Abstract

Given the heterogeneity of Attention-Deficit Hyperactivity Disorder (ADHD), there is a constant need for proper diagnosis, employing more accurate, efficient, patient-friendly, and clinician-friendly tools. We developed a Greek version of the National Institute for Children's Health Quality (NICHQ) Vanderbilt ADHD Diagnostic Parent Rating Scale (VADPRS) for follow-up assessment and examined its reliability and validity. A total of 100 schoolchildren (65 girls and 35 boys) between 7 and 12 years old with ADHD were included in the study. The internal consistency and test-retest reliability were examined via principal component analysis. Three-component structure in our study accounts for 74.63% of the tool's variance. The questions' loadings onto the factors and the internal reliability of the three factors were satisfactory. Finally, the reliability of all factors presented was found adequately high, confirming the high internal consistency of the items. Our results suggest that the Greek version of the VADPRS-follow-up may be used as an accurate psychometric instrument for the purposes of diagnosing ADHD. Furthermore, the combined use of the VADPRS-follow-up with other widely accepted tools, such as the ADHD Rating Scale-IV, may present clinical utility in collating parent information concerning the behavioural phenotype of their children.

Abstract

Multiple Sclerosis Intimacy and Sexuality Questionnaire-19 (MSISQ-19) explores optimally impact of MS on sexual activity/satisfaction/intimacy. The present study aims to provide the only validation of the Greek Version of MSISQ-19, and compare results to validation studies in other languages. The original/English version of the MSISQ-19 was translated into Greek according to standardized guidelines, while validity/reliability, correlations with other scales and sexual dysfunction prevalence were tested. Subjects were requested to complete all questionnaires and MSISQ-19, being re-tested three weeks later. Construct-validity of the Greek version of the MSISQ-19 was confirmed with principal-component-analysis. Bartlett's test assessed correlation-adequacy between items. Pearson's correlation explored internal-construct-validity between subscales and overall score, and external–construct-validity with disease-status variables, cognitive testing and patient-reported outcomes regarding fatigue, depression/anxiety, MS impact, and quality of life. 201 PwMS (130 female). Mean age was 39.3 ± 11.8 years with median disease-duration 11.7 ± 7.9 years. 79.1% RRMS, PPMS (10.4%) and SPMS (10.4%). Cronbach's alpha coefficient was 0.949. MSISQ-19 correlations between items were large. Significant associations of sexual dysfunction were identified with age (rho = 0.392, p < 0.01), years of education (rho=–0.199, p = 0.006), the Expanded Disability Status Scale (rho = 0.518, p < 0.01) and MS duration (rho = 0.354, p < 0.01). Correlations were disclosed with the Brief International Cognitive Assessment for MS (rho=-0.247, p < 0.05), Modified Fatigue Impact Scale (rho = 0.374, p < 0.05), Depression Anxiety Stress Scale (rho = 0.375, p < 0.05), Multiple Sclerosis Impact Scale (rho = 0.442, p < 0.05), and EuroQoL-five-dimensional instrument (rho = 0.375, p < 0.05). Internal consistency of the Greek version of the MSISQ-19 was confirmed with Cronbach's alpha. Test-retest reliability (31 PwMS) was excellent with intraclass-correlation-coefficients > 0.90. Besides Greek MSISQ-19 satisfactory validity/reliability/reproducibility and being first to include cognitive-testing, authors estimated sexual-dysfunction prevalence affecting half PwMS. This study provides the only validation of the Greek Version of the MSISQ-19. The latter was found with satisfactory validity, reliability and reproducibility. 50% of the Greek PwMS sample was found to be afflicted with sexual dysfunction. 46. Pythagorean Self-Awareness Intervention for Multiple Sclerosis Patients: A Quasi-Experimental Pragmatic Trial

Abstract

Objective: Multiple sclerosis (MS) is an autoimmune disorder of the central nervous system affecting patients' well-being and quality of life. Pythagorean Self-Awareness Intervention (PSAI) is a novel non-pharmaceutical intervention with significant benefits both in MS and other chronic diseases. In this study, the longstanding effectiveness of PSAI was investigated. Method: This was a two-arm quasi-experimental pragmatic trial in relapsing-remitting MS patients (23 in the PSAI and 21 in the control group). PSAI patients received an 8-week training period and then they performed PSAI at home for another 16 weeks. Assessments took place at baseline, 8 weeks, and 24 weeks. These included cognition, fatigue, perceived stress, and hair cortisol. Results: Significant group x time interactions favoring PSAI were found during the first 8-week period for information processing speed, fatigue, and perceived stress. However, only verbal memory was found to be significantly improved in the PSAI group during the 24-week follow-up period. There were no significant group x time differences with respect to hair cortisol. No side effects were noted and compliance was excellent. Conclusions: PSAI was mostly effective during the first 8-week training period. Its benefits worn out during the non-training period, albeit we observed a delayed significant improvement of verbal memory. Our findings will help to further refine the technique, either by extending the training period and/or by including booster sessions, throughout the PSAI treatment. This study provided Class III evidence for PSAI.
