Introduction: Simulations with standardized patients (SP) have long been used for teaching/assessing communication skills. The present study describes and evaluates an experiential training methodology aimed at medical students and based on interviews with standardized simulated patients. The training was focused on developing basic communication skills and taking medical histories. Methods: Longitudinal observational study of a cohort of third-year medical students. Three interviews with SP were carried out and videotaped. These interviews were assessed by the students, the SPs and the professors of the relevant subject areas. Results: 83 students conducted the interviews. The self-evaluations performed by the students showed an improvement between the first and third interviews, as demonstrated by the increase of 6.7% (CI 95%=3.6-10.0%) (p, Einführung: Simulationen mit standardisierten Patienten (SP) werden seit langem für die Unterricht/Bewertung von Kommunikationsfähigkeiten eingesetzt. Die vorliegende Studie beschreibt und evaluiert eine erfahrungsbasierte Trainingsmethodik, die sich an Medizinstudenten richtet und auf Interviews mit standardisierten simulierten Patienten basiert. Das Training konzentrierte sich auf die Entwicklung grundlegender Kommunikationsfähigkeiten und die Erfassung von Krankengeschichten. Methoden: Längsschnittbeobachtungsstudie an einer Kohorte von Medizinstudenten im dritten Studienjahr. Es wurden drei Interviews mit SP durchgeführt und auf Video aufgenommen. Diese Interviews wurden von den Studierenden, den SPen und den Professoren der entsprechenden Fachbereiche ausgewertet. Ergebnisse: 83 Studenten führten die Interviews durch. 5. Ibero-American consensus on learning outcomes for the acquisition of competencies by medical students through clinical simulation.

- Abstract
OBJECTIVE: Our aim was to reach expert consensus on specific learning outcomes (LOs) that can be achieved through clinical simulation aimed at developing the competencies that medical students need to be able to successfully manage patients and assume general clinical responsibilities. MATERIALS AND METHODS: The six-member scientific committee peer-reviewed Spanish reference documentation (in line with the Bologna Process) on required competencies in medical undergraduate students to select an initial set of 16 competencies that could feasibly be developed through simulation and a corresponding set of 75 LOs. Snowball sampling was used to identify candidates for an international panel of simulation experts. Applying a set of pre-defined criteria, 19 panelists from seven Spanish-speaking regions were recruited to participate in a modified two-round Delphi procedure based on electronic questionnaires and aimed at reaching formal consensus on appropriate LOs for simulated medical training. RESULTS: Final agreement between the panelists was high: no mean score fell below 7.26 of a maximum of 9, and all 75 LOs were agreed on, 74 in the first round and only one requiring the second round. The 16 LOs with mean scores in the top 25th percentile were selected as a set of core LOs to attain via simulation. CONCLUSIONS: This Ibero-American consensus on observable and measurable LOs, reflecting competencies that can feasibly be developed via clinical simulation, is a framework that aims at helping medical schools' plans and delivering specific kinds of undergraduate medical training through simulation. There is extensive information of the beneficial effects of virgin olive oil (VOO), especially on cardiovascular diseases. Some VOO healthy properties have been attributed to their phenolic-compounds (PCs). The aim of this review is to present updated data on the effects of olive oil (OO) PCs on the gut microbiota, lipid metabolism, immune system, and obesity, as well as on the crosstalk among them. We summarize experiments and clinical trials which assessed the specific effects of the olive oil phenolic-compounds (OOPCs) without the synergy with OO-fats. Several studies have demonstrated that OOPC consumption increasesBacteroidetesand/or reduces theFirmicutes/Bacteroidetesratio, which have both been related to atheroprotection. OOPCs also increase certain beneficial bacteria and gut-bacteria diversity which can be therapeutic for lipid-immune disorders and obesity. Furthermore, some of the mechanisms implicated in the crosstalk between OOPCs and these disorders include antimicrobial-activity, cholesterol microbial metabolism, and metabolites produced by bacteria. Specifically, OOPCs modulate short-chain fatty-acids produced by gut-microbiota, which can affect cholesterol metabolism and the immune system, and may play a role in weight gain through promoting satiety. Since data in humans are scarce, there is a necessity for more clinical trials designed to assess the specific role of the OOPCs in this crosstalk.

