6. Factores asociados al inicio tardío de atención prenatal en gestantes que discontinuaron las visitas

Introducción: La atención prenatal temprana es crucial para prevenir complicaciones que impliquen un mayor riesgo de morbimortalidad materno-fetal o neonatal. Objetivo: Determinar los factores sociodemográficos, personales e institucionales asociados al inicio tardío de la atención prenatal en gestantes con menos de 6 atenciones. Métodos: Estudio analítico transversal; la población fue de 252 registros de mujeres que durante su gestación tuvieron menos de 6 atenciones prenatales en un hospital público peruano, se excluyó a quienes tuvieron 0 atenciones; los datos fueron obtenidos mediante encuestas. Se aplicó la prueba de ji cuadrado y exacta de Fisher para incluir en el modelo multivariado aquellas que obtuvieron p< 0,2. Para obtener el modelo se usó la regresión de Poisson con varianza robusta, con p< 0,05. Resultados: El 78,9 % de las gestantes tuvieron atención prenatal tardía. En el análisis multivariado se identificaron como factores asociados al inicio tardío, a la edad materna mayor de 35 años (RPa: 1,260; IC: 1,099-1,444), empleo inflexible (RPa: 1,169; IC: 1,024-1,334), sin apoyo de la pareja (RPa: 2,068; IC: 1,556-2,749) y una relación negativa con la familia (RPa: 1,384; IC: 1,165-1,644). Conclusiones: Tener más de 35 años de edad, un empleo inflexible, falta de apoyo de la pareja y tener una relación negativa con la familia, son factores que incrementan la prevalencia de inicio tardío de la atención prenatal en esta población.

9. Efecto de la desviación positiva en el diagnóstico y tratamiento de la sífilis gestacional, y prevención de la sífilis congénita: estudio de cohortes

Objetivos: describir la aplicación de desviación positiva (DP) y sus resultados en el diagnóstico y tratamiento de la sífilis gestacional (SG), y prevención de la sífilis congénita (SC). Materiales y métodos: estudio de cohortes descriptivas. Se incluyeron mujeres atendidas en la Subred Sur (SRS) de Bogotá, con diagnóstico de SG en 2017-2018. La cohorte de 2018 fue expuesta a desviación positiva (DP), buscando identificar mejores prácticas para prevenir la SC, cómo aplicarlas y superar las barreras existentes. Se describe la adherencia a los indicadores de implementación de la Guía Colombiana para el manejo de SG y la incidencia de SC. Resultados: se confirmaron 284 casos de SG. El inicio oportuno de tratamiento pasó de 81,8 a 90,2 %, la solicitud de prueba de sensibilidad a penicilina disminuyó de 38 a 12,9 %, el tratamiento completo se incrementó de 43,8 a 82,6 %. El 97,5 % recibió al menos una dosis de penicilina en 2018. La proporción de incidencia de SC disminuyó de 41,9 a 29,6 %. El 45 % de las pacientes fueron diagnosticadas con SG en el tercer trimestre, en ellas la proporción de incidencia de SC fue 59 %. Conclusiones: este estudio describe los efectos de la aplicación de DP, como un enfoque innovador para mejorar la adherencia al tratamiento oportuno y adecuado de la SG y prevenir la SC. Se requieren estudios prospectivos con grupo control para evaluar de manera más sólida los beneficios de esta metodología en los indicadores de proceso y resultado, así como su impacto en la reducción de la transmisión materno-infantil de la sífilis.

Objetivos: Determinar las características de la atención prenatal de las gestantes atendidas en la Micro red Ascensión en el periodo 2019 y 2020. Material y método: observacional, retrospectivo, transversal; la población 619 gestantes, se utilizó datos secundarios registrados en el VEA obstétrico y el CNV. El reporte fue descargado en la planilla Excel, y se analizó mediante estadística descriptiva en el SPSS. Resultados: Características personales; las gestantes en ambos periodos se encuentran en la edad óptima de reproducción, son solteras y convivientes, con nivel educativo secundaria, procedencia Ascensión y la mayoría cuenta con SIS. Características de la APN; en ambos periodos el inicio de la APN fue en el 1er trimestre, más de la mitad tienen más de 6 APN, administración de micronutrientes, ecografías, visitas domiciliarias incompletos en ambos periodos, la batería de laboratorio, la protección inmunitaria, el VIF, el plan de parto fue completos en el periodo antes de la pandemia e incompleto durante la pandemia, el parto fue institucional en ambos periodos en mayor porcentaje. Conclusiones: La atención prenatal es importante a fin de prevenir complicaciones futuras y en tiempos de pandemia, el trabajo integrado se ve afectado.

