Abstract

The number of children and adolescents seen in emergency departments (EDs) and primary care settings for mental health problems has skyrocketed in recent years, with up to 23% of patients in both settings having diagnosable mental health conditions. 1 -4 Even when a mental health problem is not the focus of an ED or primary care visit, mental health conditions, both known and occult, may challenge the treating clinician and complicate the patient's care.4 Although the American Academy of Pediatrics (AAP) has published a policy statement on mental health competencies and a Mental Health Toolkit for pediatric primary care providers, no such guidelines or resources exist for clinicians who care for pediatric mental health emergencies. 5, 6 Many ED and primary care physicians report paucity of training and lack of confidence in caring for pediatric psychiatry patients. The 2 clinical reports support the 2006 joint policy statement of the AAP and the American College of Emergency Physicians on pediatric mental health emergencies, 7 with the goal of addressing the knowledge gaps in this area. Although written primarily from the perspective of ED clinicians, it is intended for all clinicians who care for children and adolescents with acute mental health and behavioral problems. They are organized around the common clinical challenges pediatric caregivers face, both when a child or adolescent presents with a psychiatric chief complaint or emergency (part I) and when a mental health condition may be an unclear or complicating factor in a non-mental health ED presentation (part II). Part I of the clinical reports includes discussions of Medical Clearance of Pediatric Psychiatric Patients, Suicide and Suicidal Ideation, Restraint of the Agitated Patient Including Verbal, Chemical, and Physical Restraint, and Coordination of Care With the Medical Home, and it can be accessed online at www.pediatrics.org/cgi/doi/10.1542/peds. 2016-1570. Abstract

Emergency physicians have used point-of-care ultrasonography since the 1990 s. Pediatric emergency medicine physicians have more recently adopted this technology. Point-of-care ultrasonography is used for various scenarios, particularly the evaluation of soft tissue infections or blunt abdominal trauma and procedural guidance. To date, there are no published statements from national organizations specifically for pediatric emergency physicians describing the incorporation of point-of-care ultrasonography into their practice. This document outlines how pediatric emergency departments may establish a formal point-of-care ultrasonography program. This task includes appointing leaders with expertise in point-of-care ultrasonography, effectively training and credentialing physicians in the department, and providing ongoing quality assurance reviews.

Abstract

Abstract Acute cough, a common complaint in young children, is often the result of a viral upper respiratory infection. Cough and cold remedies generate billions of dollars in annual sales in the United States, despite a lack of evidence of their efficacy and multiple warnings by the US Food and Drug Administration. The current article begins with the best available evidence for common over‐the‐counter (OTC) and prescription antitussive remedies in children. The article concludes with a discussion of the pros and cons for the use of antitussives in children with cough. In general, OTC antitussive medications should not be routinely used in children under 2 years of age. In certain cases, antitussives with minimal adverse profile and some evidence of benefit may be recommended after informed counseling.

Abstract

Background: An increased risk of contracting HIV infection, suboptimal adherence, and a loss to follow-up have been observed in migrants, particularly if those individuals are transgender or sex workers. A clear picture of the HIV epidemic among migrants is complex due to the lack of specific national data. Aims: We developed a qualitative study that describes the barriers and facilitators (cultural, social, and personal) in HIV testing and the continuum of care for a group of migrant transgender women who are sex workers. Methods: A semi-structured interview was conducted with a group of migrant transgender women who are sex workers living with HIV or with unknown HIV serostatus residing in the Florentine metropolitan area. Results: We included 12 participants: 3 had unknown HIV serostatus and 9 were living with HIV in follow-up at the Clinic of Infectious and Tropical Diseases, Careggi University hospiral, Florence, Italy. Among barriers, the perceived stigma due to their identity as migrants and transgender people, the language lack of ability and the legal position in the host country played a significant role. Moreover, the interviewees claimed having no alternative to sex work: for those individuals, changing their lifestyle condition is perceived as difficult or impossible due to social prejudices. Conversely, the interviewees considered support services, such as cultural mediators/interpreters and street units, as facilitators to HIV testing, access to care, and continuum of care. Having regular and accessible ART and the availability of a more consistent health care system, represent reasons for HIV-positive migrants living with HIV to move to Italy. Conclusions: Knowledge of this population’s personal experience regarding the barriers and factors that facilitate access to the HIV care system is essential for planning public health interventions capable of responding to the real needs of patients. Abstract

