3. Separation of ultrafine particles from class F fly ashes

Abstract

In this study, ultrafine particles were recovered from Çatalağzı (CFA) and Sugözü (SFA) thermal power plant fly ashes using a specific hydraulic classification technology. Since fly ashes have a high tendency to be flocculated in water, settling experiments were first designed to determine the more effective dispersant and the optimum dosage. Two different types of the superplasticizers (SP) polymers based on sulphonate (NSF, Disal) and carboxylate (Glenium 7500) were used as the dispersing agents in these settling experiments. Hydraulic classification experiments were then conducted to separate ultrafine fractions from the fly ash samples on the basis of the settling experiments. According to the settling experiments, better results were achieved with the use of Disal for both CFA and SFA. The classification experiments showed that the overflow products with average particle sizes of 5.2 μm for CFA and 4.4 μm for SFA were separated from the respective as-received samples with acceptable yields and high enough recoveries of -5 μm (ultrafine) particles. Overall results pointed out that the hydraulic classification technology used provided promising results in the ultrafine particle separations from the fly ash samples.

Abstract

Pulmonary hypertension (PH) is associated with decreased survival in patients with pulmonary fibrosis and combined pulmonary fibrosis and emphysema. Main pulmonary artery (PA) diameter and PA diameter/ascending aortic diameter (PA/AA) ratio, as measured on CT, have recently emerged as specific markers for PH. Our single-center retrospective study found that PA/AA ratio > 1 is associated with decreased survival in individuals with pulmonary fibrosis, with or without emphysema. Abstract

Rate of growth of the amount of data generated using the high throughput sequencing (HTS) platforms now exceeds the growth stipulated by Moore's Law. The HTS data is expected to surpass those of other "big data" domains such as astronomy, before the year 2025. In addition to sequencing genomes for research purposes, genome and exome sequencing in clinical settings will be a routine part of health care. The analysis of such large amounts of data, however, is not without computational challenges. This burden is even more increased due to the periodic updates to reference genomes, which typically require re-analysis of existing data. Here we propose Coin-Application Mediator Interface (Coinami) to distribute the workload for mapping reads to reference genomes using a volunteer grid computer approach similar to Berkeley Open Infrastructure for Network Computing (BOINC). However, since HTS read mapping requires substantial computational resources and fast analysis turnout is desired, Coinami uses the HTS read mapping as proof-of-work to generate valid blocks to main its own cryptocurrency system, which may help motivate volunteers to dedicate more resources. The Coinami protocol includes mechanisms to ensure that jobs performed by volunteers are correct, and provides genomic data privacy. The prototype implementation of Coinami is available at http://coinami.github.io/.

14. Treatment outcomes and adherence to treatment in patients with immune thrombocytopenia in two Ethiopian teaching hospitals: a retrospective cohort study

Abstract

Abstract The treatment of immune thrombocytopenia (ITP) is challenging and treatment outcomes depend on numerous unknown and patient-specific factors. Corticosteroids are the cornerstone of ITP treatment, but they are associated with many side effects. In this retrospective cohort study, treatment outcomes and treatment adherence in patients with ITP were investigated in 214 ITP patients from November 15, 2022 to March 15, 2023. Multinomial regression analysis models were used to identify predictive factors for treatment outcomes. A p value of less than 0.05 was considered statistically significant. Most study participants were female 161 (75.5%), and the majority 172 (80.4%) of them were taking prednisolone only. In terms of treatment adherence, 178 (83.2%) of the study participants adhered well to their ITP medications. The complete response rate at 3 months was 139 (65.0%). Predictive factors for partial response were increased negative impact of ITP on health-related quality of life (AOR = 1.221, 95% CI 1.096–1.360), being treated at Tikur Abessa Sepcialazed Hospital (AOR = 0.431, 95% CI 0.197–0.941) and the presence of heavy menstrual bleeding (AOR = 2.255, 95% CI 0.925–5.497) compared to patients with complete response. Hepatitis B virus-infected ITP patients (AOR = 0.052, 95% CI 0.004–0.621) were also a predictive factor for no response compared to complete response.

