Background/aim: Difficult mask ventilation and difficult intubation are more common in obese patients. Ultrasound is a reliable and noninvasive method for evaluating the airway. The aim of this study was to investigate the contribution and availability of anterior neck soft tissue (ANS) thickness at different levels, tongue volume (TV), hyomental distance (HMD), the ratio of preepiglottic distance to distance between the epiglottis and the midpoint of vocal cords (PE/E-VC) measured by ultrasonography in predicting difficult airway in morbidly obese patients. Materials and methods: Between March 2020 and November 2020, patients aged =18 years with a body mass index (BMI) of =40 kg/m2 who underwent elective surgery under general anesthesia were included in this prospective study at Fırat University Hospital. During the preoperative evaluation of patients, ultrasound was used to measure and record TV, ANS thickness at different levels, HMD, and ratio of PE/E-VC. Patients with difficult intubation were identified using the Cormack-Lehane classification system. Patients whohad difficulties with balloon mask ventilation were recorded. Subsequently, the parameters of patients with easy and difficult intubation were compared. In addition, the parameters of patients with easy and difficult mask ventilation were also compared. Results: The preepiglottic ANS thickness at the level of the thyrohyoid membrane and the PE/E-VC value in obese patients with difficult intubation were significantly greater than in obese patients with easy intubation (p < 0.001). In addition, TV (p < 0.001), preepiglottic ANS thickness at the thyrohyoid membrane level (p < 0.001), ANS thickness at the thyroid isthmus level (p = 0.002), ANS-suprasternal notch thickness (p = 0.004), and PE/E-VC (p = 0.005) values were significantly greater in obese patients with difficult mask ventilation. Conclusion: Ultrasound may be a useful tool for predicting difficult airway and difficult mask ventilation. For this purpose, ANS thickness at different levels, PE/E-VC, and TV values measured by ultrasound can be used. [ABSTRACT FROM AUTHOR]

Objective: Shear-wave elastography (SWE) can be used to obtain quantitative information about tissue stiffness. This study compared SWE measurements of patients with adenomyosis, leiomyoma, and normal myometrium. Materials and Methods: This study included 56 women who were scheduled to undergo hysterectomy. Measurements of the uterine myometrium and leiomyomas, if any, of the patients were preoperatively collected through SWE. A total of 69 SWE measurements were included. Postoperatively, the patients were categorized according to the results of histopathologic examinations: group 1 included patients with normal myometrium; group 2 included those with adenomyosis; and group 3 included those with leiomyomas. The Emean values of the kPa obtained from the SWE measurements were compared with the histopathologic results. Results: The SWE measurements of groups 2 and 3 were significantly higher than those of group 1. There were no significant differences between groups 2 and 3 in mean SWE measurements. The success rate of SWE in differentiating normal myometrium and adenomyosis was 92%, with a sensitivity of 93.1% and a specificity of 82.1%. Conclusion: This study revealed that using ultrasound elastography to diagnose adenomyosis is successful in distinguishing that condition from normal myometrium, but SWE is not useful for distinguishing adenomyosis leiomyoma. Biz safra kesesi tümörünü taklit eden intramural safra kesesi hematomlu bir olguyu ve radyolojik bulgularını sunuyoruz., Intramural gallbladder hematoma is a rare cause of abdominal pain. Symptoms are generally non-specific. Therefore, imaging methods are important for diagnosis. But despite simultaneous radiological imagings, it may be difficult to diagnose a gallbladder hematoma. Because it has similar imaging findings with gallbladder neoplasm and tumefactive biliary sludge. We present a case with intramural gallbladder hematoma mimicking gallbladder tumor and its radiological findings.

