1. Unidirectional barbed sutures vs. interrupted intracorporeal knots in thoracoscopic repair of congenital diaphragmatic hernia in pediatrics

Abstract

BackgroundIntracorporeal suturing knots continue to be one of the most challenging and time-consuming steps in the thoracoscopic repair of congenital diaphragmatic hernia (CDH). Barbed unidirectional knotless sutures are designed to shorten surgical procedures by eliminating the need to tie knots. This work aimed to compare unidirectional barbed sutures and interrupted intracorporeal knots in the thoracoscopic repair of CDH in pediatrics regarding the time required to suture, operative time and complications.MethodsThis retrospective study included 139 patients presented with Bochdalek CDH. Patients were classified into early (neonatal) and late presentations. The hernia defect was repaired by unidirectional Barbed sutures (V-LocTM and StratafixTM sutures) in group B or by Conventional interrupted intracorporeal knots in group C.ResultsIn both early and delayed presentations, the time required to suture (15 and 13 min in group B, 33 and 28 min in group C for neonatal and delayed presentation respectively) was significantly shorter in group B. Complications (visceral perforation, wound infection, and recurrence) insignificantly differed between group B and group C of early presentation. No patients suffered from major complications in both groups.ConclusionsBoth unidirectional barbed sutures and intracorporeal knots were safe and effective. However, unidirectional barbed sutures are a time-saving choices for CDH thoracoscopic repair in early and late presentations.

Abstract

Abstract Background Central venous line insertion in neonates is an important and lifesaving procedure. It can carry significant risks and complications, including death, at the time of insertion or later. We aimed to retrospectively assess the modified Seldinger technique for open placement of a central venous catheter in neonates, regarding its safety, feasibility, operative time, and preservation of the patency of the internal jugular vein. This study was conducted on 120 neonates from March 2018 to March 2020. We closely monitored the pulse for the detection of arrhythmia or bradycardia, which might be caused during the insertion of the guide wire or the tip of the catheter. Post-operative X-ray was done immediately after the end of the procedure for all cases, to determine the site of the central venous catheter and to detect the presence of pneumothorax. Results Arrhythmia was observed in 9 cases (7.5%), and blood oozing in 5 cases (4.1%). There were 3 cases of pneumothorax (2.5%), 2 cases of neck hematoma in two cases (1.6%), 6 cases of internal jugular vein thrombosis (5%), and dislodging of the catheter in 3 cases (2.5%). There were no cases of arterial puncture, failure of cannulation, or haemothorax in our study. Conclusions The modified Seldinger technique insertion for open central venous line in neonates is a safe, accessible, and feasible method, especially in centers that lack the experience of ultrasound-guided insertion in neonates.

4. Thoracoscopic T2-T3 versus T4 sympathectomy for primary palmar hyperhidrosis in children and adolescents: a randomized comparative study

Abstract

Abstract Background Controversy exists regarding the appropriate level of sympathectomy for primary palmar hyperhidrosis (PH) as different levels are associated with variable postoperative patients’ satisfaction and potential complications. The aim of this study was to investigate the safety, efficacy, and outcome of T2-T3 thoracoscopic sympathectomy (TS) versus T4 TS in this age group. Results This prospective study included 32 patients (17 males and 15 females) with primary PH treated at the department of pediatric surgery, in our institution during the period from February 2019 to February 2020. Inclusion criteria included moderate and/or severe degrees of PH not responding to conservative measures. The patients were divided randomly into two groups: group I treated by T2-T3 TS and group II underwent only T4 TS. All patients were evaluated regarding operative details and postoperative outcome. Follow-up ranged from 6 to 24 months. Group I included 18 patients (14 operated on both sides and 4 operated on one side), and group II included 14 patients (11 operated on both sides and 3 operated on one side). The ages ranged between 5 and 18 years (mean 14.25 ± 3.14 years). The difference in mean age among both groups (14.5 versus 13.9 years) was not statistically significant. The mean operative time was significantly longer in group I (22.4 versus 17.2 min, p value 0.046). The hospital stay (1 day) was similar for both groups. Postoperative compensatory hyperhidrosis (CH) was more frequent in group I (n=7, 50% versus n=5, 45.5%), but the difference was not statistically significant. Postoperative over dryness occurred in 5 patients in group I (28.6%) and temporary Horner’s syndrome in one patient (7.14%). No over dryness or Horner’s syndrome occurred in any patient in group II. The QOL score has improved in both groups; the degree of improvements was better in group II. Conclusion Both T2-T3 TS and T4 TS are effective in treating primary palmar hyperhidrosis in children and adolescents. T4 TS is preferred than T2-3 TS due to less frequent postoperative complications and better patients’ satisfaction.

