: This cross-sectional study was carried out between July and September 2010, among the clinical medical students (400L, 500L and 600L). 130 subjects were selected by simple random sampling from the sample frame of 800 using the Abramson winpepi program software 2004. Data was collected with the aid of semi-structured self-administered questionnaire. The data was collated and analyzed with SPSS statistical software version 15.0.1 and Microsoft Excel (2007). Cross tabulation of results was done with chi-square test and statistical significance was set at p value = 0.05. Results: The selected subjects were from the clinical classes namely 400L (N=32, 24.2%), 500L (N= 45, 35.2%) and 600L (N= 53, 40.6%). Most were males (N= 80, 60.6%) and the rest were females (N= 50, 37.9%). The majority of the respondents (95%) originated from the rural communities in their states of origin but less than 5% of them grew up in the rural communities earlier indicated. 8.5% (N=11) (p-value of 0.0001) of the respondents agreed to practice in the rural community with others indicating a preference for practice in semi-urban and urban areas. 66.1% (N=86) of the respondents would request for reposting if posted to rural communities for medical practice after graduation (p- value =0.03). Conclusion: Most students expressed preference for practice in the urban areas. Intense efforts should be made by all stakeholders in encouraging rural medical practice. [ABSTRACT FROM AUTHOR]
