2. Association between intimate partner violence and nutritional status of married Nepalese women

Abstract Background Intimate partner violence (IPV) is physical, sexual, or psychological harm perpetrated by a spouse or an intimate partner. Its detrimental effects on women’s physical, mental, sexual, and reproductive health are well-documented. However, its impact on nutritional status is not well-studied, and previous studies have led to contradictory findings. This study aimed to explore the association between intimate partner violence and the nutritional status of married Nepalese women. Methods The study used the 2016 Nepal Demographic Health Survey data, which employed a modified version of the Conflict Tactics Scale to determine women’s exposure to IPV. Anemia and low body mass index (BMI) were used as proxies of nutritional status. Multinomial regression was used to analyze the relationship between BMI and IPV; multivariable logistic regression was used to analyze the association between anemia and IPV. Results The prevalence of underweight, overweight/obesity, and anemia were respectively 13.9%, 25.1%, and 38.7%. The prevalence of physical, sexual, and emotional IPVs experienced in the preceding year were respectively 9.8%, 4.6%, and 7.6%. Likewise, the prevalence of lifetime physical, sexual, emotional, and controlling behavior IPVs were respectively 21.8%, 7.4%, 12.3%, and 32.1%. The low intensity of emotional IPV (AOR 1.62; CI: 1.02–2.56) and moderate intensity of physical IPV (AOR 3.70; CI: 1.64–8.35) experienced in the preceding year, and low intensity of lifetime emotional IPV (AOR 1.69; CI: 1.11–2.58) were associated with an increased risk of overweight/obesity. Moderate intensity of sexual IPV (AOR 2.59; CI: 1.099–6.108) experienced in the preceding year was associated with an increased risk of underweight BMI. The low intensity of lifetime controlling behavior (AOR1.25; CI: 1.03–1.53) was associated with an increased risk of anemia. Conclusions Emotional and Physical IPVs are significantly associated with an increased risk of overweight/obesity. Sexual IPV is significantly associated with an increased risk of underweight BMI, and controlling behavior is significantly associated with an increased risk of anemia. Seeking help could offset the detrimental effects of IPV; therefore, IPV screening should be a part of regular healthcare assessment for married women, and appropriate rehabilitation should be offered to IPV survivors.

3. Intracholecystic Papillary Neoplasm with Low-grade Intraepithelial Neoplasia: A Rare Entity

Intracholecystic Papillary Neoplasm (ICPN) is a recently described, rare, mass forming, epithelial neoplasm originating in the mucosa of the gall bladder. It usually protrudes in the lumen and is generally non invasive, but may show dysplastic changes which may be low-grade or high-grade or may be associated with invasive carcinoma. Authors, hereby, describe a case of ICPN in a 57-year-old female patient who reported to Surgery Outpatient Department (OPD) with the chief complaint of pain in abdomen since last five days. On ultrasonography, gall stones were detected for which she underwent cholecystectomy. Histopathological Examination (HPE) revealed a papillary growth in the body of gall bladder measuring 1.0 cm in diameter. Sections from the growth showed back-to-back arranged glands exhibiting a papillary pattern. The glands were lined by columnar epithelium with basally located nuclei. Pyloric metaplasia was noted focally. No foci of invasion were seen. Papillary hyperplasia was noted in adjacent areas along with features consistent with chronic cholecystitis. Thus, a diagnosis of ICPN with low-grade intraepithelial neoplasia was made. The patient recovered well postsurgery and is on regular follow-up.

