Amaç: Elektrokonvülzif tedavi (EKT), etkin ve yaşam kurtarıcı bir tedavi yöntemi olmakla beraber, etkisi ve yan etkileri hala tartışılmaktadır. Çalışmamızda EKT’nin apne döneminde oksijen (O2) uygulamasının Near Infrared Spektroskopi ile serebral oksijenizayon üzerine ve EKT sonrası komplikasyonlar üzerine etkisini değerlendirmeyi amaçladık. Yöntem: Bu ileriye dönük, gözlemsel çalışmaya ilk kez EKT planlanan 40 hasta katılmıştır. Tüm hasta-lara serebral oksimetre ve bispektral indeks sensörleri (BİS) uygulandı. Hastalar nazal kanül ile 6 L/dk O2 uygula-ması yapılan hastalar Grup O (s=20), O2 uygulanmayan hastalar Grup K (s=20) olarak rastgele ikiye ayrıldı. Nöbet sırasında ve nöbetten 10 dakika sonra hemodinamik veriler, end-tidal karbondioksit (EtCO2), bölgesel serebral doku oksijen satürasyonu (rSO2 ) ve BİS değerleri ile EKT sonrası komplikasyonlar kaydedildi. Sonuçlar: Nöbet sırasında ölçülen rSO2’deki değişim ve demografik özellikler açısından gruplar arasında anlamlı bir fark saptanmadı (p>0.05). EKT sırasında desatürasyon (SpO2, Objective: Electroconvulsive therapy (ECT) is an effective and life-saving treatment modality, while its effects and side effects are still being discussed. In our study, we aimed to demonstrate the effect of O2 administration on the cerebral oxygenation measured by Near Infrared Spectroscopy and the complications after ECT in the apnea period of ECT. Methods: This prospective, observational study was carried out with 40 patients whom ECT had been planned and who were undergoing ECT for the first time. Cerebral oximeter sensors and Bispectral index (BIS) monitoring were applied in all patients as well as routine monitoring. The participants were randomly divided into two groups. The patients who received administration of 6 L/min O2 through nasal cannula were included in Group O (n=20), and the patients who were not administered O2 were included in Group C (n=20). Hemodynamic data after induction, during the seizures and at the 10th min after the seizures, EtCO2, regional cerebral tissue oxygen saturation (rSO2), BIS values and complications after ECT were recorded in both groups. Amaç: Çalışmamızda Zonguldak İlinde yaşayan lise öğrencilerinde internet bağımlılığı (İB) ile dikkat eksikliği hiperaktivite bozukluğu (DEHB) ve tütün, alkol kullanma/deneme arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Yöntem: Çalışmaya üç lisede okuyan 771 öğrenci alınmıştır. Öğrenciler araştırmacıların hazırladığı sosyodemografik veri formu, Chen İnternet Bağımlılığı Ölçeği (CİBÖ), DSM-IV'e dayalı Erişkin Dikkat Eksikliği Bozukluğu/Dikkat Eksikliği Hiperaktivite Bozukluğu (DE/DEHB) Tanı ve Değerlendirme Envanteri ile değerlendirilmiştir.Bulgular: Olguların 61'inde (%7.9) İB, 90'ında (%11.7) DEHB saptandı. Giriş: Yaşlı hastalardaki bilişsel bozulma, işlevsel kayıp için bir risk faktörü olarak tanımlanmıştır. Bu işlevsel bozukluk; demans, depresyon, anksiyete veya medikal hastalıkların belirtisi olabilmektedir. Bu çalışma; hastanede yatan yaşlı hastalarda bilişsel bozukluk sıklığını saptamak ve hastaların bilişsel durumları ile sosyodemografik değişkenler, günlük yaşam aktiviteleri, anksiyete ve depresyon arasındaki ilişkinin incelenmesi amacıyla yapılmıştır. Yöntem: Kesitsel ve tanımlayıcı özellikteki bu çalışmanın örneklemi Bülent Ecevit Üniversitesi Sağlık Uygulama ve Araştırma Merkezi’nde yatarak tedavi gören 65 yaş ve üstü 243 hastadan oluşmaktadır. Hastaları değerlendirmek için Sosyodemografik veri formu, Standardize Mini Mental Test (SMMT), Günlük Yaşam Aktiviteleri Ölçeği, Lawton– Brody Enstrumental Günlük Aktiviteler Ölçeği, Geriatrik Depresyon Ölçeği (GDÖ) ve Beck Anksiyete ölçeği