Abstract

Humans estimate sound-source directions by combining prior beliefs with sensory evidence. Prior beliefs represent statistical knowledge about the environment while sensory evidence is acquired from auditory features such as interaural disparities and monaural spectral shapes. Models of directional sound localization often impose constraints on the contribution of these features to either the horizontal or vertical dimension. Instead, we propose a Bayesian model that more flexibly incorporates each feature according to its spatial precision and integrates prior beliefs in the inference process. We applied the model to directional localization of a single, broadband, stationary sound source presented to a static human listener in an anechoic environment. We simplified interaural features to be broadband and compared two model variants, each considering a different type of monaural spectral features: magnitude profiles and gradient profiles. Both model variants were fitted to the baseline performance of five listeners and evaluated on the effects of localizing with non-individual head-related transfer functions (HRTFs) and sounds with rippled spectrum. The model variant with spectral gradient profiles outperformed other localization models. This model variant appears particularly useful for the evaluation of HRTFs and may serve as a basis for future extensions towards modeling dynamic listening conditions.

Abstract

This paper describes an open-source Python framework for handling datasets for music processing tasks, built with the aim of improving the reproducibility of research projects in music computing and assessing the generalization abilities of machine learning models. The framework enables the automatic download and installation of several commonly used datasets for multimodal music processing. Specifically, we provide a Python API to access the datasets through Boolean set operations based on particular attributes, such as intersections and unions of composers, instruments, and so on. The framework is designed to ease the inclusion of new datasets and the respective ground-truth annotations so that one can build, convert, and extend one's own collection as well as distribute it by means of a compliant format to take advantage of the API. Abstract

Measuring and understanding spatial hearing is a fundamental step to create effective virtual auditory displays (VADs). The evaluation of such auralization systems often requires psychoacoustic experiments. This process can be time consuming and error prone, resulting in a bottleneck for the evaluation complexity. In this work we evaluated a probabilistic auditory model for sound localization intended as a tool to assess VAD's abilities to provide static sound-localization cues to listeners. The outcome of the model, compared with actual results of psychoacoustic experiments, shows the advantages and limitations of this systematic evaluation. Aim To assess the prevalence, risk and management of hyperglycemia in patients with acute coronary syndrome (ACS). Methods and Results Design: a multicenter prospective observational study of a representative sample of patients with ACS consecutively admitted to intensive cardiac care units (ICCU). Setting: 31 out of 61 ICCUs in Lombardy, the most heavily populated Italian region. From May 2009 to April 2010 1260 patients (69.4% male; mean age 68 ± 13 years) were included in the study: 301 (23.9%) were known diabetic patients (D) and 265 (21.0%) had hyperglycemia (H) (blood glucose >180 mg/dL) at hospital admission, 174 with a history of diabetes (D+H+) and 91 without (D−H+). On the first day after admission intravenous insulin infusion was prescribed to 72 D+H+ (41.4%) and 10 D−H+ (11.0%), according to different protocols. Approximately one third of D+H+ patients (59) and one fifth (17) of D−H+ maintained mean blood glucose higher than 180 mg/dL during the first day in the ICCU. Patients with diabetes or hyperglycemia had a higher incidence of major adverse cardiovascular events or death in hospital. However, at multivariable analysis neither diabetes nor blood glucose at admission was associated with a poor prognosis whereas mean blood glucose on the first day was an independent negative prognostic predictor (OR 1.010, 95% CI 1.002–1.018, p = 0.016). Conclusion Hyperglycemia is frequent in patients with ACS and is independently associated with a poor in-hospital prognosis if it persists in first day. Unfortunately, however, this condition is still poorly treated, with far from optimal blood glucose control.

Abstract

In a previous experiment we have measured the subjective perception of auditory lateralization in listeners who were exposed to binaural piano tone reproductions, under different conditions (normal and reversed-channel listening, manual or automatic tone production by a Disklavier, and disclosure or hiding of the same keys when they were autonomously moving during the automatic production of a tone.) This way, participants were engaged in a localization task under conditions also involving visual as well as proprioceptive (that is, relative to the position and muscular effort of their body parts) identification of the audio source with the moving key, even when the binaural feedback was reversed. Their answers, however, were clustered on a limited region of the keyboard when the channels were not reversed. The same region became especially narrow if the channels were reversed. In this paper we report about an acoustic analysis of the localization cues conducted on the stimuli that have been used in the aforementioned experiment. This new analysis employs a computational auditory model of sound localization cues in the horizontal plane. Results suggest that listeners used interaural level difference cues to localize the sound source, and that the contribution of visual and proprioceptive cues in the localization task was limited especially when the channels were reversed., + ID: 561471 + PeerReviewed

Abstract

An extensive piano sample library consisting of binaural sounds and keyboard vibration signals is made available through an openaccess data repository. Samples were acquired with high-quality audio and vibration measurement equipment on two Yamaha Disklavier pianos (one grand and one upright model) by means of computer-controlled playback of each key at ten different MIDI velocity values. The nominal specifications of the equipment used in the acquisition chain are reported in a companion document, allowing researchers to calculate physical quantities (e.g., acoustic pressure, vibration acceleration) from the recordings. Also, project files are provided for straightforward playback in a free software sampler available for Windows and Mac OS systems. Abstract

