Abstract

The formation of bacterial biofilms in the human body and on medical devices is a serious human health concern. Infections related to bacterial biofilms are often chronic and difficult to treat. Detailed information on biofilm formation and composition over time is essential for a fundamental understanding of the underlying mechanisms of biofilm formation and its response to anti-biofilm therapy. However, information on the chemical composition, structural components of biofilms, and molecular interactions regarding metabolism- and communication pathways within the biofilm, such as uptake of administered drugs or inter-bacteria communication, remains elusive. Imaging these molecules and their distribution in the biofilm increases insight into biofilm development, growth, and response to environmental factors or drugs. This systematic review provides an overview of molecular imaging techniques used for bacterial biofilm imaging. The techniques included mass spectrometry-based techniques, fluorescence-labelling techniques, spectroscopic techniques, nuclear magnetic resonance spectroscopy (NMR), micro-computed tomography (µCT), and several multimodal approaches. Many molecules were imaged, such as proteins, lipids, metabolites, and quorum-sensing (QS) molecules, which are crucial in intercellular communication pathways. Advantages and disadvantages of each technique, including multimodal approaches, to study molecular processes in bacterial biofilms are discussed, and recommendations on which technique best suits specific research aims are provided. 2. Enhancing Written Feedback: The Use of a Cover Sheet Influences Feedback Quality

Abstract

Feedback can be effective to student learning if the feedback practice meets several success criteria. It appears, however, that it is not easy to put insights from theory into practice. Using a cover sheet to provide structured feedback may provide a solution. Just how cover sheets influence feedback practice is, however, still largely unknown. The present study offers an in-depth evaluation of the effects of the implementation of cover sheets on feedback practice. The study described in this article gathered data from almost 1000 feedback instances, from tutor and student interviews and from a student questionnaire. The analysis shows that the use of the cover sheet led to an increased use of feed up, feed forward and feedback on process level. Tutors and students valued the use of the cover sheet as positive, and the cover sheet helped students resolve issues better than with annotations alone. The study described in this article adds to the field of research by providing empirical data for how a cover sheet influences educational practice. The study furthermore shows that a cover sheet can be used to enhance student feedback literacy and it offers background for an easy to introduce educational intervention.

6. Clinical outcomes and complications of S53P4 bioactive glass in chronic osteomyelitis and septic non-unions: a retrospective single-center study

Abstract

Introduction: Dead space management following debridement surgery in chronic osteomyelitis or septic non-unions is one of the most crucial and discussed steps for the success of the surgical treatment of these conditions. In this retrospective clinical study, we described the efficacy and safety profile of surgical debridement and local application of S53P4 bioactive glass (S53P4 BAG) in the treatment of bone infections. Methods: A consecutive single-center series of 38 patients with chronic osteomyelitis (24) and septic non-unions (14), treated with bioactive glass S53P4 as dead space management following surgical debridement between May 2015 and November 2020, were identified and evaluated retrospectively. Results: Infection eradication was reached in 22 out of 24 patients (91.7%) with chronic osteomyelitis. Eleven out of 14 patients (78.6%) with septic non-union achieved both fracture healing and infection healing in 9.1 ± 4.9 months. Three patients (7.9%) developed prolonged serous discharge with wound dehiscence but healed within 2 months with no further surgical intervention. Average patient follow-up time was 19.8 months ± 7.6 months. Conclusion: S53P4 bioactive glass is an effective and safe therapeutic option in the treatment of chronic osteomyelitis and septic non-unions because of its unique antibacterial properties, but also for its ability to generate a growth response in the remaining healthy bone at the bone-glass interface.

10. CENSORS: A Combined EIS-NVSS Survey Of Radio Sources. I. Sample definition, radio data and optical identifications

Abstract

A new sample of radio sources, with the designated name CENSORS (A Combined EIS-NVSS Survey Of Radio Sources), has been defined by combining the NRAO VLA Sky Survey (NVSS) at 1.4 GHz with the ESO Imaging Survey (EIS) Patch D, a 3 by 2 degree region of sky centred at 09 51 36.0, -21 00 00 (J2000). New radio observations of 199 NVSS radio sources with NVSS flux densities S(1.4GHz) > 7.8mJy are presented, and compared with the EIS I-band imaging observations which reach a depth of I~23; optical identifications are obtained for over two-thirds of the ~150 confirmed radio sources within the EIS field. The radio sources have a median linear size of 6 arcseconds, consistent with the trend for lower flux density radio sources to be less extended. Other radio source properties, such as the lobe flux density ratios, are consistent with those of brighter radio source samples. From the optical information, 30-40% of the sources are expected to lie at redshifts z >~ 1.5. One of the key goals of this survey is to accurately determine the high redshift evolution of the radio luminosity function. These radio sources are at the ideal flux density level to achieve this goal; at redshifts z~2 they have luminosities which are around the break of the luminosity function and so provide a much more accurate census of the radio source population at those redshifts than the existing studies of extreme, high radio power sources. Other survey goals include investigating the dual--population unification schemes for radio sources, studying the radio luminosity dependence of the evolution of radio source environments, and understanding the radio power dependence of the K-z relation for radio galaxies., Comment: Accepted for publication in MNRAS. 28 pages plus 36 reduced resolution jpeg figures. Abstract