Polyphenols have an important protective role against a number of diseases, such as atherosclerosis, brain dysfunction, stroke, cardiovascular diseases, and cancer. Cardiovascular diseases are the number one cause of death worldwide: more people die annually from cardiovascular diseases than from any other cause. The most important behavioural risk factors of heart disease and stroke are unhealthy diet, physical inactivity, tobacco use, and excess alcohol intake. The dietary consumption of polyphenols has shown to be inversely associated with morbidity and mortality by cardio- and cerebrovascular diseases. It is well-known that the protective effects of polyphenols in vivo depend on the grade how they are extracted from food and on their intestinal absorption, metabolism, and biological action with target tissues. The aim of this review was to summarise the relation between polyphenols of different plant sources and stroke in human intervention studies, animal models, and in vitro studies.

Objective. To evaluate the long-Term effects of a Mediterranean diet (MeDiet) intervention on the plasma concentrations of inflammatory and plaque stability-related molecules in elderly people at high risk for cardiovascular disease. Design and Setting. 66 participants from primary care centers affiliated with the Hospital Clinic of Barcelona were randomized into 3 groups: MeDiet plus extra virgin olive oil (EVOO) or nuts and a low-fat diet (LFD). At baseline and at 3 and 5 years, we evaluated the changes in the plasma concentrations of 24 inflammatory biomarkers related to the different stages of the atherosclerotic process by Luminex®. 13. Effects of White Wine Consumption on Weight in Rats: Do Polyphenols Matter?

INTRODUCTION: Effects of white wine and the role of wine polyphenols on weight gain in rats of different age were examined in the 4-week-voluntary-consumption trial. METHODS AND MATERIALS: Biochemically characterized standard (low polyphenols, W) and macerated (high polyphenolic content, PW) white wines were compared. One- and three-month-old Sprague-Dawley male rats (n = 78) were used. Each age group was subdivided into water-only-drinking controls (C), W, and PW-drinking animals. Daily wine and total liquid consumption, food intake, and body weight were measured, and energy intake and feed efficiency index were calculated. RESULTS: In both age categories, wine-drinking animals consumed less food and gained less weight in comparison to C (181 ± 2, 179 ± 6, and 201 ± 5 in younger animals and 32 ± 5, 28 ± 6, and 47 ± 4 grams in older animals, resp.), regardless of wine type. Total energy intake was the lowest in PW-drinking animals. CONCLUSION: Wine-drinking animals gained less weight in comparison to C, regardless of the wines' polyphenol content. Although our results are indicative of the major role of nonphenolic constituents of the wines (probably ethanol), the modifying role of wine phenolics on weight gain cannot be excluded as the group consuming PW had lower total energy intake than other groups.

