In this study, the relationship between ideological context and residential planning is examined through the ideology in the Soviet Union in the 1920s being supported by the residential architecture. Besides economic difficulties experienced after the October Revolution and World War I, the topics like technological deficiencies and encouraging women's labor force participation affected the solutions addressed by many architects within the scope of residential architecture. To offer a solution for these problems, the architectural design competition was held in 1926. In most of these designs, spaces such as kindergartens, libraries, common kitchens, common dining rooms, etc. were emphasized as common places. The Narkomfin Communal House was designed for bureaucrats of the Russian Ministry of Finance in Moscow in 1928. The residents of the Narkomfin Communal House were led to common spaces in their daily life for eating, reading, doing exercises, etc. F-type apartments in Narkomfin Communal House were thought as transitional type to communal life. However, the assumption that the demand for F-type apartments would increase and the demand for K-type apartments would decrease over time did not realise. In this study, the new definition that was brought to residential planning by ideological context is examined within instances., Çalışmada, ideolojik bağlam ve konut planlaması ilişkisi 1920'lerde Sovyetler Birliği'ndeki konut mimarisinin ideolojiyi desteklemesi üzerinden ele alınmaktadır. Ekim Devrimi ve I. Dünya Savaşı sonrasında yaşanan ekonomik zorlukların yanı sıra teknolojik eksiklikler, kadınların işgücüne katılımının sağlanması konut mimarisi kapsamında birçok mimarın ele aldığı çözümleri etkilemiştir. Bu sorunlara çözüm önermek için 1926'da mimari tasarım yarışması düzenlenmiştir. Bu tasarımların çoğunda anaokulları, kütüphaneler, ortak mutfaklar, ortak yeme- içme mekânları gibi ortak alanlar vurgulanmıştır. Narkomfin Komünal Konutu, 1928 yılında Moskova'da Finans Bakanlığı bürokratları için tasarlanmıştır. Narkomfin Komünal Konutu kullanıcıları günlük yaşamlarında yemek yemek, kitap okumak, egzersiz yapmak vb. için ortak alanlara yönlendirilmiştir. Narkomfin Komünal Konutu’nda F tipi konut birimleri komünal hayata geçiş birimi olarak düşünülmüştür. Ancak, zaman içinde F tipi konut birimlerine olan talebin artacağı ve K tipi konut birimlerinin azalacağı varsayımı gerçekleşmemiştir. Bu çalışmada, ideolojik bağlamın konut planlamasına getirdiği yeni tanım örnekler üzerinden irdelenmektedir.

Community-acquired pneumonia (CAP) is a common type of respiratory tract infections with high morbidity and mortality. Prognostic role of CRP/Albumin ratio in CAP patients still is unknown. The aim of this study was to investigate the role the CRP/albumin ratio in predicting 30-day mortality and ICU requirement in hospitalized patients with CAP. Material and Method: The study included patients with CAP.Clinical records and plain radiographic images of the patients were retrieved from hospital database and were reviewed for each patient. Results: The 179 CAP patients who were hospitalized were included. Tarihi yapılar dönemin kültürel, sosyal, ekonomik özelliklerini beraberinde getiren toplumsal zenginliktir. Tarihi ibadet yapıları ise kentlerin simge yapıları olan önemli mekânlardır. Zaman içinde değişen şartlar, tarihi yapıların özgün işlevlerinin ortadan kalkmasına neden olmakta, yapıların yeniden kullanılabilmeleri için bazı müdahaleler gerektirebilmektedir. Çalışmada, tarihi ibadet yapılarına uygulanan müdahale yaklaşımları dönüşüm olgusu çevresinde, ek tasarım, cephe korunarak iç mekânın dönüşümü ve tamamlama uygulaması şeklinde gruplara ayrılarak irdelenmektedir. Bu yaklaşımlar, Dünya’dan ve ülkemizden seçilen örnekler üzerinden ele alınmıştır. Değişen süreçte, tarihi yapıların kimliğine zarar vermeden, tarihi yapıya yeni anlamlar katarak yapının dönüştürülmesi müdahalesiyle ilgili uygu-lanmış örnekler ışığında bilgiler veren bir çalışma oluşturulmuştur. Çalışmanın amacı, tarihi ibadet yapılarında dönüştürme uygulamalarının örnekleriyle incelenmesi sonucu koruma bilincinin yerleşmesini sağlamak ve müdahalelerde çağdaş bir bakış açısı oluşturmaya çalışmaktır., Historical buildings are social richness that brings the cultural, social and economic characteristics of the period. Historical places of worship are important places that are symbols of cities. Changing conditions over time cause the original functions of historical buildings to disappear, and some interventions may be required for the buildings to be reused. In the study, the intervention approaches applied to historical places of worship are examined by dividing them into groups as additional design, the transformation of the interior by preserving the facade, and the application of completion, around the phenomenon of transformation. These approaches are discussed through selected examples from the world and our country. In the changing process, a study has been created that gives information in the light of applied examples about the intervention of transforming the historical structure by adding new meanings to the historical structure without harming the identity of the historical buildings. The aim of the study is to establish the awareness of conservation and to try to create a contemporary perspective in interventions as a result of examining the conversion practices in historical places of worship.

