Bu çalışma elit düzey dağcılarda yeme bozukluğu ile yeme farkındalığının saptanması ve sedanter bireylerle karşılaştırılması amacıyla Haziran 2021- Şubat 2022 tarihleri arasında 30 dağcı (%65),2), 16 sedanterin (%34,8) katılımıyla Bursa ve İstanbul'da yapılan bir çalışmadır. Veriler e-posta yoluyla katılımcılara iletilmiş; anket formu uygulanmıştır. Katılımcıların sosyo-demografik özelliklerini belirlemek için yaş, cinsiyet, vücut ağırlığı, fiziksel aktivite bilgileri istenilmiştir. Anket formu 3 bölümden oluşmaktadır; ilk bölüm "Münih Yeme Bozuklukları" soruları, ikinci bölümünde "Yeme Farkındalığı Ölçeği (YFÖ)" ve son bölümünde ise " 3 Günlük Besin Tüketim Kaydı (BTK)" yer almıştır. Katılımcılara uygulanan anketler ve besin tüketim kaydından elde edilen veriler SPSS istatistik programında Mann Whitney-U Test, Pearson Korelasyon ve Ki- Kare testi yapılarak değerlendirilmiştir. Dağcılar ve sedanterler Münih Yeme Bozuklukları Ölçek ve alt boyutları ( görünüş ve ağırlık ile meşguliyet, tıkınma ve kusma, uygun olmayan telafi edici davranış) bakımından karşılaştırıldığında aralarında anlamlı farklılığa saptanmamıştır. Yeme Farkındalığı Ölçek ve alt boyutları (düşünmeden yeme, duygusal yeme, yeme kontrolü, farkındalık, yeme disiplini, bilinçli beslenme, enterferans) bakımından karşılaştırıldığında aralarında anlamlı fark bulunmamıştır. Dağcılar ve sedanterler arasında beden kitle indeksi bakımından fark bulunmamıştır (p=0,351). Dağcılık, dağlık bölgelerde yapılan buz, kaya tırmanışını, kamp ve dağ yürüyüşünü kapsayan; dağcılık malzeme ve tekniklerini kullanarak yapılan spordur. İçerisinde mücadele, risk, zorluk ve dayanıklılık barındıran bu spor dalında beslenme; hayatta kalma, konsantrasyon, sportif performans, motivasyon açısından büyük öneme sahiptir. Bu açıdan incelendiğinde dağcıların daha yüksek beslenme farkındalığı ve daha az yeme bozukluğu riskine sahip olması beklenirken sedanterler ile aralarında fark saptanmamıştır. Ayrıca elit dağcıların yeme farkındalığı ve bozukluğuna sahip olup olmadıkları konusunda herhangi bir çalışmaya rastlanmamıştır. Literatürdeki sporcularda yeme farkındalığı ve yeme bozukluğu çalışmaları konunun anlaşılması bakımından yetersiz kalmaktadır. Bu nedenle bu çalışmanın amacı elit dağcıların yeme farkındalığı düzeylerini ve yeme bozuklukları sıklıklarını araştırmaktır. This study was conducted in Bursa and Istanbul with the participation of 30 mountaineers (65.2%) and 16 sedentary (34.8%) between June 2021 and February 2022, to determine eating disorders and awareness of eating in elite mountaineers and to compare them with sedentary individuals. The data was communicated to the participants via e-mail; a questionnaire form was applied. To determine the sociodemographic characteristics of the participants, age, gender, body weight, and physical activity information we are requested. The study is based on 3 basic. The first part the Munich Eating Disorders questionnaire, the second part the Eating Awareness Scale, and the last part the 3-Day Food Consumption Record. The data obtained from the questionnaires applied to the participants and the food consumption record were evaluated by using the Mann-Whitney U Test, Pearson Correlation, and Chi-Square test in the SPSS statistical program. When mountaineers and sedentary were compared in terms of the Munich Eating Disorders Scale and sub-dimensions (preoccupation with appearance and weight, binge and vomiting, in appropriate compensatory behavior), no significant difference was found between them. When the Eating Awareness Scale and its sub-dimensions (eat without thinking, emotional eating, eating control, awareness, eating discipline, conscious eating, interference) were compared, no significant difference was found between them. There was no difference in body mass index between mountaineers and sedentary persons (p=0.351). Mountaineering includes ice climbing, rock climbing, camping and hiking in mountainous regions; ıt is a sport made using mountaineering materials and techniques. Nutrition in this sport, includes struggle, risk, difficulty, and endurance; It has great importance in terms of survival, concentration, sportive performance, and motivation. From this point of view, while mountaineers are expected to have higher nutritional awareness and less risk of eating disorders, no difference was found between them and sedentary. In addition, no study has been found on whether elite mountaineers have eating awareness and disorder. Eating awareness and eating disorder studies in athletes in the literature are in sufficient in terms of understanding the subject. Therefore, this study aims to investigate the eating awareness levels and the frequency of eating disorders in elite mountaineers.

