Introduction and research questions from the rich literature on happiness and subjective well-being we know that money does not buy happiness (Diener, 1999; headey, Muffels & Wooden, 2008). this is known as easterlin’s paradox, which posits that beyond a certain level of gross domestic product (gDp) per capita wealthier nations are not better off in terms of subjective well-being than less wealthy nations, whereas within countries absolute income pays off in terms of happiness though at a diminishing rate (easterlin, 1974, 2001, 2003). the easterlin paradox is explained by the mechanism of habituation or adaptation, according to which people are presumed to be in a sort of ‘hedonic treadmill’: due to rising aspirations, increases in wealth do not lead to steady increases in happiness. the paradox is challenged in a seminal paper by Stevenson and Wolfers (2008), who used the World values Survey, the …

Abstract

Biologicals have become an important component in the treatment of Crohn's disease in children. Their increased and long term use raises safety concerns. We describe safety and cost of infliximab in Belgian pediatric Crohn's disease patients. All patients on infliximab as part of the present or past treatment for Crohn's Disease until January 1st 2011 were selected from an existing database. Information on disease phenotype, medication and adverse events were extracted. Adverse events occurred in 25.9% of patients exposed to infliximab of which 29.6% were severe. In total 31.7% of patients stopped infliximab therapy. The main reasons for discontinuation were adverse events in 45.4% and loss of response in 30.3%. No malignancies or lethal complications occurred over this 241 patient year observation period. Immunomodulators were concomitant medication in 75% of patients and were discontinued subsequently in 38.4% of them. The cost of infliximab infusions per treated patient per year in the Belgian health care setting is approximately 9 474 euro, including only medication and hospital related costs. Even though infliximab is relatively safe in pediatric CD on the short term, close follow-up and an increased awareness of the possible adverse reactions is highly recommended. Adverse reactions appeared in 25.9% of all patients and were the main reason for discontinuation. Treatment cost has to be balanced against efficacy and modifications in disease course. In the Belgian health care system, the medication is available to all patients with moderate to severe CD.

Abstract

Pesten is een maatschappelijk probleem en komt in verschillende sectoren voor. Er is nog weinig onderzoek verricht naar pesten in de Buitenschoolse Opvang (BSO). In dit onderzoek zijn 25 pedagogisch medewerkers geïnterviewd om hun visie en aanpak op pesten te onderzoeken. De interviews zijn gebruikt voor een kwalitatieve data-analyse. De resultaten toonden aan dat pedagogisch medewerkers weinig tot geen pestgedrag op de BSO signaleerden. Of dit daadwerkelijk het geval is blijft onduidelijk en moet nader onderzocht worden. Mogelijk signaleerden of herkenden medewerkers pestgedrag onvoldoende. Zij gaven bijvoorbeeld geen eenduidige definitie van pesten. Het structurele beleid van organisaties omtrent pesten bleek onvolledig en kan aangescherpt worden. Aanbevelingen richten zich op het trainen van pedagogisch medewerkers in het signaleren van pestgedrag en op aandacht voor pesten in het curriculum van opleidingen. Daarnaast wordt een nauwere samenwerking tussen basisonderwijs, BSO en andere organisaties voorgestaan om pesten adequaat aan te pakken.

44. Fate of Carbohydrates and Lignin during Composting and Mycelium Growth of Agaricus bisporus on Wheat Straw Based Compost

Abstract

In wheat straw based composting, enabling growth of Agaricus bisporus mushrooms, it is unknown to which extent the carbohydrate-lignin matrix changes and how much is metabolized. In this paper we report yields and remaining structures of the major components. During the Phase II of composting 50% of both xylan and cellulose were metabolized by microbial activity, while lignin structures were unaltered. During A. bisporus’ mycelium growth (Phase III) carbohydrates were only slightly consumed and xylan was found to be partially degraded. At the same time, lignin was metabolized for 45% based on pyrolysis GC/MS. Remaining lignin was found to be modified by an increase in the ratio of syringyl (S) to guaiacyl (G) units from 0.5 to 0.7 during mycelium growth, while fewer decorations on the phenolic skeleton of both S and G units remained.

Abstract

Wireless video-endoscopy of the small bowel is a relatively new diagnostic tool with only limited paediatric experience. We report our observations in 2 children. The capsule was easily swallowed and passed uneventful through the digestive tract of a child with unexplained anaemia not older than 5 years, although the literature mentions 10 years as minimal age. In a second child, 8 years old, with recurrent episodes of acute abdominal pain, the capsule confirmed an ileo-ileal intussusception. The absence of any exposure to radiation and the non-invasive character of this technique render it of extreme interest for paediatric use. Unfortunately, the Belgian social security does not yet reimburse it. Further investigations will be needed to evaluate if video-endoscopy may replace contrast radiological examinations in children with unexplained small bowel disease.
