UNLABELLED: PRéCIS:: Noninferiority of efficacy was demonstrated for a preservative-free latanoprost-timolol fixed combination compared with a BAK-containing formulation at 84 days after treatment in patients with open-angle glaucoma or ocular hypertension. PURPOSE: The purpose of this study was to compare the effect on intraocular pressure and safety of preservative-free latanoprost-timolol fixed combination (T2347) to benzalkonium chloride-preserved latanoprost-timolol fixed combination in patients with open-angle glaucoma or ocular hypertension. METHODS: Phase III, randomized, parallel-group, investigator-masked study in 10 countries. A total of 242 patients aged 18 years or older with open-angle glaucoma or ocular hypertension in both eyes controlled with a preserved latanoprost-timolol fixed combination (15.7±2.4 mm Hg overall before inclusion) were randomized at day 0 with no washout period to receive the preservative-free alternative T2347 (N=127) or remain on the preserved comparator (N=115) for 84 days. Intraocular pressure changes from day 0 were measured at 9:00 am (±1 hour) on day 42 and day 84, and noninferiority of T2347 to the preserved comparator was analyzed statistically at day 84. Safety parameters were also reported. RESULTS: The mean change in intraocular pressure from baseline to day 84 was -0.49±1.80 mm Hg for preservative-free T2347 and -0.49±2.25 mm Hg for the preserved comparator. These results met the noninferiority limits. Similar results were observed at day 42. There was no difference between groups in the incidence of adverse events or ocular signs. The total ocular symptoms score was better for T2347 than BPLT upon instillation at day 84 (45.9%/44.3%/9.8% of patients with improvement/no change/worsening vs. 33.6%/47.3%/19.1%; P=0.021), reflecting improvements in individual symptoms such as irritation/burning/stinging (P

The article presents the results of a comparative analysis of central retinal thickness, macular volume and retinal nerve fiber layer thickness obtained with Stratus OCT 3000, Cirrus HD-OCT 4000 and Spectralis OCT. Statistically significant differences in central retinal thickness and macular volume were revealed. The absence of a difference pattern in retinal nerve fiber layer thickness measurements on different tomographs was found., В статье представлены результаты сравнительного анализа толщины центральной зоны сетчатки, макулярного объёма и толщины слоя нервных волокон сетчатки, полученные на приборах Stratus OCT 3000, Cirrus HD-OCT 4000 и Spectralis OCT. Выявлены статистически значимые различия показателей толщины центральной зоны сетчатки и макулярного объёма. Обнаружено отсутствие закономерности различий в измерениях толщины слоя нервных волокон сетчатки на разных томографах.

Item does not contain fulltext, Exfoliation syndrome (XFS) is the most common known risk factor for secondary glaucoma and a major cause of blindness worldwide. Variants in two genes, LOXL1 and CACNA1A, have previously been associated with XFS. To further elucidate the genetic basis of XFS, we collected a global sample of XFS cases to refine the association at LOXL1, which previously showed inconsistent results across populations, and to identify new variants associated with XFS. We identified a rare protective allele at LOXL1 (p.Phe407, odds ratio (OR) = 25, P = 2.9 x 10-14) through deep resequencing of XFS cases and controls from nine countries. A genome-wide association study (GWAS) of XFS cases and controls from 24 countries followed by replication in 18 countries identified seven genome-wide significant loci (P < 5 x 10-8). We identified association signals at 13q12 (POMP), 11q23.3 (TMEM136), 6p21 (AGPAT1), 3p24 (RBMS3) and 5q23 (near SEMA6A). These findings provide biological insights into the pathology of XFS and highlight a potential role for naturally occurring rare LOXL1 variants in disease biology.