kullanılmıştır. Bulgular: Hastaların 106’sı (%43,6) kadın, 137’si (%56,4) erkektir. Hastalar, SMMT 23/24 kesme puanına göre iki gruba ayrılmıştır. Bilişsel işlev bozukluğu; ileri yaş, kadın cinsiyet, düşük eğitim ve sosyo-ekonomik düzey, kırsal kesimde yaşayan hastalarda istatistik olarak anlamlı olarak daha sık bulunmuştur. Bilişsel işlev bozukluğu olan grubun temel ve yardımcı günlük yaşam aktivitelerinde ve beslenmelerinde daha çok sorun olduğu, anksiyete ve depresyon puanlarının daha yüksek olduğu saptanmıştır. Çalışmamızda, bilişsel yıkım ve GDÖ’ne göre olası depresyon sıklığı sırasıyla %56 ve %48 olmasına rağmen; hastaların sadece %10,5’inin daha önce psikiyatri doktoruna başvurduğu, %9.3’ünün psikiyatrik bir tedavi aldığı öğrenilmiştir. Sonuç: Bilişsel işlevler bozuldukça günlük yaşam aktiviteleri gerilemekte, beslenme bozulmakta ve bağımsız iş yapabilme kapasitesi olumsuz yönde etkilenmektedir. İleri yaş, kadın cinsiyet, düşük eğitim düzeyi ve sosyoekonomik düzey, kırsal kesimde yaşamak bilişsel işlevlerde gerileme ile yakından ilişkilidir. Yaşlılardaki bilişsel gerileme depresyon ve anksiyete ile de ilişkili olabilir. Çalışmamızın sonuçlarına göre; tıbbi hastalığı olan yaşlılardaki bilişsel sorunlar, depresyon ve diğer psikiyatrik sorunların fark edilmemesinin, yaşlı hastaların zihinsel sağlığını olumsuz etkileyebileceği kanaatindeyiz. Yaşlı hastalarla karşılaşan tüm hekimlerin tarama testlerini kullanarak bilişsel yıkım ve depresyonu taraması önemlidir., Introduction: Cognitive impairment in elderly patients, which may be a sign of dementia, depression, anxiety or medical diseases, has been determined as a risk factor for functional loss. In this study, we aimed to investigate the frequency of cognitive impairment and to investigate the relationship of cognitive status with sociodemographic variables, daily living activities, anxiety and depression in elderly inpatients. Method: The sample of this cross-sectional and descriptive study consists of 243 patients aged 65 years and older who were hospitalized in Bülent Ecevit University Hospital. A sociodemographic questionnaire,, the Mini-Mental State Examination (MMSE), Activities of Daily Living Scale, Lawton-Brody Instrumental Daily Activities Scale, Geriatric Depression Scale (GDS) and the Beck Anxiety Inventory were used for data collection. Results: One hundred and six (43.6%) patients were female and 137 (56.4%) were male. The patients were divided into two groups according to the Mini-Mental State Examination (MMSE) 23/24 cut-off score. The cognitive decline was statistically significantly more frequent in patients who were older, female, less educated, low socioeconomic status, and living in rural areas. There were more problems in the basic and instrumental activities of daily living and nutrition in patients with cognitive decline. Anxiety and depression scores were higher in this group. In our study, although the frequency of cognitive decline and depression according to GDS were 56% and 48%, respectively; we found that only 10.5% of patients applied to the psychiatrist, and 9.3% of patients received psychiatric treatment. Conclusion: Cognitive decline may cause deterioration in the daily living activities, nutrition and capacity for independent functioning. Older age, female, low education, low socioeconomic status and living in rural area are important risk factors for cognitive impairment. Cognitive decline in older age may be associated with depression and anxiety. We assume that when cognitive decline, depression and other psychiatric problems are unidentified, it may contribute to deterioration of mental health in medically ill elderly.

