Introduction: Temporomandibular disease is a term that include disorders of the normal function of the temporomandibular joints, masticatory muscles and surrounding structures. These diseases have multifactorial etiology, which prevalence varies from 20% to 50%. The main goal of this research was to determine the presence of etiological factors and prevalence of temporomandibular diseases among students of Dental medicine at the University „Goce Delcev‟ in Stip. Materials and method: The research was conducted by using a Fonseca questionnaire among 73 students aged from 19 to 27 at the Faculty of medical science at UGD, of which 39 female and 34 male. The respondents were divided into three groups: Group 1 - from 19 to 21 years; Group 2 - from 22 to 24 years and Group 3 - from 25 to 27 years. The data were statistically analyzed using statistical program Statistica 7.1; SPSS17.0. Results: Difficulty opening the mouth, the presence of fatigue or pain in the masticatory muscles, pain in the ear or surrounding area, difficulty moving the lower jaw from side to side, pain or stiffness in the neck, TMJ clicking sounds when chewing or opening, as well as clenching and gnashing of teeth, are symptoms that are predominant in the female population. Conclusion: Early diagnosis of temporomandibular disorders is important to prevent major damage to the temporomandibular joints. The Fonseca questionnaire has its importance in the early diagnosis of these disorders in a young population, because many respondents have noticed the presence of symptoms and etiological factors but do not pay attention to them because they do not feel pain. Една од најважните компоненти на ортодонтската дијагноза и планирањето на третманот е евалуација на профилот на меките ткива на пациентот. Цел: Целта на оваа студија беше да се проценат просечните аголни варијабили што го дефинираат профилот на лицето на меките ткива кај примерок со различен тип на малоклузии. Материјал и метод: Анализирани се фотографии од профилот на лицето на 15 учесници (6 мажи и 9 жени) на возраст меѓу 20 и 31 година, со малоклузија I, II и III класа. Резултат: Просечните аголни мерења независно од типот на малоклузија се фацијален агол 168,7°, назофронтален агол 139°, назолабијален агол 108,2° и ментолабијален агол 123,7°. Аголот на фацијален конвексност кај I, II и III класа ги има следните резултати 169,2°, 164,9° и 172°. Кај II класа за жени 168,8° и мажи 162,2°; кај III класа 172,3° и 170,8°. Просечните мерни вредности за назофронталниот агол кај I, II и III класа се 135,6°, 136.6° и 144,9°. Кај II класа за жени 141,1° и мажи 133,6°; III класа 147.7° и 143°. Средните вредности за назолабијалниот агол се 104,5°, 110,8° и 109,3°. Кај II класа, жени 111,9° и мажи 110,1° додека пак кај III класа 113,1° и 106,8°. Вредностите на ментолабијалниот агол се 120,4°, 109° и 141,8°. Овој агол кај II класа изнесува 110,1° за жени и 108,3° за мажи. Кај III класа вредностите се 130,4° и 149,5°. Кај учесниците со II и III класа, сите измерени агли се поголеми кај жените отколку кај мажите. Само ментолабијалниот агол кај III класа е поголем кај мажите. Во однос на типот на малоклузијата, добиени се најголеми вредности кај III класа, со исклучок на назолабијалниот агол каде најголеми вредности се добиени кај II класа. Заклучок: Просечните вредности добиени од овој примерок може да се користат за споредба со резултати од други студии,како и за дијагноза и планирање на третман.

Enucleation of the cystic lesion results in bone defect that can be repared spontaneously in short or long period depending on the size of the lesion, without additional and assisted defect regeneration. Aim: This study has purpose to evaluate the spontaneous bone healing of the defect after completed cystic lesion enucleation. Material and method: The study sample consisted of 22 patients, 14 males and 8 females. After the completed oral surgery treatment and the whole cyst enucleation, control rtg images were made to follow the bone regeneration and the cyst defect reduction. Control examinations were done 6 and 12 months after the surgery treatment with roentgen assessment by comparing the images from before and after the surgical treatment, analysis of the created bone trabeculеs аnd the density of every following image. Results: the gathered results from the control examinations about the type of healing gave the following information: the bone healing was evidented in 18 participants from the total number or (82%), from which 11 participants are males (50%) and the rest 7 participants are females (32%). Fibrous regeneration was registered in 4 cases from the participants (18%), from which 3 participants are males (13.5%) and one is female (4.5%). Conclusion: Тhe physiological process of coagulation is base for spontaneous bone formation. In most of the cases the reparation is with creating bone trabecules and in smaller percent is with fibrous regeneration. In cases when the defect is significantly large, if it is surrounded with appropriate bone walls, the size of the defect is not a problem for spontaneous bone healing. It is evident that the defect reduction starts from the defect margins going towards the middle, what causes reduction on the defect size, and highest increase of bone density is happening in the period after 6 months.

Aim: The aim is to assess the position of the lips in relation to the three reference lines in individuals with class I, II and III malocclusion. Material and method: The sample includes 15 individuals (9 females and 6 males), aged 22 years without a history of previous orthodontic treatment, divided into three groups based on the Angle classification. The first group consists of subjects with class I malocclusion, the second group with class II malocclusion and the third group with class III malocclusion. Profile photographs were taken using a Nikon Z6 II-24.5 MP digital camera with a NIKKOR Z 85mm f/1.8 lens and a Godox AD200 flash. Reference planes used are the Steiner S-line, the Rickett E-line and the Burstone B-line. Results: The mean value of the position of the upper and lower lip was calculated. Rickett E-line in the first group (UL=-3.7; LL=-1.8), second group (UL=-4.87; LL=- 4.75), third group (UL=-6.5; LL=-4), while in relation to the whole sample the following results were obtained (-5.03; -3.52). S-line in the first group (UL=-0.5, LL=-0.2), second group (UL=-1.6; LL=-2.7), third group (UL=-3.1; LL=-1.8), whole sample (-1.7; -1.6). Burstone B-line in the first group (UL=3.75, LL=1.8), second group (UL =1.62; LL=-1.2), third group (UL=0.8; LL=0.51), whole sample (2.06; 0.37).Regarding the E line, the greatest retrusion of the upper lip was observed in the subjects with class III malocclusion, the highest values of the lower lip were obtained in class II malocclusion. When evaluated according to Steiner’s reference line, similar results were obtained. During the B-line analysis, the largest deviations from the reference values were observed in relation to the position of the upper lip in the subjects with class III malocclusion and the lower lip in the class II malocclusion. Conclusion: The position of the lips has been found to be related to the type of malocclusion.