Abstract

BackgroundInfection prevention and control (IPC) is a set of practices that are designed to minimize the risk of healthcare-associated infections (HAIs) spreading among patients, healthcare workers, and visitors. Implementation of IPC is essential for reducing infection incidences, preventing antibiotic use, and minimizing antimicrobial resistance (AMR). The aim of the study was to assess IPC practices and associated factors in Pediatrics and Child Health at Tikur Anbessa Specialized Hospital.MethodsIn this study, we used a cross-sectional study design with a simple random sampling method. We determined the sample size using a single population proportion formula with the assumption of a 55% good IPC practice, a 5% accepted margin of error, and a 15% non-response rate and adjusted with the correction formula. The final sample size was 284 healthcare workers. The binary logistic regression model was used for analysis. The World Health Organization (WHO) Infection Prevention and Control Assessment Framework (IPCAF) tool was used to assess IPC core components.ResultA total of 272 healthcare workers participated in the study, with a response rate of 96%. Of the total participants, 65.8% were female and 75.7% were nurses. The overall composite score showed that the prevalence of good IPC practices among healthcare workers was 50.4% (95% CI: 44.3–56.5). The final model revealed that nursing professionals and healthcare workers who received IPC training had AORs of 2.84 (95% CI: 1.34–6.05) and 2.48 (95% CI: 1.36–4.52), respectively. The final average total IPCAF score for the IPC level was 247.5 out of 800 points.ConclusionThe prevalence of good IPC practice was suboptimal. The study participants, who were nursing professionals and healthcare workers who received IPC training, showed a statistically significant association with the IPC practice level. The facility-level IPCAF result showed a “Basic” level of practice based on the WHO categorization. These evidences can inform healthcare workers and decision-makers to identify areas for improvement in IPC practice at all levels. Training of healthcare workers and effective implementation of the eight IPC core components should be strengthened to improve suboptimal practices.

Item does not contain fulltext Rigorous organization and quality control (QC) are necessary to facilitate successful genome-wide association meta-analyses (GWAMAs) of statistics aggregated across multiple genome-wide association studies. This protocol provides guidelines for (i) organizational aspects of GWAMAs, and for (ii) QC at the study file level, the meta-level across studies and the meta-analysis output level. Real-world examples highlight issues experienced and solutions developed by the GIANT Consortium that has conducted meta-analyses including data from 125 studies comprising more than 330,000 individuals. We provide a general protocol for conducting GWAMAs and carrying out QC to minimize errors and to guarantee maximum use of the data. We also include details for the use of a powerful and flexible software package called EasyQC. Precise timings will be greatly influenced by consortium size. For consortia of comparable size to the GIANT Consortium, this protocol takes a minimum of about 10 months to complete.

Abstract

Abstract Introduction Pain is unpleasant sensory and emotional experiences associated with actual and/or potential tissue damage. It is the most common and prevalent reason for emergency departments (ED) visits with prevalence over 70% in the world. Aim of the Study The study aimed to assess the adequacy and appropriateness of pain management at Aabet Hospital, Addis Ababa, Ethiopia. Methods A hospital-based prospective cross-sectional study was conducted at Aabet hospital from December 1, 2020 to March 30, 2021. Adult trauma patients having pain (at least score 1 on Numeric Rating Scale) with Glasgow Coma Scale score > 13 were eligible to participate in the study. The pain intensity was evaluated at the time of admission (o minute) and then at 60, 120, 180, and 240 minutes. The time of the first analgesics was registered. The adequacy and the appropriateness of the pain management were calculated through pain management index (PMI). Results Two hundred thirty-two (232) participants were included in this study of which 126 (54.3%) were admitted due to road traffic accident followed by fall 44(19%). Only 21 (9.1%) study participants received the first analgesic treatment within 30 minutes while 27(11.6%) participants had no treatment at all within 240 minutes. The mean pain intensity score at admission was 5.55 ± 2.32 and reduced to 4.09 ± 2.69. Nearly half 110 (47.4%) of the study participants were treated inadequately (PMI (-) score). There was a weak and negative correlation between PMI and time to analgesia (r = − .159, p = 0.0001). The type of analgesia used, the time to analgesia, and the degree of pain may predict 65% of the variance in PMI score (R2 = 0.65, P = .001). Conclusion From the results of this study, it can be concluded that acute pain in trauma patients was under and inappropriately treated.

