Methods: This was a multicenter, observational, retrospective study in patients aged $18 years who switched from intravenous (IV) epoetin alfa to IV darbepoetin alfa in October 2007 (Month 0) and continued on hemodialysis for at least 24 months. The dose was adjusted to maintain Hb within 1.0 g/dL of baseline. Results: We included 125 patients (59.7% male, mean [standard deviation (SD)] age 70.4 [13.4] years). No significant changes were observed in Hb levels (mean [SD] 11.9 [1.3] g/dL, 12.0 [1.5], 12.0 [1.5], and 12.0 [1.7] at Months −12, 0, 12 and 24, respectively, P=0.409). After conversion, the erythropoiesis-stimulating agent (ESA) dose decreased significantly (P200 IU/kg/week) doses of epoetin and 291:1 in patients requiring low doses. Conclusion: In patients on hemodialysis receiving ESAs, conversion from epoetin alfa to darbepoetin alfa was associated with an approximate and persistent reduction of 65% of the required dose. To maintain Hb stability, a conversion rate of 300:1 seems to be appropriate for most patients receiving low doses of epoetin alfa (≤200 IU/kg/week), while 350:1 would be better for patients receiving higher doses. 2. On the flow separation mechanism in the inverse Leidenfrost regime

Abstract

The inverse Leidenfrost regime occurs when a heated object in relative motion with a liquid is surrounded by a stable vapour layer, drastically reducing the hydrodynamic drag at large Reynolds numbers due to a delayed separation of the flow. To elucidate the physical mechanisms that control separation, here we report a numerical study of the boundary-layer equations describing the liquid-vapour flow around a solid sphere whose surface temperature is above the Leidenfrost point. Our analysis reveals that the dynamics of the thin layer of vaporised liquid controls the downstream evolution of the flow, which cannot be properly described substituting the vapour layer by an effective slip length. In particular, the dominant mechanism responsible for the separation of the flow is the onset of vapour recirculation caused by the adverse pressure gradient in the rearward half of the sphere, leading to an explosive growth of the vapour-layer thickness due to the accumulation of vapour mass. Buoyancy forces are shown to have an important effect on the onset of recirculation, and thus on the separation angle. Abstract

We provide external morphological observations of the American genus Pseudophilothrips, which includes two major thrip pests of avocados in Mexico and Central America. The evaluation was based on two taxonomic definitions of the genus that are typically used to identify the thrips specimens using light microscopy. A new definition of the genus Pseudophilothrips is presented with a key to the species. We propose synonyms for two species of Pseudophilothrips that were formerly in the genus Liothrips, with new observations of the arrangements of setae on the head and thorax using scanning electron microscopy. This analysis is intended as a starting point for future taxonomic and systematic studies of Liothrips lineages. [ABSTRACT FROM AUTHOR]

Abstract

The eruption of the Cumbre Vieja volcano started on September 19, 2021, on the island of La Palma, following a seismic swarm that affected the southern half of the island. The lava flowed westward and reached the coastline 8 days later in the area between Punta de la Bombilla and Tazacorte harbour. Four oceanographic cruises on board the research vessels Ramón Margalef and Ángeles Alvariño (Spanish Institute of Oceanography, IEO-CSIC) aimed to (i) characterizing the petrologic features prior to the arrival of the lava flows into the coastline, (ii) assessing the variations of the water-magma system when the lava flowed into the sea, and (iii) evaluating the main changes in the system after the end of the eruption. High-resolution bathymetric maps obtained with the multibeam echosounder EM710, combined with submarine high- definition ROV LIROPUS images and rocks samples collected from the sea bottom, showed that lava flowed down the shelf and upper slope as block (aa) and pillow lava flows, being conditioned by small gullies on the upper slope. Significant morphological changes have been defined in the subaerial northern and southern lava deltas (areas of 5,4 and 43 ha, respectively), and on a submarine area of ~30 ha. Volcanic material has been identified as far as 1.2 km from the original coastline at ~305 m seawater depth. Preliminary petrographic studies determine that the began as tephrites and later evolved intro basanites with olivine, pyroxene, amphibole and plagioclase phenocrysts within a glassy matrix with variable number of vesicles. The total volume of lava reaching the submarine environment is estimated of around 3 Mm3 with maximum thickness ca. 40 m along previous submarine slope gullies. Integrated with multidisciplinary analyses of the water column and ecosystem changes, this study will improve the knowledge of the magmatic evolution from depth up to eruption and will be useful for the risk management of future eruptive events.