Amaç: Bu çalışmanın amacı, psikiyatrik ayaktan hasta popülasyonunda Gece Yeme Sendromunun (GYS) şiddetini ölçmek için kullanılan Gece Yeme Anketinin (GYA) Türkçe sürümünün geçerlilik ve güvenilirliğini değerlendirmektir. Yöntem: GYA'nin Türkçe sürümünün faktör yapısı, iç tutarlılığı ve geçerliliği incelenmiştir. GYA'nin Türkçeye çevirisinin ardından İngilizceye tekrar çeviri yapılmıştır. Düzeltme ve semantik uyarlama sonrası anketin anlaşılabilirliği değerlendirilmiştir. GYS tanısı psikiyatrik görüşme yapılarak konulmuştur. GYA 433 hastaya, iki hafta sonra yapılan tekrar testi ise 141 hastaya uygulanmıştır. Sonuçlar: Cronbach alfa katsayısı 0.69 bulunan anket yeterli iç tutarlılık göstermektedir. Test-tekrar test toplam puanları arasındaki intraclas korelasyon katsayısı 0.96 olarak bulunmuştur. Psikiyatrik değerlendirme sonucunda 97 hastada GYS saptanmıştır. ROC eğrisi altında kalan alan 0.90 olarak bulunmuştur. GYA puanında 18 değeri sınır değer olarak kabul edildiğinde duyarlılık %90.7, seçicilik %73.8 oranındadır. GYA'nin doğrulayıcı faktör analizinde özgün ankete benzer şekilde dört faktörlü yapı elde edilmiştir. Tartışma: Bu çalışmaya göre GYA'nın Türkçe sürümünün geçerli bir test olduğu, iyi tanısal özellik gösterdiği ve iç tutarlılığının yeterli olduğu söylenebilir, Objective: The purpose of this study was to evaluate the reliability and validity of Turkish Version of the Night Eating Questionnaire (NEQ) in psychiatric out-patient population as a measure of severity of the night eating syn-drome (NES). Methods: We examined the factor structure, internal consistency and validity of the NEQ in Turkish. First, we translated NEQ into Turkish and than back-translation into English; correction and semantic adaptation and assessment of the understanding of the questionnaire. We made the diagnosis of NES according to psychiatric examination. We administered the NEQ in 433 patients and the re-test in 141 participants two weeks later. Result: The instrument showed satisfactory internal consistency with an overall Cronbach ? of 0.69.Intraclass correlation for the test re-test total score was 0.96. The ninety seven patients had NES with psychiatric examination. The area under ROC curve defined as 0,904. For cutt-of point of 18 in NEQ both sensitivity and spesificity were as 90.7% and 73.8%. The confirmatory factor analysis of the NEQ yielded a similar four factor solution as the original scale. Conclusions: We found that Turkish version of NEQ is valid and has good diagnos tical performance, and high internal consistency.