Abstract Background Glucose-6-phosphate dehydrogenase (G6PD) deficiency is currently a threat to malaria elimination due to risk of primaquine-induced haemolysis in G6PD deficient individuals. The World Health Organization (WHO) recommends G6PD screening before providing primaquine as a radical treatment against vivax malaria. However, evidence regarding the prevalence and causing mutations of G6PD deficiency in Nepal is scarce. Methods A cross-sectional, population-based, prevalence study was carried out from May to October 2016 in 12 malaria-endemic districts of Nepal. The screening survey included 4067 participants whose G6PD status was determined by G6PD Care Start™ rapid diagnostic test and genotyping. Results The prevalence of G6PD deficiency at the national level was 3.5% (4.1% among males and 2.1% among females). When analysed according to ethnic groups, G6PD deficiency was highest among the Janajati (6.2% overall, 17.6% in Mahatto, 7.7% in Chaudhary and 7.5% in Tharu) and low among Brahman and Chhetri (1.3%). District-wise, prevalence was highest in Banke (7.6%) and Chitwan (6.6%). Coimbra mutation (592 C>T) was found among 75.5% of the G6PD-deficient samples analysed and Mahidol (487 G>A) and Mediterranean (563 C>T) mutations were found in equal proportions in the remaining 24.5%. There was no specific geographic or ethnic distribution for the three mutations. Conclusions This study has identified populations with moderate to high prevalence of G6PD deficiency which provides strong evidence supporting the WHO recommendations to screen G6PD deficiency at health facility level before the use of primaquine-based radical curative regimen for Plasmodium vivax.

In this manuscript, the time-fractional diffusion equation in the framework of the Yang–Abdel–Cattani derivative operator is taken into account. A detailed proof for the existence, as well as the uniqueness of the solution of the time-fractional diffusion equation, in the sense of YAC derivative operator, is explained, and, using the method of α-HATM, we find the analytical solution of the time-fractional diffusion equation. Three cases are considered to exhibit the convergence and fidelity of the aforementioned α-HATM. The analytical solutions obtained for the diffusion equation using the Yang–Abdel–Cattani derivative operator are compared with the analytical solutions obtained using the Riemann–Liouville (RL) derivative operator for the fractional order γ=0.99 (nearby 1) and with the exact solution at different values of t to verify the efficiency of the YAC derivative operator.

9. Cytomorphological profile of lymph nodes at a tertiary care centre in Uttar Pradesh: A retrospective study

Fine Needle Aspiration Cytology (FNAC) is a quick, reliable, minimally invasive and cost effective diagnostic modality for evaluation of superficial lymph nodes and has been accepted as a first line investigation in most centres. The aim of the study was to identify the patterns of pathologies in aspiration cytology of lymph nodes in a tertiary care hospital located in Uttar Pradesh, India. The study was undertaken as a retrospective study using data in the Department of Pathology, G.S Medical College and Hospital, Uttar Pradesh, India. 235 lymph node FNACs that were performed during the two year span of 2018-19 were analysed. Females comprised 59.6% of the total patients who underwent lymph node FNAC procedure, 95 (40.4%) were males. The youngest patient was a 3 month old and the oldest happened to be 78 years. Lymph node enlargement was traced to malignant processes exclusively above 40 years of age. Uptill that age, tuberculosis seems to be the major cause underlying lymphadenopathy, mostly presenting as granulomatous lymphadenitis. Out of all cases which showed granulomatous or necrotic features, 27% showed the presence of AFB, hence confirming tuberculosis. A vast majority of the FNACs were performed for cervical lymphadenopathy (92.8%). 42.1% of the FNACs revealed reactive morphology. This diagnosis was closely followed by granulomatous lymphadenitis in 29.4% patients. Together with a definite diagnosis of tuberculous lymphadenitis, patients with granulomatous lymphadenitis comprised 42.6% of all diagnoses. 12 patients received a diagnosis of suppurative lymphadenitis, 9 received necrotizing lymphadenitis. 12 patients were diagnosed as metastatic carcinoma and 1 was suggestive of lymphoma. Most of the lymph nodes whose sizes were under 1 cm were reactive on cytology. Lymphadenopathy due to malignant etiology presented with lymph nodes measuring at least 2 cm. Granulomatous/tuberculous lymphadenitis presented in lymph nodes of varying sizes, from as small as 0.5 cm to larger ones.

