Aim: The aim of this study was to investigate the administration of analgesics and anxiolytics to people living in nursing homes, taking into account cognitive status. Methods: For this cross-sectional study, we used data from 404 residents who lived in nine randomly selected nursing homes in the Czech Republic and participated in the Czech-Austrian Long-Term Care Research Project called DEMDATA. Information about medication administration was obtained from medical records. Cognitive impairment was evaluated by the Mini-Mental State Examination, and pain was measured by the Pain Assessment in Advanced Dementia. To measure anxiety, we used the Behavioral Pathology in Alzheimer’s Disease Rating Scale in residents with severe-to-moderate dementia and also the standardized questionnaire EuroQol in other residents. Results: In all, 68% (95% CI 62–74) of residents with pain did not receive any analgesic medication and 31% (95% CI 25–38) of residents were administered some analgesics and continued to report pain. The lowest reported occurrence of pain, as well as the lowest prevalence of analgesic administration, was in residents with moderate-to-severe dementia. We found that an alarming percentage of residents in the nursing homes were not treated sufficiently. Conclusion: This study indicates that treatment effects should be better monitored. In my thesis I tried to find out if laboratory mice were also bred to a high milk secretion like dairy cattle. I especially saw over swiss mice, MF1 mice, deer mice and voles. I looked for parameters like litter size, litter mass, pup mass, length of pregnancy, length of lactation, milk energy output, food intake, food energy, metabolizable energy intake, assimilated energy, body temperature and limitation and compared these data. I found out that cold exposure can affect litter size, litter mass, milk energy output, food intake, metabolizable energy intake and assimilated energy in the reproduction of mice. The shaving of mice had an influence on the litter mass, pup mass, milk energy output, food intake and on the metabolizable energy intake. The second lactation affected reproductive performance too. Litter size played an important role in reproduction. Furthermore, the food was important. Most of the older scientific papers had been assigned to the peripheral limitation, whereas the more recent work was attributed to HDL-hypothesis. However, there were some authors who could not distinguish between peripheral or HDL-hypothesis in their scientific work. Some peer re-viewed papers did not confirm the HDL-hypothesis. A possible limitation to calcium should not be ignored. Thus, despite my literature compilation, the results on mouse limitation remain inconsistent, and it would be preferable if further research could be made to find out the reason for these different outcomes. „Turbo“ mice may theoretically be Swiss mice or MF1 mice. Similarly, such "turbo" mice would need to be kept at a low ambient temperature of 5 - 10 °C in order to have larger food intake and produce more milk. On one hand these mice should be fed with high fat diet (diet with 60 % ckal fat content) so that their pups can reach a higher body mass at weaning and the quantity of milk is increased. On the other hand, the "turbo" mice should be fed a diet with a low digestible energy content (diet with less than 25 % digestible energy than the standard diet), so that they can increase their food intake, which can equally lead to a higher body mass of the pups. In order to produce the largest possible amount of milk, a "turbo" mouse would have to have a large litter size and it should be in the second lactation. It would be important to select mice for increased milk yield. Further research on this topic should follow, especially todetermine which temperatures would be optimal so that both litter mass and litter size as well as food intake and milk energy output can be maximized. The "turbo" mouse would require warm temperatures (21 °C) to produce a large litter size and a large litter mass, which would be not exactly in line with the HDL-hypothesis. However, it is questionable whether a "turbo" mouse is really defined by the HDL-hypothesis. Since I have found another limitation, the limitation of calcium in the deer mice. In my opinion there is no "turbo" mouse in relation to a very high reproduction line because calcium can be limiting. Further studies on mice will be needed to determine how strongly and how exactly calcium limitation affects the reproductive performance of mice., Diplomarbeit - Veterinärmedizinische Universität Wien - 2020, In meiner Diplomarbeit habe ich versucht herauszufinden, ob auch Labormäuse, ähnlich wie Milchrinder, auf eine hohe Milchleistung gezüchtet werden können. Vor allem bei den Swiss Mäusen, den MF1 Mäusen, den Hirschmäusen und den Wühlmäusen habe ich Parameter wie die Wurfgröße, die Wurfmasse, das individuelle Jungtiergewicht, die Trächtigkeits- und Laktationsleistung, die Milchenergie, die Futteraufnahme und die Futterenergie, die metabolisierbare Energieaufnahme, die Effizienz der Verdauung, die Körpertemperatur sowie die Limitierung der Mäuse genauer angesehen und die Daten miteinander verglichen. Ich fand heraus, dass eine Kälte Exposition die Wurfgröße, die Wurfmasse, die