Hearing loss is the most common sensory deficit and one of the most common congenital abnormalities. The estimated prevalence of moderate and severe hearing loss in a normal newborn is 0.1–0.3%, while the prevalence is 2–4% in newborns admitted to the newborn intensive care unit. Therefore, early detection and prompt treatment are of utmost importance in preventing the unwanted sequel of hearing loss on normal language development. The problem of congenital deafness is today addressed on the one hand with hearing screening at birth, on the other with the early (at around 3 months of age) application of hearing aids or, in case of lack of benefit, by the cochlear implant. Molecular genetics, antibody tests for some viruses, and diagnostic imaging have largely contributed to an effective etiological classification. A correct diagnosis and timely fitting of hearing aids or cochlear implants is useful for deaf children. The association between congenital deafness and "mutism", with all the consequences on/the consideration that deaf mutes have had since ancient times, not only from a social point of view but also from a legislative point of view, continued until the end of the nineteenth century, with the development on one side of new methods for the rehabilitation of language and on the other of sign language. But we need to get to the last decades of the last century to have, on the one hand, the diffusion of "universal newborn hearing screening", the discovery of the genetic causes of over half of congenital deafness, and on the other hand the cochlear implants that have allowed thousands of children born deaf the development of normal speech. Below, we will analyze the evolution of the problem between deafness and deaf-mutism over the centuries, with particular attention to the nineteenth century. [ABSTRACT FROM AUTHOR]

Objectives: For 50 years, the Thomas Wickham-Jones (TWJ) Foundation has promoted the advancement of otology and audiology in the UK and Republic of Ireland through a series of overseas Fellowships and other grants., Methods: The paper examines the history of the Foundation since its establishment in 1974, drawing upon the Foundation's archives and personal recollections. The analysis is located within a framework concerning the factors that shape the success or failure of a foundation including vision, strategy, information, leadership and finance., Results: The activities of the TWJ Foundation are charted over five decades, and the governance of the Foundation is detailed. Particular attention is given to the Major Fellowships offered, at first in North America, and to their subsequent development into the Foundation's current rotation., Conclusion: The paper offers an assessment of the TWJ Foundation's impact on the training of otologists in the British Isles and concludes with a brief self-reflective analysis.

In 2019, the Manchester Centre for Audiology and Deafness celebrates its 100th anniversary. To mark the centenary, this special issue is a collection of papers that showcases current research in Manchester Centre for Audiology and Deafness. The Editorial provides a brief history and description of the Centre and an overview of the special issue.

In the 1940s, Georg von Békésy discovered that in the inner ear of cadavers of various vertebrates, structures responded to sound with a displacement wave that travels in a basal-to-apical direction. This historical review examines this concept and sketches its rôle and significance in the development of the research field of cochlear mechanics. It also illustrates that this concept and that of tonotopicity necessarily correlate, in that travelling waves are consequences of the existence of an ordered, longitudinal array of receptor cells tuned to systematically changing frequencies along the auditory organ.

The incidence of sensorineural hearing loss ranges from 1 to 3 per 1,000 live births in term healthy neonates, and 2-4 per 100 in high-risk infants, a 10-fold increase. Early identification and intervention with hearing augmentation within 6 mo yields optimal effect. If undetected and without treatment, significant hearing impairment may negatively impact speech development and lead to disorders in psychological and mental behaviors. Hearing screening programs in newborns enable detection of hearing impairment in the first days after birth. Programs to identify hearing deficit have significantly improved over the two decades, and their implementation continues to grow throughout the world. Initially based on risk factors, these programs identified only 50-75% of infants with hearing loss. Current recommendations are to conduct universal hearing screening in all infants. Techniques used primarily include automated auditory brainstem responses and otoacoustic emissions that provide noninvasive recordings of physiologic auditory activity and are easily performed in neonates and infants. The aim of this review is to present the objectives, benefits, and results of newborn hearing screening programs including the pros and cons of universal vs. selective screening. A brief history and the anticipated future development of these programs will also be discussed.

Background: Voice production relies on the integrated functioning of a three-part system: respiration, phonation and resonance, and articulation. To commemorate the 500th anniversary of the great anatomist Andreas Vesalius (1515-1564), we report on his understanding of this integral system., Methods: The text of Vesalius' masterpiece De Humani Corporis Fabrica Libri Septum and an eyewitness report of the public dissection of three corpses by Vesalius in Bologna, Italy, in 1540, were searched for references to the voice-producing anatomical structures and their function. We clustered the traced, separate parts for the first time., Results: We found that Vesalius recognized the importance for voice production of many details of the respiratory system, the voice box, and various structures of resonance and articulation. He stressed that voice production was a cerebral function and extensively recorded the innervation of the voice-producing organs by the cranial nerves., Conclusions: Vesalius was the first to publicly record the concept of voice production as an integrated and cerebrally directed function of respiration, phonation and resonance, and articulation. In doing so nearly 500 years ago, he laid a firm basis for the understanding of the physiology of voice production and speech and its management as we know it today., (Copyright © 2017 The Voice Foundation. Aural surgery is a branch of nineteenth-century medicine and surgery providing specialized treatment for ear diseases. During the 1830s, faced with a "popular prejudice" against the curability of deafness as well as intraprofessional rivalries and continuous accusations of quackery, aurists found their surgical authority questioned and their field's value threatened. In an attempt to bolster aural surgery's reputation, in 1841, the aurist John Harrison Curtis (1778-1856) introduced his new diagnostic instrument, the cephaloscope, which could not only improve diagnosis but also provide approaches for regulating aural knowledge, thus strengthening aural surgery's authority. This article examines the motives underlying Curtis's introduction of the cephaloscope and the meanings it held for the occupational group at large.