Естетските вредности денес се разликуваат од оние што биле стандард до пред неколку децении. Лицевата естетика еволуира од едноставна микроестетика на забите постигната со ортодонтски третман до макро естетика на лицето како и начинот на кој забите влијаат на лицевите пропорции. Овие пропорции се оценуваат како перцепции за лицева убавина. Естетската перцепција е променлив фактор и зависи од поединецот и е под големо влијание од неговото лично искуство и социјалното опкружување, тоа е една од причините зошто професионалното мислење за евалуација на лицевата естетика не може секогаш да се совпаѓа со перцепцијата и очекување на пациенти или општата популација. Естетска и атрактивна насмевка е една од најважните компоненти коja влијаe на целокупната лицева естетика. При ортодонтски третман покрај основните карактеристики на насмевка, големината, обликот и бојата на забите, фактори кои придонесуваат за добивање на една атрактивна насмевка се однесуваат на карактеристиките на гингивата (боја, контура, текстура, гингивална видливост), присуство на smile arc, видливост на забите како и просторот на букалните коридори. Маргарет Волф Хангерфорд славно изјавила „Убавината е во окото на гледачот“, естетската перцепција е субјективна. Целта на овој труд е да се образложат букалните коридори како една од осумте главни компоненти на насмевката и да се елаборира нивното влијание врз ортодонтската дијагноза и планирање на третман. Во литературата постојат многу обиди за проучување на влијанието на ортодонтскиот третман врз лицевата естетика особено естетиката на насмевката. Постојат различни начини според кој информациите се собираат, анализираат и објавуваат. Пребарувањето за селекција на трудови е направено во базата на податоци на PubMed, преку предтходно дефинирана стратегија и се опфатени трудови од 1958 до 2022 година. Користени се помошни алатки односно селекција на трудови во последните 5 години, кои го опишуваат влијанието на ортодонтскиот третман врз лицевата естетика и атрактивноста на насмевката како и перцепција на насмевката од различен панел (ортодонти, стоматолози, општа популација). Минималниот простор на букалните коридори може да се смета како една од преферираните естетски карактеристики, оние лица со големи букални коридори може да се сметаат како субјекти за ортодонтски третман од естетски аспект. A group of phenotypically similar bacteria, collectively known as mutant streptococci, are considered as the main bacterial components responsible for the onset and development of cavities. The aim of our study is to identify the salivary bacteria (Lactobacillus spp., Streptococcus mutans, Streptococcus sobrinus, Streptococcus salivarius and Streptococcus mitis) and analyze their interdependence with the dental status. The study included 71 children (26 female and 45 male) at the age of 12 years. According to their dental health status, they were divided into: control group - 31 examinees without caries, missing teeth (extractions) and dental fillings (DMF = 0); and experimental group - 40 examinees with caries, missing teeth (extractions) and dental fillings. In all examinees clinical and microbiological examinations were carried out. The lactobacilli in the saliva were determined with a diagnostic test CRT-bacteria (Vivadent, Schaan, Lihtenstein). Undivided sputum samples with sterile swabs were planted on Mitis Salivarius Agar (Fluka, a substrate with sucrose, glucose, trypan blue and crystal violet) which is recommended for the isolation of mixed cultures of streptococci, in particular: Streptococcus mitis, Streptococcus mutans, Streptococcus salivarius, Enterococcus faecalis, etc. Between the detected bacteria in the saliva - Streptococcus mutans, Streptococcus sobrinus, Streptococcus salivarius, Streptococcus mitis and the existence of dental caries there is a significant correlation (p < 0.001). Between the presences of Lactobacillus spp. in the saliva and the existence of dental caries there is a significant correlation (p < 0.01). The salivary bacterial parameters can be used as serious screening factors and can seriously participate as an instrument in the assessment of the dental caries risk. Dental and oral care are important for institutionalized elderly because oral diseases can have a negative impact not only onquality of life but also on general health. Maintaining adequate oral hygiene among the institutionalized elderly is most important,especially on patients that need help for basic or additional oral and dental care. Most often responsible for maintaining oral hygiene are the persons responsible for long-term care of the institutionalized elderly - nurses and paramedics. The education of stuff responsible for institutionalized elderly includes the usage of appropriate lectures with adequate audiovisual effects,appropriate presentations and exercises and discussions among the participants. For adequate dental care of the institutionalized elderly, it is necessary for the elderly, as well as the staff responsible for their care, to be highly aware and motivated to pay enough attention to oral health and hygiene, as well as wearing dentures. Oral health programs and protocols can also be used to educate institutionalized elderly, as well as non-dental staff caring for the institutionalized elderly such as GPs, nurses, nutritionists, and social workers, to change the approach to oral health and its promotion. When the patient is unable to maintain oral hygiene on his / her own, it is necessary to educate the family members and the staff responsible for them regarding their knowledge and training in maintaining oral hygiene. Background: The aim of this study was to evaluate the effect after implementation of a set of preventive measures trough the “National Strategy for prevention of oral diseases of children aged 0-14 years in Republic of North Macedonia (RNM) from 2008-2028” and the resulting need for orthodontic treatment in 12 years old Macedonian children. Material and method: This research was made in all 8 regions in RNM, including examined representatives from urban and rural areas equally.Using the WHO methodology for preparing Oral health studies, 7169 children, born in the year of 2007, were examined (33.3% from all the children in that generation).3117 of them were boys and 3050 were girls. The examinations were made in dental offices, by calibrated dentists-pedodontists, all employed in the public health sector. The decay-missing-filled (DMF)index was used as an appropriate measure for the detection of dental caries where 12-year-old children were considered as target group. Impressions were taken from 100 children (50 boys and 50 girls) with premature tooth loss to analyze the space condition and dental arch space changes. Results: After 11 years of implementation of preventive measures, evaluation showed impressive results – significant decrease of DMF index (from 6.88 to 2.43). Orthodontic analysis in the examined group with premature tooth loss showed greater extend of space loss and an increase in orthodontic treatment need. Conclusion: The implementation of the National strategy for prevention of oral diseases in RNM was an effective approach for preventing caries-related premature tooth loss in 12 years old children, preserving relevant dental arch length.