Abstract

In Ethiopia, cancer accounts for about 5.8% of total national mortality, with an estimated annual incidence of cancer of approximately 60,960 cases and an annual mortality of over 44,000 persons. This is likely an underestimation. Survival rates for pediatric malignancies are likewise suboptimal although exact figures are unknown since a national cancer registry is unavailable. The World Health Organization (WHO) provides recommendations for the creation of cancer registries to track such data. Here we describe our pharmacist-led, pre-implementation assessment of introducing an enhanced national pediatric cancer registry in Ethiopia. Our assessment project had three specific aims around which the methods were designed: 1) characterization of the current spreadsheet-based tool across participating sites, including which variables were being collected, how these variables compared to standards set by the WHO, and a description of how the data were entered and its completeness; 2) assessment of the perceptions of an enhanced registry from hospital staff; and 3) evaluation of workflow gaps regarding documentation. The hospital staff and leadership have generally positive perceptions of an enhanced pediatric cancer registry, which were further improved by our interactions. The workflow assessment revealed several gaps, which were addressed systematically using a three-phase implementation science approach. The assessment also demonstrated that the existing spreadsheet-based tool was missing WHO-recommended variables and had inconsistent completion due to the workflow gaps. A pediatric oncology summary sheet will be implemented in upcoming trips in patient charts to better summarize the patients' journey starting from diagnosis. Abstract

Abstract Background Insulin therapy can be inconvenient, painful, burdensome, and restrict patients' daily activities and health related quality of life (HRQOL) due to improper injection techniques or the nature of administration. Objective This study aimed to assess insulin injection practice, HRQOL and predictors among patients treated with insulin at Tikur Anbessa Specialized Hospital (TASH). Methods An institutional-based cross-sectional study was conducted among diabetes patients on insulin therapy from May to June 2022. A structured questionnaire was used to collect patient characteristics and insulin injection practice. The validated Amharic version of an EQ-5D-5L tool was used to assess the HRQOL. The data was analyzed using SPSS version 26. The patient data were summarized using descriptive statistics. One-way ANOVA using Kruskal–Wallis H tests was used to assess factors that predict insulin handling practice scores. Multivariate linear regression analysis was used to assess factors affecting HRQOL among diabetes patients treated with insulin. The EQ5D-5L utility scores of the patients were calculated using disutility coefficients taken from the Ethiopian general population. Statistical significance was declared at p-value 50 years of age. Among the study participants, 62.1% were only on intermediate acting insulin. A significantly higher proportion of participants 291(91.2%) in this study were taking insulin two times per day. Most of the participants 234(73.4%) had fair practice with a median insulin handling practice score of 38 out of 56. Abstract

The maintenance of normal body weight is disrupted in patients with anorexia nervosa (AN) for prolonged periods of time. Prior to the onset of AN, premorbid body mass index (BMI) spans the entire range from underweight to obese. After recovery, patients have reduced rates of overweight and obesity. As such, loci involved in body weight regulation may also be relevant for AN and vice versa. Our primary analysis comprised a cross-trait analysis of the 1000 single-nucleotide polymorphisms (SNPs) with the lowest P-values in a genome-wide association meta-analysis (GWAMA) of AN (GCAN) for evidence of association in the largest published GWAMA for BMI (GIANT). Abstract

Background. Ornamental freshwater fish releases constitute a remarkable proportion of the 100 worst invasive species worldwide. Early detection and knowledge of likely introduction vectors and pathways of potentially invasive fishes into sensitive habitats are key for their proper management, hence rapid and correct identification of their occurrence is crucial. Therefore, we tested the hypothesis that a newly-discovered catfish population was that of Clarias batrachus (Linnaeus, 1758), and that this introduction might be of single origin released by aquarium pet fish owners. Materials and methods. In total, 45 specimens of C. batrachus were captured during two electrofishing surveys on 9 and 15 March 2016 by three operators for morphometric and molecular examination. Additionally, 28 specimens were collected for assessing gonadal maturity and sex. They were also measured for standard length and total length and weighed before being dissected. We also produced COI sequences for molecular identification of the species and for tracing its origin. Results. Morphological and molecular analyses indicated that the examined specimens belong to C. batrachus, and that they were likely introduced by aquarium hobbyists, and closest to Indonesian lineage. Successful reproduction and establishment of the species are demonstrated by the occurrence of ripe females and their young of the year and juvenile individuals in the catch. Conclusion. Our findings confirmed the presence of C. batrachus in a region with extraordinarily high biodiversity, including the first evidence to indicate the successful establishment of this species in Turkey. An initial first policy and management step would be to ban the importation and keeping of this species in Turkey, thus reducing the risk of further releases. Increased public awareness for the detrimental impacts of non-native fishes would serve to support the policy and field-based management practices to control, and hopefully eradicate this highly invasive species. Abstract