Devletler, yirminci yüzyıl itibari ile hızla gelişen uluslararası ticaretin beraberinde getirdiği çifte vergilendirme sorununun önüne geçebilmek için çifte vergilendirmeyi önleme anlaşmaları yapmaktadırlar. Bazı uluslararası örgütler, bu türden anlaşmaların yeknesaklaştırılması amacı ile çalışmalar yapmakta olup bu çalışmaların en yaygın kullanılanı OECD Model Anlaşma metnidir. Ne var ki, çifte vergilendirmeyi önleme anlaşmalarına esas alınan Model Anlaşmanın uygulanması ve yorumlanması her zaman sorunsuz gerçekleşmemektedir. Bu türden sorunların önüne geçmek amacıyla da Model Anlaşmada Karşılıklı Anlaşma Usulü öngörülmüştür. Karşılıklı Anlaşma Usulünün barındırdığı eksikliklerin giderilmesi ve bu türden ihtilaflarda nihai ve bağlayıcı çözümler geliştirilmesi amacı ile tahkim kurumu 2008 yılı itibari ile OECD Model Anlaşmasında yerini almıştır. Ancak Türkiye'nin de içerisinde bulunduğu birçok devlet egemenlik hassasiyetlerinden dolayı tahkim maddesine akdettikleri çifte vergilendirmenin önlenmesi anlaşmalarda yer vermemektedirler. Bu tezde, OECD Model Anlaşmasında yer alan tahkim önerisinin söz konusu egemenlik kaygıları ve Türk Hukuku açısından bir sakınca doğurmadığı öne sürülmektedir. States has been concluding double taxation agreements in order to avoid double taxation problem brought about by international trade that has been progressing rapidly since the beginning of the 20th century. A number of international organizations have been carrying out endeavors to uniform these agreements, the most-commonly-applied of which has been the OECD Modal Tax Convention. Nonetheless, the implementation and interpretation of the Modal Tax Convention providing a base for the double taxation agreements is not always free of problems. For preventing such problems, the Modal Tax Convention foresees Mutual Agreement Procedure. Also, arbitration took place in the Modal Tax Convention as of the year 2008 in order to provide for final and binding settlement for such differences and eliminate the deficits which the Mutual Agreement Procedure has. However, many states, including Turkey, do not include arbitration in the agreements they conclude because of their sovereignty-related concerns. In this thesis, it is set forth that the OECD's arbitration proposal in the Modal Tax Convention is not in contradiction with the sovereignty and law of Turkey. 205

Idiopatik trombositopenik purpura (ITP) tedavisinde kul- lanılmakta olan eltrombopag gibi trombopoetin reseptör (TpoR) agonistlerinin iskemik serebrovasküler hastalık oluşturma riski nadir görülmektedir. 5 yıldır ITP tanısı ile takip edilen sağ hemiparezi ve afazi kliniği ile acil servi- se başvuran hastaya yapılan tetkikler sonucunda iskemik serebrovasküler hastalık tanısı konuldu. Olguda serebro- vasküler hastalığın etyolojisi eltrombopag ile ilişkilendiril- di. Bu olgunun nadir görülmesi nedeniyle literatür eşliğin- de tartışıldı. Thrombopoetin receptor agonists like eltrombopag, used for Idiopathic Thrombocytopenic Purpura treatment rarely cause to the ischemic cerebrovascular disease. In this re- port, a patient, have been followed by ITP diagnosis for 5 years, admitted to the emergency service for right hemi- paresis and aphasia. It was diagnosed as ischemic cere- brovascular disease. Etiology of patients cerebrovascu- lar event was associated with eltrombopag. Because of this rare seen situation, it was argued by in the light of the literature.