Хиподонција претставува развојно нарушување каде што имаме отсуство на еден или повеќе заби. Постојат повеќе термини кои се користат за дефинирање на отсуството на еден или повеќе заби, како што се хиподонција, олигодонција или анодонција. Присуството на хиподонцијата во млечната дентиција нема значителна разлика во полот, меѓутоа кај перманентните заби истата е повеќе застапена кај женскиот пол. Етиологијата за ова развојно нарушување може да биде наследна, меѓутоа постои и комбинација на разни бројни генетски и еколошки фактори кои влијаат во настанување на истата. Хиподонција може да се појави и без наследна етиологија, исто така можна е појавата да биде самостојна, во комбинација со одредени синдроми, како и при состојби на расцепи на усни и непце. Најчесто е присутна кај максиларни латерални инцизиви , мандибуларни втори премолари како и третите молари во обете вилици. Таа може да се класифицира во следните три групи во однос на бројот на заби што недостасуваат: блага хиподонција: 1-2 заби што недостасуваат, умерена хиподонција : 3-5 заби кои недостасуваат и тешка хиподонција: 6 или повеќе заби што недостасуваат. Клинички манифестации кои се поврзани со присуство на хиподонција се поделени во две големи групи и тоа дентални карактеристики како и краниофацијални карактеристики. Постојат повеќе опции за третман на хиподонција кои зависат од степенот на присутната хиподонција. Планирањето на третманот зависи од повеќе фактор и тоа : орално здравје и мотивираност на пациентот, возраст на пациентот, позиција и состојба на присутните заби во устата на пациентот, скелетни и мекоткивни промени на профилот како и анатомските карактеристики во максила и мандибула. Генерално во сите случаи на присутна хиподонција постојат три опции на третман: прифаќање на присутната ситуација без преземање на било каква интервенција, ортодонтски третман, протететски третман како и оралнохируршки интервенции со поставување на дентални импланти. Опциите на третман ќе зависат од сериозноста на случајот. Мериленд мостовите како една од опциите за третирање на хиподонција може да се користи како избор при третман на случаи со лесна хиподонција, но исто така кај овие пациенти можеме да пристапиме со ортодонтски третман, со затворање на просторот со фиксни апарати. Во зависност од степенот на изразеност на хиподонцијата, од особена важност е да се постави точна дијагноза, како и да се постави ефикасен план на третман во соодветна координација помеѓу ортодонтот, протетичар, орален хирург како и детски стоматолог.

Material and Method: The sample consisted of 49 patients (boys and girls) who had a Class III Malocclusion with an anterior crossbite and a component of maxillary deficiency. 28 of them were treated with protraction Face mask - Delair mask (petit type) and the other 21 were presenting the control group consisted of untreated Class III Patients. In treated group pretreatment and posttreatment cephalometric radiographs from 28 patients (15 males and 13 females) were analyzed and compared with the results of cephalometric analyzes in untreated group (observation period of 1 year). Fixed retainers are most commonly used to stabilize dental elements after orthodontic treatment. Aim: The main aim of this study was to make short-term assessment periodontal tissue health status of lower incisors of patients with fixed mandibular retainers. Material and Method: Total number of examined subjects were 40 20 male and 20 female patients with fixed mandibular retainers. Before placing the retainer, an oral hygiene session was performed for all patients. All the subjects were monitored after 3 and 12 months. The following indexes were taken into consideration: gingival index (GI) plaque index (PI) the probing depth and the presence of gingival recession on the six inferior frontal teeth. Results: Mean score for the Plaque Index (PI) after three months was 1.08and after 12 months was 1.74. The mean Gingival Index (GI) after 12 months was 1.24 and after three months was 1.01. Periodontal sockets and presence of gingival recession was not found. In our study was found a statistically significant difference for the Plaque Indexes (P>0.05) in both groups with higher scores among subjects having retainers for 12 months. Conclusion: From the analysis of the data obtained in our study we can conclude that for a period from 3 to 12 months the effects of the orthodontic retainers on the periodontal health are minimal.

Orthodontic mini implants are an inseparable part of the modern treatment of dental defects. The greatest advantage of the mini-implants is that they allow the movement of several teeth without loss of anchorage. Aim: The main aim of this research was to evaluate patients’ perception of mini-implants in context of pain hygiene aesthetics and benefits of their usage. Material and Method: Total number of 12 patients with mini implants was examined. All the mini-implants were placed between maxillary first premolars and first molars. Adequate questionnaire containing questions about pain perception during mini-implant placement and use difficulty with cleaning un-aesthetic appearance difficulty with eating and the benefits was used. Results: After analyzing the data we noted that the factor with the most negative impact on orthodontic mini-implant placement is pain and discomfort during placement followed by the difficulty with cleaning. The factors that had the least negative impact were unaesthetic social appearance and difficulty with eating. Score for benefits and advantages observed by the patients was very high indicating very good satisfaction with the end result. Conclusion: Although some patients evaluated aspects of orthodontic mini-implants usage negatively the mean score for benefits observed are very high indicating good patient satisfaction with treatment. Orthodontic treatment, especially during the period of active growth, can significantly affect the development of the jaws and teeth. Treatment with extraction of molars is more complicated because the space that needs to be closed is larger than the premolar spaces and the time of treatment will be longer. However, in cases where we have a lack of space, teeth protrusion or dental asymmetry, and the extraction of permanent teeth is indicated, the damaged molars should be the first option for extraction when premolars are in better condition. The third molars in different ways are affected by orthodontic therapy. One of them is orthodontic treatment by applying the extraction of the third molars, this situation being widely explored. Extraction of third molar for orthodontic reasons is justified when it prevents the eruption of the second molar or otherwise affects the other teeth. The indications of extraction and selection of teeth to be removed depend on the specific aspects of malocclusion. Premolars are teeth that are usually extracted from orthodontic reasons, but this is not the rule and depends on individual treatment needs. The decision to correct the malocclusion with teeth extraction should be based on the ratio of the size of the dental arch with the size of the teeth, facial appearance and other factors arising from the diagnosis itself. Indications for extraction of the third molars during orthodontic treatment are: mandibular incisor crowding, if they in some way constitute a obstruction of orthodontic treatment as well as preparation for orthognathic surgery. The success of an orthodontic treatment involving the extraction of molars is directly related to the patient complaints and psychological profile, the absence of diseases and skeletal discrepancies, the professional experience as well as the fact whether the patient underwent previous orthodontic treatment with extraction. Patients with partially or completely non-erupted teeth are significantly more likely to have certain consequences: before, during or after the orthodontic treatment. Saliva, acquired pellicle, dental structures and their relation to soft tissues in the oral cavity and tongue are in direct contact with the development of dental erosions. During the erosive attack of the saliva, its protective mechanisms such as dilution of erosive agents, neutralization and buffering of acids and slows down the decomposition of the enamel through the common ionic effect of calcium and phosphates. Saliva offers protective effects that act continuously, and at the same time, and dynamic effects that act during the challenge. Salivary flow and neutralization of acids are important dynamic effects of saliva that prevent demineralization. If these two effects are compared, the acid buffer is the most important because it is associated with improved remineralization. Fluoride in the saliva (from toothpaste and tooth materials and from foods and beverages) can cause remineralization and prevent demineralization. In a healthy environment, the pH of the inactive saliva is maintained at the limits of 6.7 to 7.4. The bicarbonate system buffer (HCO3) is the largest buffer present in the saliva. Just like in the peripheral blood, the combination of sodium bicarbonate, carbonic acid and gaseous carbon dioxide is an effective way of releasing the protons (hydrogen ions) from the system. When considering the dynamics of the buffer system. Saliva contains a large number of inorganic ions including calcium, phosphates, fluoride, magnesium, sodium, potassium and chloride.