Background: Platelet count and bleeding risk are frequently used in clinical assessments of the severity of immune thrombocytopenia. On the other hand, immune thrombocytopenia also influences patients’ health-related quality of life. The immune thrombocytopenia life quality index is a new tool for evaluating health-related quality of life in immune thrombocytopenia patients. Objective: To assess the psychometric properties of the Amharic version of the immune thrombocytopenia life quality index tool for immune thrombocytopenia patients in Ethiopia. Methods: The facility-based cross-sectional study was carried out from 15 September to 30 October 2022. The school of pharmacy institutional ethics review board provided ethical clearance and approval of the study protocol (approval number: ERB/SOP/487/14/2022). Both written informed consent and family or legal guardian consent were obtained for participants aged 14–18 years and informed consent for participants aged >18 years. The tool was forward-backward translated before being pretested on five immune thrombocytopenia patients. Descriptive statistics were used to report sociodemographic and clinical data. The psychometric properties of the immune thrombocytopenia life quality index in Amharic were evaluated for acceptability, reliability, and construct validity. Results: This study included 100 participants, at the age of diagnosis of immune thrombocytopenia, the median with inter-quartile range of platelet count (×10 9 /L) was 15.00 (18.00). The majority of study participants (79, 79%) received prednisolone alone as first-line therapy. The tool psychometric test was reliable; the correlation coefficient between items ranged from 0.147 to 0.956, and the overall Cronbach’s alpha coefficient was 0.956. Furthermore, the tool was construct-valid, with factor analysis between components revealing that Item-1 had an eigenvalue greater than one and explained 72.532% of the total variance among all 10 immune thrombocytopenia life quality index domains. The immune thrombocytopenia life quality index total (alternative score) had a strong ( r = 0.9) correlation with Item-8 and a moderate ( r = 0.317) correlation with Item-5. Conclusion: The Amharic version of the immune thrombocytopenia life quality index tool is a valid and reliable tool that can be used to assess the health-related quality of life of immune thrombocytopenia patients in Ethiopia.

Abstract

This study attempted to examine the time management practices of students and the state of different factors of time management (multitasking, procrastination, task prioritizing, and technology use) at Debre Markos College of Teacher Education. Based on students’ problem of meeting deadlines to properly carry out and submit assignments, students’ time management practices, the status of the factors described, and differences among students based on sex and field of study regarding those factors were examined. The study employed descriptive survey design of the quantitative approach involving 113 students selected through a proportional simple random sampling technique. Questionnaire data were analyzed through mean, one sample t-test, independent samples t-test, and analysis of variance. Findings unveiled that students were not good enough in their time management practices. Paradoxically most students were multitasking, using technology, and procrastinating on the one hand and prioritizing their tasks, on the other. Although they do not have significant differences in their task prioritization, female students were more procrastinating than their male counterparts in their academic tasks. More male students than females were multitasking and using technology. Although patterns of moderate differences were observed in terms of fields of study, the statistical outputs imply that there were lots of variances among students both in their practice of procrastination and technology use.