The Mediterranean fever gene (MEFV) encodes the pyrin protein. The major role of pyrin appears to be in the regulation of inflammation activity and the processing of the cytokine pro-interleukin-1β, and this cytokine plays a part in migraine pathogenesis. This study included 220 migraine patients and 228 healthy controls. Eight common missense mutations of the MEFV gene, known as M694V, M694I, M680I, V726A, R761H, K695R, P369S, and E148Q, were genotyped using real-time polymerase chain reaction with 5' nuclease assays, which include sequence specific primers, and probes with a reporter dye. When mutations were evaluated separately among the patient and control groups, only the heterozygote E148Q carrier was found to be significantly higher in the control group than in the patient group (P=0.029, odds ratio [95% confidence interval] =0.45 [0.21–0.94]). In addition, the frequency of the homozygote and the compound heterozygote genotype carrier was found to be significantly higher in patients (n=8, 3.6%) than in the control group (n=1, 0.4%) (P=0.016, odds ratio [95% confidence interval] =8.57 [1.06–69.07]). However, human exposure to the pesticide leads to neurotoxicity. Glutamine is one of the principal, free intracellular amino acids and may also be an antioxidant. This study was undertaken in order to examine the neuroprotective and antioxidant potential of L-glutamine against DLM toxicity in female Wistar albino rats. Materials and methods: The rats were divided into the following groups (n=10): Group I: control (distilled water; 10 mL/kg, po one dose), Group II: L-glutamine (1.5 g/kg, po one dose), Group III: DLM (35 mg/kg, po one dose), and Group IV: DLM (35 mg/kg, po one dose) and L-glutamine (1.5 g/kg, po one dose after 4 hours). Total oxidant status (TOS), total antioxidant status (TAS), tumor necrosis factor-α, interleukin (IL)-1β, and IL-6 levels and apoptosis were evaluated in brain tissue. Results: DLM-treated animals had a significant increase in brain biochemical parameters, as well as TOS and TAS. Furthermore, the histopathological examination showed neuronal cell degeneration in the cerebral tissue. L-Glutamine treatment decreased the elevated brain levels of TOS and neuronal cell degeneration. There was no difference in tumor necrosis factor-α, IL-1β, and IL-6 levels between the groups. Objective: The aim of this study was to determine the effect of different palatal vault shapes on the dimensional stability of a glass fiber reinforced heat polymerized acrylic resin denture base material. Methods: Three edentulous maxilla with shallow, deep and medium shaped palatal vaults were selected and elastomeric impressions were obtained. A maxillary cast with four reference points (A, B, C, and D) was prepared to serve as control. Point (A) was marked in the anterior midline of the edentulous ridge in the incisive papillary region, points (B) and (C) were marked in the right and left posterior midlines of the edentulous ridge in the second molar regions, and point (D) was marked in the posterior palatal midline near the fovea palatina media (Figure 2). To determine linear dimensional changes, distances between four reference points (A-B, A-C, A-D and B-C) were initially measured with a metal gauge accurate within 0.1 mm under a binocular stereo light microscope and data (mm) were recorded. Results:No significant difference of interfacial distance was found in sagittal and frontal sections measured 24 h after polymerization and after 30 days of water storage in any of experimental groups (P>.05). Epilepsi dünyada inmeden sonra ikinci en yaygın nörolojik hastalıktır. Sağlık ve takip sistemleri gelişmiş ülkelerde dahi epileptik hastaların % 20-25'inde uygun ve güncel tedavi yaklaşımlarına rağmen nöbetler kontrol edilemez. Bu hastalarda genelde ise tüm epileptik hastalarda nöbetin önceden klinik ve/veya elektrofizyolojik ipuçları ile tahmin edilmesi düşüncesi heyecan vericidir ve bununla ilgili araştırmalar birkaç on yıldır artarak devam etmektedir. Temel amaç bir erken uyarı yöntemi ile gelecek nöbetin engellenmesidir. Nöbetin hafif geçirilmesi veya istenilen ortamda geçirilmesi de tedavi planlamasında önemlidir.Bizde beyin dalgaları üzerinden nöbet tahmininde önemli olduğunu düşündüğümüz EEG'deki diken dalgaların elektriksel niceliğini ve nasıl jeneralize olduğunu, elektriksel olarak jeneralizasyon öncesi özel bir örüntü ortaya koyup koymadıklarını inceledik. Bu amaçla değişik elektriksel algoritmalar uyguladık.