Вовед: Болката е главната грижа на пациентите пред започнување на ортодонтскиот третман со фиксни апарати. Пациентите доживуваат болка и непријатност кои се однесуваат на забниот потпорен апарат поради силата што се применува на забите. Цел: Целта на оваа студија беше да се испитаат несаканите ефекти поврзани со болката по поставувањето на фиксните протези. Материјали и методи: 20 пациенти, 10 мажи и 10 жени, на возраст од 18 до 37 години (средна возраст од 24,7 ± 4,2 години) беа вклучени и третирани во приватна стоматолошка ординација. Од пациентите беше побарано да ги пополнат прашалниците секоја вечер од поставувањето на протезите во текот на првиот месец и да го евидентираат присуството на болка (да / не) како и карактеристиките на самата болка. Карактеристиките на болката беа индицирани со употреба на да / не одговори за: постојана болка, болка во текот на ноќта и болка при џвакање или гризење на храната. Резултати: Најголем дел од пациентите пријавија болка во текот на првите 7 дена по поставувањето, по што ниту еден пациент не пријави болка. По првите 24 часа, болката е пријавена од страна на 95% од пациентите, а приближно 35% пријавија дека се буделе во текот на ноќта. Сите известија за екстремни потешкотии во џвакањето и одгризувањето на храната со цврста конзистентност во овој период. Немаше значајна разлика помеѓу двете групи (мажи и жени). Заклучок: Пациентите доживуваат болка и непријатност по поставување на ортодонтски протези. Во текот на третманот потребно е пациентите да ја променат конзистентноста на храната во нивната исхрана, а пред самото започнување на третманот потребно е тие да бидат советувани.

GOAL: Streptococcus mutans and streptococcus sobrinus are considered to be the main etiological agents for the appearance of dental caries in humans. It has been established that in children of pre-school age in whose saliva s. mutans and s. sobrinus have a significantly higher incidence of dental caries than those with the presence of only s. mutans. METHODS AND MATERIALS: In the examination we included 74 examinees at the age from 4-6 years (primary dentition), from which 31 were with a dmft=0 (control group) and 43 examinees were with dental caries (studied group). Isolation of s. mutans and s. sobrinus samples of saliva we did with genomic charge switch Forensic DNa Purification kit (Invitrogen). (Invitrogen corporation 1600 Faraday). RESULTS: In the experimental group of 43 (58, 10%) children, s. mutans was isolated in 36 (48, 60%) of the saliva and 7 (9, 50%) was not isolated s. mutans. Introduction: A malocclusion is a misalignment or incorrect relation between the teeth of the two dental arches. It is considered one of the most common dental problems together with dental caries, gingival disease and dental fluorosis. Classification according to Angle is based on irregularities of teeth and jaws in anteroposterior or sagittal direction. There are three classes of malocclusion: Class I (neutroocclusion), Class II (distoocclusion) and Class III (mesioocclusion).The purpose of this study was to analyze and to assess the status of occlusion, the molar and the canine relationship of the permanent dentition. Aim: To assess the occurance of dental malocclusion in dental medicine students. Material and methods: The study included 22 students (11 female and 11 male) aged 22 to 31 of Dental Medicine from Goce Delchev University. An intraoral examination was performed. The students who were examined were informed that they would be included in our research. Results: From the total number of students 63.6% have Angle's Class I maloclussion, 22.7% have the presence of Angle's Class II molar relationship (Class II Division 1 or Class II Division 2) and 13.6 % have the presence of Angle's Class I molar relationship and Angle's Class II canine relationship (unilateral or bilateral). Frequency of crowding was observed in 77.3%, diastema was present in 4.5%, and posterior crossbite was 9.1%. Anterior crossbite was not detected. Conclusion: The most common malocclusion among students is Angle's Class I with normal canine and molar relationship. This study gives us valuable information on identifying occlusal problems, their incidence and the need for orthodontic treatment.

The patients think that visiting the dentist is stressful and terrifying. The reasons why they are avoiding the check-ups are often the reasons why the oral conditions are getting worse. Because of that the patients needs the regular check-ups in the dental office. The regular dental check-ups lead to maintaining the oral cavity in healthy state. Materials and methods: Total number of 50 patients, male and female, aged between 25-50 years, consulting for regular dental control check-up were evaluated. All of the subjects were with some of the most common reasons for regular dental check-ups and we divide them in some groups because of their similarity. Results: High prevalence of toothache in more than 90 % of the subjects was recorded among the patients. The results showed that there is a high significant correlation between the subjective situation (what the patient feels) and objective evaluation (what the dentist detects in patients` oral cavity). The most common reasons for regular dental checks ups are: toothache, bleeding gum, professional cleaning of teeth, something that the patient thinks is happening in the mouth but there isn’t any problem, orthodontic control etc. Conclusion: We can conclude that the most common reasons for dental check-ups are usually the first sights of some dental problems.

