Abstract: A pregnant woman presented with symptomatic bioprosthetic mitral valve stenosis. We discuss the difficulties of decision making in this particular situation where two lives are at stake, the fetus’s and the mother’s, questioning whether transcatheter mitral valve-in-valve implantation can be an effective and safe option for this challenging condition. Abstract: Background There is limited evidence that fractional flow reserve (FFR) is effective in guiding therapeutic strategy in multivessel coronary artery disease (CAD) beyond prespecified percutaneous coronary intervention or coronary graft surgery candidates. Objectives The FUTURE (FUnctional Testing Underlying coronary REvascularization) trial aimed to evaluate whether a treatment strategy based on FFR was superior to a traditional strategy without FFR in the treatment of multivessel CAD. Methods The FUTURE trial is a prospective, randomized, open-label superiority trial. Multivessel CAD candidates were randomly assigned (1:1) to treatment strategy based on FFR in all stenotic (≥50%) coronary arteries or to a traditional strategy without FFR. In the FFR group, revascularization (percutaneous coronary intervention or surgery) was indicated for FFR ≤0.80 lesions. The primary endpoint was a composite of major adverse cardiac or cerebrovascular events at 1 year. Results The trial was stopped prematurely by the data safety and monitoring board after a safety analysis and 927 patients were enrolled. At 1-year follow-up, by intention to treat, there were no significant differences in major adverse cardiac or cerebrovascular events rates between groups (14.6% in the FFR group vs 14.4% in the control group; hazard ratio: 0.97; 95% confidence interval: 0.69-1.36; P = 0.85). The difference in all-cause mortality was nonsignificant, 3.7% in the FFR group versus 1.5% in the control group (hazard ratio: 2.34; 95% confidence interval: 0.97-5.18; P = 0.06), and this was confirmed with a 24 months’ extended follow-up. FFR significantly reduced the proportion of revascularized patients, with more patients referred to exclusively medical treatment (P = 0.02). Conclusions In patients with multivessel CAD, we did not find evidence that an FFR-guided treatment strategy reduced the risk of ischemic cardiovascular events or death at 1-year follow-up. Abstract: Objective To report the authors’ initial experience of transcarotid transcatheter aortic valve replacement (TAVR) managed with ultrasound-guided intermediate cervical plexus block. Design A single-center prospective study. Setting A teaching hospital in Paris, France. Participants All consecutive patients undergoing a transcarotid TAVR were included. Interventions The ultrasound-guided intermediate cervical plexus block was performed in 28 of 31 patients. In 3 patients, the procedure was scheduled under general anesthesia: 2 because of failure of previous transfemoral procedures under local anesthesia, and 1 for an emergency procedure in a pulmonary edema context. Measurements and Main Results Anesthesia and all perioperative parameters were recorded, as well as any complications after the procedure. Twenty-eight patients were managed with intermediate cervical plexus block and light sedation without any anesthesia-related complication. No conversion to general anesthesia was required. The use of vasopressor was only required in 11% of the patients. In 2 patients, a loss of consciousness after the common carotid artery cross-clamping test occurred, leading to the use of a temporary femoral-carotid shunt; no other change in consciousness was recorded during the procedure. Conclusions Ultrasound-guided intermediate cervical plexus block appeared to be an alternative anesthetic technique for carotid TAVR, providing adequate surgical conditions, continuous neurologic monitoring, and arterial pressure stability.