Türkiye, sahip olduğu tatlısu kaynakları sayesinde, önemli bir içsu balıkçılığı potansiyeline sahiptir. Türkiye tatlısu balık faunası yaklaşık 340 tür içermesine rağmen; içsu balıkçılığı, toplam avın %78'ini oluşturan başlıca (yerli türlerden inci balığı- Alburnus tarı'chi ve sazan-Cyprinus carpio, yabancı türlerden gümüşi havuz balığı-Carassius gibeliove denizlerden tatlısulara taşınan gümüş balığı-Atherina boyeri) dört türe bağımlıdır. Gümüşi havuz balığı ve gümüş balığının ekonomik önemi olmasına rağmen, her iki tür de Türkiye içsularında istilacı olarak kabul edilmektedir. Sazamn iç pazarda gördüğü talep ve ticari değeri daha yüksek ve içsu balıkçılık ekonomisine katkısı (%37), gümüş balığı (%4) ve gümüş havuz balığı (%4)'n1n katkısından daha fazla olmakla birlikte; günümüzde, sazan ve inci balığının av oranı gittikçe düşerken, gümüş balığı ve gümüşi havuz balığının av oranı artmaktadır. Gümüşi havuz balığının ve gümüş balığı iç pazarda tüketihnemekte, diğer ülkelere ihraç edilmektedir. Bu iki türün Türkiye'de tatlısu ekosistemleri ve balıkçılık açısından olumsuz etkiler yarattığı düşünülmektedir. Turkey has potentially important inland fisheries, with extensive freshwater resources. Although the freshwater ichthyofauna in Turkey is very rich, with about 340 species, inland fisheries depend primarily on species, of which are indigenous (tarek-Alburnus tarichi, carp-Cyprinus carpio), is non-indigenous (gibel carp- Carassius gibelio) and is translocated marine fish (sand smelt-Atherina boyeri), accounting for 78% of the entire catch. Both gibel carp and sand smelt species have been considered invasive species in Turkey, although they are of increasing commercial importance. Currently, the yield of sand smelt and gibel carp are increasing, whereas the yield of carp and tarek are decreasing sharply; however, demand for carp and its economic value are higher in local markets and the contribution of carp to the inland fishery economy (3 7%) is higher than that ofboth sand smelt (4%) and gibel carp (4%). Neither gibel carp nor sand smelt are consumed locally, and both species are exported to other countries. Hence, some suggest these species have adverse effects on Turkey's ecosystem and fisheries. In order to ensure sustainable inland fishery and to obtain thorough understanding of the ecological interactions there is an urgent need to fill the data gap related to the ecological, fishery, and socioeconomic structures in an exploited system.

46. Cost-effectiveness of pegfilgrastim versus filgrastim for prevention of chemotherapy-induced febrile neutropenia in patients with lymphoma: a systematic review

Abstract

Abstract Background Febrile neutropenia (FN) is a prevalent and potentially life-threatening complication in patients with lymphoma receiving myelosuppressive chemotherapy. Pegfilgrastim is more effective than filgrastim as prophylaxis for FN. However, its usage has been limited because of its higher cost. Pegfilgrastim’s value for money remains unclear. Objective To systematically review the cost-effectiveness of pegfilgrastim compared to filgrastim as a primary or secondary prophylaxis for chemotherapy-induced FN among patients with lymphoma. Methods A systematic literature search was conducted in PubMed, EMBASE, Cochrane Library databases, and Google Scholar. The most widely used economic evaluations (cost-effectiveness analysis, cost-utility analysis and cost-benefit analysis) were included in the review. Data extraction was guided by the Consolidated Health Economic Evaluation Reporting Standards checklist, and the quality of reviewed articles was assessed using the Joanna Briggs Institute (JBI) checklist. Cost-effectiveness data were rigorously summarized and synthesized narratively. Costs were adjusted to US$ 2020. Results We identified eight economic evaluation studies (two cost-utility analyses, three cost-effectiveness analyses, and three studies reporting both cost-effectiveness and cost-utility analyses). Half of these studies were from Europe (n = 4), the other half were from Iran, USA, Canada, and Singapore. Six studies met > 80% of the JBI quality assessment criteria. Cost-effectiveness estimates in the majority (n = 6) of these studies were for Non-Hodgkin Lymphoma patients receiving myelosuppressive chemotherapy with high-risk of FN (> 20%). The studies considered a wide range of baseline FN risk (17–97.4%) and mortality rates (5.8–8.9%). Reported incremental cost-effectiveness ratios ranged from US$ 2199 to US$ 8,871,600 per quality-adjusted life-year (QALY) gained, dominant to US$ 44,358 per FN averted, and US$ 4261- US$ 7251 per life-years gained. The most influential parameters were medication and hospitalization costs, the relative risk of FN, and assumptions of mortality benefit. Conclusions Most studies showed that pegfilgrastim is cost-effective compared to filgrastim as primary and secondary prophylaxis for chemotherapy-induced FN among patients with lymphoma at a cost-effectiveness threshold of US$ 50,000 per QALY gained. The findings could assist clinicians and healthcare decision-makers to make informed decisions regarding resource allocation for the management of chemotherapy-induced FN in settings similar to those studied.