La mortalidad perinatal es un indicador que refleja el impacto de la atención materno-infantil de un país. Este estudio presenta nueve casos de la mortalidad perinatal ocurridos en el municipio de Panchimalco, El Salvador. La información se obtuvo de los informes de auditorías. Las madres de los fallecidos tenían edades entre 17 a 43 años, sin uso de anticonceptivos, con controles prenatales incompletos y un promedio de edad gestacional de 31 semanas, tres partos fueron atendidos en la comunidad. La mayoría de muertes perinatales ocurrieron antes del parto de causa desconocida y los nacidos vivos fueron prematuros. Se identificaron factores como el déficit en la atención integral a la mujer. Se requiere de nuevos estudios para determinar los principales factores que influyen en las muertes perinatales en El Salvador.

15. Factores de la atención materna asociada a complicaciones obstétricas del periparto y postparto según encuesta demográfica y de salud familiar en el 2021

Objetivos: Determinar la asociación entre los factores de la atención materna y las complicaciones obstétricas del periparto y post parto. Material y métodos: Estudio cuantitativo, analítico, retrospectivo y transversal, basado en datos de la ENDES 2022. Resultados: Se analizó una muestra de 6 158 madres, el 22.2 % tuvieron complicaciones obstétricas. Las más frecuentes fueron fiebre postparto (12%) y sangrado periparto (9.9%). Los factores de la atención materna asociados con incremento de complicaciones del periparto fueron; violencia familiar (PRa=1.6; IC95%1.16-2.20; p=0.004), DM2 (PRa=4.11; IC95%1.85-9.10; p=0.001) y recibir ferroterapia (PRa=9.76; IC95%:1.88-50.64; p=0.007), por el contrario, un control prenatal brindado por personal calificado (PRa=0.29; IC95% 0.13 - 0.65; p=0.003) disminuirán estas probabilidades. Por otro lado, HTA (PRa=2.3; IC95%:1.10-4.82; p=0.027), DM2 (PRa=4), Anemia (PRa=2.11), violencia emocional (PRa= 2.22) aumentan la posibilidad de complicaciones mientras que el Control de Hb (PRa= 0.52; IC95%:0.31-2.76; p=0.014), atención posparto temprano (PRa=0.19) por personal calificado (PRa=0.66) disminuye la prevalencia de complicaciones post parto. Conclusiones: Factores de la atención materna asociados significativamente a complicaciones obstétricas del posparto y periparto fueron características del Control prenatal, las acciones de intervención y preventivo-promocionales, además de factores de riesgo como HTA, DM2, anemia y violencia familiar.

Introducción: El control prenatal (CPN) sirve para identificar riesgos en la embarazada con la finalidad de intervenirlos para llevar una gestación a término y sin complicaciones. Objetivo: Determinar si la clasificación del riesgo obstétrico (RO) en la Red de Salud Oriente Empresa Social del Estado (ESE O) cumple los criterios definidos en el plan de choque 2012 de la Secretaría de Salud Pública de Cali. Métodos: Se analizaron las historias clínicas de 165 gestantes, el cumplimiento de las normas y guías sobre la valoración del RO y seguimiento del embarazo. Resultados: Se encontró adherencia de los médicos generales (MdG) y gineco-obstetras (GO) para clasificar el RO según criterios predefinidos, el primer CPN ocurrió antes de la semana 12 en casi 50% de las gestantes, pero 70% de las instituciones atendieron el primer CPN en el segundo o tercer trimestre. No hay diferencia estadística en la clasificación de RO entre el MdG y el GO (p=0.06). La oportunidad de evaluación por el GO no fue la recomendada para gestantes de alto o muy alto riesgo; 27.8% de las gestantes no tuvo cita con el GO. Conclusión: No se observan decisiones claras dirigidas a revisar la clasificación inicial del RO, esperable en la consulta a la semana 36 del embarazo por el GO, generándose remisiones innecesarias a niveles superiores. El peso de los recién nacidos no tuvo relación con la clasificación del RO. Se requieren evaluaciones sistemáticas, de este tipo, en todas las entidades de salud con atención a la gestante.