CCD UBVRi photometry of the final helium flash object V4334 Sgr (Sakurai's Object), carried out during 1997 - 1999, is presented, and the light curve from its pre-discovery rise to the dust obscuration phase is constructed. The optical light curve can be divided into four sections, the rise to maximum, the maximum, the dust onset, and the massive dust shell phase. The color indices show a general increase with time, first because of the photospheric expansion and cooling, and later because of the dust forming events. The energy distributions for the years 1996 - 1999 show that an increasing part of the energy is radiated at infrared wavelengths. In 1996, the infrared excess is likely caused by free-free radiation in the stellar wind. Starting from 1997 or 1998 at the latest, carbon dust grains are responsible for the more and more dramatic decrease of optical radiation and the growing infrared excess. Its photometric behavior in 1998 - 1999 mimics the ``red declines'' of R CrB variables, the amplitude, however, is more extreme than any fading ever observed in an R CrB star. Evidence is given that a complete dust shell has formed around V4334 Sgr. It therefore shows similarities with dust-forming classical novae, although evolving ~20 times more slowly. Its luminosity increased by a factor 4 between 1996 and 1998. A comparison of time scales of the final helium flash objects FG Sge, V605 Aql and V4334 Sgr shows that the observed photometric and spectroscopic features are similar, while V4334 Sgr is the most rapidly evolving object to date., Comment: 27 pages, including 5 tables and 9 figures, LaTeX, accepted by the Astronomical Journal (scheduled for May, 2000)

Abstract

BACKGROUND: Studies concerning total ankle arthroplasty could be influenced by several forms of bias. Independent national arthroplasty registries represent objective data on survival and patient reported outcomes. The aim of this study was to determine survival and identify risk factors for early failure in a nationwide series of total ankle arthroplasties from the Dutch Arthroplasty Register (LROI).PATIENTS AND METHODS: Data of 810 patients, who received 836 total ankle arthroplasties between 2014 and 2020 were obtained from the Dutch Arthroplasty Register (LROI) with a median follow-up of 38 months (range 1-84 months). Survival was expressed in Kaplan-Meier analysis and associated hazard ratios for implant failure were determined. Implant failure was defined as the need for revision surgery for any reason or (pan)arthrodesis.RESULTS: During follow-up, we recorded 39 failures (4.7%) resulting in a implant survival of 95.3% with a median follow-up of 38 months (range 1-84 months). Medial malleolus osteotomy (HR = 2.27), previous surgery (HR = 1.83), previous osteotomy (HR = 2.82) and previous ligament reconstruction (HR = 2.83) all showed potentially clinically meaningful associations with a higher incidence of implant failure, yet only previous OCD treatment (HR = 6.21), BMI (HR = 1.09) and age (HR = 0.71) were statistically significant.INTERPRETATION: Excellent short-term survival (95.3%) with a median follow-up of 38 months was reported for TAA patients from the Dutch Arthroplasty Register. Patients with a lower age, a higher BMI or who had a prior surgical OCD treatment before TAA surgery appear to have a higher risk for revision after short-term clinical follow-up. Thorough patient selection with emphasis on risk factors associated with early implant failure might be essential to improve TAA survivorship.