5. Coccygectomy for chronic refractory coccygodynia in pediatric and adolescent patients

Abstract

Aims: Coccygodynia (also known as coccydynia and coccyalgia) described for the first time by Simpson in 1859. It is a disabling pain in the coccyx usually provoked by sitting or changing position from a sitting to a standing position. Treatment started with adjustment of ergonomics as specialized cushion for sitting, application of local heat, and oral analgesics. The aim of this study was to assess the results of total or partial coccygectomy for patients suffering from chronic refractory coccygodynia and not improving on conservative treatment in pediatrics and adolescents. Methods: The study included 22 patients, 8 of them with chronic refractory coccygodynia not responding to conservative management underwent partial or total coccygectomy using Key's surgical exposure after good preoperative preparation. Results: This study included 5 females (62.5%) and 3 males (37.5%), their ages ranged from 7 to 15 years. The results were excellent in 75% of patients and good in the remaining cases. The mean length of hospital stay was 1.43 ± 0.98 days. No significant intraoperative or postoperative complications were reported. The parents were satisfied in almost all cases in the long term follow up. Conclusions: Coccygodynia is a disabling problem, especially in pediatric patients. Coccygectomy is a feasible management option and has a favorable outcome when performed for an appropriate indication.

Abstract

Abstract Background Meckel’s diverticulum (MD) is the commonest congenital abnormality of the gastrointestinal tract that occurs in 2% of general population. It remains asymptomatic, but it may lead to life-threatening complications. These complications may be misdiagnosed with other gastrointestinal disorders like acute appendicitis, making its diagnosis challenging among pediatricians and pediatric surgeons. In this study, we reported five cases with different presentations of complicated MD in children. Results Five patients with different presentations of MD were reported during the period from January 2016 to January 2020. Patients’ demographics, clinical presentations, investigations, operative data, and postoperative outcome were recorded and analyzed. Conclusions The present study highlights different presentations of MD. Surgical interference is the main key of treatment of symptomatic MD either by wedge resection of a small base diverticulum or by resection anastomosis of the small intestine in wide base and inflamed diverticulum.

12. Pediatric combined laparoscopic splenectomy and cholecystectomy performed through four ports

Abstract

Combined laparoscopic splenectomy and cholecystectomy (CLSC) seems to be the appropriate solution in children that require splenectomy for various pathologies and also have cholelithiasis. This study aimed to evaluate the efficacy, safety and feasibility of CLSC in pediatrics through four ports. Nineteen pediatric patients with hematological disorders underwent CLSC by a 4-port technique, from December 2018 to November 2020. The patients were initially positioned in a right lateral position. The first port was inserted at the umbilicus, the second in the subxiphoid region to the left side of the falciform ligament, the third midway between the umbilical port and the subxiphoid port, and the fourth in the left sub-umbilical region. The operating table was tilted to the right to first perform the splenectomy, after which it was tilted to left to perform the cholecystectomy. For the cholecystectomy, the subxiphoid port was passed through the falciform ligament to the right side. The mean age of the patients was 9.96 years and the mean splenic volume was 327.54 cm3. The mean total operative time was 128.71 min. There was no intra-operative visceral or vascular injury. All procedures were completed without conversions. The mean post-operative hospital stay was 2.28 days. During the follow-up period, no complications were detected. CLSC using four ports is a safe and a feasible procedure, with better cosmetic results, and easy to perform in well-trained hands. PACTR201901796143495.

