Abstract

Background: The benefit of pharmacogenetic testing before starting drug therapy has been well documented for several single gene–drug combinations. However, the clinical utility of a pre-emptive genotyping strategy using a pharmacogenetic panel has not been rigorously assessed. Methods: We conducted an open-label, multicentre, controlled, cluster-randomised, crossover implementation study of a 12-gene pharmacogenetic panel in 18 hospitals, nine community health centres, and 28 community pharmacies in seven European countries (Austria, Greece, Italy, the Netherlands, Slovenia, Spain, and the UK). Patients aged 18 years or older receiving a first prescription for a drug clinically recommended in the guidelines of the Dutch Pharmacogenetics Working Group (ie, the index drug) as part of routine care were eligible for inclusion. Exclusion criteria included previous genetic testing for a gene relevant to the index drug, a planned duration of treatment of less than 7 consecutive days, and severe renal or liver insufficiency. All patients gave written informed consent before taking part in the study. Participants were genotyped for 50 germline variants in 12 genes, and those with an actionable variant (ie, a drug–gene interaction test result for which the Dutch Pharmacogenetics Working Group [DPWG] recommended a change to standard-of-care drug treatment) were treated according to DPWG recommendations. Patients in the control group received standard treatment. To prepare clinicians for pre-emptive pharmacogenetic testing, local teams were educated during a site-initiation visit and online educational material was made available. The primary outcome was the occurrence of clinically relevant adverse drug reactions within the 12-week follow-up period. Analyses were irrespective of patient adherence to the DPWG guidelines. Abstract

Coffee (Coffea spp.) is one of the world's most valuable agricultural export commodities produced by small-scale farmers. Its germplasm, which holds useful traits for crop improvement, has traditionally been conserved in field genebanks, which presents many challenges for conservation. New techniques of in vitro and cryopreservation have been developed to improve the long-term conservation of coffee. But a question remains as to whether these new techniques are more cost effective than field collections and more efficient at reducing genetic erosion. This study compared the costs of maintaining one of the world's largest coffee field collections with those of establishing a coffee cryo-collection at the Centro Agron6mico Tropical de Investigacion y Ensenanza (CATIE) in Costa Rica. The results indicate that cryopreservation costs less (in perpetuity per accession) than conservation in field genebanks. A comparative analysis of the costs of both methods showed that the more accessions there are in cryopreservation storage, the lower the peraccession cost. In addition to cost, the study examined the advantages of cryopreservation over field collection and showed that for species that are difficult to conserve using seeds, and that can only be conserved as live plants, cryopreservation may be the method of choice for long-term conservation of genetic diversity. doi: 10.2135/cropsci2008.12.0736

Abstract

BackgroundThe proportion of older people has dramatically increased in recent decades. Moreover, social and demographic trends show a global increase of older people at risk of loneliness and lack of social relationships. The objective of this study was to evaluate the process, the effectiveness and the cost-effectiveness of a planned 22 weekly group sessions called School of Health for Older People to reduce social isolation.MethodsThis is a mixed methods multi-approach evaluation that includes: 1) A qualitative evaluation among coordinators and participants taking part in the intervention, through in depth-interviews and focus groups, respectively. The main topics covered will be positive and negative aspects of the intervention, suggestions for its improvement, opinions on different aspects of the intervention, and perceived benefits; 2) A quantitative quasi-experimental design, comparing a group of individuals taking part in the intervention with another group with similar characteristics not receiving the intervention. Data will be collected at the beginning and at the end of the intervention. Social support will be measured through questions drawn from the Medical Outcomes Study and the National Social Life, Health, and Aging Project questionnaires. Psychological morbidity will be measured through Goldberg's General Health Questionnaire, and Health-related Quality of Life will be measured through the EuroQoL questionnaire. Information on visits to the primary care center in the years before and after the intervention will be obtained from the electronic records of the primary care centers; 3) A cost-utility analysis, which will be conducted from a health system (primary care) perspective, including direct costs of the program and the primary care health services used. The effects of the intervention will be measured on quality-adjusted life years.DiscussionThere is an urgent need for studies assessing the effectiveness and the efficiency of potential interventions to reduce social isolation among older persons. The results of this study will help to fill the knowledge gap in this area and might be especially useful for the development of social and public health policies and programs for older people in disadvantaged neighborhoods in urban areas.Trial registrationNCT03142048 retrospectively registered (April 11, 2017).