10. Patient and Provider Perspectives of Disrespect and Abuse during Childbirth in Tanzania: A Literature Review

Objective: Disrespectful and abusive (D&A) maternity care is a deterrent to facility childbirth and can thus contribute to child and maternal mortality. This study will review existing literature on D&A in Tanzania to better understand and contextualize the issue. Methods: A comprehensive literature search was conducted to find relevant publications on D&A during childbirth in Tanzania. The search was conducted on the National Center for Biotechnology Information (NCBI) and PubMed (U.S. National Library of Medicine at the National Institutes of Health). The inclusion criteria were as follows: qualitative, quantitative, and ethnographic studies conducted in Tanzania on obstetrical violence; published in English; focused on prevalence, incidence, root causes, historical trends, interventions, and policy recommendations for obstetrical violence in Tanzania. Results: Twenty-one studies were selected for this literature review. Up to 73.1% of women reported experiencing at least one form of D&A during labor. Commonly reported D&A events include non-dignified care, non-confidential care, and physical abuse. Other forms of D&A, including detention in facilities and asking for unofficial payments, are also reported. Ninety-six percent of nurses, midwives, and nursing assistants in Tanzania self-reported engaging in at least one form of D&A. Lack of training and provider support, long work hours, fear of blame, and limited infrastructure were reported as factors leading to D&A by providers. Conclusions: In Tanzania, D&A during childbirth is prevalent in healthcare systems. The direct relationship between D&A and poor maternal and fetal health is well-documented. Individual and systemic factors influence both provider and patient perspectives of D&A, providing a window into a complex and sensitive phenomenon. It is important that D&A in Tanzania be viewed holistically, and that interventions target the multifaceted nature of the issue.

- Abstract
The paper is concerned with the SIZR mathematical model for an outbreak of zombie infection with time-dependent infection rate. This class of the SIZR model involves equations that relate the susceptible S(t), the infected I(t), the zombie Z(t), and removed population R(t). The well poseness of the model is presented. The proposed model is then outstretched to the fractional order mathematical model with three different derivative operators i.e., Caputo, Caputo-Fabrizio, and Atangana-Baleanu fractional derivative operator. The conditions under which the model has a unique solution are established for different derivative operators. Using the numerical scheme which was proposed by Atangana and Toufik the numerical solutions are presented for the different fractional derivative operators.

12. Survival after Sexual Violence and Genocide: Trauma and Healing for Yazidi Women in Northern Iraq

Introduction: In August 2014, the Yazidi community of Sinjar, in the Nineveh Governorate of Northern Iraq, was brutally targeted by the so-called Islamic State of Iraq and Syria (ISIS) for annihilation through murder, torture, and the systematic and premeditated use of rape and sexual slavery of Yazidi women. In 2016, the United Nations High Commissioner for Human Rights concluded that ISIS was committing genocide, crimes against humanity, and war crimes against Yazidis. Methods: Using current international literature, which includes reviews, qualitative interviews of survivors, and reports from medical and humanitarian actors, this paper explores the short and potentially long-term physical and mental health consequences of the extreme physical and sexual violence and atrocities perpetrated against Yazidi women. Results: Yazidi women survivors of kidnapping, sex slavery, and rape experienced significant levels of physical ailments, chronic pain, and mental health conditions. All women reported feelings of guilt, stress, insomnia, and severe flashbacks. The incidence of post-traumatic stress disorder (PTSD) ranged from 42% to 90%. Sixty-seven percent suffered from a somatoform disorder, 53% had depression, 39% experienced anxiety, and 28% suffered from dissociation. Conclusions: Sexual violence against women is a common tool systematically employed during wars and genocide. In recent ISIS attacks, intentional perpetration of mass rapes of women and execution of men was a strategy to destroy an entire population. PTSD and depression are common after traumatic stress. For disaster responders and humanitarian workers, training and education to understand, try to prevent, and plan for interventions when gender-based violence and sexual exploitation occurs must become a mandatory part of emergency preparedness.