14. Male Circumcision: Contemporary Practice Pattern of the Egyptian Pediatric Surgeons

Abstract

Background: To determine the practice pattern of male circumcision among pediatric surgeons in Egypt. Aim of Study: This study proved a shortage of communi-cation between the surgeons and the parents. And it was limited by the inability to correlate the clinical practice with the outcome results and the fact that not all members of EPSA responded to e-mails or personal communication. Patients and Methods: During 33rd Congress of Egyptian Pediatric Surgical Association in 2018, the pediatric surgeons were asked to fill a 22-question survey about their practice in male circumcision. Results: Correct responses were received from 126 sur-geons; 27% of responders operated more than 150 cases per year, 35% performed circumcision at the neonatal period, 45.2% used bone cutting forceps, 88.1% preferred electrocau-tery for sealing minute bleeding vessels; (81.7% use bipolar diathermy and only 6.4% used monopolar diathermy), 8% of the responders never used sutures in circumcision. There is a wide variation in the type of anesthesia and analgesia used. 61% recommended removal of the dressing after 24 hours. 70.4% of the responders regularly reviewed the cases in the outpatient clinics. Conclusion: The current survey reflects the diversity in the practice patterns of circumcision among pediatric surgeons in Egypt. Guillotine method using bone cutting forceps still the commonest method used. Other methods are limited due to the unavailability of the devices in most institutions. The pain control measures and the post-procedure care should be improved.

Abstract

Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Abstract

In this research paper, we discuss a new accurate algorithm to solve the steady-state nonlinear Michaelis-Menten pharmacokinetics equation model problem, which depicts oxygen diffusion in a planar cell. Michaelis–Menten kinetics of enzymatic reaction was generated by diffusion equation with a non-linear term. Approximate analytical solution of nonlinear Michaelis-Menten Pharmacokinetics is constructed using homotopy analysis method for a boundary value problem. The validity of the acquired solution is demonstrated via numerical results.

Abstract

In July 2017 — while preparing this article for publication — a two-week standoff occurred between Palestinian worshippers in Jerusalem and the Israeli government, which sought to impose new restrictions on Palestinian Muslim access to the Al-Aqsa Mosque. Thus, this article has two parts. The initial version, providing a settler-colonial reading of the conflict over Jerusalem utilising three concepts: the ‘frontier’, ‘othering’ in Orientalism, and a Foucauldian application of the creation of docile subjects through an intrusive surveillance structure. The second part, the epilogue, highlights the standoff and concludes that the Palestinian self-mobilisation in Jerusalem demonstrated an ability to break loose from the intense Israeli settler-colonial dynamics.

Abstract

Background: The burden of the spread of the COVID-19 pandemic has impacted widely on the healthcare providers physically and mentally. Many healthcare providers are exposed to psychological stressors due to their high risk of contracting the virus. Aims: This study aimed to measure the level of anxiety among healthcare providers during the COVID-19 pandemic in Saudi Arabia. In addition, this study aimed to measure the level of anxiety based on demographic characteristics. Method: A cross-sectional survey was employed to recruit a convenience sample of healthcare providers. A pencil and paper self-administered questionnaires were used to collect data from demographic and generalized anxiety disorder GAD-7 data. However, this study received written informed consent from participants of the study. In addition, the study was approved by the Institutional Review Board at Dr. Sulaiman Al Habib Medical Group (IRB Log No. RC20.06.88-03). Results: A total of 650 participants were recruited, results of GAD-7 showed that 43.5%, 28.9% and 27.5% of healthcare providers in Saudi Arabia experienced mild, moderate and severe anxiety, respectively, during the COVID-19 pandemic. Results indicated that age, health specialty, nationality, and sleeping disorders before COVID-19 were associated with anxiety levels. Conclusion: The generalized anxiety among healthcare providers in Saudi Arabia was mild. Older healthcare providers were found to have a higher level of anxiety compared to other participating healthcare providers. Several factors may contribute to a higher level of anxiety including age, socioeconomic status, marital status, having chronic conditions, and sleeping disorder before the COVID-19 pandemic. Abstract

Background and Objectives: COVID-19 is a novel infectious disease caused by a singlestranded RNA coronavirus called severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We aimed to conduct a nationwide multicenter study to determine the characteristics and the clinical prognostic outcome of critically ill COVID-19 patients admitted to intensive care units (ICUs). Materials and Methods: This is a nationwide cohort retrospective study conducted in twenty Saudi hospitals. Results: An analysis of 1470 critically ill COVID-19 patients demonstrated that the majority of patients were male with a mean age of 55.9 ± 15.1 years. Most of our patients presented with a shortness of breath (SOB) (81.3%), followed by a fever (73.7%) and a cough (65.1%). Diabetes and hypertension were the most common comorbidities in the study (52.4% and 46.0%, respectively). Multiple complications were observed substantially more among non-survivors. The length and frequency of mechanical ventilation use were significantly greater (83%) in the nonsurvivors compared with the survivors (31%). The mean Sequential Organ Failure Assessment (SOFA) score was 6 ± 5. The overall mortality rate of the cohort associated with patients that had diabetes, hypertension and ischemic heart disease was 41.8%. Conclusion: Age; a pre-existing medical history of hypertension, diabetes and ischemic heart disease; smoking cigarettes; a BMI ± 29; a long mechanical ventilation and ICU stay; the need of ventilatory support; a high SOFA score; fungal co-infections and extracorporeal membrane oxygenation (ECMO) use were key clinical characteristics that predicted a high mortality in our population. Abstract