- Abstract
Background-The usefulness of cardiovascular disease (CVD) predictive equations in different populations is debatable. We assessed the efficacy of the Framingham-REGICOR scale, validated for the Spanish population, to identify future CVD in participants, who were predefined as being at high-risk in the PREvención con DIeta MEDiterránea (PREDIMED) study-a nutrition-intervention primary prevention trial-and the impact of adherence to the Mediterranean diet on CVD across risk categories. Methods and Results-In a post hoc analysis, we assessed the CVD predictive value of baseline estimated risk in 5966 PREDIMED participants (aged 55-74 years, 57% women; 48% with type 2 diabetes mellitus). Major CVD events, the primary PREDIMED end point, were an aggregate of myocardial infarction, stroke, and cardiovascular death. Multivariate-adjusted Cox regression was used to calculate hazard ratios for major CVD events and effect modification from the Mediterranean diet intervention across risk strata (low, moderate, high, very high). The Framingham-REGICOR classification of PREDIMED participants was 25.1% low risk, 44.5% moderate risk, and 30.4% high or very high risk. During 6-year follow-up, 188 major CVD events occurred. Hazard ratios for major CVD events increased in parallel with estimated risk (2.68, 4.24, and 6.60 for moderate, high, and very high risk), particularly in men (7.60, 13.16, and 15.85, respectively, versus 2.16, 2.28, and 3.51, respectively, in women). Yet among those with low or moderate risk, 32.2% and 74.3% of major CVD events occurred in men and women, respectively. Mediterranean diet adherence was associated with CVD risk reduction regardless of risk strata (P > 0.4 for interaction). Conclusions-Incident CVD increased in parallel with estimated risk in the PREDIMED cohort, but most events occurred in non- high-risk categories, particularly in women. Eggs from Bufo arenarum are surrounded by four layers of jelly that are needed for fertilization. The mechanism of sperm penetration through these layers is unknown and should not involve acrosome content, as the acrosome reaction seems to occur at the vitelline envelope. The jelly coat is composed of a gel-forming mucin and soluble proteins. One of the main soluble protein is L-HGP, a mucin-like protein. L-HGP is a disulfide-linked heterodimer with O-linked oligosaccharides and estimated [M.sub.r] between 200 and 250. L-HGP expression was detected only in the oviduct. Immunolocalization studies indicated that this protein is present in the apical region of secretory epithelial cells all along the oviduct. Immunoblot analyses on tissue homogenates and secretions of different oviductal portions showed that L-HGP is expressed in all of them and may be present in the four jelly layers. However, two isoforms were detected in different portions. The variability in molecular mass observed between them is due to a difference in the peptide core. This could be related to the gain-loss of some function of the protein depending on the jelly layer localization. Binding assays showed that L-HGP interacts with spermatozoa membranes. Moreover, incubation of spermatozoa with this protein prevents acrosome from breakdown. These combined results suggest that L-HGP may contribute to the migration of spermatozoa through the jelly layers providing antiadhesive properties and preventing acrosome reaction until the spermatozoa reach the vitelline envelope.

This article documents the addition of 268 microsatellite marker loci to the Molecular Ecology Resources Database. Loci were developed for the following species: Alburnoidesbipunctatus, Chamaeropshumilis, Chlidoniashybrida, Cyperuspapyrus, Fusariumgraminearum, Loxigillabarbadensis, Macrobrachiumrosenbergii, Odontesthesbonariensis, Pelteobagrusvachelli, Posidoniaoceanica, Potamotrygonmotoro, Rhamdia quelen, Sarotherodonmelanotheron heudelotii, Sibiraeaangustata, Takifugurubripes, Tarentolamauritanica, Trimmatostroma sp. and Wallagoattu. Abstract: Many fish species display compensatory growth (CG), a phenomenon by which fasted fish grow faster during refeeding. However, most studies use a group‐housed fish approach that could be problematic in social fish when interaction between individuals is not considered or eliminated. Additionally, the growth hormone (GH)/insulin‐like growth factors’ (IGF‐1 and IGF‐2) axis is implicated in postnatal growth in vertebrates, but its relevance in CG is not fully understood. Thus, the aim of this work was to determine whether CG occurs in a social fish, Cichlasoma dimerus, using an individually held fish approach and secondly, to evaluate the GH/IGFs expression profile during refeeding by 3 days and 3 weeks. C. dimerus showed partial CG. The feed conversion efficiency (FCE) was higher in three‐day‐refed fish, which presented higher GH plasma and mRNA levels than controls but shown no differences in liver and muscle GH receptors (GHR1 and GHR2) and IGFs mRNA levels. Surprisingly, three‐week‐refed fish exhibited GHR1 and IGF‐2 increments, but a reduction in GHR2 expression in muscle. These results show a strong association between GH levels, growth rate and FCE during refeeding, and a long‐lasting effect of refeeding on muscular expression of GHRs and IGF‐2. El almacenamiento de espermatozoides de peces por medio de la criopreservación es muy utilizado hoy en día. Si bien se sabe que el proceso de congelamiento/descongelamiento genera la fragmentación del ADN del espermatozoide, no se conocen las consecuencias que esto tiene en el desarrollo embrionario. Actualmente hay un gran interés en el desarrollo de métodos de fecundación in vitro para la especie Prochilodus lineatus utilizando semen criopreservado. En este trabajo estudiamos el desarrollo embrionario de esta especie, a fin de sentar las bases para la observación de anormalidades en el desarrollo de embriones obtenidos a partir de espermatozoides crioconservados. Los huevos producidos por esta especie son translúcidos y poseen un gran espacio perivitelino; son del tipo telolecítico y presentan una división meroblástica durante los primeros estadíos del desarrollo. Si bien el desarrollo embrionario de Prochilodus lineatus (sábalo) se llevó a cabo en menor tiempo en comparación con Danio rerio, las etapas que atravesó el embrión son muy similares. Durante el estudio se observaron los períodos de Cigoto, Clivaje, Blástula, Gástrula, Segmentación y Eclosión de la larva de sábalo. De esta manera se logró registrar los diferentes estadíos de su desarrollo, principalmente aquellos posteriores a la transición de la blástula media (TBM), a partir del cual el embrión comienza a transcribir su propio genoma. Currently, the storage of fish spermatozoa through cryopreservation is widely used. Although it is of common knowledge that the process of freezing / thawing generates DNA fragmentation of spermatozoa, the consequences of this process on embryonary development are unknown. There is great interest in developing methods for in vitro fertilization for the species Prochilodus lineatus using cryopreserved semen. In this paper we study the embryonic development of this fish, to lay the groundwork for the observation of abnormalities in the development of embryos derived from criopreserved spermatozoa. The eggs produced by this species are translucent and have a large perivitelline space, are of the telolecitic type, and presented meroblastic division during the early stages of development. Although the embrionary development of Prochilodus lineatus (Sábalo) took place in less time compared with Danio rerio (Zebrafish) the embryo goes through very similar stages. During the study it was possible to observe the periods of zygote, cleavage, blastula, gastrula, segmentation and hatching of the larvae of Sábalo. The different stages of development were successfully recorded, especially those after the transition of blastula media (TBM), when the embryo begins to transcribe its own genome.