Milchenergie, die Nahrungsaufnahme die metabolisierbare Energieaufnahme und die Effizienz der Verdauung die Reproduktion von Mäusen beeinflussen könnte. Auch das Rasieren von Mäusen hatte einen Einfluss auf die Wurfmasse, die Jungtiermasse, die Milchenergie, die Nahrungsaufnahme und auf die metabolisierbare Energieaufnahme. Die zweite Laktation hatte ebenfalls Einfluss auf die Reproduktionsleistungen. Die Wurfgröße spielte auch eine wichtige Rolle bei Fortpflanzung. Das Futter war ebenfalls von Bedeutung. Die meisten älteren wissenschaftlichen Arbeiten waren eher der „peripheren“ Limitierung zugeordnet, wohingegen die neueren Arbeiten der HDL-Hypothese zugeschrieben wurden. Allerdings gab es auch einige Autoren, die in ihrer wissenschaftlichen Arbeit nicht genau zwischen der „peripheren“ oder der HDL-Hypothese unterscheiden konnten. Auch konnten neuere wissenschaftliche Originalbeiträge die HDL-Hypo-these in ihren Experimenten mit Swiss Mäusen nicht bestätigen. Eine eventuelle Limitierung auf Kalzium sollte auch nicht außer Acht gelassen werden. So bleiben trotz meiner Recherche die Ergebnisse in Bezug auf die Limitierung von Mäusen widersprüchlich und es wäre wünschenswert, wenn hier weitere Untersuchungen gemacht werden könnten, um herauszufinden was genau der Grund für diese unterschiedlichen Ergebnisse ist. In Bezug auf eine „Turbo“ Maus stellte ich fest, dass sie wahrscheinlich zu den Swiss Mäusen oder den MF1 Mäusen gehören müsste. Ebenso würden solche „Turbo“ Mäuse damit sie einen größeren Nahrungskonsum haben und eine größere Milchmenge erzeugen, bei niedrigen Umgebungstemperaturen (5 - 10 °C) gehalten werden. Auch sollten einerseits diese Mäuse mit fettreicher Nahrung (60 % ckal Fettgehalt) gefüttert werden, um eine höhere Milchmenge zu erhalten, damit ihre Jungtiere ein höheres Körpergewicht beim Absetzen erreichen können. Andererseits sollte der „Turbo“ Maus ein Futter mit geringem verdaulichem Energiegehalt (25 % weniger verdauliche Energie als Standarddiät mit 13,4 kJ/g) gefüttert werden, damit sie ihre Nahrungsaufnahme steigern kann, was gleichermaßen zu einem höheren Körpergewicht der Jungtiere führen kann. Um eine möglichst große Milchmenge zu erzeugen, müsste die „Turbo“ Maus eine große Wurfgröße haben und sie sollte sich in der zweiten Laktation befin-den. Es wäre genauso von Vorteil auf eine erhöhte Milchleistung zu selektieren. Weitere Forschungsarbeiten zu diesem Thema sollten folgen, vor allem um zu bestimmen welche Temperaturen optimal wären, damit sowohl das Wurfgewicht, die Wurfgröße, die Nahrungsaufnahme und die Milchleistung maximiert werden könnte. Die „Turbo“ Maus würde wahrscheinlich warme Temperaturen benötigen (21 °C) um eine möglichst große Wurfgröße und ein großes Wurfgewicht zu erzeugen, was ein Widerspruch zur HDL-Hypothese ist. Allerdings ist fraglich, ob eine „Turbo“ Maus wirklich durch die HDL-Hypothese begrenzt ist. Da bei den Hirschmäusen eine weitere Limitierung, und zwar die Limitierung durch Kalzium gefunden wurde, gibt es meiner Meinung nach keine „Turbo“ Maus in Bezug auf eine sehr hohe Reproduktionsleistung. Da Kalzium limitierend wirken kann, sind weitere Untersuchungen an Mäusen notwendig, um festzustellen, wie stark und wie genau die Kalzium Limitierung die Reproduktionsleistung von Mäusen beeinträchtigt.

A survey was conducted among dementia experts to obtain a deeper understanding of the dementia care structures and services in this geographical area, and to identify the educational needs of health care professionals, and the availability of assistive technology.Subjects and methods: A standardized questionnaire was sent out to 15 leading dementia experts/clinicians in 10 Danube Region countries inquiring about professional groups involved in dementia care, availability and reimbursement of services, inclusion of dementia in professional education and training, acceptability of Internet-based education, and availability of assistive technology. The authors are the survey respondents.Results: The majority of individuals with dementia receive care in the community rather than in institutions. The roles of medical specialties are disparate. General practitioners usually identify dementia symptoms while specialists contribute most to clinical diagnosis and treatment. Health care professionals, particularly those who work closely with patients and carers, have limited access to dementia-specific education and training. The greatest need for dementia-specific education is seen for general practitioners and nurses. An Internet-based education and skill-building program is considered to be equivalent to traditional face-to-face but offer advantages in terms of convenience of access. Assistive technology is available in countries of the Danube Region but is significantly underused.Conclusion: Dementia care in the Danube Region can be improved by an educational and skill-building program for health care professionals who work in the frontline of dementia care. Such a program should also attempt to enhance interdisciplinary and intersectorial collaboration, to intensify the interaction between primary care and specialists, and to promote the implementation of assistive technology.Keywords: dementia, Danube Region, expert survey, dementia care structures, education, assistive technology