Abstract

Background Context Cervical disc replacement (CDR) aims to restore motion of the treated level to reduce the risk of adjacent segment disease (ASD) compared with spinal fusion. However, first-generation articulating devices are unable to mimic the complex deformation kinematics of a natural disc. Thus, a biomimetic artificial intervertebral CDR (bioAID), containing a hydroxyethylmethacrylate (HEMA)—sodium methacrylate (NaMA) hydrogel core representing the nucleus pulposus, an ultra-high-molecular-weight-polyethylene fiber jacket as annulus fibrosus, and titanium endplates with pins for primary mechanical fixation, was developed. Purpose To assess the initial biomechanical effect of the bioAID on the kinematic behavior of the canine spine, an ex vivo biomechanical study in 6-degrees-of-freedom was performed. Study Design A canine cadaveric biomechanical study. Methods Six cadaveric canine specimens (C3-C6) were tested in flexion-extension (FE), lateral bending (LB) axial rotation (AR) using a spine tester in three conditions: intact, after C4-C5 disc replacement with bioAID, and after C4-C5 interbody fusion. A hybrid protocol was used where first the intact spines were subjected to a pure moment of ±1 Nm, whereafter the treated spines were subjected to the full range of motion (ROM) of the intact condition. 3D segmental motions at all levels were measured while recording the reaction torsion. Biomechanical parameters studied included ROM, neutral zone (NZ), and intradiscal pressure (IDP) at the adjacent cranial level (C3-C4). Results The bioAID retained the sigmoid shape of the moment-rotation curves with a NZ similar to the intact condition in LB and FE. Additionally, the normalized ROMs at the bioAID-treated level were statistically equivalent to intact during FE and AR while slightly decreased in LB. De programmadirectie van het Hoogwaterbeschermingsprogramma heeft de ambitie geformuleerd om ruimtelijke kwaliteit te borgen in het planproces van alle HWBP-projecten. Op basis van een grootschalige enquête onder projectmanagers beschrijft dit artikel welke beleidsinstrumenten zij inzetten om ruimtelijke kwaliteit binnen hun projecten te borgen, en wat hiervoor belangrijke drijfveren waren. De enquêteresultaten laten zien dat het realiseren van ruimtelijke kwaliteit sterk afhangt van de geformuleerde projectdoelen. Ook de ervaring van de projectteamleden en de beleidscontext waarin zij opereren spelen een belangrijke rol. Het artikel eindigt met vijf lessen voor het versterken van ruimtelijke kwaliteit in waterveiligheidsprojecten.

Abstract

Ondanks de toenemende aandacht voor ruimtelijke kwaliteit bestaat er nog geen standaardprocedure voor het integreren van ruimtelijke kwaliteit in het planproces van waterveiligheidsprojecten. In dit artikel wordt een instrumentenpalet geïntroduceerd, bestaande uit vier instrumententypes en bijbehorende instrumenten voor het versterken van ruimtelijke kwaliteit in waterveiligheidsprojecten. Het palet is ontwikkeld met diverse waterschappen, kennis- en adviesbureaus en kan gebruikt worden als handreiking voor projectteams van waterschappen. Naast een toelichting van de instrumenten worden ook praktijkervaringen gedeeld over het gebruik van deze instrumenten. Zo valt bijvoorbeeld de ‘roep om een ruimtelijke integrator’ in het projectteam op.

Abstract

BACKGROUND: Irritable bowel syndrome (IBS) is characterised by recurrent abdominal pain related to defaecation or associated with altered stool frequency or consistency. Despite its prevalence, major uncertainties in the diagnostic and therapeutic management persist in clinical practice. METHODS: A Delphi consensus was conducted by 20 experts from Belgium, and consisted of literature review and voting process on 78 statements. Grading of recommendations, assessment, development and evaluation criteria were applied to evaluate the quality of evidence. Consensus was defined as > 80 % agreement. RESULTS: Consensus was reached for 50 statements. The Belgian consensus agreed as to the multifactorial aetiology of IBS. According to the consensus abdominal discomfort also represents a cardinal symptom, while bloating and abdominal distension often coexist. IBS needs subtyping based on stool pattern. The importance of a positive diagnosis, relying on history and clinical examination is underlined, while additional testing should remain limited, except when alarm features are present. Explanation of IBS represents a crucial part of patient management. Lifestyle modification, spasmolytics and water-solube fibres are considered first-line agents. The low FODMAP diet, selected probiotics, cognitive behavioural therapy and specific treatments targeting diarrhoea and constipation are considered appropriate. There is a consensus to restrict faecal microbiota transplantation and gluten-free diet, while other treatments are strongly discouraged. Abstract

Irritable bowel syndrome (IBS) is characterised by recurrent abdominal pain related to defaecation or associated with altered stool frequency or consistency. Despite its prevalence, major uncertainties in the diagnostic and therapeutic management persist in clinical practice. A Delphi consensus was conducted by 20 experts from Belgium, and consisted of literature review and voting process on 78 statements. Grading of recommendations, assessment, development and evaluation criteria were applied to evaluate the quality of evidence. Consensus was defined as > 80 % agreement. Consensus was reached for 50 statements. The Belgian consensus agreed as to the multifactorial aetiology of IBS. According to the consensus abdominal discomfort also represents a cardinal symptom, while bloating and abdominal distension often coexist. IBS needs subtyping based on stool pattern. The importance of a positive diagnosis, relying on history and clinical examination is underlined, while additional testing should remain limited, except when alarm features are present. Explanation of IBS represents a crucial part of patient management. Lifestyle modification, spasmolytics and water-solube fibres are considered first-line agents. The low FODMAP diet, selected probiotics, cognitive behavioural therapy and specific treatments targeting diarrhoea and constipation are considered appropriate. There is a consensus to restrict faecal microbiota transplantation and gluten-free diet, while other treatments are strongly discouraged. A panel of Belgian gastroenterologists summarised the current evidence on the aetiology, symptoms, diagnosis and treatment of IBS with attention for the specificities of the Belgian healthcare system.