Abstract: BACKGROUND There were gaps between guidelines and practice when surgery was the only treatment for aortic stenosis (AS). OBJECTIVES This study analyzed the decision to intervene in patients with severe AS in the EORP VHD (EURObservational Research Programme Valvular Heart Disease) II survey. METHODS Among 2,152 patients with severe AS, 1,271 patients with high-gradient AS who were symptomatic fulfilled a Class I recommendation for intervention according to the 2012 European Society of Cardiology guidelines; the primary end point was the decision for intervention. RESULTS A decision not to intervene was taken in 262 patients (20.6%). In multivariate analysis, the decision not to intervene was associated with older age (odds ratio [OR]: 1.34 per 10-year increase; 95% CI: 1.11 to 1.61; P = 0.002), New York Heart Association functional classes I and II versus III (OR: 1.63; 95% CI: 1.16 to 2.30; P = 0.005), higher age adjusted Charlson comorbidity index (OR: 1.09 per 1-point increase; 95% CI: 1.01 to 1.17; P = 0.03), and a lower transaortic mean gradient (OR: 0.81 per 10-mm Hg decrease; 95% CI: 0.71 to 0.92; P < 0.001). During the study period, 346 patients (40.2%, median age 84 years, median EuroSCORE II [European System for Cardiac Operative Risk Evaluation II] 3.1%) underwent transcatheter intervention and 515 (59.8%, median age 69 years, median EuroSCORE II 1.5%) underwent surgery. A decision not to intervene versus intervention was associated with lower 6-month survival (87.4%; 95% CI: 82.0 to 91.3 vs 94.6%; 95% CI: 92.8 to 95.9; P < 0.001). CONCLUSIONS A decision not to intervene was taken in 1 in 5 patients with severe symptomatic AS despite a Class I recommendation for intervention and the decision was particularly associated with older age and combined comorbidities. Abstract: Coronary bifurcations are involved in 15-20% of all percutaneous coronary interventions (PCI) and remain one of the most challenging lesions in interventional cardiology in terms of procedural success rate as well as long-term cardiac events. The optimal management of bifurcation lesions is still debated but involves careful assessment, planning and a sequential provisional approach. The preferential strategy for PCI of bifurcation lesions remains to use main vessel (MV) stenting with a proximal optimisation technique (POT) and provisional side branch (SB) stenting as a preferred approach. Final kissing balloon inflation is not recommended in all cases. In the minority of lesions where two stents are required, careful deployment and optimal expansion are essential to achieve a long-term result. Intracoronary imaging techniques (IVUS, OCT) and FFR are useful endovascular tools to achieve optimal results.

Abstract: Introduction L’angioplastie ambulatoire est devenue le gold standard pour la prise en charge des lesions coronaires simples ; la faisabilite des lesions complexes qui interessent le tronc commun reste encore peu etudiee. L’objectif de notre etude retrospective, monocentrique, est d’etudier le pronostic des patients dilates sur le tronc commun en hopital de jour par rapport aux patients hospitalises. Materiel et methode Nous avons inclus tous les patients ayant beneficie d’une angioplastie du tronc commun entre janvier 2018 et decembre 2019. Le critere de jugement principal etait un critere composite de deces cardiovasculaires, AVC ischemiques, infarctus, revascularisation du tronc commun a 30 jours. Les criteres de jugement secondaires etaient la survie sans evenement a 12 mois, les saignements graves (BARC > 2) a 30 jours. Tous les patients ont ete rappeles, a defaut la derniere consultation medicale etait renseignee. Les patients ambulatoires avaient une biologie 48 heures avant, ils etaient telemetres pendant 6 heures apres l’angioplastie, la sortie etait a la discretion de l’operateur. Resultats Au total, 229 patients ont ete inclus, dont 14 avaient beneficie d’une angioplastie en hopital de jour et 215 en hospitalisation. Le suivi moyen etait de 366 jours, 1 seul patient etait perdu de vue. Les patients ambulatoires avaient un âge median de 65 ans contre 75 ans chez les patients hospitalises (p = 0,02), aucun n’avait de syndrome coronarien (0 vs. 56 ; p = 0,02), le Syntax score median etait de 18 contre 21 (p = 0,02). Seulement trois patients ambulatoires etaient pontes (21 % contre 16 %, p = 0,71). Le nombre de stents total et la longueur des stents dans le tronc commun etait moins importants en ambulatoire (p = 0,05). L’utilisation de l’imagerie endocoronaire (21 % versus 23 %, p = 0,59), de l’atherectomie rotative (7 % versus 6 %, p = 0,57) et de la voie femorale (14 % versus 23 %, p = 0,36) etait similaire entre les deux groupes. Le critere de jugement principal est survenu chez 0 patient dans le groupe ambulatoire contre 13 patients dans le groupe hospitalise (0 % vs. 6 % ; p = 0,43). La survie sans evenements a 12 mois de l’ensemble des patients dilates sur le tronc commun etait de 87,1 %. Elle etait de 90,9 % (89,2–99,6 ; n = 1/14) dans le groupe ambulatoire contre 86,7 % (84,3–89,2 ; n = 21/215) ; p = 0,51 dans le groupe hospitalise. Un patient ambulatoire avait ete revascularise a 180 jours sur une restenose ostiale de l’artere circonflexe. Une hemorragie grave est survenue a 30 jours chez 1 patient ambulatoire contre 18 patients hospitalises ; p = 0,80. Conclusion Notre serie montre la faisabilite de l’angioplastie ambulatoire du tronc commun et permet de mieux definir le profil des patients qui pourront en beneficier.