Amaç: İş yerinde olumsuz eylemlere maruz kalma, çalışma yaşamında işin psikososyal boyutu kapsamında sağlığa olumsuz etkileri vurgulanan temel konulardan biridir. İş yerinde yıldırma, iş yerindeki stres nedenleri arasında en yaygın ve diğer bütün iş yeri stresörlerinden daha yıkıcı ve zedeleyici olanı olarak tanımlanmaktadır. Bu çalışmada iş yerinde yıldırmaya maruz kalımın değerlendirmesinde kullanılan Olumsuz Davranışlar Anketi Türkçe Formu’nun (NAQ-TR) güvenilirliğinin saptanması amaçlandı. Yöntem: Transkültürel uyarlama ilkeleri dikkate alınarak NAQ Türkçeye çevrildi ve sonra geri çevirileri yapıldı. Geri çeviriler asıllarıyla karşılaştırılarak Türkçede anlaşılabilirlik ve uygulanabilirlik yönünden uzman grubu tarafından değerlendirildi ve anket formunun son hali üzerinde görüş birliğine varıldı. Ölçek, on bir farklı iş yerinden 456 işçiye uygulandı. NAQ-TR’nin faktör yapısı ve iç tutarlılığı incelendi. Bulgular: Ölçeğin iç tutarlılığı yeterliydi ve Cronbach alfa değeri 0.91 olarak bulundu. NAQ-TR’nin faktör analizi sonucunda özgün ölçekten farklı olarak dört alt bileşen (kişisel tehdit ve izolas- yon, mesleğe yönelik tehdit, destabilizasyon ve aşırı iş yükü) saptandı. Ölçek, çalışanlarda yıldırmaya maruz kalımı değerlendirmede kullanılabilir bulundu. Sonuç: Olumsuz Davranışlar Anketi Türkçe Formu, iş yerinde yıldırmanın saptanması ve izlenmesi için güvenilir bir ölçektir ve epidemiyolojik çalışmalarda kullanılabilir., Objective: The exposure to negative acts at work is one of the main issues highlighted adverse health effects within the scope of work life's psychosocial dimension. Mobbing is defined as both the most common one among the causes of stress being at work, and also more devastating and damaging than the other stressors of the work. The aim of the present study was to examine the reliability of the Turkish version of the Negative Act Question- naire (NAQ-TR). Methods: The questionnaire was first translated into Turkish and then back translated into English and the equivalence of both versions was determined. An expert group analyzed the formulation of the translated items and they redrafted some of them according to agreedon criteria. Subsequently, it was adminis- tered to 456 workers from 11 different workplaces. The factor structure and internal consistency of the NAQ-TR was examined. Result: The instrument showed satisfactory internal consistency with an overall Cronbach α of 0.912. The factor analysis, as opposed to the original scale, yielded four factor solution (Personal bullying and isolation, Work-related bullying, destabilization and excessive workload). We found that NAQ-TR has high internal consistency. Conclusions: The Turkish version of NAQ was found to be reliable for mobbing in workplace. It can be used in epidemylogic trials.

Amaç: Türkiye’de cinsel sorunlar sık görülmelerine rağmen cinsel sorunların sıklığı ile ilgili yapılan toplum çalışmaları azdır. Bu araştırmanın amacı, Zonguldak il merkezinde yaşayan evli kadınlarda cinsel sorunların yaygınlığının ve cinsel sorunlarla ilişkili olabilecek etkenlerin belirlenmesidir. Yöntem: Araştırma evreni Zonguldak il merkezinde yaşa- yan 15-49 yaş arasında 517 evli kadını içeriyordu. Katılımcılara sosyodemografik bilgi formu ve Golombok-Rust Cinsel Doyum Ölçeği (GRCDÖ) uygulanmıştır. Bulgular: Katılımcıların yaş ortalaması 36.5±7.87 idi. GRCDÖ’ye göre kadın- arda cinsel sorun sıklığı %45.5’tir. En sık vajinismus (%67.3) ve cinsel sıklık sorunu (%60.3) saptandı. Cinsel dokun- ma sorunu (%54.5), cinsel iletişimsizlik(%54.7), cinsel kaçınma (%54.5), anorgazmi (%23.8) ise diğer sık görülen sorunlardı. Sonuç: Sonuç olarak bizim çalışmamızda yüksek oranda kadınların cinsel bilgi almamış olması, evlerinde cinsel konuların konuşulmaması ve en fazla arkadaşlarından cinsel bilgi almaları cinselliğin toplumumuzda tabu olma- yı sürdürdüğünün göstergesidir. Kadınların tamamına yakınının evlilik öncesi cinsel deneyimlerinin olmaması beka- retin önemini sürdürdüğünü göstermektedir. The