When placing dermal fillers by the cosmetic technician, frequent adverse effects such as tissue necrosis, artificial appearance or a possibility of many infections may occur. Doctors, dentists and oral surgeons learn how to safely administer dermal injections as part of their medical training. Medical professionals are taught how to use needles and cannulas safely, and they are also taught how to deal with problems that may arise from allergic reactions, infection, or an error by the injector. Diabetes mellitus is a systematic disease and has a major negative impact on different organs and systems, including oral cavity and teeth. Diabetes mellitus is a common and a serious public health problem. There are 3 types of diabetes: type 1 also known as juvenile or insulin dependent diabetes, type 2 or non-insulin-dependent diabetes mellitus and type 3 or gestational diabetes. Dentists often are faced with diabetic patients who are demanding orthodontic treatment. The main aim of this study was to evaluate the oral manifestation in orthodontic patients. Oral and dental manifestation of diabetes mellitus are: mouth dryness (xerostomia), burning mouth syndrome, dental caries, periodontitis and gingivitis, ketone breath in poorly controlled patients, teeth disposition and misalignment, brittle teeth, recurrent oral infections (e.g. oral candida, mouth ulcerations), altered taste sensation and also there is delayed healing of mucous membranes. Treatment considerations in patients with diabetes mellitus are: consideration before deciding orthodontic treatment, during the process of orthodontic intervention and diabetic emergencies during orthodontic treatment. Diabetic patients often seek for orthodontic treatment and dentists should be aware about the importance of this condition to prevent all possible complications. It is necessary for dentists to have a basic and solid knowledge about diabetes diagnosis, management, dental consequences, and the considerations to be taken during orthodontic treatment. Diabetes is not a contraindication to orthodontic treatment.

In a pursuit for perfect esthetic and natural appearance of restorations in contemporary dentistry, porcelain fused to metal restorations are gradually being replaced by all-ceramics. All-ceramic restorative systems are not a new concept. They were first introduced with the so-called “porcelain jacket crown” in the 1900s by Charles H.Land, fabricated with high-fusing feldspathic porcelain on platinum foil matrix. Because of the low strength of this porcelain, the failure rate was very high and they were slowly abandoned. In the last 10 years with the introduction of lithium-disilicate class ceramics IPSe.max System (Ivoclar Vivadent) with a strength of 470 MPa monolithic restorations made a big comeback. Monolithic or full-contour lithium-disilicate restorations can be stained and glazed for more natural tooth colour look. The ideal restoration shade is optimally reproduced by means of the IPS e.max Shade Navigation App. It facilitates the material selection, leads to results that feature optimum shade match and thus provides efficiency and reliability. This material has two forms: a homogeneous ingot with various degrees of translucency and opacity used with hot-pressed technology Emax Press and a pre-crystallized block used with CAD/CAM technology Emax CAD. Regarding the strength of the material, pressed lithium disilicate is 11% stronger than the CAD/CAM lithium disilicate according to Ivoclar’s 2011 scientific report. Pressable lithium disilicate restorations demonstrated a 98.4% survival rate. Machineable lithium disilicate restorations (IPS e.max CAD) demonstrated a 97.9% survival rate. The pressing technique demonstrated a significantly smaller marginal gap than the CAD/CAM technique. Some in vitro studies have pointed out that lithium disilicate monolithic crowns and FDPs, both CAD/CAM and hot-pressed, are more resistant to fatigue fracture compared to bilayered, hand veneered ones, showing higher fracture loads (1900 N), that are comparable to the metal-ceramic standard. Clinical outcomes of different IPS e.max lithium disilicate prostheses like single crowns, 3 unit bridges, veneers, inlays and onlays, both in the bilayered and monolithic forms, in a 4-year retrospective study reported: 0.91% for monolithic and 1.83% for bilayered single crowns (twice the rate of the monolithic); 4.55% for monolithic FDPs; 1.3% for monolithic and 1.53% for bilayered veneers; and 1.01% for monolithic inlays/onlays. This research supports the use of monolithic lithium disilicate restorations in everyday treatment of choice for high-strength and esthetic indirect treatments, whether for the anterior or posterior regions of the mouth. The material offers specific advantages to dentists such as the ability to provide strong and durable restorations with highly esthetic characteristics without metal. More clinical studies for the esthetic outcome of monolithic restorations are needed in the future. Maintaining good oral hygiene, reducing the presence of dental plaque in the mouth is an important part of the everyday lives of patients. The importance is even greater in patients undergoing orthodontic treatment. The aim of this study was to assess the oral hygiene behavior among the patients during orthodontic treatment with fixed appliances. One hundred and two orthodontic patients (65 female) and (37 male) treated with fixed appliances were studied. Patients were requested to fill out a structured questionnaire about oral hygiene behavior after placement of fixed orthodontic appliances. Results showed that 65.68 % from the patients brush their teeth twice a day. Most of them, 68.63% use orthodontic tooth brush and 78.43% use fluorid toothpaste. 33.33% from the patients use interdental brushes and 81.37% of them are using mouthwash solutions. Percentage of regular and occasional uses of dental floss were 7.84% and 21.57%. Oral hygiene instructions are necessary throughout the entire orthodontic treatment and they need to be strengthened. Patients undergoing fixed orthodontic treatment are required to be educated and motivated to maintain their oral hygiene on a high level. Ортодонтскиот третман има многу значајна улога во подобрување на лицевата естетка, денталната хармонија а со тоа влијае и на самопочитта кај пациентите. Негова главна цел е да ја подобри забната оклузијата, при што ќе резултира со добро функционирање на дентицијата односно ќе се овозможи подобра мастикација. Одржувањето на орални здравствени навики се од клучно значење за пациентите кои се под ортодонтски третман бидејќи неуспехот да се одржи добра орална хигиена може да резултира со акумулација на дентален плак преку фиксните протези и други ортодонски апарати, што доведува до несакани ефекти и предизвикува влошување на оралното здравје. За да се избегнат несаканите компликации кои можат да настанат во текот на ортодонтскиот третман од особена важност е одржување на правилна орална хигиена. Всушност, несоодветната оралната хигиена пред започнување на третманот и лошата орална хигиена за време на ортодонтскиот третман е поврзана со поголема инциденца на несакани ефекти. Орална хигиена има важна улога во оралната здравствена заштита. Веднаш по започнувањето на ортодонтскиот третман од суштинско значење е пациентот да се едуцира за правилно одржување на оралната хигена, користење на соодветни четки за заби, правилни техники на четкање на забите, употреба на интерапроксимални четки, користење на пасти за заби со флуориди, антисептични водички за испирање врз база на флуориди како и професионална нега од страна на стоматолог. Одржувањето на оралното здравје во текот на ортодонтскиот третман е комбинација од добра орална хигиена и балансирана, правилна исхрана. Се препорачува избегнување или минимизирање на употребата на шеќери, газирани пијалоци за време на оброците како и закуски меѓу оброци. Сепак мотивацијата на пациентот, неговата умешност се од витално значење за оралното здравје односно за успехот на оралната хигиена во текот на ортодонтскиот третман. Иако пациентите редовно го посетуваат ортодонтот во текот на третманот потребна е и редовна посета кај стоматологот која и во овој период е исто значајна. Ортодонтот е фокусиран на подобрување на денталната хармонија додека стоматологот се грижи забите да останат здрави со темелни прегледи и превентивна нега. Оптимално одржување на оралното здравје за време на ортодонтскиот третман треба да биде златен стандард во денешната практика. Покрај интервенциите на стоматологот потреба е и соодветна едукација и мотивација на пациентите во одржувањето на оралната хигиена во домашни услови. Ортодонтскиот третман е тимски напор во кој ортодонтот, пациентот и стоматологот имаат важна улога во добивањето на совршена насмевка.