Abstract

This paper proposes a discrete-time robust fault estimation scheme based on an interval Takagi-Sugeno Unknown Input Observer (UIO). The designed observer allows the interval estimation of states and faults by considering that the uncertainty is modelled in a deterministic bounded context. Furthermore, in the observer design, the effect of uncertainty is minimized using the H8 criterion using a LMI formulation. The proposed estimation scheme allows not only to provide the nominal value for states and faults but also an uncertainty interval where they should lie. Abstract

Fault estimation plays an important role in the fault diagnosis system since provides information about the fault magnitude and temporal evolution. In this paper, we present an approach that allows to obtain a simultaneous estimation of the fault, state and associated uncertainty intervals of a uncertain Takagi-Sugeno (TS) system. The fault estimation is obtained using a TS interval observer augmenting the system state with the fault and considering the system uncertainty in a bounded context. A set of Linear Matrix Inequalities (LMIs) have been derived to design the TS interval observer. Abstract

Antecedentes: Los trabajadores agrícolas y habitantes rurales están expuestos a productos plaguicidas. Aun cuando las políticas sanitarias se preocupan de normarlos, diversos estudios demuestran exposición en trabajadores y residentes cercanos a las zonas productivas. Entre ellos se encuentran mujeres gestantes, consideradas población particularmente vulnerable frente a plaguicidas. Objetivo: Identificar el contacto con plaguicidas, así como conductas asociadas a su uso, en mujeres gestantes de dos comunas agrícolas de la V Región en control prenatal en Centros de Salud Familiar. Método: Estudio observacional transversal, realizado en una muestra de ochenta gestantes que ingresaron a control entre mayo 2013 y abril 2014. El instrumento de recolección de datos fue una encuesta, aplicada por el equipo de salud al ingreso. Resultados: Las gestantes, en su gran mayoría, alguna vez han tenido contacto con plaguicidas, y cerca de la mitad lo tuvo en el mismo año en que se aplicó la encuesta. La exposición a plaguicidas se da tanto en los espacios productivos, como residenciales. Se observó que las medidas preventivas asociadas al contacto con agrotóxicos son insuficientes. A la vez, destaca la exposición a fumigaciones aéreas, frente a las cuales la posibilidad de prevención es aún menor. Conclusión: En comunas rurales puede existir un contacto permanente de la población femenina gestante con plaguicidas, lo cual sucede sin las medidas de protección adecuadas, incrementando el riesgo de exposición a estas sustancias. Es pertinente entonces, desarrollar una labor educativa activa para mejorar las prácticas de uso de plaguicidas. Background: Agriculture workers and rural dwellers are exposed to pesticide products. Sanitation policies attempt to regulate their use, but studies demonstrate that a significant percentage of workers and residents who are close to the productive zones are exposed to these chemicals. Pregnant women are particularly vulnerable to pesticides. Aim: To identify the risk of exposure to pesticides and practices of pesticide handling in women under pregnancy control programs in Family Health Care Centers in two rural districts of the 5th Region. Method: Cross-sectional study, conducted during May 2013 and April 2014 on eighty women, who entered pregnancy control programs in either rural district. The health professionals of the Centers collected the data in a survey. Results: Most of the pregnant women declared having been in contact with pesticides at some point. Almost half of them had this contact in the same year of their interview, that is previous or during their pregnancy. The exposure to pesticides takes place in productive locations as well as residential areas. The preventive measures associated to the contact with pesticides are not sufficient. Rural populations are also exposed to aerial fumigations, which further limits preventive measures. Conclusion: Pregnant women living in a rural district may be permanently exposed to pesticides. Exposure to agrochemicals is not associated to safe prevention practices which increase the risk of exposure. Educational programs may aid to enhance safe pesticide use.

A permanent-magnet DC motor is used as case of study. The main idea is to estimate on-line the non-load torque (To) in order to monitor the bearing health condition. The fault detection system is based on the design of a generalized Takagi-Sugeno PI (proportional-integral) observer. The main advantage of this approach is that it can be easily implemented because the observer gains are obtained by solving a set of LMIs (linear matrix inequalities). Moreover, the method can be extended to more complicated nonlinear systems by using the Takagi-Sugeno approach. A simulation is performed to show that this fault detection scheme can be applied to detect abrupt faults on rotative machinery which can lead the system to undesirable performance caused by vibrations or breakdown., Accepted version