Introduction: Safinamide is a recent antiparkinsonian drug that modulates both dopaminergic and glutamatergic systems with positive effects on motor and nonmotor symptoms of Parkinson's disease (PD). Here, we aimed to describe the efficacy and safety of safinamide in the Italian PD patients in real-life conditions. Methods: We performed a sub-analysis of the Italian cohort of the SYNAPSES study, a multi-country, multi-center, retrospective-prospective cohort observational study, designed to investigate the use of safinamide in routine clinical practice. Patients received for the first time a treatment with safinamide and were followed up for 12 months. The analysis was conducted on the overall population and in subgroups of interest: i) patients > 75 years, ii) patients with relevant comorbidities and iii) patients affected by psychiatric symptoms. Results: Italy enrolled 616/1610 patients in 52 centers, accounting for 38% of the entire SYNAPSES cohort. Of the patients enrolled, 86.0% were evaluable at 12 months, with 23.3% being > 75 years, 42.4% with psychiatric conditions and 67.7% with relevant comorbidities. Safinamide was effective on motor symptoms and fluctuations as measured through the Unified PD rating scale III and IV scores, and on the total score, without safety issues in none of the subgroups considered. Conclusion: The SYNAPSES data related to Italian patients confirms the good safety profile of safinamide even in special groups of patients. Motor fluctuations and motor impairment improved at the follow-up suggesting the significant role of safinamide in managing motor symptoms in PD patients. [ABSTRACT FROM AUTHOR]

Abstract

This article focuses on the sustainable transformation of Nunavik’s Northern Villages with regard to the notions of dwelling, urbanity and territoriality. Our participatory design research approach addressed the limits of current planning frameworks, exogenous development models, and a complex governance system by integrating the relationship to the territory and the impacts of climate change in the exploration of scenarios for alternative futures adapted to local urban realities. The research illustrates these limits and complexities with hypothetical transformation scenarios in Inukjuak and Salluit. By identifying plausible futures, we formulated sustainable planning strategies combining interrelated factors in the development of project-based interventions to incorporate traditional practices in an urbanizing context. The case of Inukjuak illustrates the importance of natural environments, urban consolidation practices, diversity of use, and socialization, while that of Salluit demonstrates the reciprocity between geomorphological, constructive, socio-cultural, and logistical design variables. Evolving Northern development challenges require that the relevance of these scenarios be examined based on alternative hypotheses and long-term horizons, in the imagination of shared strategies for sustainable planning. Design research, the use of decision-making tools and participatory frameworks are questioned in relation to their contribution to the self-determination of local communities in the “resituation” of their living environments., Cet article porte sur la transformation durable des villages nordiques du Nunavik au regard des notions d’habitation, d’urbanité et de territorialité. Notre approche de recherche en design participatif a abordé les limites des cadres de planification actuels, des modèles de développement exogènes et d’un système de gouvernance complexe en intégrant la relation au territoire et les impacts du changement climatique dans l’exploration de scénarios pour des futurs alternatifs adaptés aux réalités urbaines locales. La recherche illustre ces limites et ces complexités par des scénarios hypothétiques de transformation à Inukjuak et Salluit. En identifiant des futurs plausibles, nous avons formulé des stratégies de planification durable combinant des facteurs interdépendants dans le développement d’interventions basées sur des projets visant à intégrer les pratiques traditionnelles dans un contexte d’urbanisation. Le cas d’Inukjuak illustre l’importance des environnements naturels, des pratiques de consolidation urbaine, de la diversité des usages et de la socialisation, tandis que celui de Salluit démontre la réciprocité entre les variables de conception géomorphologiques, constructives, socio-culturelles et logistiques. L’évolution des enjeux du développement du Nord nécessite d’examiner la pertinence de ces scénarios à partir d’hypothèses alternatives et d’horizons à long terme, dans l’imagination de stratégies partagées de planification durable. La recherche en design, l’utilisation d’outils d’aide à la décision et les cadres participatifs sont questionnés par rapport à leur contribution à l’autodétermination des communautés locales dans la « resituation » de leurs milieux de vie.