Sonuç: Diffüzyon ağırlıklı görüntüleme, alkol tüketiminin neden olduğu karaciğer fibrozisinin saptanması ve takibi için kullanılabilecek noninvaziv bir görüntüleme tekniğidir., Background and Aims: This study aimed to investigate the relationship between liver apparent diffusion coefficient values and potential early-stage liver injury by comparing apparent diffusion coefficient values obtained by liver diffusion-weighted magnetic resonance imaging between individuals with a history of excessive alcohol consumption and healthy volunteers. Materials and Methods: This study enrolled 35 individuals with a history of excessive alcohol consumption and 32 healthy volunteers. In all cases, blood samples were taken to measure biochemical parameters and complete blood count. Upper abdominal magnetic resonance imaging including liver diffusion-weighted imaging was performed. Results: In total, 19 individuals with a history of excessive alcohol consumption and 20 healthy volunteers were included in the study. The mean apparent diffusion coefficient values obtained by high b value (b 1000) diffusion-weighted imaging in individuals with a history of excessive alcohol consumption were lower than those in healthy volunteers. Although no significant difference was observed between the two groups in terms of biochemical parameters, hepatic apparent diffusion coefficient values in both groups were positively correlated with international normalized ratio values and negatively correlated with gamma-glutamyl transferase and alkaline phosphatase levels (r = 0.002, r = −0.007, r = −0.049, respectively). Conclusion: Diffusion-weighted imaging is a noninvasive imaging technique that can be used for the detection and follow-up of liver fibrosis induced by alcohol consumption.

Introduction: Thyroid abscess is a serious head and neck infection that may easily be overlooked. İt is a rare disease with serious complications when the diagnosis is delayed. In this paper, we present a case of thyroid abscess who presented with fever, fatigue, neck pain and swelling, and rapidly deteriorated general condition. Case Report: A 28-year-old female patient had painful swelling and redness on the left side of the neck. She had fever, fatigue, and dysphagia. Laboratory tests revealed leukocytosis, elevated CRP, and sedimentation rate. The patient who was detected to have thyroid abscess on ultra-sonography and computed tomography was discharged with recovery through surgery and appropriate antibio-therapy. Conclusion: Thyroid gland abscess is a rare, serious clinical condition with high morbidity and mortality. Thyroid abscess should be kept in mind while performing radiological evaluation in cases presenting with swelling and redness in the neck. In these cases, quite good clinical outcomes may be achieved with early diagnosis and appropriate treatment. [ABSTRACT FROM AUTHOR]

Intraocular foreign body (IOFB) is a common association of penetrating ocular trauma. Early diagnosis and removal of IOFBs especially if they are metallic is very important to determine further management and the final result of treatment. Missed IOFB may present in different clinical aspects that may limit its detection and symptoms may only become apparent after a prolonged period of time. We report a case of a missed metallic intraocular foreign body in the anterior chamber over a 2-year period without causing severe inflammatory reaction and presented with uveitis later. A 42-year-old man presented with a progressive blurring of vision, pain, photophobia, and redness in the left eye for 3 months. He had a history of traffic accident 2 years ago and he was accepted to intensive care unit for 3 days. Three months ago, in another center, he was admitted to hospital for 1 week and intravitreal antibiotics and medical treatment were given for pain, photophobia, and redness in his left eye. In five o’ clock meridian of the angle, there was an IOFB coated with hypopyon was observed under biomicrocopic magnification. Plain X-ray and computed tomography confirmed the foreign body in the left eye. After obtaining informed consent from the patient, the foreign body was removed under local anesthesia.

