Background:  The early identification of outbreaks of both known and novel influenza-like illnesses (ILIs) is an important public health problem., Objective:  This study aimed to describe the design and testing of a tool that detects and tracks outbreaks of both known and novel ILIs, such as the SARS-CoV-2 worldwide pandemic, accurately and early., Methods:  This paper describes the ILI Tracker algorithm that first models the daily occurrence of a set of known ILIs in hospital emergency departments in a monitored region using findings extracted from patient care reports using natural language processing. We then show how the algorithm can be extended to detect and track the presence of an unmodeled disease that may represent a novel disease outbreak., Results:  We include results based on modeling diseases like influenza, respiratory syncytial virus, human metapneumovirus, and parainfluenza for 5 emergency departments in Allegheny County, Pennsylvania, from June 1, 2014, to May 31, 2015. We also include the results of detecting the outbreak of an unmodeled disease, which in retrospect was very likely an outbreak of the enterovirus D68 (EV-D68)., Conclusions:  The results reported in this paper provide support that ILI Tracker was able to track well the incidence of 4 modeled influenza-like diseases over a 1-year period, relative to laboratory-confirmed cases, and it was computationally efficient in doing so. Over the past decade, outbreaks of new or reemergent viruses such as severe acute respiratory syndrome (SARS) virus, Middle East respiratory syndrome (MERS) virus, and Zika have claimed thousands of lives and cost governments and healthcare systems billions of dollars. Because the appearance of new or transformed diseases is likely to continue, the detection and characterization of emergent diseases is an important problem. We describe a Bayesian statistical model that can detect and characterize previously unknown and unmodeled diseases from patient-care reports and evaluate its performance on historical data., Competing Interests: The authors have declared that no competing interests exist.

The prediction and characterization of outbreaks of infectious diseases such as influenza remains an open and important problem. This paper describes a framework for detecting and characterizing outbreaks of influenza and the results of testing it on data from ten outbreaks collected from two locations over five years. Outbreaks of infectious diseases such as influenza are a significant threat to human health. Because there are different strains of influenza which can cause independent outbreaks, and influenza can affect demographic groups at different rates and times, there is a need to recognize and characterize multiple outbreaks of influenza. This paper describes a Bayesian system that uses data from emergency department patient care reports to create epidemiological models of overlapping outbreaks of influenza. Clinical findings are extracted from patient care reports using natural language processing. These findings are analyzed by a case detection system to create disease likelihoods that are passed to a multiple outbreak detection system. We evaluated the system using real and simulated outbreaks. The results show that this approach can recognize and characterize overlapping outbreaks of influenza. Objectives: This study evaluates the accuracy and transferability of Bayesian case detection systems (BCD) that use clinical notes from emergency department (ED) to detect influenza cases., Methods: A BCD uses natural language processing (NLP) to infer the presence or absence of clinical findings from ED notes, which are fed into a Bayesain network classifier (BN) to infer patients' diagnoses. We developed BCDs at the University of Pittsburgh Medical Center (BCDUPMC) and Intermountain Healthcare in Utah (BCDIH). At each site, we manually built a rule-based NLP and trained a Bayesain network classifier from over 40,000 ED encounters between Jan. 2008 and May. 2010 using feature selection, machine learning, and expert debiasing approach. Transferability of a BCD in this study may be impacted by seven factors: development (source) institution, development parser, application (target) institution, application parser, NLP transfer, BN transfer, and classification task. We employed an ANOVA analysis to study their impacts on BCD performance., Results: Both BCDs discriminated well between influenza and non-influenza on local test cases (AUCs > 0.92). When tested for transferability using the other institution's cases, BCDUPMC discriminations declined minimally (AUC decreased from 0.95 to 0.94, p<0.01), and BCDIH discriminations declined more (from 0.93 to 0.87, p<0.0001). We attributed the BCDIH decline to the lower recall of the IH parser on UPMC notes. The ANOVA analysis showed five significant factors: development parser, application institution, application parser, BN transfer, and classification task., Conclusion: We demonstrated high influenza case detection performance in two large healthcare systems in two geographically separated regions, providing evidentiary support for the use of automated case detection from routinely collected electronic clinical notes in national influenza surveillance. The transferability could be improved by training Bayesian network classifier locally and increasing the accuracy of the NLP parser.