The historical environment serves as a bridge from the past to the present by bringing together various information about the social, cultural, economic, sociological characteristics, traditions, aesthetic understandings and construction techniques of various civilizations that it has hosted for centuries. Change in the historical environment is inevitable due to various reasons such as the changing needs of the society, population growth and physical damage/dysfunction of historical buildings. A qualified change in the historical environment depends on the designer taking into account the context of the historical environment and choosing the appropriate design approach with the awareness of protecting the historical environment. In order to contribute to researchers and to designers who design new buildings/additions in the historical environment, in the scope of this study, information is given about the context, the historical environment, the new building /addition in the historical environment and the design approaches. In the light of this information, examples of new buildings/additions in the historical environment from various countries have been selected and examined. Then the opinions of various researchers about these samples have been given., Tarihi çevre, yüzyıllardır içinde barındırdığı çeşitli uygarlıkların sosyal, kültürel, ekonomik, sosyolojik özellikleri, gelenekleri, estetik anlayışları, yapım teknikleri ile ilgili çeşitli bilgileri beraberinde getirerek geçmişten günümüze uzanan bir köprü görevi görmektedir. Toplumun değişen ihtiyaçları, nüfus artışı, fiziksel olarak tarihi yapıların zarar görmesi/işlevsiz kalması gibi çeşitli nedenlerle tarihi çevrede bulunan yapıların değişime uğraması kaçınılmaz hale gelmektedir. Tarihi çevrede değişimin nitelikli bir şekilde gerçekleşmesi, tasarımcının tarihi çevrenin bağlamını dikkate alması ve tarihi çevreyi koruma bilinci ile uygun tasarım yaklaşımını seçmesine bağlıdır. Tarihi çevrede yeni yapı/ek tasarlayan tasarımcılara ve araştırmacılara katkı sağlamak amacıyla çalışma kapsamında bağlam, tarihi çevre, tarihi çevrede yeni yapı/ek ve tarihi çevrede yeni yapı tasarım yaklaşımları hakkında bilgi verilmiştir. Bu bilgiler ışığında çeşitli ülkelerden tarihi çevredeki yeni yapı/ek örnekleri seçilerek incelenmiştir. Ardından bu örneklerle ilgili çeşitli araştırmacıların görüşlerine yer verilmiştir.