participants were administered to GRISS and sociodemographical questionnaires for the evaluation of Female Sexual Dysfunction. Findings: The mean age of participants was 36.5±7.87. According to GRISS, 45.5% of women had female sexual dysfunction. The most frequent sexual dysfunctions were vaginismus (67.3%) and sexual frequency problems (60.3%). Sexual non-sensuality prob- lem (54.5%), sexual non-communication (54.7%), sexual avoidance (54.5%) and anorgasmia (23.8%) were among other prevalent problems. Conclusion: As a result of our study, the inadequate sexual knowledge of women, restric- tion of sexual talk at home and information exchange with friends being the most common method of sexual know- ledge indicated that sexuality is still a taboo in our community. Amaç: Geç başlangıçlı şizofreni hastalarının kliniğinde, şizofreni hastalarına benzer şekilde paranoid sanrılar, kötülük görme sanrıları ve işitsel halüsinasyonlar görülür fakat duygulanımda küntleşme, negatif belirtiler ve ailede şizofreni öyküsü daha azdır. Bu çalışmanın amacı kliniğimizde yatarak tedavi gören geç başlangıçlı şizofreni hastalarının klinik özellikleri ve tedavi yanıtlarının gözden geçirilmesidir. Yöntem: 2001-2010 yılları arasında Karaelmas Üniversitesi Tıp Fakültesi psikiyatri kliniğinde tedavi gören 26 geç başlangıçlı şizofreni hastasının sosyodemografik özellikleri, DSM-IV tanıları, klinik özellikleri, hastalığın başlamasından bu yana geçen süre, hastanede yatış süresi, tedavide kullanılan ilaçlar ve tedavi yanıtı, vitamin B12 ve folik asit kan düzeyleri ve görüntüleme tekniklerinin sonuçları yatış dosyaları taranarak kaydedildi. Bulgular: Hastaların yaş ortalaması 54.2±6.25 (45-69), 15'i (%57.7) erkek, 11'i (%42.3) kadın, hastalık başlangıç yaşı ortalaması 41.8±2.4 (40- 48) bulundu. Hastaların %85'inde sanrı, %46'sında halüsinasyon, %30'unda ajitasyon, %27'sinde apati ve %12'sinde bilişsel sorun vardı. Beyin görüntüleme yapılmış 17 hastadan 8'inde yapısal sorunlar bildirilmişti. Hastaların %54'ünde B12 eksikliği, %4'ünde folik asit eksikliği mevcuttu. Sonuç: Geç başlangıçlı şizofreni hastalarında beyin görüntüleme ve diğer tıbbi değerlendirmeleri yapmanın önemli olduğunu düşünüyoruz. Bu durumun epidemiyoloji, fenomenoloji, genetik, biyolojik ve psikososyal yönü, süreç ve sonucunu daha iyi anlamak için daha ileri çalışmalara ihtiyaç vardır., Objective: The late onset schizophrenia patients had symptoms similar to those of schizophrenia patients like paranoid delusions, persecutory delusions and auditory hallucinations but with less affective flattening, negatif signs and family history of schizophrenia. The purpose of this paper was overwiew the clinic features and treatment response of late onset schizophrenia patients. Method: We collected data about sociodemographic features, DSM-IV diagnosis, clinic features, period of disease, inpatient periods, medication and treatment response, blood levels of vitamin B12 and folic asid, imaging technique results of 26 late onset schizophrenia patients that was treated in Psychiatry Clinic of Karaelmas University from 2001 to 2010. Results: The mean age of patients was 54.2±6.25 (range: 45-69), 15 (57.7%) were male, 11 (42.3%) were female. The mean age of onset was 41.8±2.4 (range: 40-48). Delusions were present in 85%, hallucinations were present in 46%, ajitation were present in 30%, apathy were present in 27% and cognitif problems were present in 12% of the patients. The structural abnormalities were present in 8 of 17 patients who had brain imaging. The patients who had abnormal imaging results have longer inpatient periods. The vitamin B12 and folic asid deficiency were present in 54% and 4% of the patient, respectively. Conclusion: İt is important that doing brain imaging and other medical assesments in the late onset schizophrenia. Further systematic research into its epidemiology, phenomenology, genetics, biological and psychosocial issues, course and outcome are necesary for a beter understanding of this condition.