As early as the 1930s the term Master Hearing Aid (MHA) described a device used in the fitting of hearing aids. In their original form, the MHA was a desktop system that allowed for simulated or actual adjustment of hearing aid components that resulted in a changed hearing aid response. Over the years the MHA saw many embodiments and contributed to a number of rationales for the fitting of hearing aids. During these same years, the MHA was viewed by many as an inappropriate means of demonstrating hearing aids; the audio quality of the desktop systems was often superior to the hearing aids themselves. These opinions and the evolution of the MHA have molded the modern perception of hearing aids and the techniques used in the fitting of hearing aids. This article reports on a history of the MHA and its influence on the fitting of hearing aids.

Georg von Békésy was one of the first comparative auditory researchers. He not only studied basilar membrane (BM) movements in a range of mammals of widely different sizes, he also worked on the chicken basilar papilla and the frog middle ear. We show that, in mammals, at least, his data do not differ from those that could be collected using modern techniques but with the same, very loud sounds. There is in all cases a major difference to frequency maps collected using low-level sounds. In contrast, the same cannot be said of his chicken data, perhaps due to the different roles played by the BM in mammals and birds. In lizards, the BM is not tuned and it is perhaps good that Békésy did not begin with those species and get discouraged in his seminal comparative work., (Copyright © 2012 Elsevier B.V. This paper reviews the contributions of von Békésy to psychoacoustics, comparing his findings and interpretations to those that have emerged since his work. The areas covered include the perception of pitch for pure tones and complex tones, the effect of frequency on the apparent location of pure tones, estimation of the velocity of the traveling wave on the basilar membrane using judgments of lateralization, and the relative loudness of monaural and diotic sounds. While subsequent research has failed to replicate some of his findings, other findings have stood the test of time. Georg von Békésy designed the instruments needed for his research. He also created physical models of the cochlea allowing him to manipulate the parameters (such as volume elasticity) that could be involved in controlling traveling waves. This review is about the specific devices that he used to study the motion of the basilar membrane thus allowing the analysis that lead to his Nobel Prize Award. The review moves forward in time mentioning the subsequent use of von Békésy's methods and later technologies important for motion studies of the organ of Corti. In the fifty years since Békésy was awarded the Nobel Prize, cochlear physiology has blossomed. Many topics that are now current are things Békésy could not have imagined. In this review we start by describing progress in understanding the origin of cochlear gross potentials, particularly the cochlear microphonic, an area in which Békésy had extensive experience. We then review progress in areas of cochlear physiology that were mostly unknown to Békésy, including: (1) stereocilia mechano-electrical transduction, force production, and response amplification, (2) outer hair cell (OHC) somatic motility and its molecular basis in prestin, (3) cochlear amplification and related micromechanics, including the evidence that prestin is the main motor for cochlear amplification, (4) the influence of the tectorial membrane, (5) cochlear micromechanics and the mechanical drives to inner hair cell stereocilia, (6) otoacoustic emissions, and (7) olivocochlear efferents and their influence on cochlear physiology. We then return to a subject that Békésy knew well: cochlear fluids and standing currents, as well as our present understanding of energy dependence on the lateral wall of the cochlea. Georg Békésy laid the foundation for cochlear mechanics, foremost by demonstrating the traveling wave that is the substrate for mammalian cochlear mechanical processing. He made mechanical measurements and physical models in order to understand that fundamental cochlear response. In this tribute to Békésy we make a bridge between modern traveling wave observations and those of Békésy, discuss the mechanical properties and measurements that he considered to be so important, and touch on the range of computational traveling wave models., (Copyright © 2012 Elsevier B.V. In our daily lives we hear airborne sounds that travel primarily through the external and middle ear to the cochlear sensory epithelium. We also hear sounds that travel to the cochlea via a second sound-conduction route, bone conduction. This second pathway is excited by vibrations of the head and body that result from substrate vibrations, direct application of vibrational stimuli to the head or body, or vibrations induced by airborne sound. The sensation of bone-conducted sound is affected by the presence of the external and middle ear, but is not completely dependent upon their function. Measurements of the differential sensitivity of patients to airborne sound and direct vibration of the head are part of the routine battery of clinical tests used to separate conductive and sensorineural hearing losses. Georg von Békésy designed a careful set of experiments and pioneered many measurement techniques on human cadaver temporal bones, in physical models, and in human subjects to elucidate the basic mechanisms of air- and bone-conducted sound. Looking back one marvels at the sheer number of experiments he performed on sound conduction, mostly by himself without the aid of students or research associates. Békésy's work had a profound impact on the field of middle-ear mechanics and bone conduction fifty years ago when he received his Nobel Prize. Today many of Békésy's ideas continue to be investigated and extended, some have been supported by new evidence, some have been refuted, while others remain to be tested., (Copyright © 2012 Elsevier B.V. Benjamin Martin, the English natural philosopher, and Claude-Nicolas Le Cat, the French surgeon, both published important work on auditory physiology and function in the mid-eighteenth century. Despite their different backgrounds, there was consensus between the two scholars on key principles of hearing research, most notably the importance of the inner ear in relation to auditory perception. Martin's work (1755 [1763?]) drew directly on the surgical work of Le Cat (1741) to demonstrate the importance of the auditory mechanism in listening processes. Le Cat's interest in the ear, however, came in turn from his interest in surgical anatomy. Martin used Le Cat's elegant designs as a tool for the vivid communication of auditory function to a popular, fee-paying audience. The meeting of two very different minds through intellectual agreement and material transfer demonstrates the way in which principles of hearing science were established in the Enlightenment period.

