Abstract

Atypical hemolytic uremic syndrome (aHUS) is a rare cause of thrombotic microangiopathy (TMA), characterized by microangiopathic hemolytic anemia, consumptive thrombocytopenia, and multisystem end organ involvement, most commonly affecting the kidney. Diagnosis is clinical, after exclusion of other TMA causes. Primary aHUS arises from genetic abnormalities, resulting in uncontrolled complement activity, while a variety of clinical scenarios cause secondary aHUS, including infection, pregnancy, malignancy, autoimmune disease, and medications. They can also induce a temporary complement deregulation with an overlap between both scenarios, which can make differential diagnosis difficult. Primary aHUS can be sporadic or familial and is associated with a high rate of progression to ESRD. Many aHUS patients relapse in the native or transplanted kidneys, leading to kidney failure. The introduction of eculizumab has changed the prognosis of aHUS, by inducing hematologic remission, improving or stabilizing kidney functions, and preventing graft failure. The early institution of appropriate therapy can prevent multiorgan damage, so is essential to recognize and differentiate the TMA syndromes. Eculizumab is considered now the first-line treatment, and it is recommended lifelong therapy. However, the high cost of therapy has led to make efforts to develop precise complement functional and genetic studies that help physicians to determine the appropriate duration of eculizumab therapy. Nowadays, more studies are needed to select candidates to adjustment of therapy.

BACKGROUND Proteinuria changes have a prognostic significance in proteinuric nephropathies. Proteinuria has been related to kidney transplant outcomes, but there are no information about the impact of increasing proteinuria during the first year on long-term graft and patient survival. Abstract

Background Atypical haemolytic uraemic syndrome (aHUS) is a rare, life-threatening, chronic thrombotic microangiopathy (TMA) caused by uncontrolled complement dysregulation. Eculizumab, a humanised anti-C5 monoclonal antibody, is the only approved treatment of aHUS. Purpose To assess the effectiveness and safety of eculizumab in patients with aHUS and/or TMA. Material and methods A retrospective observational study was conducted from June 2012 to September 2015 in a general university hospital, including patients diagnosed with aHUS and/or TMA. All patients received an induction period of 900 mg eculizumab weekly (weeks 1–4) followed by a maintenance period of 1200 mg in week 5 and then 1200 mg every 2 weeks. Demographic (sex and age) and clinical data (platelet count, haemoglobin, lactate dehydrogenase (LDH), haptoglobin and renal function) were systematically collected at baseline and during treatment. Effectiveness was assessed by complete response (normalisation of haematologic parameters combined with an improvement in renal function), haematologic response (normalisation of platelet count and LDH) and TMA event free status (no decrease in platelet count of >25%, no plasma exchange (PE) and no dialysis). Adverse events were registered. Results Six patients were included: 4 men, aged 34 ± 7years, 4 diagnosed with aHUS and 2 with post-transplant TMA. Five patients received PE and dialysis prior to eculizumab treatment. Clinical data at baseline were: platelet count (138 ± 65×10 9 /L), haemoglobin (8.8 ± 1.0 g/dL), LDH (320.3 ± 269.2UI/L), haptoglobin (47.3 ± 38.5 mg/dL) and creatinine (6.3 ± 2.8 mg/dL). After the induction period, complete response was achieved in 3/6 patients, haematologic response in 4/6 patients and TMA event free status in 5/6 patients. Treatment response was maintained in all patients during the follow-up period (median 33 weeks; min 7, max 156). There were no adverse events. Conclusion Eculizumab showed effectiveness and safety profiles consistent with those seen in previous clinical trials, showing that it is a well tolerated and effective drug in patients with aHUS and post-transplant TMA. Abstract

Background: Anti-human leukocyte antigen antibodies (HLA Abs) have been associated with reduced kidney allograft survival. Our aim was to analyze the prevalence and impact on allograft function of donor-specific HLA antibodies (DSA) among a cohort of kidney transplant recipients., Patients and Methods: The 321 recipients had received deceased-donor kidneys followed for a median of 70 ± 43 months. We performed a cross-sectional analysis of the presence of HLA Abs with the use of Luminex technology., Results: Fifty patients (15.6%) displayed HLA Abs after transplantation including 21 (6.7%) as de novo HLA Abs. Eight patients (2.5%) developed DSA, and 42 (13%) showed no DSA. We compared 3 groups of patients: with DSA, without DSA, and without HLA sensitization. The DSA patients were younger (P = .03) with a higher percentage of men (P = .00), and having received less frequent induction treatment with basiliximab or thymoglobulin (P = .02). Patients without DSA revealed a higher percentage of pretransplantation HLA sensitization (P = .00), more pretransplantation transfusions (P = .08), and more frequent retransplantations (P = .00). The incidence of acute rejections was higher for DSA patients (P = .02) than for the other 2 groups, behaving as an independent risk factor (relative risk, 4.7; 95% confidence interval, 1.1-18.8; P = .03). Graft survival at 5 years was lower among patients with compared to those without HLA Abs (P = .00)., Conclusions: HLA donor-specific sensitization, an uncommon situation in our study, was associated with younger male recipients and less induction treatment. An acute rejection episode was an independent risk factor for the development of DSA; therefore, we think that monitoring of HLA Abs should be included in evaluation of the early postransplantation period., (Copyright © 2011 Elsevier Inc.