Objetivo: Describir las malformaciones del sistema nervioso asociadas a embriopatía diabética en madres sin adecuado control glicémico en Latinoamérica. Material y métodos: Se realizó una revisión minuciosa de publicaciones en reconocidas bases de datos en línea en español y en inglés. Los buscadores utilizados incluyeron MEDLINE, PubMed, Scielo, BVS, HINARI, EBSCO, Google Scholar. Se encontraron artículos de tipo descriptivo, observacional, retrospectivo, guías de práctica clínica, revisión sistemática, casos y controles, y metaanálisis. Resultados: Mediante la revisión sistematizada de artículos, se obtuvo que las malformaciones del sistema nervioso central secundarias a la embriopatía diabética guardan una relacion estrecha con diversos factores de riesgo de la mujer latinoamericana. Conclusiones: Se encontró asociación entre las características epidemiológicas multifactoriales de la mujer latinoamericana y el desarrollo de malformaciones del sistema nervioso central secundarias a embriopatia diabetica, resultantes de multiples procesos fisiopatológicos desencadenados por la hipeglicemia materna.

The guide for prenatal care is the regulatory mark of obstetrics norms for the control and care of pregnant women, with emphasis on those who have the RhD negative blood group. Alloimmunization by anti-D is the most frequent and serious cause of hemolytic disease of the newborn. Prenatal care is monitoring the progress of pregnancy and aims to embrace, care for and prepare women for motherhood. In view of this, this moment needs to be safe and healthy; therefore, effective, adequate, embracing and holistic maternal care is essential. This work sought to understand the perception of pregnant women regarding the embracement obtained during prenatal care in Primary Health Care. This is a study with an exploratory qualitative design, developed in a health unit located in the municipality of Juazeiro, Bahia. Semi-structured interviews were carried out with random recruitment during the doctors' appointments and the content collected was analyzed and organized into categories according to the Collective Subject Discourse technique. Thus, it was clear that good practices in the professionals' work improve processes and provide quality, humanized, safe and effective assistance to pregnant women. [ABSTRACT FROM AUTHOR]