De verwachting van docenten en schoolleiders is dat net afgestudeerde studenten aan de lerarenopleidingen op de hoogte zijn van formatief evalueren en beschikken over een (vak)didactisch handelingsrepertoire om formatief evalueren in de praktijk te brengen.In dit artikel leest u de resultaten van een onderzoek waarbij de kennisbases van 14 eerstegraads en 18 tweedegraads lerarenopleidingen geanalyseerd zijn op de aandacht voor formatief evalueren om inzicht te krijgen in het beoogde curriculum. Met een kwantitatieve en kwalitatieve analyse van de kennisbases aan de hand van de formatieve evaluatiecyclus wordt duidelijk dat de aandacht voor formatief evalueren verschillend is tussen de kennisbases van de eerstegraads lerarenopleidingen en de kennisbases van de tweedegraads lerarenopleidingen en dat er grote verschillen zijn tussen de verschillende vakspecifieke kennisbases. In het algemeen kan gesteld worden dat er relatief weinig aandacht is voor formatief evalueren. De resultaten van dit onderzoek kunnen gebruikt worden bij de herziening van de kennisbases.

32. Complications following total ankle arthroplasty: A systematic literature review and meta-analysis

Abstract

BACKGROUND: Total ankle arthroplasty (TAA) is increasingly used as a treatment for end-stage ankle arthropathy. However, TAA may be more sensitive to complications, failure and subsequent re-operations compared to ankle arthrodesis. The aim of this systematic review and meta-analysis is to generate an overview of complications of TAA surgery.METHODS: PubMed, EMBASE and the Cochrane library were searched between 2000 and 2020 to identify all papers reporting on complications in TAA surgery. Meta-analysis was conducted based on type of complication in TAA surgery. Pooled estimates of complications were calculated using a random effects model. Risk of bias and quality was assessed using the Cochrane risk of bias and ROBINS-I tools. The confidence in estimates was rated and described according to the recommendations of the GRADE working group.RESULTS: One hundred twenty-seven studies were included in this systematic review. All combined, they reported on 16.964 TAAs with an average follow-up of 47.99 ± 29.18 months. Complications with highest reported pooled incidence were intra-operative fracture 0.06 (95 %CI 0.04-0.08) (GRADE Very low) and impingement 0.06 (95 %CI 0.04-0.08) (GRADE low) respectively.CONCLUSION: Reported complication incidence of TAA surgery is still high and remains a significant clinical problem that can be severely hampering long-term clinical survival of the prosthesis. The results of this systematic review and meta-analysis can help guide surgeons in informing their patient about complication risks. Implementation of more stringent patient selection criteria might contribute to diminishing TAA complication rates.

34. Going with the waves: The role of the vasculature during leukocyte transendothelial migration

Abstract

During the inflammatory response, the white blood cells must exit the blood vessels at the right site to migrate into the tissue. To do so, the blood vessel wall cells – endothelial cells – provide signals when an inflammation in the underlying tissue occurs. The circulating white blood cells recognize these signals by interacting with the endothelial cells of the blood vessel wall. As inflammation often occurs in the underlying tissue, white blood cells maneuver out of the blood vessel, without causing excessive leakage of other blood components. Therefore, a tightly regulated interplay occurs when the white blood cells squeeze themselves between and through the endothelial cells. This process is called transendothelial migration; the migration of white blood cells through (trans) the blood vessel wall (endothelial layer). This process is tightly regulated to make it highly efficient. In this thesis new insights in the process of transendothelial migration are discussed. We describe a phenomenon called transendothelial migration hotspots. These preferred diapedesis sites are marked by adhesion molecules but also by endothelial membrane protrusions that are used by neutrophils to exit the vasculature. Furthermore, we discuss the post-diapedesis phase, where neutrophils migrate around focal adhesions underneath the endothelium. These new insights might help us better understanding TEM but also reveal targets for therapeutic intervention in disease situations.

Abstract

Environmental impact assessment (EIA) is implemented in most countries as an administrative procedure. In this context, it is subject to the principles of administrative justice. However, to date, no empirical research has been conducted to determine the extent to which EIA decisions comply with the principles of administrative justice. In this paper, empirical data from 42 EIA cases in South Africa are used to establish EIA compliance with the administrative justice principles of lawfulness, procedural fairness and reasonableness. This is achieved by measuring EIA decisions against specially developed key performance indicators (KPIs). Overall, decisions were found to mostly comply with the principles of administrative justice. However, questions arise with regards to the quality and substance of the information feeding into the decision-making process and on which decisions are ultimately based. [ABSTRACT FROM AUTHOR]
