26. Primer epiploik apendagitis: Bilgisayarlı tomografi bulguları

Abstract

Primary epiploic appendagitis is a rare inflammatory disease caused by either torsion or spontaneous venous thrombosis of colonic epiploic appendices, with subsequent infarction and inflammation. Because of its rarity and nonspecific presentation, the clinical diagnosis is difficult and most patients are misdiagnosed as diverticulitis, appendicitis, or cholecystitis. Ultrasonography and computed tomography allow a reliable diagnosis and render invasive procedures such as colonoscopy and surgery unnecessary. We report the case of a 59 year old woman with primary epiploic appendagitis and discuss the pathognomonic computerized tomography findings of this entity in the light of the literature., Primer epiploik apendagitis kolonik epiploik apendikslerin torsiyon ya da spontan venöz tromboz sonucu iskemi ya da inflamasyonu ile karakterize nadir bir hastalıktır. Nadir olması ve nonspesifik belirtileri nedeni ile primer epiploik apendagitisin klinik tanısı zordur ve hastaların çoğu divertikülit, apandisit ya da kolesistit gibi yanlış tanılar alır. Ultrasonografi ve bilgisayarlı tomografi incelemeleri güvenilir tanı sağlayarak kolonoskopi gibi invaziv prosedürleri ve gereksiz cerrahiyi önler. Bu bildiride primer epiploik apendagitisli 59 yaşında kadın olgu sunulmakta ve bu antitenin literatüre uygun olarak patognomonik bilgisayarlı tomografi bulguları tartışılmaktadır.

Abstract

A 34-year-old man was admitted with dyspnea, cough, and fever. Thorax computed tomography revealed ground glass opacities and pneumomediastinum. The patient was diagnosed as amyopathic dermatomyositis due to skin lesions and radiological findings. Despite immunosuppressive treatment clinical deterioration and radiological progression were observed and the patient died because of severe hypoxemic respiratory failure. The patient presented with extremely rare occurrence of pneumomediastinum and subcutaneous emphysema in amyopathic dermatomyositis with a poor prognosis.