20. Factores asociados al parto domiciliario en población rural durante primer año de pandemia de COVID-19 en el Perú.

Introduction: Maternal mortality is a public health issue, with greater significance in rural populations. Home delivery (HD) persists at high rates in rural areas, increasing the risk of obstetric complications, hence the need to study the involved factors. Objective: To determine the factors associated with HD in the rural population of Peru. Methodology: A cross-sectional analytical study was conducted on women of childbearing age in rural areas using the 2020 Peruvian National Demographic and Family Health Survey (ENDES). Home delivery (HD) was defined as childbirth occurring within a residence with the assistance of institutional healthcare personnel or midwives. Weighted absolute and relative frequencies were obtained for complex sample calculations using Stata 16.0, along with crude and adjusted prevalence ratios. Results: A total of 5334 rural women were evaluated, with a mean age of 30.02 ± 7.35 years. Secondary education was reported by 46.84%, while 8.06% were illiterate. Additionally, 31.92% reported a language other than Spanish, and the frequency of home delivery was 16.76%. In adjusted multivariate analysis, factors associated with HD included: speaking a different language (aPR 2.27, 95% CI 1.13-4.56, p=0.021), residing in a city above 1000 meters above sea level (aPR 1.79, 95% CI 1.33-2.40, p<0.001), lower wealth index compared to the poorest (aPR 0.31, 95% CI 0.16-0.60, p=0.001), having access to electricity (aPR 0.53, 95% CI 0.37-0.76, p=0.001), and receiving prenatal care from an obstetrician (aPR 0.65, 95% CI 0.49-0.86, p<0.001). Conclusions: The frequency of HD in the rural population during the pandemic in Peru was high. Factors increasing HD included language diversity and residing at higher altitudes, while factors decreasing HD included wealth index, access to electricity, and obstetrician-led prenatal care. To evaluate professional records of prenatal care in electronic medical records in a city in Ceará. Retrospective, cross-sectional, and descriptive study, carried out through the analysis of medical records of pregnant women followed up at a Family Health Unit during the period from January 2021 to December 2022. The data collec-tion instrument contains sociodemographic information, obstetric history and care. The data were analyzed based on absolute and relative frequencies. The percentage of suitability of consultations decreased as more cri-teria were used to assess quality, at level 1 it presented 73.1% suitability, at level 2 57.7%, at level 3 53.8% and at level 4 50.0%. Prenatal care was in compliance with Ministry of Health guidelines. Se evaluó la percepción sobre las barreras para el acceso al control prenatal (ACP) de las embarazadas atendidas en el Centro de Salud Tipo C Dr. Arnaldo Calderón de Tosagua, Manabí, Ecuador, durante marzo-agosto 2023. Se realizó una investigación observacional, descriptiva de corte transversal. La mayoría de las embarazadas era de procedencia rural (77,78 %), tenía un nivel de escolaridad secundaria (37,91 %) y trabajaba (47,71 %). El 83,01 % de las embarazadas inició el control prenatal en el primer trimestre; el 56,86 % tuvo entre 4 y 6 controles prenatales y un período intergenésico de 1 a 2 años (47,71 %). La accesibilidad fue considerada lejos y difícil (68,83 %) y la calidad de las instalaciones fue buena (43,14 %). El horario de atención se percibió como conveniente y el tiempo de espera fue de entre 15 y 30 minutos (60,78 %). El 71,9 % tuvo dificultades para el acceso al ginecólogo. Las principales barreras individuales/psicosociales fueron embarazo no planificado, no deseado, con síntomas de depresión y desmotivación y dificultad para asistir a las consultas por el cuidado de los demás hijos. Las dificultades económicas (90,2 %), dificultades para el traslado a la institución (93,46 %) y dificultad por quehaceres domésticos (87,58 %), fueron las barreras socioeconómicas que más afectaron el ACP. También incidieron en el ACP, el difícil acceso a las citas para la consulta (98,69 %), tiempo de espera prolongado para su atención (97,38 %) y poco tiempo de duración de la consulta (96,08 %).