Abstract: International audience; Background: Fractional flow reserve (FFR) measurement requires adenosine injection. However, adenosine can induce conductive and rhythmic complications, or be contraindicated in some patients. Contrast-induced hyperemia could provide a simple first-line method (contrast-enhanced FFR; cFFR) to assess coronary lesions. In this study we evaluated the accuracy of cFFR to predict lesion significance. Methods: This prospective study included 104 patients with 138 coronary lesions. Abstract: From January 2013 to January 2017, 686 consecutive patients were referred to our centre for transcatheter aortic valve replacement, including 5 subjects with severe aortic regurgitation and acute refractory cardiogenic shock. These patients were contraindicated for surgical treatment by the heart team because of high surgical risk (median logistic EuroSCORE: 74.6/Society of Thoracic Surgeons score: 37.9). The success rate of valve implantation was 100% through transfemoral access with self-expandable devices. The observed 30-day mortality rate was 20%. Hence, the transcatheter aortic valve replacement procedure might represent a successful and life-saving intervention for treatment of patients with severe aortic regurgitation who present with acute refractory cardiogenic shock.

Abstract: Background - Transcatheter aortic valve replacement (TAVR) is standard therapy for patients with severe aortic stenosis who are at high surgical risk. However, national data regarding procedural characteristics and clinical outcomes over time are limited. Objectives - The aim of this study was to assess nationwide performance trends and clinical outcomes of TAVR during a 6-year period. Methods - TAVRs performed in 48 centers across France between January 2013 and December 2015 were prospectively included in the FRANCE TAVI (French Transcatheter Aortic Valve Implantation) registry. Findings were further compared with those reported from the FRANCE 2 (French Aortic National CoreValve and Edwards 2) registry, which captured all TAVRs performed from January 2010 to January 2012 across 34 centers. Results - A total of 12,804 patients from FRANCE TAVI and 4,165 patients from FRANCE 2 were included in this analysis. The median age of patients was 84.6 years, and 49.7% were men. FRANCE TAVI participants were older but at lower surgical risk (median logistic European System for Cardiac Operative Risk Evaluation [EuroSCORE]: 15.0% vs. 18.4%; p < 0.001). More than 80% of patients in FRANCE TAVI underwent transfemoral TAVR. Transesophageal echocardiography guidance decreased from 60.7% to 32.3% of cases, whereas more recent procedures were increasingly performed in hybrid operating rooms (15.8% vs. 35.7%). Rates of Valve Academic Research Consortium-defined device success increased from 95.3% in FRANCE 2 to 96.8% in FRANCE TAVI (p < 0.001). Abstract: Background Although solid tumors and haematological malignancies represent a significant percentage of all pericardial effusion cases, the characteristics, optimal therapeutic management strategy (percutaneous pericardiocentesis (PPC) or surgical drainage) and prognosis of these patients are poorly understood. Objectives To evaluate the features and outcomes of patients with malignant pericardial effusion (MPE) admitted for PPC or surgical drainage. Methods This single centre registry included all patients referred to our institution for symptomatic pericardial effusion between 2014 January 1 st and 2017 December 31 st . We retrospectively identified patients with suspected MPE and analyzed their clinical and procedural characteristics, overall mortality and drainage recurrence rate. Results A total of n = 68 patients (median age: 61 (15.5) y; Male gender: 43%) were included in the analysis. Malignant diseases were mainly lung (49%), breast (18 %), digestive tract (15%) cancers, hemopathies (6%) and melanoma (2%). A total of n = 44 patients underwent PPC (65%) whereas n = 24 underwent surgical management (35%). There was no significant difference in baseline characteristics between the two groups. There was no major procedure-related complication in both groups. MPE severely affected outcome: the 1–year actuarial survival was 32.1 ± 6.1%. The 1-year actuarial survival from MPE recurrence was 88 ± 5%. There was no difference between surgical drainage and PPE in terms of global survival and risk of recurrence. Cox regression analysis revealed that presence of malignant cell within the pericardial fluid was the only independent predictors of death in this cohort. Conclusion MPE is mostly related to solid tumors and is associated with poor prognosis. There is no significant difference in outcome between PPC and surgical treatment, suggesting that both strategies are valuable options to manage this affection.