- Abstract
[Background and aim]: Hypertension is a major public health problem and a leading cause of death and disability in both developed and developing countries, affecting one-quarter of the world's adult population. Our aim was to evaluate whether the consumption of gazpacho, a Mediterranean vegetable-based cold soup rich in phytochemicals, is associated with lower blood pressure (BP) and/or reduced prevalence of hypertension in individuals at high cardiovascular risk., [Methods and results]: We selected 3995 individuals (58% women, mean age 67 y) at high cardiovascular risk (81% hypertensive) recruited into the PREDIMED study. BP, weight, and dietary and physical activity data were collected. In multivariate linear regression analyses, after adjustment, moderate and high gazpacho consumption categories were associated with reduced mean systolic BP of −1.9 mm Hg [95% confidence interval (CI): −3.4; −0.6] and −2.6 mm Hg (CI: −4.2; −1.0), respectively, and reduced diastolic BP of −1.5 mm Hg (CI: −2.3; −0.6) and −1.9 mm Hg (CI: −2.8; −1.1). By multiple-adjusted logistic regression analysis, gazpacho consumption was associated with a lower prevalence of hypertension, with OR = 0.85 (CI: 0.73; 0.99) for each 250 g/week increase and OR = 0.73 (CI: 0.55; 0.98) for high gazpacho consumption groups compared to the no-consumption group., [Conclusions]: Gazpacho consumption was inversely associated with systolic and diastolic BP and prevalence of hypertension in a cross-sectional Mediterranean population at high cardiovascular risk. The association between gazpacho intake and reduction of BP is probably due to synergy among several bioactive compounds present in the vegetable ingredients used to make the recipe.