Stuart Gatehouse was one of the pioneers of cognitive hearing science. The ease of language understanding (ELU) model (Rönnberg) is one example of a cognitive hearing science model where the interplay between memory systems and signal processing is emphasized. The mismatch notion is central to ELU and concerns how phonological information derived from the signal, matches/mismatches phonological representations in lexical and semantic long-term memory (LTM). When signals match, processing is rapid, automatic and implicit, and lexical activation proceeds smoothly. Given a mismatch, lexical activation fails, and working or short-term memory (WM/STM) is assumed to be invoked to engage in explicit repair strategies to disambiguate what was said in the conversation. In a recent study, negative long-term consequences of mismatch were found by means of relating hearing loss to episodic LTM in a sample of old hearing-aid wearers. STM was intact (Rönnberg et al.). Beneficial short-term consequences of a binary masking noise reduction scheme on STM was obtained in 4-talker babble for individuals with high WM capacity, but not in stationary noise backgrounds (Ng et al.). This suggests that individuals high on WM capacity inhibit semantic auditory distraction in 4-talker babble while exploiting the phonological benefits in terms of speech quality provided by binary masking (Wang). Both long-term and short-term mismatch effects, apparent in data sets including behavioral as well as subjective (Rudner et al.) data, need to be taken into account in the design of future hearing instruments.

A tribute to noise. Noise is not only the harmful waste of the world of sounds. Some noises have contributed, or continue to contribute, an added value in three fields at least: Internal Medicine, Audiology and Music. Moreover, they are perceived naturally by the ear (Figure 1).

Noise-induced hearing loss has been documented as early as the 16th century, when a French surgeon, Ambroise Paré, wrote of the treatment of injuries sustained by firearms and described acoustic trauma in great detail. Even so, the protection of hearing would not be addressed for three more centuries, when the jet engine was invented and resulted in a long overdue whirlwind of policy development addressing the prevention of hearing loss. We present a synopsis of hearing loss prevention in the US Army and describe the current Army Hearing Program, which aims to prevent noise-induced hearing loss in soldiers and to ensure their maximum combat effectiveness.

Stuart Gatehouse was an internationally renowned auditory scientist whose work on basic research, government policy, and clinical practice has directly improved the quality of life of adult hearing aid users. He addressed issues of relevance to the lives of hearing-impaired adults, especially on the impact of a hearing loss on an individual, the management of hearing loss, and the measurement of the benefits offered by hearing aids. He also influenced practice and service delivery and made major contributions to the delivery of audiological services in the United Kingdom, including chairing a report that directly led to their modernization in Scotland. This article describes his life and career.

Stuart Gatehouse worked in the MRC Institute of Hearing Research Scottish Section for 29 years until his untimely death in 2007. The former director records a personal appreciation but also an explanation, for those who did not know him well, of the mixture of qualities that made him an outstanding applied scientist.

The international contributions of Stuart Gatehouse are reviewed in three areas: as a scientist, as an advisor to health policy makers, and as a participant in international conferences. He was able, as no other auditory scientist of his time, to bridge the gap between scientific and clinical research. His ability to apply sound scientific principles to issues of clinical importance was most apparent in his work in three main areas of his research: acclimatization to amplified speech, auditory disability and hearing aid benefit, and candidature for linear and nonlinear signal processing.

This article provides the author's perspective on the development of digital hearing aids and how digital signal processing approaches have led to changes in hearing aid design. Major landmarks in the evolution of digital technology are identified, and their impact on the development of digital hearing aids is discussed. Differences between analog and digital approaches to signal processing in hearing aids are identified.