Abstract

Tagoro is one of the few worldwide submarine volcanoes that satisfies the following features: i) is an intraplate and shallow- water volcano; ii) comprises diffuse low-temperature hydrothermal fields; and iii) its physical-chemical and biological characteristics that have been monitored since its first eruption in 2011. Despite a decade of widespread investigation, there is still a gap of knowledge on Tagoro volcano hydrothermal system. Hence, this preliminary research aims to report a precise estimation on the amount and distribution of hydrothermal vents, as well as an estimative quantification of the heat and nutrient fluxes associated therein. The hydrothermal vents distribution was determined through video imagery recorded by the Remote Operated Vehicle (ROV) Liropus-2000 during four oceanographic expeditions in 2014, 2017, 2018 and 2021 on board the R/V Ángeles Alvariño. Moreover, fluid flow velocities were estimated using a previously proposed sampling technique involving a custom-built particle-tracker device, designed to be manipulated by ROV Liropus-2000 on the hydrothermal areas. This device was deployed during oceanographic cruises in 2021 and 2022 on board the R/V Ángeles Alvariño. Lastly, hydrothermal fluids were sampled directly from vents using a piston-driven suction device coupled to the ROV Liropus-2000 during the 2022 cruise. Preliminary results highlight the remarkably vast richness and complexity of the Tagoro hydrothermal system, which is generally distributed in the main crater and main cone. The system is composed of various morphologies including diffuse- discharge fields, crevices and very fragile chimneys. Additionally, our results show important heat outflows into the ocean, ranging from 200 to 580 MW. This comprehensive study contributes to the broadening knowledge of the magnitude of hydrothermal activity at Tagoro submarine volcano and builds a reference for further studies.

Abstract

On September 2021 a fissure eruption occurred and the Cumbre Vieja volcano situated on the Canary Island of La Palma, resulting in the expulsion of a substantial amount of lava that flowed westward reaching the coastline 8 days later. Our data indicate that lava flowed down the shelf and upper slope as block (aa) and pillow lava flows, channeled by small gullies on the upper slope. Significant morphological changes have been detected in the subaerial northern and southern lava deltas (areas of 43 and 5.4 ha, respectively), and on a submarine area of ~30 ha. New volcanic materials halve been identified as far as 1.2 km from the original coastline at ~305 mbsl.