En las dos últimas décadas se ha observado una incidencia creciente de una forma peculiar de anafilaxia que sólo se produce al realizar ejercicio físico. Dentro del síndrome de anafilaxia inducida por ejercicio se incluyen dos formas clínicas bien diferenciadas: urticaria colinérgica sistémica y anafilaxia inducida por ejercicio propiamente dicha, que se puede presentar en su forma clásica o en su forma variante, más infrecuente y con manifestaciones similares a la urticaria colinérgica. La anafilaxia postprandial inducida por ejercicio o dependiente de alimentos constituye un subtipo frecuentemente identificado en estos últimos casos. Puede deberse a una alergia alimentaria subclínica que se manifiesta con el esfuerzo, aunque en muchas ocasiones no es posible encontrar un alergeno responsable. El diagnóstico se establece por la historia clínica y la realización de pruebas específicas con alergenos alimentarios. Puede ser necesario recurrir a una prueba de esfuerzo con y sin ingesta previa. El tratamiento es preventivo y se basa en evitar la comida o el alimento responsable de la alergia en las horas previas a la realización del ejercicio. Cuando no depende de alimentos se recurre a farmacoterapia profiláctica con antihistamínicos, cromonas o bicarbonato sódico. El paciente debe ser instruido sobre el uso de adrenalina en caso de nuevas reacciones. In the last two decades a growing incidence of a peculiar form of anaphylaxis that only occurs while carrying out physical exercise has been observed. Within the exercise-induced anaphylaxis syndrome two well differentiated clinical forms are included: systemic cholinergic urticaria and exercise-induced anaphylaxis in the strict sense which can be shown by a classic form or a variant form, more uncommon and with manifestations similar to cholinergic urticaria. Postprandial or food-dependent exercise-induced anaphylaxis is a frequently identified subtype of these last cases. It can be due to an asymptomatic food allergy manifested thraugh physical effort, although in many occasions it is not possible to find a responsible allergen. The diagnosis is settled on the clinical history and specific tests with food allergens. It can be necessary to perform an exercise challenge test with and without previous ingestion. The treatment is preventive and it is based on avoiding the food or the food allergen some hours before the exercise. When it does not depend on foods it is used a prophylactic farmacotherapy with antihistamines, cromones or sodium bicarbonate. The patient should be well educated on the use of epinephrine in the event of new reactions

Background: Epidemiological evidence suggests a cardioprotective role of α‐linolenic acid (ALA), a plant‐derived ω‐3 fatty acid. It is unclear whether ALA is beneficial in a background of high marine ω‐3 fatty acids (long‐chain n‐3 polyunsaturated fatty acids) intake. In persons at high cardiovascular risk from Spain, a country in which fish consumption is customarily high, we investigated whether meeting the International Society for the Study of Fatty Acids and Lipids recommendation for dietary ALA (0.7% of total energy) at baseline was related to all‐cause and cardiovascular disease mortality. We also examined the effect of meeting the society's recommendation for long‐chain n‐3 polyunsaturated fatty acids (≥500 mg/day). Methods and Results: We longitudinally evaluated 7202 participants in the PREvención con DIeta MEDiterránea (PREDIMED) trial. Multivariable‐adjusted Cox regression models were fitted to estimate hazard ratios. ALA intake correlated to walnut consumption (r=0.94). During a 5.9‐y follow‐up, 431 deaths occurred (104 cardiovascular disease, 55 coronary heart disease, 32 sudden cardiac death, 25 stroke). The hazard ratios for meeting ALA recommendation (n=1615, 22.4%) were 0.72 (95% CI 0.56–0.92) for all‐cause mortality and 0.95 (95% CI 0.58–1.57) for fatal cardiovascular disease. The hazard ratios for meeting the recommendation for long‐chain n‐3 polyunsaturated fatty acids (n=5452, 75.7%) were 0.84 (95% CI 0.67–1.05) for all‐cause mortality, 0.61 (95% CI 0.39–0.96) for fatal cardiovascular disease, 0.54 (95% CI 0.29–0.99) for fatal coronary heart disease, and 0.49 (95% CI 0.22–1.01) for sudden cardiac death. The highest reduction in all‐cause mortality occurred in participants meeting both recommendations (hazard ratio 0.63 [95% CI 0.45–0.87]). Conclusions: In participants without prior cardiovascular disease and high fish consumption, dietary ALA, supplied mainly by walnuts and olive oil, relates inversely to all‐cause mortality, whereas protection from cardiac mortality is limited to fish‐derived long‐chain n‐3 polyunsaturated fatty acids.