Abstract

Abstract Background Invasive candidiasis and/or candidemia (IC/C) is a common fungal infection leading to significant health and economic losses worldwide. Caspofungin was shown to be more effective than fluconazole in treating inpatients with IC/C. However, cost-effectiveness of caspofungin for treating IC/C in Ethiopia remains unknown. We aimed to assess the cost-utility of caspofungin compared to fluconazole-initiated therapies as primary treatment of IC/C in Ethiopia. Methods A Markov cohort model was developed to compare the cost-utility of caspofungin versus fluconazole antifungal agents as first-line treatment for adult inpatients with IC/C from the Ethiopian health system perspective. Treatment outcome was categorized as either a clinical success or failure, with clinical failure being switched to a different antifungal medication. Liposomal amphotericin B (L-AmB) was used as a rescue agent for patients who had failed caspofungin treatment, while caspofungin or L-AmB were used for patients who had failed fluconazole treatment. Primary outcomes were expected quality-adjusted life years (QALYs), costs (US$2021), and the incremental cost-utility ratio (ICUR). These QALYs and costs were discounted at 3% annually. Cost data was obtained from Addis Ababa hospitals while locally unavailable data were derived from the literature. Cost-effectiveness was assessed against the recommended threshold of 50% of Ethiopia’s gross domestic product/capita (i.e.,US$476). Deterministic and probabilistic sensitivity analyses were conducted to assess the robustness of the findings. Results In the base-case analysis, treatment of IC/C with caspofungin as first-line treatment resulted in better health outcomes (12.86 QALYs) but higher costs (US$7,714) compared to fluconazole-initiated treatment followed by caspofungin (12.30 QALYs; US$3,217) or L-AmB (10.92 QALYs; US$2,781) as second-line treatment. Caspofungin as primary treatment for IC/C was not cost-effective when compared to fluconazole-initiated therapies. Fluconazole-initiated treatment followed by caspofungin was cost-effective for the treatment of IC/C compared to fluconazole with L-AmB as second-line treatment, at US$316/QALY gained. Our findings were sensitive to medication costs, drug effectiveness, infection recurrence, and infection-related mortality rates. At a cost-effectiveness threshold of US$476/QALY, treating IC/C patient with fluconazole-initiated treatment followed by caspofungin was more likely to be cost-effective in 67.2% of simulations. Conclusion Our study showed that the use of caspofungin as primary treatment for IC/C in Ethiopia was not cost-effective when compared with fluconazole-initiated treatment alternatives. The findings supported the use of fluconazole-initiated therapy with caspofungin as a second-line treatment for patients with IC/C in Ethiopia.

Abstract

Background/Aim: C. albicans remains the most common pathogen responsible for invasive candidiasis. On the other hand, increased rates of candidiasis have been reported worldwide, caused by other Candida species (such as K. marxianus and C. lusitaniae). Considering these uncommon yeasts may be crucial pathogens in the future, it is preferable to describe the in-vitro activities of antifungal agents as potential options for their treatments. This study aimed to evaluate the in-vitro activity of nine different antifungal agents that are routinely used to contribute to the treatment of the infections caused by K. marxianus and C. lusitaniae. Methods: The study included 21 K. marxianus and eight C.lusitaniae strains isolated from various clinical specimens of patients with suspected invasive fungal infection. Conventional identification was confirmed using the molecular methodology of DNA sequencing analysis. Antifungal susceptibilities of the isolates were tested using the Sensititer Yeast One Test Panel Y06 kit, a colorimetric microdilution test. Results: For K. marxianus, amphotericin B had the highest geometric mean MIC (1 µg/mL) and voriconazole had the lowest geometric mean MIC (0.010 µg/mL). For C. lusitaniae, flucytosine had the highest geometric mean MIC (8 µg/mL) and voriconazole had the lowest geometric mean MIC (0.011 µg/mL). Conclusion: Considering that these two species, rare causes of invasive candidiasis nowadays, may become important pathogens in the future, it is reasonable to investigate the in-vitro activities of antifungal agents that can be used in their treatment.