Çalışmamızın sonucunda;Öncelikle içerisinde dikenlerin olduğu EEG işaretinin zaman-frekans karakteristiğini ortaya koyduk ve nöbet anına yaklaştıkça işaretin içerdiği enerji miktarının yavaş yavaş düşüş gösterdiğini, bu düşüşün özellikle belirli kayıt bölgelerinde daha fazla olduğunu saptadık.Kayıt bölgeleri arasında zamansal ve uzaysal faz ve enerji sekronizasyon endeksinin de düştüğünü gözledik. Kayıt bölgeleri arasındaki EEG işaretinin faz ve enerji senkronizasyondaki düşme eğilimlerinin, nöbete yol açmayan dikenlerin kısmi veya tam jeneralize olmaları halinden önce ortaya çıktığını tespit ettik.Ancak diken jeneralizasyonundan önce, 0.2 ve altına düşen senkronizasyon endeksi değerinin nöbete yol açmayan diken jeneralizisyonundan hemen sonra eski değerine, 0.4-0.5 civarına tekrar çıktığını saptadık.Epileptik EEG'de görülen üstte anlatılan karakteristik yapıya epileptik olmayan bir EEG'de rastlamadık. Following the stroke, epilepsy is the second wide-ranging neurological disease in the world. In spide of the valuable and updated treatment approachs, even in the countries in those the health care and patient management systems are well developed, the seizures are not controlled in 20-35% of patients. The concept of estimation of seizure before occurrence through clinical and/or electrophysiological signs manifasted in such patients is challenging and in the last few decades the research in this direction is increasingly continuing. Herein, the main purpose is to prevent seizures by an early alarming system. However a safe seizure or a seizure at a desired place is important in the planning of the treatment.Using the electromagnetic brain waves that we have considered as important in seizure prediction, a variety of signal processing algorithms were conducted on the EEG for investigation of the electrical properties of spikes emerged in the EEG in terms of how they do generalize, either there is an marker or particular pattern before their generalization.In this study, firstly the time-frequency characteristic of EEG and the spikes were represented. It was found that as the seizure time approaches the energy content of the electrical signal is gradually reduces until the occurrence of seizure. This reduction was much evident in the particular sites that were considered as epileptic focus.The temporal and spatial synchronization between the recording sites were quantified in terms of signal?s phase and energy content. A nonuniform reduction in the phase and energy synchronisation index was observed before the partial or absolute spike generalization those were not leading to seizure. It was found that the energy synchronisation index is decay to below 0.2 just before the emergence of spike generalization and after the generalization this index was raises back to its normal value; around 0.3-0.4. Interestingly these characteristics found in the epileptic EEG was not encountered in the nonepileptic EEG.From an alternative point of wiev, if the whole brain medium considered as an electromagnetic system, one may consider that minor and/or major ubnormal structures within the brain might have affected the funcion of the system and this had reflected into the EEG records. The spikes found in the EEG, in this case, may be considered as the reflections of the controlling mechanizm (in the form of exitation or inhibition) for preventig the brain from unexpected circumstances. In other words, in case of considering the brain is an electromagnetic system each time of the reduction observed in the energy index we thought that the structures that provides energy for brain functioning are controlled somehow with the involvement of the controlling mechanisms. If this is the case, we wonder if the the required energy sources for sustaining brain functioning in a reliable way dos not function well or the need for energy is extremely increased? is the energy provided to the system is a rapid correcting action? Therefore we have considered the spikes are the sign of the struggle of brain for maintaining its function reflected into EEG.If the occurance of spikes could be considered as a result of such a process, a supeculation for the occurrence of seizure may be made: The seizure is the reflection of a secondary controlling mechanism in case of the failure or insufficiency of the primary controlling mechanism.The results found in this study implied that, the markers attached to spikes need to be considered in the seizure prediction algorithms based on EEG. 81

