No. of bitstreams: 0 Previous issue date: 2020-10-23 Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Background Periodontal disease and diabetes mellitus (DM) are highly prevalent and interrelated diseases, resulting in altered host response microbiota. Thus, this study aimed to evaluate the impact of DM on local levels of lipopolysaccharide (LPS) and lipoteichoic acid (LTA) and their relationship with cytokines and matrix metalloproteinases' (MMPs) profile. Methods This case-control study included diabetic (n = 15) and non-diabetic (n = 15) subjects presenting Stage 3-4, Grade C, Periodontitis. Gingival crevicular fluid (GCF) was collected, and LPS and LTA levels were analyzed by enzyme-linked immunosorbent assay (ELISA), while IFN-gamma, IL-10, IL-17, IL-1 beta, IL-4, MMP-2, and MMP-9 were measured by LUMINEX/MAGpix. Mann-Whitney and Spearman's correlation tests were used to compared and to correlate variables (p < 0.05). Results Higher levels of LTA, LPS, IL-10, IL-1 beta, and MMP-2 (p < 0.05) and lower levels of IL-17 were found in the DM group (p < 0.05). Non-diabetic subjects presented higher LPS, IFN-gamma, IL-17, and MMP-2 levels and lower IL-10 concentration (p < 0.05). No significant correlation was seen between LPS and cytokine profile in non-diabetic. Local levels of LTA were positively correlated with IL-17 and MMP-2 and negatively with IL-10. Conclusion LTA and LPS drove the inflammatory profile through the modulation of cytokines and MMPs in a different manner in DM and non-diabetic subjects. 4. Effect of triclosan containing-dentifrice on subgingival microbiota in children from generalized aggressive periodontitis parents

Generalized aggressive periodontitis (GAgP) cause a rapid and severe destruction periodontal tissues and it is characterized by an altered microbiological profile and familial aggregation. Studies involving children from GAgP individuals identified early clinical and microbiological changes in these individuals, suggesting that this aspect could be related to disease susceptibility. The aim of this study was to evaluate the effect of Triclosan toothpaste in the concentration of periodontopathogens present in the subgingival biofilm in children from GAgP parents, compared to children from periodontally health parents. Subgingival biofilm were collected from incisors and first molars. The bacterial DNA were extracted and the total amounts of A. actinomycetemcomitans (Aa), Fusobacterium nucleatum, P. gingivalis and T. forsythia were evaluated by qPCR. At baseline, children from GAgP parents presented higher plaque index (PI), gingival index (GI), and bleeding on probing (BoP) as well as higher concentration of Aa subgingivally. The PI was controlled by both toothpastes. Triclosan Toothpaste reduced the GI for both groups and aditional reduction in PD ana BoP were demontrated in the GAgP group. No effect of triclosan tothpaste was observed in pathogens concentration. In conclusion, Triclosan toothpaste was more effective than placebo toothpaste to control the clinical condition in children from GAgP parents, however both treatments did not change the microbiological condition.

Periodontitis is an inflammatory disease with a multifactorial etiology, triggered by the host's immune-inflammatory response to the periodontopathogens, manly gram-negative bacteria, present in the subgingival biofilm. Systemic conditions, such as diabetes mellitus (DM), can alter this response through immunological and microbiological changes. Studies have shown that DM promotes a dysbiotic pattern of biofilm, which may alter endotoxin content in the subgingival environment. Thus, the present study aims to evaluate the relationship between cytokine profile and lipopolysaccharide levels in the subgingival environment of diabetic and non-diabetic subjects.

To assess the panoramic radiomorphometric indices and fractal dimension in women with celiac disease.The sample consisted of 20 women with celiac disease and 20 healthy women (control group). The mandibular cortical index classification, panoramic mandibular index, mental index, and fractal dimension were evaluated on panoramic radiographs. One-way ANOVA with post hoc Tukey test was used for comparison of the linear measurements and fractal dimension between the celiac and control groups, adopting a significance level of 5% RESULTS: There was no significant difference in panoramic radiomorphometric indices or fractal dimension between the celiac and control groups.Panoramic radiomorphometric indices and fractal dimension revealed no significant bone changes in women with celiac disease.

