Kussmaul disease is characterized by recurrent pain and swelling in the major salivary glands that arise from obstruction of salivary ducts by mucofibrinous plugs. In this case report, a 15-year-old girl presented with complaints of pain and recurrent swelling in both parotid regions exacerbated by food intake, which had been persisting for 2 years. History revealed allergic asthma and allergic rhinitis. Head and neck examination revealed diffuse swelling and tenderness in the both parotid glands, mucofibrinous plug drainage from the opening of the bilateral Stensen's ducts, diffuse nasal polyposis, and bilateral otitis media with effusion. The serum eosinophil and the serum immunoglobulin E levels were high while the erythrocyte sedimentation rate and C-reactive protein levels were normal. Computed tomography of the head and neck revealed diffuse ductal ectasia with the enlarged Stensen's duct in the parotid gland and diffuse increased density in all the paranasal sinuses. Yellowish mucofibrinous plugs were seen within the Stensen's duct on sialendoscopy. Histopathological examination of the mucofibrinous plugs demonstrated Charcot-Leyden crystals with profuse eosinophils. In conclusion, mucofibrinous plugs and the enlarged salivary ducts on sialendoscopy may be a sign of Kussmaul disease diagnosis. Sialendoscopy may be a useful diagnostic method and therapeutic approach along with other relevant treatment methods for Kussmaul disease. [ABSTRACT FROM AUTHOR]

Objective: To analyze the demographic characteristics and the pathological results of neck dissection in primary parotid gland (PG) cancer patients, and to investigate the effects of histopathological parameters (perineural invasion, lymphovascular invasion, and extracapsular spread), neck metastasis, stage and lymph node ratio (LNR) on survival. Methods: Patients who underwent parotidectomy for malignant PG tumors between 2000 and 2019 years were retrospectively reviewed from the medical records. Thirty patients who were treated with parotidectomy and neck dissection were included in the study. Lymph node ratio was calculated as the ratio of the number of metastatic lymph nodes (LN) to the total number of excised LNs. Tumor stage, regional LN metastasis, LNR, perineural invasion, lymphovascular invasion, and extracapsular spread were reviewed for the effects on survival with the Kaplan-Meier analysis. Results: The study included 17 (57%) male and 13 (43%) female patients. Their mean age was 67.93±16.90 years (range, 50-85 years). The average number of the excised LN was 26.03±11.79 (range, 3-50). Mean LNR was 0.16±0.26. The Kaplan-Meier analysis showed that neck metastasis (p=0.001) and LNR (p<0.001) were associated with shorter survival times compared to perineural invasion (p=0.818), lymphovascular invasion (p=0.154), extracapsular spread (p=0.410) and stage (p=0.294). In multivariate COX regression analysis, only LNR had a statistically significant difference (p=0.027) compared to the other parameters. Conclusion: The present study suggests that LNR and neck metastasis are associated with shorter survival times in PG cancers. Lymph node ratio can be used as a prognostic marker in these patients. Papillary thyroid carcinoma (PTC) is the most common type of thyroid malignancy and it can cause cystic metastasis to the neck region. Branchial cleft cysts (BCCs) are among the most commonly encountered congenital cystic neck masses. PTC metastasis into BCC has been rarely reported in the literature. In this case report we report a patient presenting with a cystic neck mass for which fine needle aspiration biopsy was reported as benign cytology and eventually was diagnosed as PTC metastasis to the BCC. The importance of investigating the thyroglobulin levels in fine needle aspiration materials of cystic neck masses for early diagnosis of possible PTC metastasis is specifically emphasized. This study aimed to evaluate the antimicrobial effects of the medicinal plant Ecballium elaterium, which is topically applied as a traditional medicine for the treatment of rhinosinusitis. Pure and extract forms of E elaterium were applied to the nasal cavity of rats for the treatment of Streptococcus-pneumoniae–induced rhinosinusitis. The nasal mucosa, soft palate, and trachea of the rats were harvested in the first hour, third day, and third week for histopathologic evaluation. This experiment revealed the anti-inflammatory effects of E elaterium and showed that it reduced fibrosis. The anti-inflammatory effect of all forms of E elaterium was found to reach its maximum level on the third day, decreasing by the third week. We also observed that the pure form of E elaterium caused severe epithelium loss in the first hour after application. The mechanism of the anti-inflammatory effect of different levels of extract forms needs to be further analyzed with different doses and duration of treatment. [ABSTRACT FROM AUTHOR]