ABSTRACTASSESMENT OF THE FOCAL LIVER LESIONS WITH TWO- DIMENSIONALAND THREE-DIMENSIONAL COLOR DOPPLER ULTRASONOGRAPHY,POWER DOPPLER ULTRASONOGRAPHY AND DYNAMIC FLOW IMAGINGIn this study, vascular pattern of focal liver lesions (FCL) were evaluated bytwo- dimensional (2D) and three-dimensional (3D) color Doppler ultrasonography(CDUS), power Doppler ultrasonography (PDUS) and Dynamic flow imaging (DFI).Between January 2004-January 2005, 33 female and 29 male patients (total62) patients between ages of 16-78 years (mean 58.2±14.1 years) were included in tothe study. 14 benign (22.6%) and 48 malignant (%77.6) lesions were diagnosed bypathologic examination or typical radiological findings. In benign lesions, 12 lesions(85.7%) were hemangiomas, 1 (7.15%) lipoma and 1 (7.15%) inflammatorypseudotumor. In malign lesions, 33 lesions (68.7%) were hepatocellular carcinomaand 15 (31.3%) metastases.B-mode, CDUS, PDUS and DFI examinations were performed by ToshibaSSA 770A/80 Aplio device (Tokyo-Japan) using 2-5 MHz multifrequency wide-bandconvex shaped transducer. All patients were examined with 2D and 3D CDUS,PDUS and DFI. The vascular pattern on Doppler sonography was classified intothree types: avascular (Type I), peripheral vascular (Type II), and intratumoralvascular (Type III).All kind of Doppler imaging study were made 51 of 62 patients. The samevascular pattern were found in 38 (74.5 %) of 51 lesions (n= 20 type II, n=17 typeIII, n=2 type I). Significant differences were found in between 2D CDUS and 3DPDUS (p=0.005), 2D PDUS and 3D PDUS (p=0.003), 2D DFI and 3D DFI(p=0,001).33D Doppler imaging is better than 2D Doppler techniques for detectingintratumoral flow. There was no significant difference in between 3D DFI and other3D Doppler modalities. But visually, 3D DFI more clearly depicted intratumoralvascularity than the other Doppler modalities.Key words: Three-Dimensional Doppler, Focal Liver Lesions, Power Doppler,Dynamic Flow.4 80

- Abstract
Purpose: To evaluate the role of diffusion-weighted magnetic resonance imaging (DWMRI) in differentiating benign and malignant thyroid nodules using a 3 Tesla (T) MRI scanner. Materials and Methods: Twenty-eight nodules in 25 patients and 14 healthy control cases were included in the study. DWMRI was acquired with 6 b values with a 3T MRI scanner. The apparent diffusion coefficient (ADC) values of the nodules were calculated from reconstructed ADC map images and were compared with the final histopathological diagnoses. Results: The mean ADC value of the benign nodules was 1548 ± 353.4 (×10
−6 mm2 /s), and the mean ADC of the malignant nodules was 814 ± 177.12 (×10−6 mm2 /s). The normal thyroid tissue had a mean ADC value of 1323.43 ± 210.35 × 10−6 mm2 /s (958-1689 × 10−6 mm2 /s) in the healthy control group. The ADC values were significantly different among the three groups ( P = 0.001). An ADC value of 905 × 10−6 mm2 /s was determined to be the cutoff value for differentiating benign and malignant nodules, with 90% (55.5-98.3) sensitivity and 100% (81.3-100.0) specificity. Conclusion: This study suggests that the ADC values of nodules measured with a 3T MRI scanner could help in differentiating benign thyroid nodules from malignant nodules. J. Magn. Reson. Anahtar Kelimler: alkol, karaciğer, diffüzyon ağırlıklı manyetik rezonans görüntüleme Alcohol consumption is known to be associated with many pathological conditions, especially in liver disease, which can result in death. Alcohol consumption can cause a wide spectrum of diseases as, alcoholic hepatitis, liver cirrhosis and hepatocellular cancer by inducing liver fibrosis. The aim of this study, investigate the relationship with liver ADC values and potential early stage liver injury by comparing the values of the ADC obtained by liver diffusion-weighted MR imaging with alcoholics and healthy volunteers. In our study, we accepted 30 individuals with a history of excessive alcohol consumption and 25 healthy volunteers. In all cases, blood sampled for biochemical parameters and complete blood count and liver diffusion-weighted MR imaging was performed. In our study, 19 individuals with a history of excessive alcohol consumption and 20 healthy volunteers were included. Although statistically not significant difference between two groups, mean ADC values obtained by high b value (b 1000) diffusion-weighted imaging in alcoholics were lower than the control group(p= 0,47). Although no significant difference was observed between the two groups in terms of biochemical parameters; liver ADC values in both groups, positively correlated with INR, negatively correlated with GGT and ALP values(r= 0,002, r= -0,007, r= -0,049 respectively). As a result; we think that diffusion-weighted magnetic resonans imaging is a noninvasive imaging technique that can be used for detection and follow-up of liver fibrosis inducing by alcohol consumption. Keywords: alcohol, liver, diffusion-weighted magnetic resonans imaging 59