Community-acquired pneumonia (CAP) is an important clinical condition with regard to patient mortality, patient morbidity, and healthcare resource utilization. The assessment of the likely clinical course of a CAP patient can significantly influence decision making about whether to treat the patient as an inpatient or as an outpatient. That decision can in turn influence resource utilization, as well as patient well being. Predicting dire outcomes, such as mortality or severe clinical complications, is a particularly important component in assessing the clinical course of patients. We used a training set of 1601 CAP patient cases to construct 11 statistical and machine-learning models that predict dire outcomes. We evaluated the resulting models on 686 additional CAP-patient cases. The primary goal was not to compare these learning algorithms as a study end point; rather, it was to develop the best model possible to predict dire outcomes. A special version of an artificial neural network (NN) model predicted dire outcomes the best. Using the 686 test cases, we estimated the expected healthcare quality and cost impact of applying the NN model in practice. The particular, quantitative results of this analysis are based on a number of assumptions that we make explicit; they will require further study and validation. Nonetheless, the general implication of the analysis seems robust, namely, that even small improvements in predictive performance for prevalent and costly diseases, such as CAP, are likely to result in significant improvements in the quality and efficiency of healthcare delivery. Therefore, seeking models with the highest possible level of predictive performance is important. Consequently, seeking ever better machine-learning and statistical modeling methods is of great practical significance.

Medical records can form the basis of retrospective studies, be used to evaluate hospital practices and guidelines, and provide examples for teaching medicine. Each of these tasks presumes the ability to accurately identify patient subgroups. We describe a method for selecting patient subgroups based on the text of their medical records and demonstrate its effectiveness. We also describe a modification of the basic system that does not assume the existence of a preclassified training set, and illustrate its effectiveness in one retrieval task.

The poinsettia (Euphorbia pulcherrima) is a much-maligned plant which is thought by the public and some health professionals to be extremely toxic. Despite pronouncements by public health officials to the contrary, the poinsettia continues to be recognized as a poisonous plant. To determine if there was any validity to the toxicity claims, 849,575 plant exposures reported to the American Association of Poison Control Centers were electronically analyzed. Poinsettia exposures accounted for 22,793 cases and formed the subset that was analyzed to critically evaluate the morbidity and mortality associated with poinsettia exposures. There were no fatalities among all poinsettia exposures and 98.9% were accidental in nature, with 93.3% involving children. The majority of exposed patients (96.1%) were not treated in a health care facility and 92.4% did not develop any toxicity related to their exposure to the poinsettia. Most patients do not require any type of therapy and can be treated without referral to a health care facility.

Plant exposures are the fourth most common cause of poisoning and 86% of those exposures involve the pediatric population. The large number of plant-related exposures and the lack of knowledge about plant toxicity has led to plant paranoia and considerable educational efforts to reduce the number of exposures. These efforts are often dictated by misconceptions and folklore. AAPCC TESS data on all plant exposures for the years 1985-1994 were electronically analyzed by plant species and state to establish a frequency distribution in the US. Aggregate national data was also tabulated. A total of 912,534 plant exposures were analyzed to tabulate the top 30 plant exposures for each state. Philodendron species were the most common exposures, followed by Dieffenbachia species, Euphorbia pulcherrima, Capsicum annuum and Ilex species. There were considerable differences between states relating to indoor vs outdoor plants and native vs introduced varieties. Plant exposures are common and poison information centers devote significant clinical service and educational effort to manage them and to enhance public awareness. Recognition of specific plant species and exposure frequency in a poison center region can be a basis for staff education and the development of appropriate poison prevention education brochures. This can direct better utilization of poison center's resources.