Aggressive periodontitis(AgP) is a disease characterized by the presence of periodontal pathogens and family aggregation of cases, possibly related to the vertical transmission of microorganisms. Thus, the project has evaluated the pattern of colonization of some pathogens in children of parents affected by generalized aggressive periodontitis, compared to children of periodontally healthy parents. Five edentulous babies and 15 primary, mixed and permanent dentate children from families with parents affected by AgP were selected and, by a age and gender-matched approach, similar population from eriodontally heath individuals were also selected. From all subjects, saliva was collected, had a DNA extracted and sequenced for Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Treponema denticola (Td), Tannerella forsythia (Tf) identification. Studies have demonstrated that children from aggressive periodontitis (AgP) parents presented precocious alterations in their periodontal condition, and the use of chemical agents in association to plaque control could be useful to control these alterations. This study aimed to evaluate the effect of Triclosan toothpaste to modulate the clinical and subgingival condition in children from AgP parents. Fifteen children from AgP parents and 15 from periodontally healthy parents were included in this crossover placebo study. Children were randomly allocated into triclosan or placebo therapy, using selected toothpaste for 45 days. After 15 days of wash-out, groups were crossed, changing the used toothpaste. Clinical examination and saliva, crevicular gingival fluid (GCF), and subgingival biofilm collection were performed at baseline and 45 days of each phase. GCF cytokines’ levels were analyzed by Luminex/MAGpix platform and subgingival and salivary periodontal pathogens’ levels by qPCR. At baseline, AgP group presented higher plaque index (PI), gingival index (GI), and bleeding on probing (BoP), higher Aggregatibacter actinomycetemcomitans (Aa) abundance in saliva and subgingival biofilm, and lower levels of INF-ɣ, IL-4, and IL-17 in GCF. Placebo therapy only reduced PI in both groups. Triclosan toothpaste reduced PI and GI in both groups. Triclosan promoted reduction of BoP and probing depth (PD), Aa salivary, and IL-1β levels in AgP group. In health group, triclosan reduced INF-ɣ and IL-4 concentration. Triclosan toothpaste demonstrated to be more effective than placebo toothpaste to control the periodontal condition in children from AgP parents, by reducing the BoP, PD, salivary Aa, and IL-1β. Triclosan toothpaste can improve oral conditions in higher-risk population for AgP. This study was registered at ClinicalTrials.gov with the identifier NCT03642353.

No. of bitstreams: 0 Previous issue date: 2019-12-01 BACKGROUND: To assess the clinical and microbiological responses of amoxicillin + metronidazole (AMX + MET) versus clarithromycin (CLM) as adjuncts to one-stage full-mouth ultrasonic debridement (FMUD) in the treatment of generalized aggressive periodontitis (GAgP). METHODS: For this parallel, double-masked, pilot randomized clinical trial, 46 patients with GAgP were selected and randomly assigned into two groups: AMX+MET group (n = 23): FMUD associated with AMX (500 mg three times a day) and MET (400 mg three times a day) for 7 days; and CLM group (n = 23): FMUD associated with CLM (500 mg twice a day) for 7 days. Clinical parameters were evaluated at baseline, 3, and 6 months post-treatment. The levels of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, and Fusobacterium nucleatum from subgingival biofilm were determined by quantitative polymerase chain reaction. RESULTS: Both treatments significantly improved all clinical parameters compared with baseline and promoted a significant reduction of A. actinomycetemcomitans and P. gingivalis counts (P > 0.05). CLM succeeded in decreasing T. forsythia at 6 months (P 0.05). CONCLUSIONS: The results suggest that CLM is not superior than AMX + MET in the treatment of GAgP. However, this antibiotic led to good clinical outcomes and may be a possible alternative to AMX+MET in the treatment of severe periodontitis in young patients. Future studies with larger sample sizes are needed to confirm this statement (NCT02969928).