- Abstract
Background/aim: Difficult mask ventilation and difficult intubation are more common in obese patients. Ultrasound is a reliable and noninvasive method for evaluating the airway. The aim of this study was to investigate the contribution and availability of anterior neck soft tissue (ANS) thickness at different levels, tongue volume (TV), hyomental distance (HMD), the ratio of preepiglottic distance to distance between the epiglottis and the midpoint of vocal cords (PE/E-VC) measured by ultrasonography in predicting difficult airway in morbidly obese patients., Materials and Methods: Between March 2020 and November 2020, patients aged ≥18 years with a body mass index (BMI) of ≥40 kg/m2 who underwent elective surgery under general anesthesia were included in this prospective study at Fırat University Hospital. In addition, the parameters of patients with easy and difficult mask ventilation were also compared., Results: The preepiglottic ANS thickness at the level of the thyrohyoid membrane and the PE/E-VC value in obese patients with difficult intubation were significantly greater than in obese patients with easy intubation (p < 0.001). In addition, TV (p < 0.001), preepiglottic ANS thickness at the thyrohyoid membrane level (p < 0.001), ANS thickness at the thyroid isthmus level (p = 0.002), ANS-suprasternal notch thickness (p = 0.004), and PE/E-VC (p = 0.005) values were significantly greater in obese patients with difficult mask ventilation., Conclusion: Ultrasound may be a useful tool for predicting difficult airway and difficult mask ventilation. For this purpose, ANS thickness at different levels, PE/E-VC, and TV values measured by ultrasound can be used., Competing Interests: Conflict of interest: The authors declare that they have no conflicts of interest., (© TÜBİTAK.)

- Abstract
Median arcuate ligament syndrome is caused by the compression of the celiac artery by a fibrous band called the median ligament, which originates from the crus of diaphragm. The prevalence of Median arcuate ligament syndrome has been reported as 10%-24% among patients; however, the etiology is unclear. The components of the clinical triad include a chronic post-prandial pain, epigastric murmur, and weight loss. Diagnosis is based on clinical and radiological findings. Median arcuate ligament syndrome has been reported in a small portion of patients undergoing pancreaticoduodenectomy. Most patients have been diagnosed prior to surgery.

- Abstract
Acrania is a developmental abnormality characterized by a partial or complete absence of calvaria with complete but abnormal development of brain tissue. Acrania is a relatively common malformation and affects about 1 in 1000 newborns. Meroacrania refers to absence of the cranium with the exception of the occipital bone. Brain stem and cerebellum develop normally, but cerebral parenchyma tissue is covered with a thin membrane and severely dysmorphic supratentorial brain is also seen. The other system findings are normal. Magnetic resonance imaging findings of one neonate with meroacrania have been reported in medical literature. Other radiographic and computed tomography findings have not yet been reported. We report a female neonate with meroacrania with discussion of etiology, pathogenesis, radiological findings, and differential diagnosis.

- Abstract
Purpose: To the best of our knowledge, there are only a few previous studies on the effects of overweight on extracranial carotid arteries. We aimed to investigate the effects of overweight on flow velocities, luminal diameters and intima media thicknesses (IMTs) of the common and internal carotid arteries (CCA, ICA)., Materials and Methods: A prospective study on color duplex sonography of the extracranial arteries was performed in 71 adults, ages between 25-58 years old. The body mass index (BMI) was found to be normal in 24 (group 1) and high in 47 (group 2) subjects of the study group. Flow velocity (FV), luminal diameter and IMT of carotid arteries of all the participants were measured., Results: There were significant differences between group 1 and group 2 in the following parameters; luminal diameters of the right CCAs were significantly larger in group 2, peak-systolic FV of ICAs were significantly lower in group 2 than in group 1 and ICA end-diastolic FVs were significantly lower in group 2 than in group 1., Conclusion: The present data shows that there is a correlation among carotid artery luminal diameter, FVs and overweight. In the overweight subjects, the increased luminal diameter and decreased FVs can point at the early stage of atherosclerosis. The influence of adiposity on atherosclerosis is very complex and varies with gender and age, therefore we need larger series and further investigation.