Abstract

Des atlas paysagers – des recueils photographiques présentant des adéquations entre territoire et habiter – peuvent engager les populations locales dans un processus participatif et contribuer à l’aménagement de l’habitat. Suivant une approche perceptive empruntée à l’école territorialiste, deux méthodes exploratoires de collecte de photographies ont interpelé des acteurs aux connaissances contextuelles et expertes pour l’assemblage d’atlas. Ils illustrent des informations déterminantes pour un aménagement local approprié : l’état de « l’enregistrement inachevé » qu’est le paysage – la « manifestation tangible du territoire ». Les photographies issues d’un concours en ligne et d’excursions par l’auteur permettent d’identifier des caractères paysagers qui ajoutent à la compréhension de l’urbanité des communautés du Nunavik. Ces caractères, observés de façon synchronique, concernent des aspects formels et syntaxiques qui émanent de pratiques du territoire dans le contexte local. Complétées par des observations diachroniques et une approche morphotypologique, ces connaissances permettraient d’adapter des pratiques actuelles en design urbain pour s’adjoindre à un « urbanisme inuit » informé des règles de formation inhérentes à la phase actuelle d’occupation du territoire. Les méthodes explorées ici, croisées d’activités de cartographie communautaire, pourraient constituer le rôle de comités citoyens dans la production de l’habitat et la formulation de scénarios stratégiques autodéterminés, partagés et co-construits., Landscape atlases – photographic collections linking the territory and its occupation – can engage local populations in a participatory process and contribute to the planning of their habitat. Following a perceptive approach borrowed from the territorialist school, two exploratory methods of photograph collection challenged agents with contextual and expert knowledges for the assembly of atlases. They illustrate essential information for an adapted local planning: the “record” of an ever-transforming landscape – the “tangible manifestation of the territory”. Photographs from an online competition and from excursions by the author allow the identification of recurring landscape characters, which inform on the urbanity of Nunavik communities. These characters, observed synchronically, concern formal and relational aspects which define the profound ways of dwelling in the local territorial context. Completed with diachronic observations and a morphotypological approach, this knowledge could incorporate current practices of urban design into an “Inuit urbanism”, informed by the rules of formation obtaining in the current phase of occupation of the territory. These methods, along with community mapping activities, could constitute the role of citizen committees in the production of their habitat and the formulation of shared, self-determined, co-constructed strategic scenarios.

Abstract

We evaluated the effectiveness and safety of elexacaftor/tezacaftor/ivacaftor (ELX/TEZ/IVA) in three subjects carrying the Phe508del/unknown CFTR genotype. An ex vivo analysis on nasal epithelial cells (NEC) indicated a significant improvement of CFTR gating activity after the treatment. Three patients were enrolled in an ELX/TEZ/IVA managed-access program, including subjects with the highest percent predicted Forced Expiratory Volume in the 1st second (ppFEV1) < 40 in the preceding 3 months. Data were collected at baseline and after 8, 12 and 24 weeks of follow-up during treatment. All patients showed a considerable decrease of sweat chloride (i.e., meanly about 60 mmol/L as compared to baseline), relevant improvement of ppFEV1 (i.e., >8) and six-minute walk test, and an increase in body mass index after the first 8 weeks of treatment. No pulmonary exacerbations occurred during the 24 weeks of treatment and all domains of the CF Questionnaire-Revised improved. No safety concerns related to the treatment occurred. This study demonstrates the benefit from the ELX/TEZ/IVA treatment in patients with CF with the Phe508del and one unidentified CFTR variant. The preliminary ex vivo analysis of the drug response on NEC helps to predict the in vivo therapeutic endpoints.