Desmoplastic neurotropic melanoma (DNM) is known as a rare variant of cutaneous melanoma. The authors defined the term ‘neurotropic' which is used to refer to the associated nerve infiltration or neural differentiation. A 74-year-old female applied to the plastic surgery clinic with a one-year history of a nodule in the left infraorbital skin. The lesion was excised by the surgeons and was sent to the pathology department. The tumor with spindle cells in a scar-like stroma was detected microscopically and diagnosed as a ‘dermatofibroma’. Eight months after surgery, a deep-seated nodule recurred at the same place. This nodule was re-excised. In this sample, we observed hypercellularity, atypical mitoses and nerve infiltration of the spindle tumor cells having strong positive staining with S-100 protein and negative staining with HMB-45, thus, the ultimate diagnosis was DNM. The differential diagnosis of this lesion includes many benign and malignant entities. This is crucial because of the potential for recurrence and metastasis of the lesion. OBJECTIVE: This study planned to evaluate the effects of vitamin A (VA) and steroid treatment on nerve healing. Study design: we investigated electrophysiological, electron microscopic (EM), immunohistochemical, and biochemical findings in rat sciatic nerve model. MATERIALS and METHODS: In total, 112 female rats were divided into 16 groups, each consisting of 7 animals. Subjects were evaluated according to compound muscle action potential (CMAP) findings, ultrastructural staging of myelinated axons with EM, nitric oxide (NO) and malondialdehyde (MDA) findings in serum and tissue, and immunohistochemical findings in the first week and first month. RESULTS: NO and MDA levels in serum and tissue were found to be statistically lower after high-dose methylprednisolone (MP), normal-dose MP, high-dose MP+VA, normal-dose, and MP+VA treatment modalities compared to controls (p<0.05). The most similar EM findings compared to control animals were seen in the normal-dose MP and high-dose MP+VA treatment groups. The best amplitude values were seen in the high-dose MP and continuous normal-dose MP therapy groups in the first month. In the normal-dose MP and high-dose MP+VA treatment groups, intense staining was found compared to the control and sham groups according to the sciatic nerve immunohistochemical examination. CONCLUSION: This study revealed noticeable VA effects on the inflammatory process during the healing of traumatic nerve paralysis by electron microscopy and immunohistochemistry. However, similar findings were not established by electrophysiology. Therefore, more experimental studies are needed to further investigate the efficacy and the mechanism of action of VA on steroid administration and the inflammatory process. [ABSTRACT FROM AUTHOR]

Inflammatory myofibroblastic pseudotumor (plasma cell granuloma) is a soft tissue lesion consisting of myofibroblasts, mature lymphocytes, histiocytes, plasma cells, eosinophils, and extracellular collagen. Various sites in the body may harbor these lesions. Lungs, omentum, intestines, mesentery, and urinary system are the most susceptible areas. It is usually seen in children and young adults. The lesion is rarely detected in the head and neck region. The orbit and the upper respiratory system are the most common localizations in the head and neck region. Sinonasal tract is a rare site of involvement. The differential diagnosis includes squamous cell carcinoma (spindle cell variant), inflammatory fibrosarcoma, leiomyosarcoma, schwannoma, and nonspecific inflammation. Our patient who had a sinonasal mass showed a benign tumor consisting of spindle tumor cells and inflammatory cells histopathologically. This case was presented due to its rare existence to this site.

Objective: This study was made to emphasize the importance of cystic echinococcosis in the Mersin province and to contribute to informarion regarding the geographic distribution of infection in Turkey., Methods: Seven different pathology laboratory records in the Mersin province between the period 2011-2012 were evaluated according to age, sex and organ involvement., Results: In study period, a total of 119 cases of cystic echinococcosis were determined. Male patients constituted 52.94% of the cases, and female, 47.05%. The age distribution of these cases was between 5 and 76 years. Most of the cases appeared in 41-50 (22.68%) year old patients. The most common localizations for cystic echinococcosis was determined in the liver (n: 53, 44.53%) and lung (n: 39, 32.77%), followed by the kidney (n: 4, 3.36%), spleen (n: 3, 2.52%) and gall bladder (n: 3, 2.52%). In 3 cases (2.52%), multiorgan involvement was seen., Conclusion: Cystic echinococcosis is an important health problem in Turkey. The stray dogs, which play an important role in the contagiousness of the disease, should be kept under control and treated. Also, slaughtering of animals must be preformed under the supervision of veterinary surgeons and destruction of infected organs should be carried out under proper conditions in slaughterhouses.