Amaç: Bu çalışmada, çocuklarda akut apandisit tanısında ortalama trombosit hacmi (MPV)’nin rolüolup olmadığının araştırılması amaçlandı. Yöntemler: Bu çalışmada hasta dosyaları retrospektif olarak tarandı. Klinik ve patolojik olarak akut apandisit tanısı alan 311 olgunun MPV değerleri, günübirlik operasyonlar için başvuran sağlıklı çocuklardan seçilen kontrol grubunun (n=314) MPV değerleri ile karşılaştırıldı. Özet 1 :Yenilenebilirbir gelecek için ekolojik sorunların bilinmesi yeni bir mimarlık tanımını daberaberinde getirmiştir. Öncelikli olarak, sürdürülebilir bir yaşam biçimi ileuyumlu bu tanım ile içinde yaşayanların da katılımının sağlandığı ve ekolojikayak izinin büyük ölçüde azaltıldığı bir mimarlık hedeflenmektedir. Bu amaçlaçevresel, ekonomik ve işlevsel kaygıları ortadan kaldıracak, iklimi ve yerelçevreyi dikkate alan tasarımlar izlerini oluşturmaya başladılar. Birkaç onyıliçinde tasarım yöntemlerini ve planlama süreçlerini değiştiren bu yeni nesiltasarımlar, iklim ve araziye uyum, kütle organizasyonu, gün ışığı ve ısıtasarımı, enerji sistemleri, malzeme özellikleri gibi kendi temel ilkelerinibelirledi. Objective: Pneumonia is a leading cause of mortality worldwide, especially in the elderly. The use of clinical risk scores to determine prognosis is complex and therefore leads to errors in clinical practice. Pneumonia can cause increases in the levels of cardiac biomarkers such as N-terminal pro-brain natriuretic peptide (NT-proBNP). The prognostic role of the NT-proBNP level in community acquired pneumonia (CAP) remains unclear. The aim of this study was to evaluate the prognostic role of the NT-proBNP level in patients with CAP, as well as its correlation with clinical risk scores. Methods: Consecutive inpatients with CAP were enrolled in the study. At hospital admission, venous blood samples were collected for the evaluation of NT-proBNP levels. The Pneumonia Severity Index (PSI) and the Confusion, Urea, Respiratory rate, Blood pressure, and age ≥ 65 years (CURB-65) score were calculated. The primary outcome of interest was all-cause mortality within the first 30 days after hospital admission, and a secondary outcome was ICU admission. Results: The NT-proBNP level was one of the best predictors of 30-day mortality, with an area under the curve (AUC) of 0.735 (95% CI: 0.642-0.828; p < 0.001), as was the PSI, which had an AUC of 0.739 (95% CI: 0.634-0.843; p < 0.001), whereas the CURB-65 had an AUC of only 0.659 (95% CI: 0.556-0.763; p = 0.006). The NT-proBNP cut-off level found to be the best predictor of ICU admission and 30-day mortality was 1,434.5 pg/mL. Conclusions: The NT-proBNP level appears to be a good predictor of ICU admission and 30-day mortality among inpatients with CAP, with a predictive value for mortality comparable to that of the PSI and better than that of the CURB-65 score. RESUMO Objetivo: A pneumonia é uma das principais causas de mortalidade no mundo, especialmente em idosos. O uso de escores de risco clínico para determinar o prognóstico é complexo e, portanto, leva a erros na prática clínica. A pneumonia pode causar aumento nos níveis de biomarcadores cardíacos, como o N-terminal pro-brain natriuretic peptide (NT-proBNP, pró-peptídeo natriurético cerebral N-terminal). O papel prognóstico do nível de NT-proBNP na pneumonia adquirida na comunidade (PAC) continua incerto. O objetivo deste estudo foi avaliar o papel prognóstico do nível de NT-proBNP em pacientes com PAC, bem como sua correlação com escores de risco clínico. Métodos: Pacientes consecutivos internados com PAC foram incluídos no estudo. Na internação hospitalar, foram coletadas amostras de sangue venoso para avaliação dos níveis de NT-proBNP. Foram calculados o Pneumonia Severity Index (PSI, Índice de Gravidade de Pneumonia) e o escore Confusão mental, Ureia, frequência Respiratória, Blood pressure (pressão arterial) e idade ≥ 65 anos (CURB-65). O desfecho primário de interesse foi mortalidade por todas as causas nos primeiros 30 dias após a admissão hospitalar, e um desfecho secundário foi admissão na UTI. Resultados: O nível de NT-proBNP foi um dos melhores preditores de mortalidade em 30 dias, com uma área sob a curva (ASC) de 0,735 (IC95%: 0,642-0,828; p < 0,001), assim como o PSI, que teve uma ASC de 0,739 (IC95%: 0,634-0,843; p < 0,001), enquanto CURB-65 teve uma ASC de apenas 0,659 (IC95%: 0,556-0,763; p = 0,006). O nível de corte do NT-proBNP que mostrou ser o melhor preditor de admissão na UTI e de mortalidade em 30 dias foi de 1.434,5 pg/ml. Conclusões: O nível de NT-proBNP parece ser um bom preditor de admissão na UTI e de mortalidade em 30 dias entre pacientes internados com PAC, com um valor preditivo para mortalidade comparável ao do PSI e superior ao do CURB-65.