Objectives: Long-term video-electroencephalogram (EEG) monitoring (VEM) is a diagnostic system used for many purposes, including the precise categorization of epileptic seizures, excluding non-epileptic seizures, and finding the seizure onset zone. The aim of this study was to demonstrate the importance of the use of VEM in the diagnosis and differential diagnosis of epilepsy. Methods: Data of patients who were hospitalized in the video-EEG unit of Dicle University Neurology Department between 2012 and 2016 were retrospectively evaluated. The records of 245 patients that were of at least 24-hours duration were included in the study. Results: The mean duration of recording was 3.3±1.3 days. Clinically observed seizures were detected in 37.5% (n=92) of the patients. Of those, 21.2% (n=52) were evaluated as epileptic seizures and 16.3% (n=40) were defined as non-epileptic seizures. The proportion of psychogenic non-epileptic seizures was 14% (n=36). The mean length of the recording of the first seizure attack was 1.6 days. Interictal EEG abnormalities were found in 13.4% (n=33) of the patients. The mean duration of the disorder was 7.3 years. Conclusion: Medical history, physical examination, and routine EEG procedures can be misleading factors in the diagnosis of epilepsy. VEM is a crucial technique to differentiate diagnoses in patients with treatment-resistant epilepsy and to precisely diagnose the seizure type and the epileptic syndrome. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: Pseudotumor cerebri syndrome (PTCS) is characterized by symptoms and signs of increased intracranial pressure without ventriculomegaly, intracranial tumor or mass. This study aimed to explore and analyze 34 patients with PTCS according to age, sex, symptoms of the disorder, cranial magnetic resonance images findings, etiology, and treatment. Materials and Methods: A total of 34 patients who were diagnosed as having PTCS and followed up between January 2011 and August 2016 by Dicle University Medical School Neurology Department were included in this study. PTCS was diagnosed in accordance with the modified Dandy criteria. Results: Thirty -four patients were identified as having PTCS. Twenty-one (91.2%) had headache, 19 (55.9%) had blurred vision, 6 (17.6%) had diplopia, 2 (5.9%) had vertigo, 1 (2.9%) had tinnitus, and 1 (2.9%) had numbness of the face. Twenty-seven patients were diagnosed as having idiopathic intracranial hypertension, 21 (61.8%) had no etiologic factors. Six (17.6%) patients were obese, one of whom had recently gained weight and another had polycystic ovary syndrome. Seven patients were thought to have secondary PTCS with the following etiologic factors: 2 (5.9%) patients had Hashimoto's thyroiditis, 1 (2.9%) had a history of all-trans retinoic acid intake due to a malignancy, 1 (2.9%) had choroid plexus granuloma, 2 (5.9%) had sinus venous thrombosis, and 1 (2.9%) had Familial Meditteranian Fever. Conclusion: Although PTCS was described many years ago, its physiopathology and exact treatment procedures are not clearly understood. The most important target of its treatment is to prevent loss of vision and improve symptoms. With a better understanding of its pathophysiology, effective treatment protocols will be developed. The aim of this article is to describe the relationship between consciousness and epilepsy. Epilepsy is a neurological disorder which can be seen all over the world. It can be diagnosed by the brain's electrical activity (EEG). The determination of epileptic attacks by EEG signals is quite common in both clinical and research fields. During epileptic seizures, the brain dynamics that make up the graph consist of abnormalities in EEG signals. Purpose:The aim of this study was to investigate the role ofEcballium elaterium(EE) on sepsis-induced lung injury.Materials and Methods:A total of 30 male rats were divided into three groups as follows: control, sepsis, and treatment groups (sepsis + EE) with each group containing 10 rats. A rat model of sepsis induced by cecal ligation and puncture (CLP) was used. In the treatment group, rats were injected intraperitoneally with 2.5 mg/kg EE after CLP. Interleukin-6 (IL-6), tumor necrosis factor (TNF)-α, total antioxidant status (TAS), total oxidant status (TOS), and oxidative stress index (OSI) values after a 24-hr period were measured via cardiac puncture. Animals were harvested after the procedure and biochemical analysis was done and histopathological changes of the tissue sections of lungs were examined thereafter.Results:A statistically significant decrease was observed in the IL-6 (p< .05), TNF-α (p< .05), and TOS (p< .01) levels in the sera of the treatment group compared to those of the sepsis group. Following the treatment, the TOS (p= .01) and