- Abstract
Introduction. Ischemic maculopathy is the main cause of irreversible vision loss due to retinal vein occlusion (RVO). Fluorescein angiography (FA), which is the “gold standard” for evaluating retinal capillary plexuses, does not allow for the visualization of separate intraretinal capillary networks. Optical coherence tomography angiography (OCT-angiography) enables the possible visualization of four capillary plexi and allows for the quantitative analysis of microcirculation to quantitatively estimate capillary network density and non-perfusion areas. Aim. To investigate microcirculation changes using OCT-angiography data and to compare the changes with opthalmoplethysmography indices in patients with RVO. Material and methods. The study included 12 patients with RVO. In all patients, a routine ophthalmic examination was performed, and ocular blood flow was estimated using FA, OCT-angiography, and ophthalmoplethysmography. Results. Ischemia in the macular area was detected in four patients (25%) according to FA results, and in eight (67%) accor ding to OCT-angiography data. Compared with the unaffected eye, significant decrease in the density of both superficial and deep capillary plexuses as well as a decrease in “flow area” and enlargement of foveal avascular zone were observed. A significant close direct correlation was established between capillary density in the superficial capillary plexus (r > 0.8) and the deep capillary plexus (r > 0.7), choroidal thickness, and opthalmoplethysmography indices (r > 0.6). Conclusion. Compared with FA, OCT-angiography is a more sensitive method to detect macular capillary perfusion. In cases with RVO, the combination of the above mentioned methods with ophthalmoplethysmography allows for the comprehensive evaluation of regional hemodynamics., Введение. Ишемическая макулопатия — основная причина необратимого снижения зрения при окклюзии вен сетчатки (ОВС). «Золотым стандартом» оценки ретинальной сосудистой сети является флюоресцентная ангиография (ФАГ), однако она не позволяет раздельно визуализировать интраретинальные сосудистые сплетения. Оптическая когерентная томография-ангиография (ОКТ-ангиография) даёт возможность визуализировать четыре капиллярных сплетения и позволяет проводить количественный анализ микроциркуляции с расчётом плотности капиллярной сети и зон отсутствия капиллярной перфузии. Цель: изучить у больных с ОВС особенности изменения микроциркуляции по данным ОКТ-ангиографии и сопоставить их с показателями офтальмоплетизмографии. Материалы и методы. В исследование включено 12 пациентов с окклюзией вен сетчатки. Всем пациентам проводилось стандартное офтальмологическое обследование с дополнительной оценкой регионарной гемодинамики с помощью ФАГ, ОКТ-ангиографии и офтальмоплетизмографии. Результаты. У больных с ОВС по данным ФАГ ишемия в центральной зоне выявлена у 4 пациентов (25 %), а по данным ангиографии ОКТ — у 8 (67 %). При сравнении с непоражённым глазом выявлено значимое снижение плотности капилляров в поверхностном и глубоком капиллярных сплетениях, уменьшение зоны перфузии хориокапилляров и расширение фовеолярной аваскулярной зоны в глубоком капиллярном сплетении. Получена значимая тесная прямая связь между плотностью капилляров в поверхностном (r > 0,8) и глубоком (r > 0,7) капиллярных сплетениях сетчатки, толщиной хориоидеи и показателями офтальмоплетизмографии (r > 0,6). Выводы. ОКТ-ангиография является более чувствительным методом диагностики макулярной капиллярной перфузии по сравнению с флюоресцентной ангиографией. Сочетание обсуждаемых методов с выполнением офтальмоплетизмографии позволяет провести комплексную оценку регионарной гемодинамики при окклюзии вен сетчатки.

10. Choroidal thickness changes in age-related macular degeneration different forms and stages

- Abstract
Choroidal thickness measurement results of 124 patients (210 eyes) with age-related macular degeneration (AMD) obtained by enhanced depth imaging of spectral domain optical coherence tomography (EDI-OCT) are presented. There were no significant differences in choroidal thickness between patients with dry and wet AMD forms and healthy people of the same age group (р = 0.67; р = 0.26). A statistically significant choroidal thinning in patients with late AMD stages as compared to each of healthy people and dry and wet AMD patients was found (р < 0.0001)., В работе представлены результаты измерения толщины хориоидеи у 124 пациентов (210 глаз) с возрастной макулярной дегенерацией (ВМД) на спектральном оптическом когерентном томографе в режиме увеличенной глубины изображения (EDI-OCT). Значимых различий между толщиной хориоидеи у пациентов с сухой и влажной формами ВМД и у здоровых лиц той же возрастной группы не выявлено (р = 0,67; р = 0,26). Установлено статистически значимое истончение хориоидеи у пациентов с поздними стадиями ВМД по сравнению со здоровыми людьми вне зависимости от формы ВМД (р < 0,0001).

The article presents a review of studies dedicated to the role of genetic factors in age-related macular degeneration (AMD) pathogenesis. The polymorphisms of Y402H gene of the complement factor Н, HTRA1, ARMS2/LOC387715, and PLEKHA1 increase the risk of AMD development. More detailed description is done also for other genes, involved into this disease, which were identified so far. Possible schemes of influence of mutations in these genes on AMD development and progression, В статье представлен обзор исследований, посвящённых роли генетических факторов в патогенезе возрастной макулярной дегенерации (ВМД). Полиморфизмы Y402H гена фактора комплемента Н, HTRA1, ARMS2/LOC387715 и PLEKHA1 увеличивают риск развития ВМД. Более подробно описаны и другие гены, которые были идентифицированы на сегодняшний день. Рассматриваются возможные схемы влияния мутаций в этих генах на возникновение и прогрессирование ВМД.

BP was monitored before and after 10 surgical interventions of various complexity (2003 – 2010) in GSK (a patient of 76 – 84 years) suffering from hypertension disease since 1959. Treatment up to 09.2005 was performed traditionally (T, medications in mornings and evenings), later scheduled according to chronobiological principles, based on the 24-hour blood pressure (BP) profile. Changes in the parameters of the 24- and 12-hour components were analyzed during the 7 days preceding and the 7 days following each intervention. MESORs and acrophases did not change regularly after surgeries The magnitude (extent of predictable daily change determined from the composite model) did not change or even decreased on the days of operations but later began increasing (which could provoke sudden BP peaks), returning to initial values only within 6 – 7 days. In surgeries performed after T, inconsiderable increases in magnitude did not last more than 3 days or was absent altogether. As BP peaks may lead to many unfavorable effects, their reduction or disappearance after a chronobiologically scheduled treatment was implemented should be considered as a positive factor. Unfortunately, monitoring making this approach feasible is not realistic now, even in specialized surgical hospitals. © 2014 Nova Science Publishers, Inc.
