A 16-year-old high school wrestler was examined because of a complaint of pain at his right elbow when training with weights. The condition had begun about two months before, but was unrelated to a specific traumatic injury. X-rays revealed a displacement (olecranon avulsion) of the olecranon growth plate, an area of bony growth in children. This area is composed of a cartilaginous plate interpositioned between the shaft of the bone (diaphysis) and a small end piece of bone (the epiphysis). The olecranon growth plate is obliterated, and the body epiphysis fuses to the diaphysis at about 14 years in girls, and at about 16 years in boys. This patient was conservatively treated with a soft cast, but healing was incomplete. Ultimately, the boy had surgery where the ends of the bone were wired together in a conventional manner; this surgical treatment resulted in a complete cure. Adolescents who complain of persistent elbow pain should be thoroughly examined and tested to rule out this type of injury and to prevent the problems that can result from these avulsions. Avulsion fractures of the tibial tubercle are very uncommon injuries, with only 170 cases reported since 1853 [i],[ii],[iii]. They account for less than 3% of all physeal injuries in children [iv]. These injuries are extremely rare in adults, with only case reports in the literature. The mechanism of injury in adults is likely to be related to direct injury to the tubercle, rather than by indirect contraction of the quadriceps muscle as seen in children [v]. We described a case report of an 88-year-old female who suffered an avulsion fracture of the tibial tubercle after a simple fall. We performed an open reduction and internal fixation of the tibial tubercle using a cannulated screw and tension-band wire. Avulsion fracture of the base of the second metacarpal is an uncommon injury because of the stability of the carpometacarpal joints. We present a case of fracture of the index metacarpal base with avulsion of the extensor carpi radialis longus tendon. All the previous reports have described fall on flexed wrist as mechanism of injury. This case represents the injury as a result of motor vehicle accident (dashboard injury). It was treated with open reduction and internal fixation using a screw and suture anchor. After surgery, the patient regained full function with normal range of movement and power of the wrist. One of the most common hand injuries incurred by skiers is skier's thumb. The condition may occur when the thumb is forcibly abducted, that is, pulled away from the hand's central axis, by the pressure of the skier's pole as the skier falls. The resulting injury is usually either to the ulna collateral ligament, a main ligament of the wrist, or to the ligament connecting the metacarpal bone in the hand to the proximal phalanx, the bone at the base of the finger. A system of diagnosis and classification is presented. X-rays of the thumb are important because the injury can be complicated by the fracture of the thumb. A case is discussed in which an avulsion fracture, separation of ligament with bone, of the thumb was observed. Treatment generally involves immobilization by means of cast or splint, followed by periods of support by elastic bandages. The treatment for each class of injury should be adjusted according to experience. For the most serious type of injury, where the ligaments have been avulsed or transected, surgery is generally required. In many cases physical therapy is also recommended. Prognosis for return of function is generally excellent, and most individuals are able to regain sufficient power in their hands to return to the sport.