The main goal is to improve the dental occlusion, which will result in good functioning of the dentition, that is, better mastication will be enabled. Maintaining oral health habits is crucial for patients undergoing orthodontic treatment because the failure to maintain good oral hygiene can result in the accumulation of dental plaque over the braces and other orthodontic appliances, which leads to side effects and causes deterioration of the oral health. To avoid unwanted complications that may occur during orthodontic treatment it is important to maintain proper oral hygiene. In fact, inadequate oral hygiene before the orthodontic treatment and poor oral hygiene during orthodontic treatment is associated with a greater incidence of adverse effects. Oral hygiene plays an important role in oral health care. Immediately after initiating orthodontic treatment, it is essential that the patient is educated for the proper maintenance of oral hygiene, using appropriate toothbrushes, proper teeth brushing techniques, use of interproximal brushes, use of fluoride toothpastes and adjunctive fluoride mouthwashes, chlorhexidine mouthwashes as well as professional care by a dentist. Maintaining oral health during orthodontic treatment is a combination of good oral hygiene and a balanced, proper diet. It is recommended to avoid or minimize the use of sugars, carbonated drinks during meals, energy drinks, and between meal snacks. However, patient motivation and dexterity are vital to oral health, or to the success of oral hygiene during orthodontic treatment. Although patients regularly visit the orthodontist during the treatment, a regular visit to the dentist is required, which is also significant in this period. The orthodontist is focused on improving dental harmony while the dentist takes care to keep your teeth healthy with thorough dental checkups and preventive care. Optimal maintenance of oral health during orthodontic treatment should be a gold standard in today's practice. In addition to the interventions of the dentist, there is also an appropriate education and motivation of the patients in the maintenance of oral hygiene at home. Ортодонтскиот третман на малоклузиите и краниофацијалните абнормалности придонесува за правилна поставеност на забите и на тој начин овозможува подобра оклузијата како и подобар однос на вилиците. Со ортодонтскиот третман исто така е овозможено и подобро одржување на оралната хигиена кај пациентите. Покрај ова исто така е овозможена подобра мастикација, фонација и естетика на лицето на пациентот, при што ортодонтскиот третман придонесува за општото и оралното здравје на пациентот, влијае на самодоверба односно подобрување на квалитетот на неговиот живот. Како и секоја друга медицинска интервенција, покрај придобивките од ортодонтскиот третман постојат и одредени ризици, негативни ефекти. Меѓутоа овие негативни ефекти кои се поврзани со самиот ортодонтски третман се значително помали во споредба со останатите хирушки и нехирушки интервенции. Сепак, најчестите негативни ефекти од ортодонтскиот третман можат да бидат локални односно интраорални и екстраоралнии како и системски. Еден од предизвиците на најголем дел од ортодонтите е да го завршат ортодонтскиот третман со најмалку ефекти врз корените и пародонтот на забите. Ортодонтскиот третман најчесто е контраиндициран кај пациенти со активна пародонтална болест или лошо пародонтолошко здравје, бидејќи во таков случај шансата за понатамошно влошување на состојбата е повисока. To get a quality impression is of great importance in prosthodontic therapy. The impression is a negative of the condition in the patient’s mount and it should be dimensionally stable in order to gain a good working model – positive, on which the prosthodontic construction will be made. Due to the fact that the indirect restorations are produced in a dental laboratory, it is of big significance to precisely fit the prosthodontic appliance without the need for their correction. This mostly depends from the quality of the impression and consequently the model cast with the condition in the patient’s mouth. Thus, allows faster and more efficient dental practice and at the same time to produce restoration which will help in prophylaxis of the periodontal tissue. Over the past 100 years, elastomeric impression materials have been continuously developed and optimized for improved precision, patient comfort and ease of use. However, there are also some factors that adversely affect the impression taking itself. All this can affect the size of the crown and it can be too big or too small. If we get a wider crown, intolerant marginal gaps appear, and in another situation, if the crowns are narrower, they do not reach the preparation line. Biological tolerance for marginal differences is almost unknown; however, there is positive correlation between marginal accuracy and damage to periodontal tissue and as well as the appearance of secondary caries in the case of imprecise fit. This problem mainly relates to restorations with subgingival and paragingival finish lines. Therefore, taking a good impression in the preparation margin is a necessary precondition to achieve a good marginal adaptation. Standardization and compliance with workflow protocols can reduce the number of repetitions and the need for corrections. In addition, the final success is strongly influenced by the clinical condition and is individual for each patient. Clinical parameters affecting the impressions are: periodontal status and oral hygiene of the patient, time period between preparation and taking impression, tooth preparation and anesthesia. The dentist has to be careful and to know how to identify them, and at the same time correct the potential complications