Abstract

Objetivo. Este artículo tiene como propósito analizar la relación existente entre los estilos de aprendizaje, el género y el rendimiento académico del estudiantado del curso Fundamentos de Economía en la Universidad Tecnológica de Bolívar (UTB). Metodología. El enfoque del estudio es cuantitativo, con un tipo de investigación descriptiva-correlacional. Se aplicó el cuestionario Honey-Alonso de estilos de aprendizaje (CHAEA) a un grupo de 166 estudiantes. Los datos se procesaron con el paquete estadístico Stata versión 13.0 para Windows. Para establecer las correlaciones y la probabilidad se diseñaron y ejecutaron dos modelos: uno de tipo lineal de probabilidad y un modelo probabilístico de tipo probit. Resultados. Se evidencia significancia estadística al 10% respecto a las preferencias de los estilos activo y reflexivo. De igual modo, se observa una asociación entre la preferencia por el estilo reflexivo y el aumento de las posibilidades de aprobar el curso por el estudiantado. Por el contrario, la probabilidad de aprobar el curso es menor cuando la preferencia es por el estilo activo. Conclusión. El estudio permitió evidenciar la relación entre el género y el estilo de aprendizaje pragmático; también se observó que el estudiantado con preferencias por el estilo reflexivo tiene una mayor probabilidad de mejorar el rendimiento académico en el curso de Fundamentos de Economía. Además, quienes tienen preferencia por el estilo activo, su probabilidad de mejorar el rendimiento académico es menor. Objective. This article aimed to analyze the relationship between learning styles, gender, and students’ academic performance in the Fundamentals of Economics course at the Universidad Tecnológica de Bolívar (UTB). Methodology. The study used a quantitative approach with a descriptive-correlational research design. It administered the Honey-Alonso Learning Style Questionnaire (CHAEA) to 166 students. The data was processed with the Stata (version 13.0) statistical package for Windows. Two models were designed and applied to establish correlations and probability: a linear probability model and a probabilistic probit model. Results. Statistical significance is evident at 10% with respect to the preferences of active and reflective styles. Similarly, there is a possible association between the preference for reflective style and the increased chances for students to pass the course. Conversely, the probability of passing the course is lower when the preference is for the active style. Conclusion: The study evidenced the relationship between gender and pragmatic learning style. It also showed that students with preferences for the reflective learning style have a higher probability of improving their academic performance in the Fundamentals of Economics course. In addition, those who prefer the active learning style are less likely to enhance their academic performance. Objetivo. O presente artigo tem como objetivo analisar a relação entre os estilos de aprendizagem, gênero e o desempenho acadêmico dos estudantes do curso Fundamentos de Economia da Universidad Tecnológica de Bolívar (UTB). Metodologia. A abordagem do estudo é quantitativa, com um tipo de pesquisa descriptivo-correlacional. Foi aplicado o questionário de Estilos de Aprendizagem Honey-Alonso (CHAEA) a um grupo de 166 estudantes. Os dados foram processados com o pacote estatístico Stata versão 13.0 para Windows. Para estabelecer as correlações e a probabilidade, foram desenhados e aplicados dois modelos: um do tipo linear de probabilidade e um modelo probabilístico do tipo probit. Resultados. Foi evidenciada uma significância estatística ao 10% respeito às preferências dos estilos ativo e reflexivo. Da mesma maneira, se observou uma possível associação entre a preferência pelo estilo reflexivo e o aumento das probabilidades de aprovação do curso pelo estudante. Ao contrário, a probabilidade de aprovação do curso é menor quando a preferência é pelo estilo ativo. Conclusão. O estudo possibilitou demonstrar a relação entre gênero e estilo pragmático de aprendizagem. Observou-se também que estudantes com preferências pelo estilo reflexivo têm maior probabilidade de melhorar o desempenho acadêmico no curso de Fundamentos de Economia. Além disso, aqueles que têm preferência pelo estilo ativo, sua probabilidade de melhorar o desempenho acadêmico é menor.

Abstract

Hyperkalaemia (HK) is one of the most common electrolyte disorders and a frequent reason for nephrological consultations. High serum potassium (K+) levels are associated with elevated morbidity and mortality, mainly due to life-threatening arrhythmias. In the majority of cases, HK is associated with chronic kidney disease (CKD), or with the use of renin-angiotensin-aldosterone system inhibitors (RAASis) and/or mineral corticoid antagonists (MRAs). These drugs represent the mainstays of treatment in CKD, HF, diabetes, hypertension, and even glomerular diseases, in consideration of their beneficial effect on hard outcomes related to cardiovascular events and CKD progression. However, experiences in relation to the Randomised Aldactone Evaluation Study (RALES) cast a long shadow that extends to the present day, since the increased risk for HK remains a major concern. In this article, we summarise the physiology of K+ homeostasis, and we review the effects of dietary K+ on blood pressure and cardiovascular risk in the general population and in patients with early CKD, who are often not aware of this disease. We conclude with a note of caution regarding the recent publication of the SSaSS trial and the use of salt substitutes, particularly in patients with a limited capacity to increase K+ secretion in response to an exogenous load, particularly in the context of "occult" CKD, HF, and in patients taking RAASis and/or MRAs.

