A case of adenocarcinoma of the fundus of the bladder arising in the urachal canal is presented to remind the urologists of the occurrence of this type of tumor. The appearance of a single growth at the vertex should put the examiner on his guard as to the possibility of this mucoid neoplasm. Moreover, the attachment of thick mucus to the tumor site, and its presence in the urine should be noted as being strongly suggestive of the actual condition. Early diagnosis and radical operation are the prerequisites of successful treatment.

During the past few years, I have needed a suitable meatotomy knife for the proper performance of meatotomy in male babies, young children, and even in adults with pin-point or very fine meatus. I have hitherto employed a sharp-pointed urethral knife with a very narrow blade and with a long handle made for use with the endoscope. This was just a makeshift to accomplish the purpose and its use was objectionable because of the sharp point which had to be passed within the urethra. A few cases of retention of urine in babies could be relieved only by the passage of a small ureteral catheter, and would usually recur until I used the urethral knife just mentioned. K. H., a youth, aged 18, referred by Dr. G. B. Reitz, Feb. 8, 1922, complaining of frequency and urgency of urination of three months' duration, had pyuria, but no burning, hematuria or urethritis. There was no complaint of pain in either the back or abdomen. Cystoscopic examination revealed a moderately severe cystitis. Catheters were passed to both renal pelves without obstruction, and with intermittent drip of clear urine from both sides. Renal functional tests were negative. A diagnosis was then made of cystitis, and treatment was outlined to be given by the family physician. After treatment over a period of a few months, the symptoms improved considerably but later recurred. Cystoscopic examination in November, 1922, then revealed the presence of a right-sided pyelitis, with slight delay in the function of the right kidney. The left kidney was normal.