Periferik ödem çoğunlukla sistemik hastalıklar ve non-steroid, antihipertansif, steroid, immün baskılayıcılar gibi çeşitli ilaçlarla ilişkilidir. Atipik bir antipsikotik olan olanzapin nadir olarak periferik ödeme neden olur. Olanzapine bağlı ödemin oluş nedenleri hakkındaki açıklamalar varsayım düzeyinde devam etmektedir. Bu olgu sunumunda bir şizofreni hastasında olanzapin tedavisinin başlanmasından kısa süre sonra ortaya çıkan bir periferik ödem olgusu sunulmuş ve bu fenomenin teorik açıklamaları tartışılmıştır., Peripheral edema is most commonly associated with multiple medical etiologies and drugs including nonsteroidal anti-inşamatory, antihypertensives, steroids, and immunosuppresive agents. The atypical antipsychotic olanzapine is rarely related with peripheral edema. However relationship of olanzapine and edema remains as a hypothesis. In this case report a peripheral edema developing shortly after initiation of treatment in a patient with schizophrenia is reported and theoretical explanations of this phenomenon are discussed.(

Amaç: Çalışmanın amacı postpartum dönemde depresif belirtilere neden olabilecek ve düzeyini etkileyen risk etkenlerini ortaya çıkarmaktır. Gereç ve Yöntemler: Örneklemi oluşturan 97 kadın kadın doğum kliniğimizden toplanmıştır. Bu kadınlara doğumdan sonraki birinci ve altıncı haftada Edinburgh Doğum Sonrası Depresyon ölçeği uygulanmıştır. Çalışma grubu ölçeğe göre depresif olan ve olmayan grup olarak ikiye bölünmüştür. İki grup arasında sosyodemografik ve doğumla ilgili özellikler karşılaştırılmış; ayrıca birinci ve altıncı haftadaki depresif belirti düzeyinin korelasyonuna bakılmıştır. Bulgular: Doksanyedi kadının 19'u ölçeğe göre depresif bulunmuştur (%19,5). Çalışma durumu dışında iki grup arasında sosyodemografik ve doğumla ilgili özellikler açısından fark bulunmamıştır. Depresif semptom düzeyi çalışan kadınlarda anlamlı derecede daha yüksek bulunmuştur (p=0.02). Birinci ve altıncı haftadaki depresif belirti düzeyi arasında anlamlı derecede pozitif korelasyon saptanmıştır (r=0,825 p=0,01). Sonuç: Postpartum depresyon önemli bir sağlık sorunudur. Özellikle çalışan kadınlarda depresyon riski daha yüksektir. Doğumdan sonraki erken dönemde var olan depresif belirtiler daha sonraki haftalarda devam edebilir. Klinisyenler bu durumun önemi konusunda farkında olmalıdır., Objective: The aim of the study is to find out the risk factors which may effect the development of depressive signs during postpartum period. Material and Methods: A sample of 97 women have been recruited from our obstetric clinic. These women, in the first and sixth weeks after delivery, were administered Edinburgh Pospartum Depression Scale (EPDS). Study group were divided into two groups according to scale : depressive and non-depressive group. Sociodemographic and obstetric features were compared between two groups, and correlation first and sixth week depressive levels were analysed. Results: Nineteen of 97 women (%19,5) were depressive according to EPDS. It was not found difference between two groups in terms of sociodemograhic and obstetric features except occupation. Depressive symptom level was statisticaly significantly higher in working women (p=0.02). It was found a significant correlation between the first and sixth weeks depressive symptom level (r=0,825 p=0,01). Conclusion: Postpartum depression is an important health problem. Especially working women have higher risk for development of depression. Depressive signs in early days in postpartum period may continue in later weeks. Clinicians should be aware of importance of this condition.