A novel series of [1,2,4]triazolo[4,3-a]quinoxaline derivatives of different heteroaromatization members were synthesized. The newly synthesized molecules were explored for their potential antimicrobial activities against a panel of pathogenic organisms. Among these derivatives, the chalcone compound 6e with a methoxy substituent exhibited broad potent antimicrobial activity against most of the bacterial and fungal strains. Furthermore, the analysis of the SAR disclosed that the linker and terminal aromatic fragments perform critical roles in exerting antibacterial activity. The molecular docking calculations were executed on two of the most bacterial targets, ATP-binding sites of DNA gyrase B, and the folate-binding site of DHFR enzymes. The results presented good binding data to the pockets of both enzymes showing different linkers contributions through the hydrogen-bonding and aromatic stacking interactions that stabilize the compounds in their pockets taking 6e compound as representative of most active analogs. In addition, good pharmacokinetic profiling data for the 6e compound was obtained and compared to reference drugs. Accordingly, our findings suggest that [1,2,4]triazolo[4,3-a]quinoxaline scaffold is an interesting precursor for the design of potent antimicrobial agents with multitarget inhibition.

27. Exploring the effectiveness of Continuing Medical Education programs accredited by UAE University on Health care professional’s knowledge: Expectations vs. satisfaction

Abstract

It is common notion that Continuing Medical Education (CME) has positive effects on the knowledge and competencies of health care professionals (HCP), however, this notion has not yet been fully proven in the United Arab Emirates (UAE) health care context as there is very limited research conducted in this area to confirm this notion. The aim of this research study is to investigate the effects of CME on the knowledge and competency of HCP in the UAE based the UAE University (UAEU) accredited postgraduate CME programs in the Emirates of Abu Dhabi for healthcare professionals. Measuring the effectiveness of educational programs is a consensus by all education theorists, the first two level of Kirkpatrick training evaluation model was used to in this study. Based on a mixed method approach, quantitative data was collected from 302 health care professionals (doctor, nurse, pharmacist, others) who attended 4 CME accredited programs by UAEU, in Al Ain city and qualitative data were collected from 14 health care professionals using semi structured interviews with participants who agreed to be interviewed. The research results showed that the UAEU accredited CME programs were effective in terms of exceeding health care professionals learning expectations, the achievement of educational programs learning objectives, and significant increase in HCP knowledge in the topics discussed in these programs. However, the results also indicate that the need for training, patient no compliance and insurance cost were the major barriers to transfer learning from CME experience to their work place; 95% of the participants specified these as barriers. The study also provides recommendations for improving CME programs. These programs should be designed to meet the special and individual learning needs of HCPs, and the outcomes should be evaluated using professional appraisals. Abstract

AbstractIntroduction: Changing health care requires changing medical education. In this position paper it is suggested that subsequent innovations in medical education each had their specific strengths and shortcomings. What they have, however, in common is that they place the medical student and their competencies at their center. Innovation in medical education is inward looking. Discussion: The authors propose a perspective on the medical curriculum in which the patient, their family, and the surrounding community take center stage. They argue that present medical education cannot adequately respond to the great challenges to population health: an aging population, the obesity epidemic, and future pandemics of new diseases due to population growth, urbanization, and antimicrobial resistance, particularly because these challenges cannot be dealt with by the medical sciences alone but need deep understanding of the social sciences as well. In addition, the practice of health care is changing: effective health care demands a close partnership between the health care system and the medical school which is mostly lacking, cooperation with other health professions is becoming more and more necessary in response to the increasing complexity of health care, patients and their families are required to play a more active role in their health, medical error threatening patient safety is becoming to be seen as a huge problem, and the emergence of artificial intelligence in education and practice, all requiring transformation of medical education. Conclusion: The present contribution suggests eight such transformations necessary to create a truly patient- and population-centered medical curriculum. [ABSTRACT FROM AUTHOR]