that will affect the final prosthesis. The most common mistakes that occur during the process of taking impression are: inadequate marginal details, marginal tears, creating bubbles in the impression, selecting an inappropriate tray, separation from the tray, distorting the tray and surface contamination. Complications that arise in the process of taking impression are often confusing for the dentist and the dental technician. Of utmost importance for the ultimate success of prosthetic therapy is to know the possible causes of the failure and of course preventing them. By taking the necessary precautionary measures, damage to the impression will be avoided and we will have overall improvement in the fit of the final prosthetic construction. Од огромно значење при протетичка терапија е земање на квалитетен отпечаток. Отпечатокот претставува негатив од состојбата во устата на пациентот, кој треба да биде димензионално стабилен бидејќи со негово излевање ќе ни послужи како добра основа за добивање на работен модел – позитив, на кој ќе се изработи протетичката конструкција. Поради фактот дека индиректните реставрации се произведуваат во заботехничка лабораторија, од огромно значење е прецизно пасување на конечната изработка, без потреба за нивна корекција, што е условено од совпаѓањето на моделот со ситуацијата во устата на пациентот. На тој начин од една страна е овозможено побрзо и поефикасно работење, а притоа ваквите изработки помагаат и во профилакса на периодонтот. Во текот на изминатите 100 години, еластомерните отпечаточни материјали континуирано се развиваа и се оптимизираа за подобрена прецизност, удобност на пациентот и леснотија на користење. Сепак, постојат и одредени фактори кои негативно влијаат на самото отпечатување. Сето тоа може да влијае на големината на коронката и истата може да биде преголема или премала. Доколку добиеме поширока коронка, се појавуваат нетолерантни маргинални празнини и во друга ситуација доколку коронките се потесни, истите не ја достигнуваат границата на препарација. Биолошката толеранција за маргиналните разлики е речиси непозната, сепак докажана е и поврзаноста меѓу маргиналната точност и оштетувањето на пародонтот и појавата на секундарни кариеси во случај на непасување на протетичката изработка. Ова проблематика главно се однесува на реставрации кај субгингивална и парагингивална препарација. Затоа, добро отпечатување во пределот на демаркационата линија е неопходен предуслов за да се постигне добра маргинална адаптација. Стандардизација и запазување на протоколите на работа може да го намали бројот на повторувања и потребата од корекции. Покрај тоа, конечниот успех е под силно влијание на клиничката состојба и е индивидуален за секој пациент. Клинички параметри кои влијаат на отпечатувањето се: пародонтален статус и оралната хигиена на пациентот, временски период помеѓу препарацијата и отпечатувањето, препарација на забите и анестезија. Стоматологот мора да внимава и да знае да ги идентификува, а воедно и да ги корегираат потенцијалните компликации при отпечатување што ќе влијаат на конечната протетичка изработка од впечатоците. Најчести грешки кои се појавуваат при отпечатувањето се: дефекти и раскинување во маргиналната регија, создавање на меури во отпечатокот, избор на несоодветна лажица за отпечатување, одвојување на отпечатокот од лажицата, изобличување на лажицата и површинска контаминација. Компликациите кои настануваат во процесот на отпечатување често пати се конфузни за стоматологот и за забниот техничар. Од големо значење за конечниот успех на протетичката терапија е да се знаат можните причини за настанатиот неуспех и секако нивно превенирање. Преку преземање на неопходните мерки на претпазливост ќе се избегне оштетување на отпечатокот и целокупно подобрување во пасувањето на конечната протетичка изработка. Мускулоскелетни нарушувања (МСН) се сериозни здравствени проблеми кои влијаат на мускулите, зглобовите, лигаментите, тетивата, 'рскавицата, крвните садови и нервите. Голем ризик за развој на мускулоскелетни проблеми имаат стоматолозите, поради нивната позиција кога работат во ординациите. Резултатите на МСН се: ограничени движења, непријатност и болка во тригер точки. Затоа наша цел ни беше да ги истражиме факторите на ризик поврзани со овие нарушувања кои предизвикуваат делумен хендикеп при работењето. Ние ја проценивме распространетоста на симптомите на МСН кај стоматолозите и преваленцата на симптоми пријавени од страна на испитаниците беше многу висока: 92,4% од испитаниците изјавиле дека имаат најмалку еден симптом на МСН. Како објаснување за оваа состојба е дека искусни стоматолози се подобри во нивните работни техники и позиции, кои ги применуваат со цел да се избегнат мускулоскелетни проблеми, во споредба со нивните помалку искусни колеги т.е. денталните студенти. МСН обично се развиваат како резултат на кумулативната изложеност на различни фактори на ризик. Најчесто споменатите фактори на ризик се: ограничени несоодветни работни места, поле на работа, повторувачки движења, неергономски инструменти и недостаток на обука. MСН е значаен професионален здравствен проблем во стоматологијата. Со цел да се зголеми квалитетот на животот без МСН и да се обезбеди висококвалитетна медицинска услуга за стоматолози и дентални работници потребно е менаџирање на факторите на ризик. Стоматолозите треба да се едуцираат за ергономските позиции за секојдневна работа, исто така треба да се спроведат ергономски обуки пред и по дипломирање за поефикасно користење на телото и стоматолошката опрема, а соодветно на тоа да го имплементираат знаењето. Исто така, тие мора да одржуваат соодветен ергономски став, да се ослободат од психолошкиот стрес со редовни секојдневни вежби, да се одмораат и да ја подобрат работната средина.