Obstetric violence or disrespect and abuse in childbirth is a worldwide phenomenon that takes on various forms, from absence of informed consent to physical harm. The objective of this review is to assess prevalence and root causes of obstetric violence in different countries, and potential solutions to address disrespect and abuse in childbirth. The review finds that obstetric violence is rooted in a patriarchal understanding of gender stereotypes and is exacerbated by power dynamics between health professionals and patients, especially for minorities. Obstetric violence has a long-lasting impact on women’s lives and can jeopardize subsequent decisions to access healthcare services.

17. Association Between Intimate Partner Violence and Undernutrition Among Married Nepalese Women of Reproductive Age

BackgroundIntimate partner violence (IPV) is well recognized as a human rights violation and a serious global health issue that has affected about a third of worldwide women at any given time. It is defined as physical, sexual, or psychological harm caused by a spouse or an intimate partner. Its short and long-term detrimental effects on women’s physical, mental, sexual, and reproductive health are well-documented. However, its effect on nutritional status is not well-studied, and previous studies have led to contradictory findings. This study aimed to explore the association between intimate partner violence and undernutrition among married Nepalese women of reproductive age.MethodThe 2016 Nepal Demographic Health Survey data was used in this study. This study used a modified version of the Conflict Tactics Scale to determine women’s exposure to IPV, and anemia and low body mass index as the proxies of undernutrition. Prevalence of IPV, anemia, and underweight were calculated across sociodemographic characteristics, and multivariate logistic regression was used to analyze the association between IPV and undernutrition.ResultsAmong 3476 women included in this study, the prevalence of physical, sexual, and emotional IPVs were 21.75%, 7.42%, and 12.31% respectively. 13.78% of women were underweight and 38.95% were anemic. The adjusted odds ratio of physical, sexual, and emotional IPV survivors of being underweight were 1.06 (95% CI: 0.95–1.17), 0.96 (95% CI: 0.73–1.25), and 1.14 (95% CI: 0.89–1.43) respectively. The adjusted odds ratio of being anemic for physical, sexual, and emotional IPV survivors was 1.01 (95% CI: 0.93–1.09), 1.24 (95% CI: 1.02–1.52), and 0.92 (95% CI: 0.76–1.11) respectively.ConclusionNone of the three IPV types was significantly associated with being underweight. Among the three IPV types, only sexual IPV (SIPV) was significantly associated with anemia, meaning the increase in SIPV score by 1, which is experiencing an additional form of SIPV, increased SIPV survivors' odds of being anemic by 24%. Therefore, IPV screening should be a part of regular healthcare assessment for undernourished married women, especially for those who are anemic, and appropriate support should be offered to IPV survivors.

Lipoleiomyoma is a rare, benign lipoid tumor of the uterus. Histologically, these tumors are composed of variable amount of smooth muscles with lobules of adipocytes separated by thin brous tissue. They primarily occur in obese postmenopausal women and often remain asymptomatic. Imaging plays a crucial role in determining the intrauterine location along with the fatty component of these tumors. Tuberculosis is a global health problem mainly affecting people in developing countries although muscular involvement is rarely seen. Authors hereby, describe a case of tuberculosis of triceps muscle in a 35-year-old immunocompetent female who presented with swelling in right upper arm. Radiological examination ruled out involvement of underlying bones. Excision biopsy was performed and on histopathological examination cyst wall showed presence of foamy macrophages in sheets, inammatory cells and few scattered epithelioid cells but no well-formed granulomas or giant cells were seen. ZN stain for acid fast bacilli was positive. Based on the above ndings a diagnosis of tuberculosis of triceps muscle was made. The patient responded well to antituberculosis therapy and is currently on follow up.