OSI (p< .05) levels in the lung tissue of rats indicated a statistically significant decrease compared to those of the sepsis group. The histopathological follow-up undertaken after the administration of the EE treatment to septic rats showed significantly lower values of alveolar wall thickness (p< .001), interstitial edema (p= .018), and neutrophil infiltration (p= .047).Conclusion:EE treatment may have beneficial effects on sepsis-induced lung injury, and therefore has potential for clinical use. Migraine pathogenesis involves a complex interaction between hormones, neurotransmitters, and inflammatory pathways, which also influence the migraine phenotype. The Mediterranean fever gene (MEFV) encodes the pyrin protein. The major role of pyrin appears to be in the regulation of inflammation activity and the processing of the cytokine pro-interleukin-1β, and this cytokine plays a part in migraine pathogenesis. This study included 220 migraine patients and 228 healthy controls. Eight common missense mutations of the MEFV gene, known as M694V, M694I, M680I, V726A, R761H, K695R, P369S, and E148Q, were genotyped using real-time polymerase chain reaction with 5' nuclease assays, which include sequence specific primers, and probes with a reporter dye. When mutations were evaluated separately among the patient and control groups, only the heterozygote E148Q carrier was found to be significantly higher in the control group than in the patient group (P=0.029, odds ratio [95% confidence interval] =0.45 [0.21-0.94]). In addition, the frequency of the homozygote and the compound heterozygote genotype carrier was found to be significantly higher in patients (n=8, 3.6%) than in the control group (n=1, 0.4%) (P=0.016, odds ratio [95% confidence interval] =8.57 [1.06-69.07]). However, there was no statistically significant difference in the allele frequencies of MEFV mutations between the patients and the healthy control group (P=0.964). In conclusion, the results of the present study suggest that biallelic mutations in the MEFV gene could be associated with a risk of migraine in the Turkish population. Moreover, MEFV mutations could be related to increased frequency and short durations of migraine attacks (P=0.043 and P=0.021, respectively). Future studies in larger groups and expression analysis of MEFV are required to clarify the role of the MEFV gene in migraine susceptibility. [ABSTRACT FROM AUTHOR]

Background: Deltamethrin (DLM) is a broad-spectrum synthetic dibromo-pyrethroid pesticide that is widely used for agricultural and veterinary purposes. However, human exposure to the pesticide leads to neurotoxicity. Glutamine is one of the principal, free intracellular amino acids and may also be an antioxidant. This study was undertaken in order to examine the neuroprotective and antioxidant potential of L-glutamine against DLM toxicity in female Wistar albino rats. Materials and methods: The rats were divided into the following groups (n=10): Group I: control (distilled water; 10 mL/kg, po one dose), Group II: L-glutamine (1.5 g/kg, po one dose), Group III: DLM (35 mg/kg, po one dose), and Group IV: DLM (35 mg/kg, po one dose) and L-glutamine (1.5 g/kg, po one dose after 4 hours). Total oxidant status (TOS), total antioxidant status (TAS), tumor necrosis factor-α, interleukin (IL)-1β, and IL-6 levels and apoptosis were evaluated in brain tissue. Results: DLM-treated animals had a significant increase in brain biochemical parameters, as well as TOS and TAS. Furthermore, the histopathological examination showed neuronal cell degeneration in the cerebral tissue. L-Glutamine treatment decreased the elevated brain levels of TOS and neuronal cell degeneration. There was no difference in tumor necrosis factor-α, IL-1β, and IL-6 levels between the groups. Conclusion: L-Glutamine may reduce the toxic effects of DLM in the cerebral tissue through antioxidant properties. [ABSTRACT FROM AUTHOR]

In this study, it is aimed to show that almost all of the institutions of the service sector and especially the banking sector examined in this study are directly related to the innovative works in order to make a difference in the competition and to be permanent with the advancement of technology and changing customer expectations and needs. In this study, a research based on case study was carried out. Detailed analysis was conducted on the 4 banks identified in the banking sector. As a result of the evaluation, it was concluded that it is not possible for a non-innovative institution operating in the service sector to be able to maintain and be structured.