Abstract: Background Transcatheter aortic valve implantation (TAVI) has revolutionized the treatment of severe symptomatic aortic stenosis. However, cardiac tamponade remains a threatening complication with few reported data. Purposes To investigate the incidence, causes, predictive factors and outcomes of peri-procedural tamponade among TAVI patients. Methods This study included the patients who consecutively underwent TAVI in two high-volume centres between 2006 and 2017. Baseline characteristics of the patients, procedural details and hospital outcomes were prospectively recorded. Results A total of 1957 patients were included (Age: 81.8 ± 2.1 years; male gender, 46%; logistic Euroscore: 16.9 ± 2.9%). A peri-procedural tamponade occurred in n = 51 patients (2.6%). The incidence decreased significantly over time: 5.8% for the 2006–2008 period, 3.6% for the 2009–2011 period, 3.5% for the 2012–2014 period and 1.7% for the 2015–2017 period (P = 0.03). The causes were: left ventricular perforation by the stiff wire in n = 12 patients (24%), right ventricular perforation by pacing leading in n = 9 patients (17%), annular rupture in n = 12 patients (24%). There was no reported cause in 35% of the cases. The patients were treated by percutaneous pericardiocentesis in n = 20 cases (39%), cardiac surgery in n = 28 cases (55%) and hemodynamic support only in n = 3 cases (6%). Patient age (OR = 1.06 (95% CI: 1.01–1.11) per year), baseline left ventricle ejection fraction (OR = 1.03 (1.00–1.07) per %), previous chest radiation (OR = 3.60 (1.5–8.5)) and non-transfemoral access (OR = 2.70 (1.25–8.84)) were independent predictors of peri-procedural tamponade. The occurrence of tamponade severely impaired outcome: the actuarial 30-day survival was 66.4 ± 6.6% in tamponade patients vs. 96.8% ± 0.4% in the others (P Conclusions Cardiac tamponade during TAVI is a rare complication whose incidence decreased over past years but remains associated with a poor immediate outcome.

Abstract: Background— The pathomechanisms underlying very late stent thrombosis (VLST) after implantation of drug-eluting stents (DES) are incompletely understood. Using optical coherence tomography, we investigated potential causes of this adverse event. Methods and Results— Between August 2010 and December 2014, 64 patients were investigated at the time point of VLST as part of an international optical coherence tomography registry. Optical coherence tomography pullbacks were performed after restoration of flow and analyzed at 0.4 mm. A total of 38 early- and 20 newer-generation drug-eluting stents were suitable for analysis. VLST occurred at a median of 4.7 years (interquartile range, 3.1–7.5 years). An underlying putative cause by optical coherence tomography was identified in 98% of cases. The most frequent findings were strut malapposition (34.5%), neoatherosclerosis (27.6%), uncovered struts (12.1%), and stent underexpansion (6.9%). Uncovered and malapposed struts were more frequent in thrombosed compared with nonthrombosed regions (ratio of percentages, 8.26; 95% confidence interval, 6.82–10.04; P P P Conclusions— The leading associated findings in VLST patients in descending order were malapposition, neoatherosclerosis, uncovered struts, and stent underexpansion without differences between patients treated with early- and new-generation drug-eluting stents. The longitudinal extension of malapposed and uncovered stent was the most important correlate of thrombus formation in VLST.

Abstract: Aims The optimal management strategy of acute hemodynamic compromise due to severe aortic regurgitation (AR) remains controversial. Few cases are reported in the literature, where LV assistance device was used as a bridge to surgical valve replacement. However, conventional surgery in this situation carries a high percentage of adverse events. Thus, transcatheter aortic valve replacement (TAVI) might represent an alternative option for management of AR patients with unstable hemodynamics. This pilot series investigate the feasibility and short-term outcome of urgent TAVI in this setting. Methods and results From January 2013 to January 2017, n = 686 consecutive patients were referred to our centre for TAVI procedures, including n = 16 patients (2.3%) with severe AR. Among these patients, n = 5 subjects presented with acute refractory cardiogenic shock. All 5 patients were proposed candidates for a percutaneous transcatheter procedure according to heart team consensus because of some contraindication to conventional aortic valve replacement or high surgical risk, with a median Logistic EuroSCORE of 74.5 (IQR, 33.55) and median Society of Thoracic Surgeons score of 37.9 (IQR, 19.8). AR was related to aortic bioprosthesis degeneration in n = 3 cases and native aortic valve lesion in n = 2. All procedures were performed through transfemoral access using a self-expandable device. The success rate of valve implantation was 100%. All patient improved the creatinine clearance and arterial blood lactate level after the procedure. The observed 30 days mortality rate was only of 20% with only 1 death 18 days after TAVI related to sepsis and multiple organ failure ( Table 1 ). Conclusions This pilot series suggests that TAVI procedure can be a successful and lifesaving intervention for treatment of severe AR presenting with acute refractory cardiogenic shock. Further data from multicentre series are required to confirm these results.