Prenatal care is intended to preserve and improve maternal and child health; however, the quality of this service and the attendance rates vary, with developing countries having the lowest rates. There are cultural gaps that prevent adequate access to prenatal care, putting the maternal and child health at risk. Our objective was to carry out an updated review on cultural gaps and their impact on prenatal care. This narrative review was conducted by searching all articles published until December 17, 2022, in English databases such as PubMed and Scopus, and Spanish databases such as SciELO. There are various situations that force pregnant women not to attend prenatal appointment, including machismo, reported as a common cause of non-attendance to prenatal checkups in developing countries and especially in rural communities; poor education in families, so women do not attend prenatal checkups due to ignorance about pregnancy care and risks, as well as their beliefs and customs about pregnancy; difficulty accessing health care services due to geographic issues; mistreatment from health professionals while receiving prenatal care or lack of inclusion in the health center; immigration status and the resulting problems with access to health care. Recognizing these situations is as important as knowing their consequences since, once identified, different solution alternatives can be sought from the primary health care. Health personnel must be trained to help the population understand the importance of prenatal care by knowing and respecting the customs and traditions of each patient, and not undermining their ideologies. Objetivo: Evaluar la efectividad de una estrategia para promover y lograr la lactancia materna exclusiva durante los primeros seis meses de vida. Materiales y métodos: Estudio cuasiexperimental aleatorizado no enmascarado. Se incluyeron primigestantes adultas con edad gestacional entre 12 y 27 semanas, en Cali, Colombia, entre 2014 y 2017. El grupo control recibió atención estándar; el grupo de intervención, además, se sometió a una estrategia prenatal y posnatal que incorporó las tecnologías de la información y comunicación. Objetivos: describir la frecuencia del no cumplimiento del control prenatal en gestantes de 35 años o más del departamento del Cauca, Colombia, y hacer un análisis exploratorio de los factores asociados. Materiales y métodos: estudio descriptivo de corte transversal. Se incluyeron gestantes entre 35 y 41 años afiliadas a la Entidad Administradora de Planes de Beneficios Asociación Indígena del Cauca Entidad Promotora de Salud-I (EAPB AIC-I) e inscritas al programa de control prenatal, entre 2016 y 2018. Se excluyeron pacientes con registros sin información completa. Se analizaron variables sociodemográficas, clínicas y la frecuencia de no asistencia adecuado al control prenatal a través de estadística descriptiva, y se calcularon Odd Ratios con sus intervalos de confianza para los factores asociados. Resultados: en 1016 pacientes entre 35 y 41 años evaluadas se encontró una frecuencia de no cumplimiento de mínimo seis controles prenatales de 61,3 %. El antecedente de aborto (OR ajustado: 0,46; IC 95 % 0,33-0,64,) y gravidez de cinco o más (OR ajustado: 3,22; IC 95 % 1,50-6,91) fueron los factores asociados. Conclusiones: el no cumplimiento de controles prenatales por gestantes de 35 o más años inscritos en la EAPB AIC-I del Cauca es alto. Se requieren nuevos estudios cualitativos que evalúen factores culturales y sociales presentes en estas comunidades que afectan la adherencia al control prenatal, así como estudios prospectivos que confirmen el análisis exploratorio de los factores asociados a la no adherencia. Es importante que las empresas aseguradoras del régimen subsidiado realicen actividades de promoción en estas comunidades para incrementar su cumplimento.

Objetivo: identificar el riesgo de depresión durante el embarazo en gestantes de riesgo habitual incluidas en el control prenatal y los factores asociados. Método: estudio transversal, realizado con 201 gestantes, en el consultorio de prenatal de riesgo habitual de una maternidad universitaria. Para la recolección de datos se utilizó un formulario electrónico que contenía un instrumento de caracterización y la Escala de Riesgo de Depresión del Embarazo. La variable dependiente fue el riesgo de depresión en el embarazo. El análisis estadístico se realizó mediante el cálculo de la razón de posibilidades (Odds Ratio) y utilizando las pruebas de Chi-cuadrado y exacta de Fischer. Resultados: entre las participantes, 68,2% tenían mayor riesgo de depresión durante el embarazo. Hubo asociación estadísticamente significativa entre mayor riesgo de depresión durante el embarazo y la variable ocupación (p=0,04), o sea, la ausencia del trabajo (OR = 2,00) duplicó la probabilidad de ocurrencia. Conclusión: la alta prevalencia de riesgo de depresión durante el embarazo destaca la necesidad de planificación, priorización e integración de la salud mental en los servicios de salud prenatal, especialmente en el ámbito de la Atención Primaria de Salud, por parte de los gestores de salud y de los formuladores de políticas.

Antecedentes: La mortalidad materna es un indicador de calidad en la atención de las embarazadas y se considera un problema de salud pública por las repercusiones que tiene para los infantes y sus familias. Su principal factor desencadenante es el embarazo de alto riesgo. Objetivo: Identificar las condiciones que llevaron al diagnóstico de embarazo de alto riesgo y describir las alteraciones de salud en las gestantes de un hospital de nivel III. Métodos: Estudio retrospectivo en embarazadas que consultaron por embarazo de alto riesgo durante el 2017. Resultados: La edad media de las pacientes fue 26 años; 54% procedían del área urbana; el 72.7% pertenecían al régimen subsidiado, el antecedente personal más frecuente fue la cirugía previa (20.9%); la media de controles prenatales fue de 4.7 iniciados alrededor de la semana 11.7. La condición que originó el diagnóstico de embarazo de alto riesgo fue el control prenatal insuficiente con un 25.6%, determinado durante la gestación. El tipo de parto más común fue el vaginal espontáneo con un 38.8% y no se presentaron muertes maternas. Conclusión: De acuerdo con perfil epidemiológico de las gestantes con embarazo de alto riesgo, se resalta el control prenatal insuficiente como principal causa, lo que sugiere reforzar o rediseñar las estrategias de atención por parte de los actores involucrados que permitan implementar controles de calidad en la caracterización y cuidado de las gestantes del Departamento.