Резиме: Главна цел на овој труд е да се прикаже можнста за третман и надополнување на виличните коскени дефекти со помош на биоматеријали како што се вештачките коски и мембрани, како и успешноста од нивната употреба. Според нашето мислење како и на повеќе автори ширум светот е дека употребата на вештачката коска е најдобро да биде комбинирана со автографт , но во сите случаи кога тоа не е изводливо, сепак вештачката коска треба да биде заштитена со колагена мембрана фиксирана над коскениот графт. Вештачката коска наоѓа широка примена за регенерација на виличните коскени дефекти , пред се поради можноста истите да се надополнат или да стимулираат природна коскена регенерација. Механизмот на интегрирање на ваквите биоматеријал е врз основа на самата остеоинтеграција, остеоиндукција и остеокондукција. Сосема познато е дека постојат различни видови на вештачки коски кои пред се можат да бидат од природно потекло или да станува збор за синтетски материјали. Додека мембраните генерално се поделени на нересорптивни и ресорптивни. Со тоа всушност се прави еден поинаков, современ пристап кон третманот на коскени дефекти од различно потекло коешто се прави со употреба на автогени, алогени или ксеногени графтови, а сега и со употреба на алопластични или синтетски материјали. Како автоген графт најчесто употребувана е спонгиозната коска од карлицата (crista iliaca), од ребро или од потколеницата (tibia). Гранулите на коскениот супституент се атхерираат меѓусебно, со што формираат пластична но порозна маса. Материјалот се стврднува во стабилен скелет во контакт со крвта. Многу истражувања за примената на калциум сулфатот за репарација на коскениот дефект довеле до заклучок дека калциум сулфатот е извонреден коскен супституент кој осигурува коскена формација и дава резултати еднакви или подобри од употребата на било кои други коскени графтови. Од друга страна пак употребата на автогена коска во комбинација со ксенографт, била користена за да се забрза коскената регенерација и да се добијат добри мекоткивни контури. Therefore, before starting the orthodontic treatment, a thorough assessment of the periodontal health and the level of the attached gingiva is recommended, it is particularly important to emphasize the need to maintain good oral hygiene in order to achieve the best treatment. Caring for dentures and oral tissues is important for both and oral and general health in the patients. Dentures in the patient's mouth causes hyperemia, mechanical irritation, retention of food and moisture that constitute the ideal substrate for the growth and development of microorganisms. If the patients do not maintain the hygiene of the dentures as well as the hygiene of the complete orofacial system, may be come to the oropharyngeal diseases as well as systemic diseases of the airways and the blood vessels. Biofilm could damage the adjacent mucosa, the first step in the pathogenesis of denture - stomatitis is the adherence of Candida albicans to the biofilm surfaces of dentures. denture hygiene habits, denture age and frequency of denture use and cleaning were factors that related to the occurrence of denture stomatitis. Non-maintain proper oral hygiene causes a halitosis in the patient which is an unpleasant situation. maintaining of proper hygiene on the dentures helps to prevent the coloring of the dentures base and artificial teeth. According to some studies, there's a lack of preventive hygiene programs for maintain proper oral hygiene and hygiene of dentures. About dental hygiene patients had limited knowledge and that the patients were totally unconscious of the measures of cleaning the dentures. For the hygiene of the dentures, also greatly influenced the material and the processing of the dentures. Only water to clean their dentures along with the toothbrush used a large number of the patients. The hygiene of the dentures was carried out with chemical and mechanical means. According the studies minimal patients used the chemical substance to cleaner their dentures. The purpose of this study was to present guidelines for maintenance of oral hygiene in patients with dentures to improve oral hygiene, because oral hygiene is the most important measure for the promotion and prevention of oral health. Мускулоскелетни нарушувања (МСН) се сериозни здравствени проблеми кои влијаат на мускулите, зглобовите, лигаментите, тетивата, 'рскавицата, крвните садови и нервите. Голем ризик за развој на мускулоскелетни проблеми имаат стоматолозите, поради нивната позиција кога работат во ординациите. Резултатите на МСН се: ограничени движења, непријатност и болка во тригер точки. Затоа наша цел ни беше да ги истражиме факторите на ризик поврзани со овие нарушувања кои предизвикуваат делумен хендикеп при работењето. Ние ја проценивме распространетоста на симптомите на МСН кај стоматолозите и преваленцата на симптоми пријавени од страна на испитаниците беше многу висока: 92,4% од испитаниците изјавиле дека имаат најмалку еден симптом на МСН. МСН обично се развиваат како резултат на кумулативната изложеност на различни фактори на ризик. Најчесто споменатите фактори на ризик се: ограничени несоодветни работни места, поле на работа, повторувачки движења, неергономски инструменти и недостаток на обука. MСН е значаен професионален здравствен проблем во стоматологијата. Со цел да се зголеми квалитетот на животот без МСН и да се обезбеди висококвалитетна медицинска услуга за стоматолози и дентални работници потребно е менаџирање на факторите на ризик. Стоматолозите треба да се едуцираат за ергономските позиции за секојдневна работа, исто така треба да се спроведат ергономски обуки пред и по дипломирање за поефикасно користење на телото и стоматолошката опрема, а соодветно на тоа да го имплементираат знаењето. Исто така, тие мора да одржуваат соодветен ергономски став, да се ослободат од психолошкиот стрес со редовни секојдневни вежби, да се одмораат и да ја подобрат работната средина.

Background: The best make up on our face is smiling. All ceramics crown or bridges is the solutions for a beautiful smile that we all wish. All ceramics represent the highest achievements of modern dentistry. Aesthetic dentistry would be unthinkable without all ceramics. The purpose of this study was to show our management of smile design with all ceramics bridge. Case report: A 37 years old male patient visit our dental office searching for solution for his unsatisfactory smile appearance. Thought clinical examination, missing tooth, tooth discoloration after root canal treatment, low smile line and caries were found. All the solutions were offered to the patient, and a bridge was chosen all ceramics dental bridge as the best solution. A preparation on the three frontal teeth was made. Impressions were taken and sent to the laboratory where dental bridge with three anchor crown and one lateral pontic were made. After try-in the bridge were cemented by light curing cement. After follow-up there were no signs of changes in the bridge. Conclusion: А patient was satisfied with the restoration done. Aim: The main aim is to present the composite laminates, such as modern means used in aesthetic dentistry for the correction of minimal irregularities. Also with this case we want to present the advantages of dental laminates. Clinical case: It is the patient who was not happy with his smile. We made composite laminates for 12 and 22 in maxilla and to improve his smile, and we got a very satisfied patient and perfect smile. Conclusion: The advantages of the laminates are those that they are the latest choice for the patients that are looking for a perfect smile from a reason that with a proper care they can last a long time, and the need care as natural teeth, making is fast, and the results-perfect smile.