Abstract: BackgroundFew data is available concerning the latest generation Edwards SAPIEN 3- S3 in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation (TAVI).AimsTo evaluate periprocedural and 30-day clinical outcomes of TAVI using the Edwards SAPIEN 3 prosthesis) via transiliofemoral approach.MethodsBetween September 2014 and March 2015, consecutive high-risk or non-operable patients with severe aortic stenosis had TAVI using Edwards SAPIEN 3 prosthesis in Institut Mutualiste Montsouris. Valve Academic Research Consortium endpoints were used.ResultsOf 142 patients who underwent TAVI using Edwards SAPIEN prosthesis, 66 were treated with SAPIEN 3 via transfemoral access (mean age 84±7.1 years; 70% female). About 65% and 48% of patients had, respectively, severe peripheral artery diseases and calcified iliofemorale arteries. Multi-detector computed tomography estimated an aortic annular diameter of 25,07±2mm. Mean logistic EuroSCORE was 15,8±10,8. The device success rate was 98.5%. Abstract: Aims: Time-domain optical coherence tomography (TD-OCT) allows assessment of the anatomy and features of unstable coronary artery lesions, including discrimination between the thrombotic and atherosclerotic plaque components. We sought to investigate the feasibility of thrombus quantification and its monitoring in patients with high thrombotic burden acute coronary syndromes (ACS). Methods: Patients were suitable for inclusion if they presented with an ACS that was successfully revascularized by manual thrombo-aspiration with a residual large thrombus burden on coronary angiography and initial TD-OCT analysis. These patients underwent a second procedure with TD-OCT analysis after several days of optimal anti-thrombotic therapy and benefited eventually from an intra-coronary stent. Coronary lesion stenosis degree was determined by quantitative coronary angiography (QCA). TD-OCT analysis included measurement of thrombus score (as defined by ESC consensus document), thrombus volume, length and minimal luminal area (MLA), which were quantified by serial area measurement within the athero-thrombotic culprit lesion. Results: Seventeen patients fulfilled inclusion criteria. The OCT image quality was suitable for thrombus quantification in n=14 subjects (86% men/ age=57.9±4.5y/ 93% STEMI). Low molecular weight heparin anticoagulation and dual antiplatelet therapy were given to all patients between the two procedures (mean delay: 3.9.±0.3 days). Coronary lesion stenosis degree was determined by quantitative coronary angiography (QCA) methods. Thrombus volume, maximal surface and minimal luminal area (MLA) were quantified by serial area measurement within the athero-thrombotic culprit lesion in 1 mm intervals before any stenting. Results Eleven patients underwent 2 consecutive TD-OCT examinations. The OCT image quality was suitable for thrombus quantification in n=9 subjects (89% men/ age=62.4±5.7y/ 89% STEMI). All patients were under anticoagulant and dual antiplatelet therapy between the two procedures (mean delay: 4.1±0.4 days). No adverse events were observed following OCT analysis. We measured a progressive reduction of thrombus burden between the two analysis, as assessed by the decrease of thrombus volume (5.3±1.7 vs.11.0±3.4 mm3, p=0.004), length (7.4±1.0 vs. 10.9±1.8 mm, p=0.02) and increase of MLA (2.5±0.6 mm2 vs. 1.8±0.3 mm2, p=0.02). However, the degree of stenosis analyzed by QCA didn’t significantly decrease over time (49.7±4.6% vs. 55.7±6.6%, p= 0.15). The thrombus volume reduction was time dependent, as witnessed by the high correlation between the percentage of thrombus volume decrease and the delay between analysis (R=0.87, p=0.002). The observed thrombus volume reduction rate was evaluated to 12% of the initial volume per day under medical therapy. Conclusion TD-OCT assessment of thrombus volume in patients with ACS is feasible, safe and could allow in vivo clot regression monitoring.