Background Glucose-6-phosphate dehydrogenase (G6PD) deficiency is currently a threat to malaria elimination due to risk of primaquine-induced haemolysis in G6PD deficient individuals. The World Health Organization (WHO) recommends G6PD screening before providing primaquine as a radical treatment against vivax malaria. However, evidence regarding the prevalence and causing mutations of G6PD deficiency in Nepal is scarce. Methods A cross-sectional, population-based, prevalence study was carried out from May to October 2016 in 12 malaria-endemic districts of Nepal. The screening survey included 4067 participants whose G6PD status was determined by G6PD Care Start™ rapid diagnostic test and genotyping. Results The prevalence of G6PD deficiency at the national level was 3.5% (4.1% among males and 2.1% among females). When analysed according to ethnic groups, G6PD deficiency was highest among the Janajati (6.2% overall, 17.6% in Mahatto, 7.7% in Chaudhary and 7.5% in Tharu) and low among Brahman and Chhetri (1.3%). District-wise, prevalence was highest in Banke (7.6%) and Chitwan (6.6%). Coimbra mutation (592 C>T) was found among 75.5% of the G6PD-deficient samples analysed and Mahidol (487 G>A) and Mediterranean (563 C>T) mutations were found in equal proportions in the remaining 24.5%. There was no specific geographic or ethnic distribution for the three mutations. Conclusions This study has identified populations with moderate to high prevalence of G6PD deficiency which provides strong evidence supporting the WHO recommendations to screen G6PD deficiency at health facility level before the use of primaquine-based radical curative regimen for Plasmodium vivax.

Ectopic thyroid tissue is a rare developmental abnormality. A case is reported here of ectopic thyroid tissue that presented as a midline mass, mimicking a thyroglossal duct cyst. The thyroid gland was not visualized in its normal position. This report highlights the importance of imaging in the preoperative evaluation of a midline neck mass.

Wet snow avalanches in India are common during the mid- and late winter in the Pir Panjal Range (2000–3000ma.s.l.) and during the late winter in the Great Himalayan Range (3000 ma.s.l. and above). Although it is well known that the presence of liquid water in snow makes the flow behaviour of wet snow avalanches different from that of dry snow avalanches, there exist few actual flow measurements with wet snow. The aim of this investigation is to understand the dynamics of wet snow avalanches by conducting medium-scale experiments (volumes of 3, 6 and 11 m3) on the Dhundi snow chute in Himachal Pradesh, India. We measured flow velocities using video data, as well as optical velocity sensors installed on the side walls and running surface. Measurement results relating to the slip velocity of the front and tail of the moving snow mass, as well as the average slip velocity, are presented. In addition, we use the results of the vertical velocity profile measurements to calculate the effective viscosity of snow at two locations within the flow. We identified a shear thinning type of behaviour, suggesting that a single avalanche rheology cannot describe wet snow avalanche behaviour.

Objective : To study the clinico-pathological profile of significant pediatric peripheral lymphadenopathy and to arrive at an etiological diagnosis.Methods : Prospective study in a tertiary care hospital setting. One hundred consecutive children reporting to pediatric OPD from 1 January 1995 to 31 December 1998, aged 1 month to 12 years were studied.Results : The commonest aetiology diagnosed was tubercular lymphadenitis in 35% cases, followed by chronic tonsillopharyngitis in 15% cases. Lymphomas, AIDS and infectious mononucleosis constituted 3,2 and 1 cases each. Aetiology could not be ascertained in 44 (44%) children even after detailed haematological, microbiological, radiological and serological investigations. FNAC’s sensitivity and specificity as compared to ’gold standard of excision lymph node biopsy was 94% and 100% respectively.Conclusion : Tubercular lymphadenitis was the commonest treatable entity of significant pediatric peripheral lymphadenopathy. A majority of the cases even after thorough evaluation, remained undiagnosed. FNAC as a diagnostic modality is almost as sensitive and as specific as excision lymph node biopsy when an adequate aspirate is examined by expert eyes.