Objective. To characterize the state of sexual and reproductive health (SRH) in the adolescent (10-19) and adult (20-49) population. Materials and methods. Using the Encuesta Nacional de Salud y Nutrición 2022, we estimated prevalence and 95% confidence intervals for SRH indicators. Results. Regarding adolescents, 73.2% have heard of contraceptives, 88.1% know that the condom is used only once and 60.4% that it prevents pregnancies and sexually transmitted infections. Also, 22.8% have initiated sexual life and 73.2% used a condom in the first intercourse. Regarding adult population, 42.7 and 38.0% did not use contraception in the first and last sexual relationship and 53.4 and 40.7% used a condom in the first and last sexual relationship. We found differences in maternal health care between adolescents and adults. Conclusion. Having updated information on SRH allows us to focus attention on people's needs. [ABSTRACT FROM AUTHOR]

The objective of the study was to investigate nursing consultations processing in prenatal care in Family Health Strategies in a city in Minas Gerais. This is a descriptive, qualitative study carried out with eight nurses with three to five years of experience in Primary Care. Data were collected between August and September 2021, through an interview, analyzed using Thematic Analysis. It was observed that the consultation is subsidized by guidelines from the Ministry of Health and its own instrument. The registration is done on the medical record, in the pregnant women's booklet, and in digital spreadsheets. Recurrent nursing problems were low adherence to prenatal care, lack of family support, and unbalanced nutrition. Work overload and lack of nursing protocols hinder nurses' work. It is concluded that the Systematization of Nursing Care is partially applied; however, identifying nursing problems during consultations is essential for comprehensive care. [ABSTRACT FROM AUTHOR]

Congenital malformations (CM) are structural/functional alterations that occur in the fetus, leading to a socio-economic and emotional impact. This is a retrospective, documentary study with a quantitative approach aiming at identifying the relationship of risk factors and maternal epidemiological profile with the occurrence of CM. It was based on the analysis of 37 medical charts of pregnant women in a municipality in the metropolitan region of Curitiba-PR, between 2019 and 2021. The data revealed a profile: pregnant, multiparous, obese women between 18 and 36 years old, with comorbidities, with no preconception evaluation. The most affected organic systems were the cardiovascular and the nervous systems or multiple systems. An increased risk of fetal malformations was observed in obese women over 25 years old, who used alcohol or tobacco. Although a single cause of CM in that period and place has not been identified, an association was established between maternal epidemiological factors and characteristics of fetal malformations, highlighting the need for preconception health education. Objective: To understand the experiences of men in the transition to paternity during the prenatal period. Methodology: Qualitative, exploratory, descriptive, cross-sectional and retrospective study. Theoretical sampling, with the participation of 10 men experiencing, for the first time, the partner's pregnancy. Data collection performed using semi-structured interview. Data analysis using content analysis technique, semantic categorization and inductive approach. Results: The theme of (de)constructing bridges for transition emerges from the data and seeks to clarify the conditions that favour or otherwise inhibit the experience of a positive transition experience by men, along the path to paternity. It encompasses seven categories: support received, seeking information, previous experience with babies, feeling prepared to be a father, having a child is a life goal, meaning attributed to the transition and exclusion from the father figure. Conclusion: Men take a proactive stance during pregnancy and try to prepare for and overcome the transition experience, but encounter several obstacles in the process. What is required of modern man (greater sensitivity and exercise of a more responsible and participatory paternity) is not reflected in clinical practices, which perpetuate the exclusion of the father figure, weakening the exercise of paternity.