21. Optimizing Mandibular Advancement Maneuvers during Sleep Endoscopy with a Titratable Positioner: DISE-SAM Protocol

Abstract

Mandibular advancement devices (MAD) are an effective alternative treatment to CPAP. Different maneuvers were performed during drug sleep-induced endoscopy (DISE) to mimic the effect of MAD. Using the Selector Avance Mandibular (SAM) device, we aimed to identify MAD candidates during DISE using a titratable, reproducible, and measurable maneuver. This DISE-SAM protocol may help to find the relationship between the severity of the respiratory disorder and the degree of response and determine the advancement required to improve the collapsibility of the upper airway. Explorations were performed in 161 patients (132 males; 29 females) with a mean age of 46.81 (SD = 11.42) years, BMI of 27.90 (SD = 4.19) kg/m(2), and a mean AHI of 26.51 (SD = 21.23). The results showed no relationship between severity and MAD recommendation. Furthermore, there was a weak positive relationship between the advancement required to obtain a response and the disease severity. Using the DISE-SAM protocol, the response and the range of mandibular protrusion were assessed, avoiding the interexaminer bias of the jaw thrust maneuver. We suggest prescribing MAD as a single, alternative, or multiple treatment approaches following the SAM recommendations in a personalized design.

Abstract

The global upper ocean has been warming during the last decades accompanied with a chlorophyll-α (Chl-α) and productivity decrease. Whereas subtropical gyres show similar trends, Eastern Boundary Upwelling Systems are thought to increase in productivity due to increased trade winds. This study analyzes recent trends in sea surface temperature (SST), Chl-α, net primary production (NPP) and meridional wind stress in the Eastern North Atlantic subtropical gyre (NASE) in order to examine if the global trends can be detected in open ocean and upwelling areas and how the ocean biota responds. Satellite data of such variables of the last 15-40 years were analyzed to calculate mean trends in upwelling areas in the Canary upwelling system and open ocean areas around the Azores, Madeira and the Canary Islands. Our results show significant warming in the area with a maximum of 2.7°C per century for the Azores. Moreover, a general decreasing trend for Chl-α and NPP seems to be more evident in the permanent upwelling areas, which will be responsible for a loss of 0.13% of the global NPP per century. Our results also highlight a significant expansion of the oceanic desert area of 10% with an increase in unproductive days of up to 84 days in the last 20 years. The competitive relationship of stratification and wind stress in the Canary upwelling system might be a more plausible explanation for the decrease in Chl-α and NPP in upwelling areas linked to the increase in upwelling favorable wind stress and the surface warming. Abstract

Disorders of mineral metabolism, including secondary hyperparathyroidism, are thought to contribute to extraskeletal (including vascular) calcification among patients with chronic kidney disease. It has been hypothesized that treatment with the calcimimetic agent cinacalcet might reduce the risk of death or nonfatal cardiovascular events in such patients.In this clinical trial, we randomly assigned 3883 patients with moderate-to-severe secondary hyperparathyroidism (median level of intact parathyroid hormone, 693 pg per milliliter [10th to 90th percentile, 363 to 1694]) who were undergoing hemodialysis to receive either cinacalcet or placebo. All patients were eligible to receive conventional therapy, including phosphate binders, vitamin D sterols, or both. The patients were followed for up to 64 months. The primary composite end point was the time until death, myocardial infarction, hospitalization for unstable angina, heart failure, or a peripheral vascular event. The primary analysis was performed on the basis of the intention-to-treat principle.The median duration of study-drug exposure was 21.2 months in the cinacalcet group, versus 17.5 months in the placebo group. The primary composite end point was reached in 938 of 1948 patients (48.2%) in the cinacalcet group and 952 of 1935 patients (49.2%) in the placebo group (relative hazard in the cinacalcet group vs. the placebo group, 0.93; 95% confidence interval, 0.85 to 1.02; P=0.11). Hypocalcemia and gastrointestinal adverse events were significantly more frequent in patients receiving cinacalcet.In an unadjusted intention-to-treat analysis, cinacalcet did not significantly reduce the risk of death or major cardiovascular events in patients with moderate-to-severe secondary hyperparathyroidism who were undergoing dialysis. Abstract

Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Abstract

Objetivo: Estimar el grado de asociación entre el apiñamiento anteroinferior y la presencia de terceros molares inferiores en modelos de estudio y radiografías panorámicas de pacientes atendidos en el Postgrado de Ortodoncia de la Facultad de Odontología de la Universidad de Cartagena y centros de atención odontológica y radiológica de la ciudad de Cartagena. Diseño del estudio: Estudio de corte transversal. Emplazamiento: Facultad de Odontología de la Universidad de Cartagena de Indias (Colombia) y otros centros de atención odontológica de la ciudad. Participantes: 366 modelos de estudio y radiografías panorámicas de pacientes seleccionados por estrictos criterios de inclusión y exclusión. Mediciones principales: Se utilizó un instrumento que incluyó edad, sexo, presencia o ausencia de terceros molares, posición de terceros molares según la clasificación de Winter, estadio de formación de los terceros molares según la clasificación de Nolla; y magnitud de apiñamiento según la clasificación de Harfin. Los datos fueron analizados a partir de distribuciones de frecuencia y proporciones; se realizó análisis inferencial (prueba chi²) utilizando el programa SPSS statistic v22 IBM y se efectuó análisis multivariante utilizando el análisis de correspondencias simple en el programa estadístico R. Resultados: La prevalencia de apiñamiento anteroinferior fue de 70,2%. Se presentó asociación estadísticamente significativa entre la magnitud de apiñamiento anteroinferior con el estadio de formación del tercer molar inferior izquierdo. Conclusiones: La prevalencia de apiñamiento anteroinferior en los sujetos fue alta; el estadio de formación del tercer molar es un factor determinante en la aparición de apiñamiento anteroinferior. Participants: 366 study casts and panoramic radiographs of patients selected by strict inclusion and exclusion criteria. Main Surveyed Data: An instrument which included age, sex, presence or absence of third molars,third molars position by classification of Winter, stage formation of the third molars by classification of Nolla and the magnitude of crowding byclassification of Harfin. Data were analyzed based on frequency distributions and proportions; inferential analysis was performed using the chi² test with the SPSS statistic v22 IBM program. In addition, multivariate analysis was performed using simple correlation analysis in the R statistical program. Results: The prevalence of anterior crowding was 70.2%. Statistically significant association between the magnitude of anterior crowding with the stage formation of the lower left third molar was presented. Conclusions: Prevalence of anterior crowding in the subjects was high; stage formation of the third molar is a determining factor in the appearance of anterior crowding.

Abstract

To follow-up loci discovered by the International Genomics of Alzheimer's Disease Project, we attempted independent replication of 19 single nucleotide polymorphisms (SNPs) in a large Spanish sample (Fundacio ACE data set; 1808 patients and 2564 controls). Our results corroborate association with four SNPs located in the genes INPP5D, MEF2C, ZCWPW1 and FERMT2, respectively. Of these, ZCWPW1 was the only SNP to withstand correction for multiple testing (P = 0.000655). Furthermore, we identify TRIP4 (rs74615166) as a novel genome-wide significant locus for Alzheimer's disease risk (odds ratio = 1.31; confidence interval 95% (1.19-1.44); P = 9.74 x 10(-9)).