Aim: Insulin is the preferred treatment for the control of diabetes in hospital, but it raises the risk of hypoglycaemia, often because oral intake of carbohydrates in hospitalized persons is lower than planned. Our aim was to assess the effect on the incidence of hypoglycaemia of giving prandial insulin immediately after a meal depending on the amount of carbohydrate ingested. Methods: A prospective pre–post intervention study in hospitalized persons with diabetes eating meals with stable doses of carbohydrates present in a few fixed foods. Foods were easily identifiable on the tray and contained fixed doses of carbohydrates that were easily quantifiable by nurses as multiples of 10 g (a 'brick'). Prandial insulin was given immediately after meals in proportion to the amount of carbohydrates eaten. Results: In 83 of the first 100 people treated with the 'brick diet', the oral carbohydrate intake was lower than planned on at least one occasion (median: 3 times; Q1–Q3: 2–6 times) over a median of 5 days. Compared with the last 100 people treated with standard procedures, postprandial insulin given on the basis of ingested carbohydrate significantly reduced the incidence of hypoglycaemic events per day, from 0.11 ± 0.03 to 0.04 ± 0.02 (P < 0.001) with an adjusted incidence rate ratio of 0.70 (95% confidence interval 0.54–0.92; P = 0.011). Conclusions: In hospitalized persons with diabetes treated with subcutaneous insulin, the 'brick diet' offers a practical method to count the amount of carbohydrates ingested, which is often less than planned. Prandial insulin given immediately after a meal, in doses balanced with actual carbohydrate intake reduces the risk of hypoglycaemia. Abstract

To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Abstract

Skilled interactions with sounding objects, such as drumming, rely on resolving the uncertainty in the acoustical and tactual feedback signals generated by vibrating objects. Uncertainty may arise from mis-estimation of the objects’ geometry-independent mechanical properties, such as surface stiffness. How multisensory information feeds back into the fine-tuning of sound-generating actions remains unexplored. Participants (percussionists, non-percussion musicians, or non-musicians) held a stylus and learned to control their wrist velocity while repeatedly striking a virtual sounding object whose surface stiffness was under computer control. Sensory feedback was manipulated by perturbing the surface stiffness specified by audition and haptics in a congruent or incongruent manner. The compensatory changes in striking velocity were measured as the motor effects of the sensory perturbations, and sensory dominance was quantified by the asymmetry of congruency effects across audition and haptics. A pronounced dominance of haptics over audition suggested a superior utility of somatosensation developed through long-term experience with object exploration. Large interindividual differences in the motor effects of haptic perturbation potentially arose from a differential reliance on the type of tactual prediction error for which participants tend to compensate: vibrotactile force versus object deformation. Musical experience did not have much of an effect beyond a slightly greater reliance on object deformation in mallet percussionists. The bias toward haptics in the presence of crossmodal perturbations was greater when participants appeared to rely on object deformation feedback, suggesting a weaker association between haptically sensed object deformation and the acoustical structure of concomitant sound during everyday experience of actions upon objects.

Abstract

The perceived properties of a digital piano keyboard were studied in two experiments involving different types of vibrotactile cues in connection with sonic feedback. The first experiment implemented a free playing task in which subjects had to rate the perceived quality of the instrument according to five attributes: Dynamic control, Richness, Engagement, Naturalness, and General preference. The second experiment measured performance in timing and dynamic control in a scale playing task. While the vibrating condition was preferred over the standard nonvibrating setup in terms of perceived quality, no significant differences were observed in timing and dynamics accuracy. Abstract

Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Abstract

Background The interpretation of serial electrocardiographic (ECG) changes in hypertensive subjects is uncertain. We tested the hypothesis that serial changes in repolarization and voltage are independent determinants of outcome. Methods The Hypertrophy at ECG And its Regression during Treatment (HEART) Survey was a prospective observational study performed at 61 centers. We studied 711 subjects with hypertension and ECG left-ventricular hypertrophy (LVH) at entry. Tracings from 496 subjects at entry and one or more visits during follow-up were available for central reading. Results The prevalence of ECG LVH progressively decreased by 49.6% at 3 years. The crude rate of a prespecified primary composite end point of cardiovascular events was 4.17 per 100 subjects per year (95% confidence interval [CI], 3.27 to 5.33). We used Cox regression models of ECG LVH indexes as time-varying covariates at baseline and at follow-up. Time-varying LVH, defined as an absence of ST-T alterations (“strain”), was associated with a lower event rate hazard ratio (HR), 0.47; 95% CI, 0.28 to 0.78; P = .0035), whereas the LVH changes defined in terms of ECG voltages did not achieve significance (HR, 0.91; 95% CI, 0.74 to 1.13; P = .39). The crude event rate in subjects with versus without in-treatment ST-T alterations on the last available ECG before the event or before censoring was 8.38 versus 3.17 per 100 subjects per year ( P Conclusions In this study of subjects with hypertension and ECG LVH at entry, serial changes in repolarization significantly predicted the prognosis, independent of voltage change (which was not significantly predictive in this study). The persistence or new development of ST-T alterations identifies subjects at very high risk of cardiovascular events.