Amaç: Süregen günlük baş ağrısında az araştırılmış bir konu olan psikiyatrik komorbiditenin yaygınlığını araştırmayı amaçladık. Yöntem: Süregen günlük baş ağrılı 115 hasta çalışmaya alındı. Psikiyatrik değerlendirme DSM-IV eksen I bozuklukları için yapılandırılmış klinik görüşme kılavuzuna göre yapıldı (SCID-CV). Beck Anksi­yete ve Depresyon ölçekleri uygulandı. Bulgular: 55 hasta süregen migren, 56 hasta süregen gerilim tipi ve dört hasta yeni günlük sürekli baş ağrısı tanısı aldı. Süregen migren hastalarının %45.5’inde komorbidite saptandı: 6'sı depresyon (%10.9), 11’i distimi (%20), 5'i panik bozukluğu (%9.0), 2'si yaygın anksiyete bozukluğu (%3.6) ve 1'i obsesif kompulsif bozukluk (%1.8) tanısı aldı. Süregen gerilim tipi baş ağrısı hastalarının %55.4’ünde komorbidite vardı: 10’u depresyon (%17.8), 13’ü distimi (%23.2), 4'ü panik bozukluğu (%7.1), 3'ü yaygın anksiyete bozukluğu (%5.3) ve 1'i somatizasyon bozukluğu (%1.7) idi. İlaç fazla kullanımı oranı süregen günlük baş ağrısında (%69.5) yüksek bulundu. İlaç fazla kullanımı olan süregen gerilim baş ağrısı grubunda komorbidite oranı ilaç fazla kullanımı olmayan gruba göre yüksek bulundu (%70’e %38.5 p=0.018). Sonuç: Psikiyatrik komorbiditenin süregen günlük baş ağrısı hastalarında yaygın olduğunu ve ilaç fazla kullanımı açısından risk etkeni olabileceğini düşünüyoruz. Baş ağrısı hastalarında psikiyatrik komorbiditenin dikkate alınması gerektiğine inanıyoruz., Objectives: Psychiatric comorbidity in headache disorders has been studied rarely, therefore in this study we aimed to investigate the prevalence of psychiatric comorbidity in chronic daily headache. Method: We recruited 115 consecutive patients with chronic daily headache. Psychiatric evaluation of the patients was made according to Structured Clinical Interviewing for DSM-IV Axis I Disorders Clinical Version (SCID-CV). Anxiety and depression levels were scored with Beck anxiety and depression scale. Results: Chronic migraine was diagnosed in 55 patients, chronic tension type headache in 56 patients and new daily persistant headache in 4 patients. 25 patients (45.5%) with chronic migraine had psychiatric comorbidity; including major depression in 6 patients (10.9%), dystimia in 11 (20%), panic disorder in 5 (9.0%), obsessive compulsive disorder in 1 (1.8%), and generalized anxiety disorder in 2 (3.6%). 31 patients (55.4%) with chronic tension type headache had psychiatric comorbidity; including major depression in 10 patients (17.8%), dysthimia in 13 (23.2%), panic disorder in 4 (7.1%), generalized anxiety disorder in 3 (5.3%), and somatization disorder in 1 (1.7%). Medication overuse ratio was 69.5% in chronic daily headache patients. Psychiatric comorbidity ratio was significantly higher in chronic tension type headache with medication overuse patients than in patients without medication overuse (70% vs 38.5% p=0.018). Conclusion: We thought that these results demonstrate that psychiatric comorbidity is highly prevalent in chronic daily headache patients and may be a risk factor for medication overuse.
